Application of Flap in Repairing Defect of Orbital Tumor

Introduction: Orbital tumors can extensively damage the upper and lower eyelid, peri-orbital skin, periorbital peri-bulbar tissue and eyeball. The complete removal of the tumor and the tissue destroyed by the tumor often leaves a large tissue defect area. Therefore, the application of flap in repairing defect after exenteration of orbital tumor is of paramount importance. The objective of this study is to investigate the clinical value of skin fl ap in repairing defect after exenteration of orbital tumor. Methods: From January 2008 to December 2017, there were 101 patients with orbital tumors, including 47 malignant and 54 benign tumors. All patients underwent excision of tumor while skin flap was used to repair tissue defects of different degrees. Postoperative observation showed the survival and functional recovery of tissue flap. Results: The skin flaps of the patients were completely alive after operation, the defect was completely closed, and the flaps were viable as of color of the flaps. Quality of life score significantly improved after flap surgery. Conclusion: The flap is used to repair the skin defect after orbital tumor surgery, which is helpful to improve the quality of life of patients with orbital tumor.

Background Fatigue and pain are prevalent symptoms of multiple sclerosis (MS) and frequent complaint in MS patients, which reduce their quality of life. This study aimed to assess the effect of massage therapy on pain and fatigue in MS Patients. Method The original and Persian databases were searched included PubMed, web of science, embase, ovid, scopus, and the Cochrane Library, SID, and Iranedex from inception to November 2020. Studies that reported the effect of massage on fatigue and pain were included. Two investigators extracted all relevant data, independently. For deriving analysis, mean difference (MD) and standardized mean difference (SMD) were used. Result Ten studies were eligible acoording criteria. The effect of massage on fatigue showed significant improvement (−1.62; 95% CL −2.40, −0.83; p < .00001), also results of the systematic review showed a significant reduction in pain severity. Conclusion Massage as a complementary and non-pharmacological therapy might have been associated with alleviating fatigue and pain in M.S. patients. Based on the current study, massage intervention for MS patients could have possible clinical value for palliating pain and fatigue and improving quality of life; however, this matter needs further and more significant trial studies.

Background:Objectives:Analyse the effect of secukinumab in terms of the patient´s own variables, specifically: fatigue, sleep, pain and quality of life in patients with psoriatic arthritis or spondyloarthritis.Methods:A multicentric longtitudinal observational prospective study was carried out at 6 months in patients who begin treatment with secukinumab. At the start and after 6 months the following data was collected on the outcome: pain through an visual analogue scale (VAS), fatigue using the FACIT-fatigue scale, sleeping problems using the insomnia severity index (ISI) and quality of life with the EuroQol-3L-5D and the PsAQoL.The sample can be described in terms of the distribution of the variables through measures of central tendency.It was analysed if the change after 6 months was statistically relevant using Student´s t-test for paired data in the case of FACIT, VAS, PsAQoL and ISI and chi-squared for the dimensions of the EQ-5D. The size of the effect of each of the measurements taken was calculated using Cohen’s D. the results are given grouped by disease and globally. The analysis was carried out using Stata v12 (College Station Tx, USA)Results:In table 1, the changes in the scales of normal distribution can be seen. Apart from general VAS, all the scales experience significant relevant changes. The PROs preferred by the patient with the best therapeutic response is the quality of sleep. The adjustment of the regression models does not produce changes in the results, apart from small adjustments to the condidence intervals (final column table 1). The subdomain in which the most significant change in the EQ-5D is produced is in that of pain and discomfort.Conclusion:After 6 months patients who begin treatment with secukinumab, present with improvements in all sizes of the effects of the treatment in the various studied scales. The improvement achieves global and generalised statistical significance after 6 months of study. Objective: To explore the effectiveness of pain nursing interventions for patients in ENT. Methods: The 200 ENT patients admitted from March 2018 to March 2020 were divided into observation group (n=100) and control group (n=100). The control group was used conventional clinical nursing methods, and the observation group received pain nursing intervention as a comparison with the control group. Results: The observation group had 83 cases, 14 cases and 3 cases of mild, moderate and severe pain, respectively, while the control group are of 43 cases, 43cases and 14 cases. Nursing intervention can significantly reduce the pain of patients after otolaryngology surgery, improve the quality of life of patients, and has high clinical value for application.

Objective: To assess the effect of tourniquet use on bleeding, bruising and quality of life in varicose vein surgery. Method: A prospective randomised trial of 49 patients undergoing varicose vein surgery was carried out. All patients were having single leg surgery as a day case. Blood loss was assessed by weighing swabs. Bruising was measured at 10 days, and quality of life assessed with SF36 questionaires. Synthesis: In the tourniquet group blood loss was significantly reduced but not the area of bruising. The SF36 scores showed deterioration in the non-tourniquet group at 10 days but this was not present at 6 weeks. We saw no complications of tourniquet use but the study was limited by disappointing recruitment reducing the power. Conclusion: Tourniquet use may be of clinical value. A larger study is required to assess the significance.

OBJECTIVECraniopharyngiomas are rare and benign tumors of the sellar and/or parasellar region. Primary treatment involves resection followed by adjuvant radiotherapy. While the grade of resection was frequently analyzed following surgery, the neurological outcome and especially neuropsychological deficits and quality of life have been neglected for many decades. Therefore, the authors retrospectively analyzed their patient series and prospectively assessed neuropsychological outcome and quality of life following resection of craniopharyngiomas in adults.METHODSIn total, 71 patients (39 men and 32 women) with a mean age of 49 years were enrolled in the retrospective analysis. In addition, 36 of the 71 patients were included in the prospective arm of the study and underwent neurological and neuropsychological testing as well as quality of life (36-Item Short-Form Health Survey; SF-36) assessment. Factors influencing outcome were identified and correlations calculated.RESULTSResection was performed mostly using a pterional (41.6%, 47/113 surgical procedures) or bifrontal translamina terminalis (30.1%, 34/113 surgical procedures) approach. Following surgery, visual acuity was significantly improved (> 0.2 diopters) in 32.4% (23/71) of patients, or remained stable in 45.1% (32/71) of patients. During long-term follow up, 80.3% (57/71) of patients developed pituitary insufficiency, particularly involving the corticotropic and thyrotrophic axes. In total, 75% (27/36) of patients showed neuropsychological deviations in at least 1 test item. In particular, attentiveness, cognitive speed, and short-term memory were affected. Referring to the SF-36 score, quality of life was affected in both the mental and physical score in 19.4% (7/36) and 33.3% (12/36), respectively. The risk factors that were identified were a tumor volume larger than 9 cm3, tumor extension toward/into the third ventricle or the brainstem, and resection using a bifrontal translamina terminalis or left-sided approach.CONCLUSIONSThis study demonstrated that resection of craniopharyngiomas is frequently associated with postoperative neuropsychological deficits and hence an impaired quality of life. In addition to tumor size and extension toward/into the third ventricle or the brainstem, selection of the surgical approach may play a crucial role in the patient’s neuropsychological outcome and quality of life.

Despite remarkable advancements in medicine, cancers remain one of the most challenging pathologies for doctors. Head and neck neoplasms especially raise serious management problems due to the often advanced stage at the moment of diagnosis, associated with difficult curative interventions that impose significant changes in the quality of life of patients. Whenever possible, a complete removal of the tumor with disease free margins significantly increases the rate of survival. During surgery, assessing the limit of resection up to healthy tissue is crucial in order to attempt a curative intervention. The purpose of this paper is to evaluate the use of the methylene blue staining test in assessing the margins of resection during interventions for head and neck cancers.

Objective: To investigate the effect of advanced nursing on the quality of life and the prevention of complications in patients with breast cancer undergoing PICC (Peripherally Inserted Central Catheter) chemotherapy. Methods: From January 2017 to January 2020, 98 patients with breast cancer undergoing PICC chemotherapy in our hospital were divided into control group and study group by random sampling. The patients were given routine nursing and advanced nursing intervention on the basis of routine nursing. The psychological status, complications and quality of life of the patients were compared. Results: There was no significant difference in HAMA, HAMD score and QLQ-30 score before nursing (P > 0.05). The scores of HAMA and HAMD in the study group after nursing were lower than those in the control group (P < 0.05), the scores of QLQ-30 were higher than those in the control group (P < 0.05), and the complication rate was lower than that in the control group (P < 0.05). Conclusion: The intervention of advanced nursing mode in patients with breast cancer treated with PICC chemotherapy can effectively alleviate the influence of negative emotion on chemotherapy, reduce the occurrence of complications and improve the quality of life. This mode has high clinical value.

Introduction: Circulatory system diseases have high medical and social significance, therefore, the study of the quality of life associated with health for cardiological patients is of great practical importance. A large number of methods for assessing the quality of life are known, but not all methods have a methodological justification in cardiology and practical approbation. The aim of the work was to assess the clinical value of the WHOQOL-100 questionnaire for a group of patients with cardiovascular diseases. Materials and methods: The work methodology is presented by the analysis of RISC, Springer Link, Oxford University Press, New England Medical Journal, British Medical Journal, Elsevier SCOPUS database, EBSCO platform, as a result, the WHOQOL100 method was justified for use. Results: It has been established that the psychological field (the ratio of positive and negative emotions) and the sphere of social relations with an emphasis on practical social support are components of the quality of life, which can be considered as key vectors of social rehabilitation and criteria for the social effectiveness of treating patients with cardiovascular diseases. Conclusion: The international questionnaire WHOQOL-100 is recommended for use for cardiological patients with the subsequent analysis of the whole profile of quality of life.
