Abstract: Ulcus decubitus is a serious dermal (skin) problem, especially for the unconscious patients with the lack of mobilization hospitalized in the ICU. It appears in the area which is localizedwith the necrotic tissues; it also usually appears on the surface of protruding bones, resulted fromthe long term pressure which increases the capillary pressure. This study was purposed to analyzethe effectiveness of the use of VCO to the patient with the risk of having ulcus decubitus in thecentral ICU of Rumkital Dr. Ramelan Surabaya (Surabaya Navy Hospital). The design used in thisstudy was case study done by investigating a problem through one case comprising of a single unit.The variable used in this study was the use of VCO to the patient with the risk of ulcus decubitus.The data were analyzed descriptively done by using inductive approach. The result of this studyconcludes that the use of VCO to Mr. M who has the risk of impaired skin integrity is effective toprevent decubitus.

Intorduction:Incontinence was a problem that often occurs in the elderly and this condition can worsen the elderly situation with the appearance of severe damage to the integrity of the skin,Virgin Coconut Oil (VCO) contains medium chain saturated fat that easily enters deep skin layers and maintains skin elasticity and suppleness. The aim of this study was to determine the effect of virgin coconut oil on skin integrity in the elderly with incontinence.Methods: The design of  research was a quasi-experimental with non equivalent control group design. The population in this study were all elderly who experience incontinence. The sampling technique used was consecutive sampling with a total of 10 people in the experimental group and 10 people in the control group. To assess skin integrity before and after treatment is used was The Ghent Global IAD (GLOBIAD) tool. The using of virgin coconut oil is done every time after the stool incontinence episode and after bathing. Virgin coconut oil has been applied to the damaged skin and also to other vulnerable skin. Results: Based on the Wilcoxon test results obtained p value smaller than 0.05 which is 0.010 so that there was a significant difference in the value of skin integrity before and after the treatment. Conclusions:There was a significant effect on the use of virgin coconut oil on skin integrity in the elderly with incontinence.

The use of drugs that are less soluble in water will become clinically less efficient, this is caused by the low penetration of the drug into the body. A microemulsion is a dispersion system such as an emulsion that can increase the solubility of drugs that are sometimes difficult to dissolve in water. Microemulsions have long-term stability, clear, transparent, and good penetration capabilities. In this study, a microemulsion formulation with active ingredients griseofulvin and virgin coconut oil was conducted as an oil phase. The resulting microemulsion evaluates physical stability during 35 storage days. The results showed that the microemulsion preparation remained stable during storage time without changes in color, odor, and consistency, while the pH and dosage viscosity experienced less significant changes. The consequences of the centrifugation test at 3700 rpm for 5 hours and freezing tests for 24 days showed stable preparation and cannot be separated. The diffusion test results obtained by FG2 had the largest permeation percentages of 3.6136%, FG3 2.8724%, and the smallest FG1 2.0477%.Keywords: microemulsion, griseofulvin, stability, diffusion test

Meningkatnya permintaan tahu di pasaran menyebabkan para pengrajin tahu lebih giat memprodusi tahu, sehingga meningkatkan jam kerja mereka. Hal ini menimbulkan permasalahan baru yaitu munculnya berbagai gangguan kesehatan pada kulit. Salah satu penyebab gangguan kesehatan kulit adalah limbah cair tahu yang bersifat asam. Selama ini pengrajin tahu menangani masalah tersebut dengan mengolesi lotion anti nyamuk, sebelum mereka bekerja. Sebenarnya ada cara mengatasi gangguan kesehatan kulit dengan aman menggunakan bahan alami yaitu minyak kelapa yang ditambah ekstrak bawang merah. Berdasarkan latar belakang tersebut maka perlu dilakukan penelitian dengan fokus pada pemanfaatan minyak kelapa fermentasi yang diperkaya ekstrak bawang merah untuk membantu menyelesaikan permasalahan gangguan kesehatan kulit pada pengrajin tahu. Tujuan penelitian yaitu mengidentiÞ kasi berbagai jenis gangguan kesehatan kulit yang dialami oleh pengrajin tahu akibat limbah cair tahu; mengetahui potensi minyak kelapa fermentasi yang diperkaya bawang merah (Allium cepa) mampu mengatasi masalah gangguan kesehatan kulit pengrajin tahu dan mengetahui potensi minyak kelapa fermentasi yang diperkaya bawang merah (Allium cepa) untuk dapat diterima oleh pengrajin tahu. Metode penelitian adalah eksperimen semu, dengan rancangan One Group Pre-Test and Post-Test, dengan satu kelompok yang diamati, namun pengamatan dilakukan sebelum dan setelah perlakuan. Kesimpulan penelitian yaitu jenis gangguan kesehatan kulit akibat limbah cair tahu di Kampung Krajan Mojosongo Surakarta adalah dermatitis iritan, gatal-gatal, dan panu. Minyak kelapa fermentasi yang diperkaya ekstrak bawang merah (Allium cepa) mampu mengatasi masalah dermatitis iritan, dan gatalgatal ringan pada kulit tangan pengrajin tahu. Minyak kelapa fermentasi yang diperkaya bawang merah (Allium cepa) dapat diterima oleh masyarakat Kata kunci: limbah cair tahu, minyak kelapa fermentasi,bawang merah (Allium cepa), gangguan kesehatan kulit Demand increase of tofu in market lead to increasement of tofu production thus multiply the work hours. This give rise to health skin problem as the liquid waste of tofu is acidic material. As this condition appear, the worker apply mosquito repellent lotion over the wounds. Fermented virgin coconut oil and extract of Allium cepa are used as natural treatment in health skin problem and help to cure the wounds. This research aims to know the effect of Fermented virgin coconut oil and extract of Allium cepa towards health skin problem. This research aim to identiÞ es the heatlh skin problem had by worker of tofu production; indentiÞ es the potential of fermented virgin oil and extract of Allium cepa to solve the skin health problem of the tofu factory worker, and indentiÞ es the acceptance of society towards the potential of fermented virgin oil and extract of Allium cepa. The methode of this research:squasi-experiment were apply in this research, continued by One Group Pre-Test and Post-Test, within one group was observed on previous and after the experiment. Conclusions of this research are irritant dermatitis was the main skin problem in Kampung Krajan Mojosongo Surakarta; fermented virgin coconut oil and extract of Allium cepa were potentially help to solve the irritant dermatitis; and fermented virgin coconut oil and extract of Allium cepa were able to be accepted in society. Purpose This study examined caregiver perceptions of their child's language and literacy disorder as influenced by communications with their speech-language pathologist. Method The participants were 12 caregivers of 10 school-aged children with language and literacy disorders. Employing qualitative methods, a collective case study approach was utilized in which the caregiver(s) of each child represented one case. The data came from semistructured interviews, codes emerged directly from the caregivers' responses during the interviews, and multiple coding passes using ATLAS.ti software were made until themes were evident. These themes were then further validated by conducting clinical file reviews and follow-up interviews with the caregivers. Results Caregivers' comments focused on the types of information received or not received, as well as the clarity of the information. This included information regarding their child's diagnosis, the long-term consequences of their child's disorder, and the connection between language and reading. Although caregivers were adept at describing their child's difficulties and therapy goals/objectives, their comments indicated that they struggled to understand their child's disorder in a way that was meaningful to them and their child. Conclusions The findings showed the value caregivers place on receiving clear and timely diagnostic information, as well as the complexity associated with caregivers' understanding of language and literacy disorders. The findings are discussed in terms of changes that could be made in clinical practice to better support children with language and literacy disorders and their families.
