Medical paternalism supported by hospitals and courts has been, will be, and should be used in those few situations where clearly incompetent or irresponsible parents resist treatment that health professionals generally agree is needed and effective. If used in other circumstances, it is likely to erode family and professional autonomy and bring about an oppressive "medical Vietnam" (costly, sometimes abusive, use of technology to ensure biologic existence with little regard for quality of child and family life and competing interests). Families must be relied upon to help resolve most complex tragedies because there is no better alternative. They are likely to become stronger if they participate in decisions for care of their severely defective children. Such decisions logically include those for life or death.

Patient education is an essential component of the management of musculoskeletal disorders. However, despite clear guidance that health professionals should be providing education for patients, uncertainty remains about the optimum strategies which should be adopted—exactly what, when and how education should be offered to patients—as well the effect education has on patients. Like any intervention, evidence is needed to show that education is effective, and to justify the resources required for its production and delivery. Yet education is a complex intervention, and studying its effect is challenging. This chapter focuses on these debates, reviewing current evidence regarding methods of delivering education, the timing and content of education, and how education is evaluated. Finally, it makes practical suggestions about improving the quality of patient education in the future.

BACKGROUND According to WHO, to achieve targets for control of leprosy by 2030, it will require disease elimination and interruption of transmission at the national or regional level. India and Brazil have reported the highest leprosy burden over the decades, revealing the need for strategies and tools to help health professionals correctly manage and control the disease. OBJECTIVE The objective of this study is to assess the quality of leprosy data in Brazil by SINAN (Information System for Notifiable Diseases) and build a web application to increase the accessibility of an accurate method of classifying leprosy treatment for health professionals, especially for communities further away from the country's major diagnostic centers. METHODS Leprosy data were extracted from the SINAN database, carefully cleaned, and used to build artificial intelligence (AI) decision model based on Random Forest (RF) algorithm to predict operational classification in Paucibacillary (PB) or Multibacillary (MB). It used the software: i) Python to extract and clean the data; ii) R to train and test the AI model via cross-validation. To allow broad access, we deployed the final RF classification model in a web application that integrates the cloud service, Microsoft Azure, with a friendly layout built in Bubble.io. It used data available on the IBGE (Brazilian Institute of Geography and Statistics) and the DATASUS (Department of Informatics of the Unified Health System). RESULTS We mapped the dispersion of leprosy incidence in Brazil, 2014 to 2018, and noticed a high number of cases in central Brazil in 2014 that became even higher in 2018, in the state of Mato Grosso. Some municipalities showed discrepancies in the 80% range. We considered inconsistency the fact of not matching a set of standards for leprosy classification, according to WHO. Of a total of 21,047 discrepancies detected, the main was considered the operational rating that does not match the clinical form. After data processing, we identified a total of 77,628 cases with missing data. Regarding the quality of the AI model applied, the sensitivity was 93.97%, and the specificity was 87.09%. In most states of Brazil, human and machine confidence intervals intersect. CONCLUSIONS The proposed APP was able to recognize patterns in leprosy cases registered in the SINAN database and classify new patients as PB or MB, reducing the probability of oversight by health professionals. The collection and notification of data on leprosy in Brazil seem to lack specific validation to increase the quality of the data for implementations via AI. The AI model implemented in this work presented relatively large confidence intervals of accuracy that varied from across Brazilian states. This distortion is possibly due to the quality of the data that fed the classification model.

The paper provides an explanation for widespread student employment in Russia. The main difference between Russia and Western countries is considered to be the following: student employment during the period of studying is widely used as a signal about the quality of human capital for the future employer. The authors show that in Russia more productive students start working earlier and employment is more often connected with the future profession than in the Western countries. They suggest that the system of communication between education structures and employers should be established in order to inform the latter about the quality and features of education acquired.

Weeds are very annoying for farmers and also not very good for the crops. Its existence might damage the growth of the crops. Therefore, weed control is very important for farmers. Farmers need to ensure their agricultural fields are free from weeds for at least once a week, whether they need to spray weeds herbicides to their plantation or remove it using tools or manually. The aim of this research is to build an automated weed control robot using the Lego Mindstorm EV3 which connected to a computer. The robot consists of motors, servo motors and a camera which we use to capture the image of the crops and weeds. An automated image classification system has been designed to differentiate between weeds and crops. The robot will spray the weed herbicides directly to the area that have been detected weeds near or at it. For the image classification method, we employ the convolutional neural network algorithm to process the image of the object. Therefore, by the use of technology especially in artificial intelligence, farmers can reduce the amount of workload and workforce they need to monitor their plantation. In addition, this technology also can improve the quality of the crops.

Immigration checks are one part of the immigration control function which has a strategic role in maintaining security and upholding the country's sovereignty. Along with the times, the implementation of immigration checks has experienced several increasingly complex obstacles and challenges. The use of technology is seen as one of the solutions that can overcome these obstacles and challenges. The Directorate General of Immigration has currently implemented an innovation in the form of autogate which is considered to have provided various benefits, although from the beginning of its implementation, until now autogate has not been able to fully replace the role of immigration officers. This is because immigration checks using autogate are unable to carry out profiling, and interviews with prospective passers at the Immigration Checkpoint, but currently with the discovery of technology in the form of Artificial Intelligence, it is expected to be able to improve the quality of immigration checks with autogate.
