ABSTRACT Introduction: Epilepsy represents one of the main causes of morbidity in pediatric neurology. The early onset of epileptic seizures is related to lower long-term intellectual performance, and alterations in adaptive skills. Objective: To apply the Pediatric Evaluation of Disability Inventory Computer Adaptive Test (PEDI-CAT) report in pediatric patients with epilepsy. Methods: A descriptive, observational, prospective, cross-sectional and analytical study was carried out in the Department of Pediatric Neurology at Hospital Infantil de México Federico Gómez, in Mexico City. Patient recruitment took place between April and December 2020. Results: patients with epilepsy presented a <5 percentile in their mobility skills 22 (35.4%), daily activities19 (30.6%), social-cognitive 19 (30.6%), which means that they are below of normal range compared to children his age. Conclusions: patients with epilepsy presented a <5 percentile in the domain of daily activities, mobility and social-cognitive, which means that they are below the normal range compared to children of their age. Therefore, epilepsy should be considered a disorder that affects the adaptive capacity of patients and not just a pathology whose manifestation is seizures, its early detection with the use of scales such as PEDI-CAT translates into the identification and planning of strategies effective therapeutics to improve the quality of life of patients Keywords: Epilepsy, Adaptation, Psychological, Neurodevelopmental Disorders.

For many decades, epilepsy has remained a serious medical and social problem that requires constant attention not only from neurologists, psychiatrists, but also from medical psychologists and social workers. The priority in the treatment of epilepsy is not only to prevent seizures, but also to help the patient maintain social functioning and restore quality of life. Achieving drug remission of epileptic seizures is only part of the management of the disease, it is necessary to help restore the patient's psychosocial adaptation and form a correct understanding of their disease and related limitations in daily functioning. A comprehensive examination which involved 65 patients with epilepsy of both sexes aged 18–40 years was conducted in compliance with the principles of bioethics and medical deontology. The mean age of the subjects ranged from (34.0±1.6) years, duration of the disease in patients did not exceed 10 years. All subjects had disorders of the emotional sphere of non-psychotic level, which were recorded in the interictal period of the disease. Selected comprehensive system of correction of affective disorders in patients with epilepsy helped to achieve following results, especially: reduction of anxiety and depressive disorders (73.5 %), reducing the level of motor and ideational inhibition (65.2 %), reducing the intensity and duration of depressive affect (72.5 %), restoration of interest in activities that previously interested patients (69.5 %), improvement of psychosocial adaptation (81.3 %), managed to achieve the transition of maladaptive types of attitudes to the disease to adaptive (66.5 %) and improve quality of life (83.2 %). A study of affective disorders in patients with epilepsy allowed us to make following conclusions: epilepsy is often combined with affective disorders. Depressive disorders are more common among all affective disorders. Epilepsy and affective disorders interact with each other and complicate clinical prognosis. Keywords: epilepsy, affective disorders, rehabilitation, psychoeducation.

Background: Carbamazepine and valproate are widely used in the treatment of epileptic seizures. However, these agents exhibit certain adverse effects including hematopoietic disorders (carbamazepine) and severe hepatotoxicity (valproate). Purpose: To determine the extent of monitoring of the hematologic effects of carbamazepine as well as the extent of monitoring of the hepatic effects of valproate in patients with epilepsy receiving treatment with these agents. Method: A cross-sectional antiepileptic drug use study using case notes of patients with epilepsy managed at the neurologic clinics of 2 tertiary medical facilities in Nigeria between January and December 2017. Results: Carbamazepine was the most frequently prescribed antiepileptic drug (48.24%), followed by valproate (29.34%) and levetiracetam (9.24%). Pretreatment monitoring of hematologic effect was carried out in only 61.11% of patients placed on carbamazepine therapy while follow-up monitoring was done in 3.7% of these patients. Also, in patients placed on valproate therapy, pretreatment and follow-up monitoring of the hepatic effect was done in only 33.71% and 19.0% of the patients, respectively. Conclusions: The extent of monitoring of the hematologic effects of carbamazepine, as well as the hepatic effects of valproate in the cohort studied, is poor.

Pregnancy is the most dangerous period in terms of interruption of even persistent and long-term remission. At the same time increasing the dose of anticonvulsant increases the risk of teratogenic effects. Aim. to assess the possibility of using progesterone to prevent relapse of epileptic seizures during pregnancy. Materials and methods. 38 pregnant patients with epilepsy with clinical remission before pregnancy, with relapse of epileptic seizures in I trimester of pregnancy, age 31.81.4 years. Dydrogesterone in a dose of 10 to 60 mg/day was prescribed after the relapse of remission. Anticonvulsant dosage was not changed. The blood progesterone concentration and EEG control was carried out. Results. During pregnancy, the level of progesterone in the blood gradually increased from 77.8 nmol/l at 78 weeks of pregnancy to 521.1 nmol/l at 3637 weeks of pregnancy, without exceeding the limits. EEG results did not deteriorate. None of the patients had seizures during pregnancy. Conclusion. Progesterone therapy is an adequate and safe alternative to increasing the dose of anticonvulsants in case of recurrent seizures during pregnancy.

Background: Epilepsy is fairly a frequent occurrence in the elderly. It is commonly diagnosed after the episode of two or more unprovoked seizures. Unprovoked seizures in elderly are recurrent rather than younger individuals. This study was designed to estimate the concrete burden of frequent causes of epilepsy. Methods: A descriptive cross-sectional study with a total of 153 patients diagnosed case of epilepsy were included in this study at Jinnah Medical College Hospital from February 2018-August 2018. Mean was calculated for age, duration of disease of the patients. Causes of epilepsy, gender, and education was calculated and presented as percentages. Electrolyte readings were taken i.e., Sodium, Calcium and Magnesium levels and imaging was planned to rule out stroke, primary neurodegenerative disorders and tumors. Post stratification Chi square test was applied and p-value less than or equal to 0.05 was considered significant. Results: The mean age of the patients was 63.91±5.68 years and mean duration of the disease was 4.61± 1.07 months. The common causes of epilepsy were found to be cerebrovascular disease 56.9%, cryptogenic 54.2%, neurodegenerative disorder 20.3%, traumatic head injury 11.8%, metabolic abnormalities or electrolyte disturbances 10.5% and brain tumor 7.8%. Conclusion: Elderly patients with first seizure should present to a facility designed in a way that neurologist, cardiologist, rehabilitation and geriatrics work together to identify and treat the condition in a better way. Keywords: Epilepsy; Seizures; Cerebrovascular Disease; Neurodegenerative Disorder.

Abstract Objectives Patients with epilepsy and their family have diverse beliefs about the cause of their illness that generally determine their treatment-seeking behavior. In this study, our aim was to find out different beliefs about epilepsy that lead to different help-seeking patterns, which act as barrier to the intended modern medical management of epilepsy. Materials and Methods One hundred and fifty consecutive consenting patients accompanied by a reliable informant/family member fulfilling the International Classification of Epileptic Seizures (ICES), simplified version, were included. Demographic and clinical data of all the eligible subjects was collected. Perceived cause of illness and help-seeking pattern were explored from patient/informant by administering proper instruments. Results Respondents identified varied causes of epilepsy and explored multiple help-seeking options before reaching tertiary care centers. We observed that, generally, epileptic patients/relatives who had belief in causes like supernatural causes sought help from nonprofessional personnel and those attributed their symptom to bodily pathology had professional help-seeking. Conclusions The belief in supernatural causes not being conformed to the biomedical models of the epileptic disorders increases the treatment gap.
