Authentic Conversations About Self-Care with Fourth-Year Veterinary Medical Students

Expanding literature on well-being within veterinary medicine has been instrumental in recognizing the prevalence of psychological distress among students and practitioners, promoting awareness and advocacy for well-being within teaching institutions, professional associations, and the workplace. However, greater focus on distress has also been critiqued for overemphasizing illness and reactive interventions, and a call has been made for more balanced conversations about veterinary well-being, with focus on strengths and proactive interventions. This best-practices article highlights a proactive, strengths-focused intervention, aimed at increasing fourth-year students’ awareness of self-care while in training and in their transition to the profession. Authentic conversations about self-care is a required part of clinical training at Kansas State University College of Veterinary Medicine. All students attend a private meeting with a behavioral scientist, engaging in an authentic conversation about their experience of stress and coping strategies. Current practices in providing stress management interventions are restricted to pre-clinical training. Authentic conversations about self-care are presented here as an alternative to current practices, which the authors argue are not adequately addressing students’ needs during clinical training and the transition to the profession. Potential implications of providing self-care interventions during the clinical year of training include greater student engagement, increasing awareness, and self-efficacy as students make efforts to maintain well-being both in training and in the professional realm.

Abstract The emergence of resistant bacteria forced the medical and animal agriculture communities to rethink how antibiotics are used. In Arkansas, medicated feed is mostly used to treat or control Anaplasmosis in beef cattle herds. Bovine Anaplasmosis is a tick-borne disease caused by the rickettsial bacteria, Anaplasma marginale. This disease causes over $300 million in losses annually for the U.S. cattle industry. With beef cattle being the fifth largest agricultural commodity in Arkansas, it is important to know the prevalence of Anaplasmosis infection in the state. The project described is a collaborative effort between the University of Arkansas-Fayetteville campus, the University of Arkansas Extension, and the Kansas State University (KSU) College of Veterinary Medicine. Extension agents from 33 Arkansas counties were trained to recruit producers for the study and to coordinate sample collection. On the day of blood collection, Extension agents discussed the purpose of the project and appropriate disclosures with the producers. A total of 578 mature beef cattle were randomly selected from six geographical regions for sampling between the months of November 2019 and February 2020. Both whole blood and serum samples were collected from each animal. PCR testing was completed at the KSU College of Veterinary Medicine andcELISA testing was completed at the University of Arkansas Veterinary Diagnostic Laboratory. Blood samples from 335 cattle (58.7%) were positive for Anaplasmosis on at least one test with the majority of animals testing positive (229; 68.4%) on both the cELISA and PCR tests. Rates of regional prevalence ranged from 36.7% to 93.8%. The overall results were discussed with Extension agents via Zoom prior to discussing results with individual producers. Data from this study were added to previous surveillance data collected by Kansas State University and will direct Extension education efforts pertaining to Anaplasmosis management in regional beef cattle herds.

Purpose To effectively prepare students to engage in interprofessional practice, a number of Communication Disorders (COMD) programs are designing new courses and creating additional opportunities to develop the interprofessional competencies that will support future student success in health and education-related fields. The ECHO (Educational Community Health Outreach) program is one example of how the Rongxiang Xu College of Health and Human Services at California State University, Los Angeles, has begun to create these opportunities. The ultimate goal of the ECHO project is to increase both access to and continuity of oral health care across communities in the greater Los Angeles area. Method We describe this innovative interdisciplinary training program within the context of current interprofessional education models. First, we describe the program and its development. Second, we describe how COMD students benefit from the training program. Third, we examine how students from other disciplines experience benefits related to interprofessional education and COMD. Fourth, we provide reflections and insights from COMD faculty who participated in the project. Conclusions The ECHO program has great potential for continuing to build innovative clinical training opportunities for students with the inclusion of Child and Family Studies, Public Health, Nursing, and Nutrition departments. These partnerships push beyond the norm of disciplines often used in collaborative efforts in Communication Sciences and Disorders. Additionally, the training students received with ECHO incorporates not only interprofessional education but also relevant and important aspects of diversity and inclusion, as well as strengths-based practices.

<p class="3">This note from the field reviews the sustainability of an institution-wide program for adopting and adapting open and alternative educational resources (OAER) at Kansas State University (K-State). Developed in consult of open textbook initiatives at other institutions and modified around the needs and expectations of K-State students and faculty, this initiative proposes a sustainable means of incentivizing faculty participation via institutional support, encouraging the creation and maintenance of OAER through recurring funding, promoting innovative realizations of “educational resources” beyond traditional textbooks, and rallying faculty participation in adopting increasingly open textbook alternatives. The history and resulting structure of the initiative raise certain recommendations for how public universities may sustainably offset student textbook costs while also empowering the pedagogies of educators via a more methodical approach to adopting open materials.</p>

Abstract Background In a context where there is no treatment for the current COVID-19 virus, the combination of self-care behaviours together with confinement, are strategies to decrease the risk of contagion and remain healthy. However, there are no self-care measures to screen self-care activities in general population and which, could be briefly in a lockdown situation. This research aims to build and validate a psychometric tool to screen self-care activities in general population. Methods Firstly, an exploratory factor analysis was performed in a sample of 226 participants to discover the underlying factorial structure and to reduce the number of items in the original tool into a significant pool of items related to self-care. Later a confirmatory factor analyses were performed in a new sample of 261 participants to test for the fit and goodness of factor solutions. Internal validity, reliability, and convergent validity between its score with perceived stress and psychological well-being measures were examined on this sample. Results The exploratory analyses suggested a four-factor solution, corresponding to health consciousness, nutrition and physical activity, sleep, and intra-personal and inter-personal coping skills (14 items). Then, the four-factor structure was confirmed as the best model fit for self-care activities. The tool demonstrated good reliability, predictive validity of individuals’ perception of coping with COVID-19 lockdown, and convergent validity with well-being and perceived stress. Conclusions This screening tool could be helpful to address future evaluations and interventions to promote healthy behaviours. Likewise, this tool can be targeted to specific population self-care’s needs during a scalable situation.

Monthly surveillance allows us to monitor trends in the recovery of these resistant organisms and address issues of concern that could impact our patients, clients, staff, and students. Methods: The OSU-CVM ASWG collects samples from >100 surfaces within the companion animal, farm animal, and equine sections of our hospital each month. Sampling has been continuous since January 2018. Samples are collected from both human–animal contact and human-only contact surfaces using Swiffer electrostatic cloths. These samples are cultured for recovery of Salmonella spp, extended-spectrum cephalosporin-resistant Enterobacteriaceae, carbapenemase-producing Enterobacteriaceae (CPE), and methicillin-resistant Staphylococcus spp. Results: The recovery of these antibiotic resistant target organisms is low in the environment of our hospital. Recovery from human-only contact surfaces (19.8%) is very similar to recovery from human–animal contact surfaces (25.5%). We commonly recover Enterobacteriaceae (E.coli, Klebsiella spp, and Enterobacter spp) that are resistant to extended-spectrum cephalosporins (496 of 2,016; 24.6%) from the VMC environment. These antibiotic-resistant indicator bacteria are expected in a veterinary hospital setting where use the of β-lactam drugs is common. Recovery of both Salmonella spp and CPE has remained very low in our hospital environment over the past 19 months: 16 of 2,016 (0.7%) for Salmonella and 15 of 2,016 (0.8%) for CPE. Discussion: The active environmental surveillance component of our antimicrobial stewardship program has allowed us to reduce the threat of nosocomial infections within our hospital and address environmental contamination issues before they become a problem. Our consistently low recovery of resistant organisms indicates the effectiveness of our existing cleaning and disinfection protocols and biosecurity measures. Due to the nature of our patient population, we do expect to find resistant organisms in the patient-contact areas of the hospital environment. However, our similar rates of resistant organisms from human-only surfaces (eg, computer keyboards, door handles, telephones, and Cubex machines) indicates a need to improve our hand hygiene practices. These data are now supporting the implementation of a new hand hygiene campaign in our veterinary hospital.Funding: NoneDisclosures: None
