Reflections of Medical Interns on Internship in Internal Medicine

Background: Reflection and reflective practices are considered as one of the important components of medical education and imperative for self-directed lifelong learning. The objective of this study was to take reflections of interns what they learnt during posting in internal medicine and challenges they faced and what was gratifying moments. Methods: MBBS course in Nepal is of five and half years including one-year mandatory rotational internship of which 2 months posting in internal medi­cine. The Internship of first MBBS batch at Chitwan Medical College started in May 2015. At the end of last round of posting of first group within department of internal medicine, written reflections of all interns were taken. The data were streamlined manually and presented. Results: Interns updated and enhanced clinical, report interpretation, correlat­ing clinical condition with lab reports, decision making, management, docu­mentation, counselling and communication skills for a limited time posted in medical wards, OPD, ICU, CCU and hemodialysis unit. Some of the challenges were dealing patients with different attitude, to become responsible trustwor­thy doctor, responding to expectations of patient and family and to keep pa­tient informed. When patient believes and trusts on us and happily returns home were among the gratifying moments mentioned by interns. In fact, stu­dents were not taught about reflection during 4 and half years of study Conclusions: Though interns reflected but approach was not systemic as they didn’t learn about reflection during course of study. Incorporate reflection as a teaching learning and assessment methods in curriculum, implement it and make reflective practice as core professional skill.

This cross-sectional descriptive study was carried out among the teachers and interns doctors of four medical colleges of Dhaka city in last one year (2010-2011).Data was collected by conveniance sampling. In-depth interview of the teachers were taken to explore their views regarding current situation, what should be, and recommendations for future curriculum about teaching communication skills. Information was collected from interns by semi-structured questionnarie. Alocation of score for response of each item was based on 5 point Likert scale.Total respondents were 249 out of which intern doctors were 218 and 31 were teachers. Teachers were chosen from eight clinical subjects as the conveniance of the researcher. Selected 14 variables of communication skills were used in this study. By reviewing the study it was seen that most of the interns gave opinion that they were not taught communication skills in their cinical classes. It was seen in the study that majority of the teachers confessed that they don't teach communication skills in stuctured way. All of them agreed that it must be taught in stuctured way and assessed. Considering the incidents of violence in health care setups reported from all over the country, we developed a one-day workshop to orient interns towards communication, conflict management, stress management and prescription writing skills. The objective was to impart training on effective communication and conflict management skills which are crucial to reduce the incidence of workplace violence, and improve the quality of services provided in our hospital. We describe the evolution of content and assessment of the workshops conducted for interns posted in the Department of Community Medicine during a period of one year since 01.01.2018. fter extensive review of literature, we prepared interactive presentations, developed games, innovative activities and role plays in context of the doctor-patient relationship. We modified the teaching learning methods to increase participant involvement, and added the use of video clips and flip-charts. Assessment of take home messages was done using an anonymous open-ended questionnaire (n=86). Codes were identified for the responses under three domains i.e. communication skills, conflict management and prescription writing. Likert scale was used to rate statements concerning the overall feedback for the workshop in a subset of interns (n=54). The most frequently reported take-home messages were on improving listening skills (41, 48%), learning the various conflict management techniques (61, 71%) and writing generic names and correct dosages (54, 63%), in each of the three sessions held on effective communication, conflict management and prescription writing, respectively. Majority of the interns strongly agreed that the workshop was needed (31, 57%), increased their knowledge (28, 52%), the small group exercises were beneficial (36, 67%) and that they would implement the skills they learnt (34, 64%). In conclusion, interactive, small group workshops on communication skills and conflict management for interns are a felt need, and have motivated them to implement various soft skills in their place of work.

Introdução: A afasia é uma das sequelas mais importantes que ocorrem após lesão cerebral de acidente vascular encefálico (AVE). Objetivos: Identificar o perfil da linguagem oral de pacientes com AVE, atendidos pelo Serviço Assistencial Multidisciplinar Domiciliar (SAMD) na cidade de Porto Velho – RO. Materiais e Métodos: Trata-se de um estudo transversal e quantitativo, realizado na residência dos pacientes que estavam sendo atendidos pelo SAMD. Onde foi utilizado o protocolo de Teste de Reabilitação das Afasias composto inicialmente de um questionário que foi aplicados com os familiares dos pacientes para coletar dados quanto ao AVE e os Testes de Comunicação Oral para avaliar dos pacientes. Resultados: Foram avaliados 11 sujeitos com AVE e oito sujeitos apresentaram afasia emissiva do tipo Broca (100%) com presença de agramatismo e anomia (87,5%), e déficit na organização da comunicação e na memória (100%). Referente à etiologia do AVE, verificou-se que 62,5% dos sujeitos apresentaram etiologia decorrente de hipertensão, com tempo de sequelas com tempo entre um ano menos e a três anos (37,5%). Todos os sujeitos (100%) eram muito falantes antes do AVE, porem 50% continuaram muito falantes após o AVE e os outros 50% se tornaram pouco falantes. Conclusão: Os resultados evidenciam que a maioria dos sujeitos apresentaram afasia emissiva do tipo Broca, em decorrência de Acidente Vascular Encefálico Isquêmico tendo como fator etiológico a hipertensão. Desencadeando alterações de linguagem como anomia e agramatismo, afetando ainda suas habilidades de comunicação como a memória e a organização da linguagem. Introduction: Aphasia is one of the most important sequels that occur after brain injury from stroke. Objectives: To identify the oral language profile of patients with stroke, assisted by the Multidisciplinary Home Care Service (SAMD) in the city of Porto Velho - RO. Materials and Methods: This was a cross-sectional and quantitative study carried out at the residence of the patients who were being treated by the SAMD. Where the Aphasia Rehabilitation Test protocol was used, composed initially of a questionnaire that was applied with the relatives of the patients to collect data regarding the AVE and the Oral Communication Tests to evaluate the patients. Results: Eleven subjects with EVA were evaluated, and eight subjects presented Embryonic Emphasis of Broca type (100%) with presence of agramatism and anomia (87.5%), and deficits in the organization of communication and memory (100%). Regarding the etiology of the AVE, 62.5% of the subjects presented etiology due to hypertension, with sequelae time between one year less and three years (37.5%). All subjects (100%) were very talented before the AVE, but 50% remained very talented after the AVE and the other 50% became less talkative. Conclusion: The results show that most of the subjects presented Embryonic Emphasis of the Broca type, due to Ischemic Stroke, having as etiologic factor hypertension. Unleashing language changes such as anomie and agramatism, still affecting his communication skills as memory and the organization of language.

The purpose of this study was to gain a better understanding of how communication relates to teaching effectiveness in an Industrial Education setting. Teaching effectiveness was defined in terms of two criteria: supervisor evaluations of teachers and student evaluations of teachers. Results from the study underscored the importance of communication skills in the teaching/learning process. The most important measures were: teacher dynamism, teacher delivery, time spent with the students, positive reinforcement of the students, and positive attitude toward the students. These measures differentiated “good” from “poor” teaching.

Haemorrhoids are a clinical symptom of a change in the normal functional architecture of the inflamed and swollen veins known as the anal cushion. Numerous treatment opportunities are available for this communal issue, but Baron's Gum Ligation (RBL) is the utmost frequently used technique for second- and third-degree haemorrhoids because it treats hemorrhoidal disease without anaesthesia, hospitalization, minimal complications, and ooutpatient discharge is uneventful compared to conventional surgery. Purpose: We conducted this study to evaluate the effectiveness of rubber band ligation in the treatment of second- and third-degree haemorrhoids. Methodology: This prospective study was performed on 76 patients over a six-month period at the Department of General Surgery from January 2021 to June 2021 at Islam Medical College and Teaching Hospital Sialkot. Rubber rubber ligation was used in all patients. A maximum of two haemorrhoids in one session were banded, and third as needed in a check-up done two weeks later. At the end of the fourth week, post-banding assessments were made for the presence and absence of bleeding, pain, and haemorrhoidal prolapse. Results: Of the 76 patients, 60 (78.9%) were male and 16 (21.1%) were female. The M:F ratio was 5.0: 1.0. 37.85 years was the mean age of patients (range 15 to 68). The duration of symptoms in approximately 48 patients (63.2%) was less than one year old, and in 18 (23.7%) patients ranged from 1 to 3 years and 10 (13.2%) patients had symptoms for more than 4 years. Complications occurred in 23 patients, bleeding in three patients, pain in 18 patients, 2 patients have prolapsed. Conclusion: RBL is effective, safe and simple method of symptomatic treatment for 2nd and 3rd degree haemorrhoids in an outpatient setting.

BACKGROUND Mentorship programs are becoming increasingly common in undergraduate medical education all over the world. However, very few medical colleges are running mentorship programs in India. A mentorship program was introduced in our college for the first year MBBS students to help them cope up with the stress of the new environment. OBJECTIVE The aim of this study is to evaluate the mentorship program with regard to its efficacy, utility, limitation and potential for improvement after successful completion of one year of the program. METHODS It was an observational analytical study including 143 mentees and 15 mentors conducted in a tertiary care teaching hospital for five months from November 2019 to March 2020. Two sessions of FGD were carried out involving 7 students in each session. One session of FGD was conducted involving 7 mentors. The feedback was collected with a pre validated questionnaire containing 10 questions (8 close ended and 2 open ended). Feedback of closed ended questions were obtained by using a 5-point Likert scale (1 to 5) where 5 indicated ‘Strongly Agree’ and 1 indicated ‘Strongly Disagree’ RESULTS Most of the mentees (86.7%) and mentors (66.7%) mentioned that mentorship program is necessary for the welfare of the students. However, it was also noted that better communication and frequent meetings can improve the outcome of the program. CONCLUSIONS Mentorship program is deemed essential, however, a well-structured framework and dedicated time from both mentors and mentees will make the program more successful. This type of feedback evaluation is however important to validate such program.

Objectives: To see the (a) type of congenital heart disease, (b) Clinical presentation of the cases, (c) association with extra-cardiac anomalies and disease, (d) complications of different CHD, (e) outcome of patients during hospital stay. Methodology: it was a prospective study conducted in the department of pediatrics of Rajshahi Medical College & Hospital over a period of one year.115 children from birth to 12 years of age who had congenital heart disease confirmed by echocardiography were included. All patients were treated conservatively and observed for immediate out come during the hospital stay. Result: major types of CHD were VSD (42.6%), TOF (18.3%), ASD (14.8%), PDA (7.8%). Male outnumbers female child. Common symptoms were breathlessness (60%), fatigue (54.8%), cough (43.5%), poor weight gain (41.7%), recurrent chest infection (34.8%), fever (28.7%), feeding problems (26.1%), palpitation (21.7%) and bluish discoloration of lips and fingertips (20%). Murmur with or without thrill and cardiomegaly was the most important cardiac finding.
