Study of Bacterial Pathogens in Urinary Tract Infection and their Antimicrobial Sensitivity Pattern

Background: Urinary tract infection (UTI) is one of the most common bacterial infections in Bangladesh. Antimicrobial agents are used for its treatment. The increase in antibiotic resistance among uropathogens is a global problem.Objective: This study was designed to find out the prevalence of UTI, its causative agents and their antimicrobial susceptibility patterns among suspected patients of UTI attending Dhaka Dental College Hospital, Dhaka.Methodology: This cross-sectional study was carried out in Dhaka Dental College Hospital, Dhaka during the period of November 2014 to May 2017. Clinically diagnosed cases of UTI irrespective of age and sex from out-patient department and in-patient department were selected for the purpose of the study. For culture, the urine samples were inoculated on 5% sheep blood agar and MacConkey’s agar media using calibrated loop following standard bacteriological technique. After the incubation period, the plate were examined for bacterial pathogen. The disk diffusion method (Kirby Bauer’s) was used to determine the antimicrobial susceptibility of isolates. Standard inoculums were inoculated on Mueller-Hinton agar and incubated at 37oC for 24 hours.Result: Ninety three urine samples were studied. Among the 93 samples most of them were in the age group of 21 to 30 years. Out of 93 samples, 26 (27.95%) samples were found culture positive. Escherichia coli (92.30%) was found to be the predominant organism.  Regarding antimicrobial sensitivity pattern Esch. coli was found to be most sensitive to Imipenem (100%), Amikacin (87.5%), and Nitrofurantoin (83.33%).Conclusion: In conclusion young aged female are commonly suffering from UTI and Escherichia coli is the most common isolated bacteria.Bangladesh Journal of Infectious Diseases, December 2017;4(2):40-44

Background: Urinary tract infection is very common in both male and female.Objectives: The purpose of the present study was to see the antibiotic sensitivity pattern of isolated from urinary tract infected patients.Methodology: This cross sectional study was carried out in Dhaka Medical College Hospital, Dhaka for a period of 12 months. Clinically diagnosed cases of urinary tract infection irrespective of age and sex having pus cells ?5/HPF in the deposits of centrifuged urine were selected as study population. Data regarding organisms causing UTI and their antibiotic sensitivity patterns were collected. For urine culture the urine samples were inoculated on HiCrome UTI agar, CLED agar, 5% sheep blood agar and MacConkey’s agar media with a calibrated loop having diameter of 1.45 mm which contains 0.001 ml of urine. The inoculation at 37o C for 24 hours and CFU count of 105/ml of urine were considered positive for UTI. Identification of bacteria was done by standard biochemical techniques and their distinct colony characteristics. All the isolated organisms were tested for antimicrobial sensitivity against different antimicrobial agents by disc diffusion method on Mueller-Hinton agar plates.Result: Diagnosis of bacteria causing UTI with their sensitivity to different antibiotics was performed with a total of 300 samples from both male (38.66%) and female (61.33%) of different age groups. Among 300 samples 107 strains were isolated. Out of 107 identified strains, 95(31.67%) samples showed single growth and 6(2%) samples showed mixed growth. Escherichia coli (64.49%) was found to be the predominant organism. Regarding antimicrobial sensitivity pattern Esch. coli showed 98.55 to 63.77% sensitivity to imipenem, amikacin, ceftazidime and nitrofurantoin. Other isolated organisms showed 50 to 100% sensitivity to ceftazidime, amikacin, imipenem except Klebsiella, Pseudomonas and enterococci spp. which showed 40% and less sensitivity.Conclusion: In conclusion Escherichia coli is the most commonly isolated bacteria which is highly sensitive to imipenem.Bangladesh J Infect Dis 2015;2(1):13-18

The changing antimicrobial sensitivity in UTI demands use of appropriate antibiotics.Objective: This study was conducted to determine the distribution and antimicrobial susceptibility of uropathogens.Materials and method: This was a prospective study conducted in Bangladesh Medical College & Hospital, Dhaka, Bangladesh between May 2016 and June 2017 to identify the organisms causing UTI and their antibiotic susceptibility. Clean catch midstream urine samples were collected from 95 patients presenting with symptoms of UTI. Antimicrobial susceptibility was performed by disc diffusion method.Results: Out of 95 urine samples, 56 (58.9%) were found positive. The prevalence was significantly higher in females than in males (females: 58.9%; males: 41%). Age group of >48 years showed higher prevalence of UTI. The most common organisms isolated were Escherichia coli, Klebsiella, Pseudomonas, Proteus and Staphylococcus aureus. These represented 44.6%, 21.4%, 14.3%, 12.5%, and 7.14% of isolates respectively. Imipenem and Meropenem were found the most susceptible drug against isolated uropathogens.Conclusion: Most powerful antibiotics in our study were imipenem and meropenem. In conclusion, one can truly affirm that the choice of drugs in the treatment of UTI is becoming quite narrow today due to the wide scale resistance that the common UTI pathogens show to drugs which have been used previously.Delta Med Col J. Introduction: Urinary Tract Infection (UTI) is one of the most common bacterial diseases worldwide that can present as asymptomatic or symptomatic characterized by a wide range of symptoms from mild irritative voiding to bacteremia, sepsis or even death. Increase in resistance of urinary pathogens to conventional antimicrobial agents is gaining the attention of many microbiologists worldwide in respect to treatment of urinary tract infection. Since the pattern of bacterial resistance is continuously changing, the monitoring of antibiotic susceptibility pattern becomes more important.Objective: The objective of this study was to update the distribution of current urinary pathogens and to find out their antimicrobial susceptibility pattern.Methods: A Cross sectional descriptive study was conducted at Border Guard Hospital (BGB Hospital) Peelkhana, Dhaka during the period of February 2013 to September 2013. A total of three hundred ninety urine samples were collected from patients with suspected UTI. All the samples were clean catch mid stream urine. Urine samples were cultured on MacConkey agar medium and blood agar medium. Colony counts yielding single type of bacterial growth of 105 CFU/ml were deemed significant.Results: Out of 390 samples, 49(12.56%) samples were culture positive. The isolated organisms were identified by conventional methods and were subjected to determine antimicrobial susceptibility testing by Kirby Bauer’s disc diffusion method. Escherichia coli (E.coli) was found as the most prevalent isolates 31(63.26%) followed by klebsiella spp 06(12.24%), Enterobacter spp o4(8.17%), Pseudomonas aeruginosa 04(8.17%), Staphylococcus aureus (Staph aureus) 02(4.08%) and Enterococcus spp 02(4.08%). Eight antimicrobial agents namely Amoxycillin, Co-trimoxazole, Levofloxacin, Nitrofurantoin, Gentamicin, Ceftriaxone, Imipenem and Amikacin were used for antimicrobial susceptibility testing. The most effective drug found against urinary isolates was Imipenem (95.91%), followed by Amikacin (81.63%) and Ceftriaxone (69.38%). None of the drug found was 100% effective against urinary pathogens. E.coli was found to be fairly sensitive to Ceftriaxone, Gentamicin and Nitrofurantoin.Conclusion: Urinary tract infection remains one of the most common bacterial infections. Antimicrobial drug resistance is increasing among urinary pathogens. This study updated the current occurrence of urinary pathogens and their antibiotic sensitivity pattern. We recommend periodical determination of antimicrobial sensitivity pattern of urinary pathogens in a particular hospital or area.Journal of Armed Forces Medical College Bangladesh Vol.10(2) 2014

Urinary tract infection is one of the most common bacterial infections seen in clinical practice both in developed and developing countries. The causative agents of Urinary tract infection vary from place to place and they also vary in their susceptibility and resistance patterns. This descriptive study aimed to evaluate the antibiotic susceptibility patterns of pathogens isolated from routine laboratory specimens at Dhaka Medical College Hospital. A descriptive cross- sectional study was done from January 2018 to June 2018. A total of 100 mid-stream urine samples from the suspected UTI patients were tested microbiologically and antimicrobial susceptibility test were performed for the isolated pathogens using Kirby-Bauer disk diffusion method with positivity rate of UTI was 90% (90/100). Escherichia coli (39%) was the most UTI causing bacteria followed by Klebsiella 18(18%), Acinetobacter 12(12%), Staphylococci, 9 (9%), Pseudomonas 6 (6%), Enterococcus species 3 (3%) and Proteus 3 (3%). Female 83 (83%) were more affected in comparison with male. Adult female especially age range 18-30 years were found in high risk. Nitrofurantoin, Amikacin and Meropenem were recorded as most sensitive antibiogram for most of the bacteria’s whereas cephalosporin showed increased resistance. However, for uncomplicated community acquired UTI, Nitrofurantoin (oral) and Amikacin (Injectable) can be chosen as first line medication. J MEDICINE JUL 2020; 21 (2) : 93-97

Extended-spectrum β-lactamases producing Escherichia coli (ESBL-EC) lend resistance to most β-lactam antibiotics. Because of limited treatment options, ESBL-EC infections are generally more difficult to treat, leading to higher hospital costs, reduced rates of microbiological and clinical responses, and a threat to the patient’s life. This study aimed to determine the antibiotic resistance pattern of ESBL-EC isolated from patients with urinary tract infection in Morocco. This retrospective laboratory-based study was conducted at Cheikh Khalifa International University Hospital, Casablanca, from January 2016 to June 2019. A total of 670 urine samples were collected from urinary tract infection patients and processed by standard microbiological methods. In vitro susceptibility testing to different antibiotics of all identified isolates of Escherichia coli (E. coli) was performed following Kirby–Bauer’s disc diffusion method on Mueller–Hinton Agar according to the EUCAST standards. The reviewing of ESBL-EC was confirmed by the appearance of a characteristically shaped zone referred to as a “champagne cork” using the Combined Disk Test. Among a total of 438 E. coli isolated from nonrepetitive urine samples, two hundred fifty-nine (59%) were ESBL-EC, of which 200 (77%) were isolated from adult patients (over the age of 50) and the majority were female. All ESBL-EC isolates were resistant to third-generation cephalosporin and quinolones and sensitive to carbapenem and fosfomycin. Knowledge of antimicrobial resistance patterns in ESBL-EC, the major pathogen associated with urinary tract infection, is indispensable as a guide in choosing empirical antimicrobial treatment.

Urinary tract infections represents one of the most common diseases encountered in medical practice affecting people of all ages from the neonate to the geriatric age group. The aim of this study was to determine and evaluate the antimicrobial susceptibility patterns for most common strains of bacterial urine isolates taken from patients at Azadi teaching hospital laboratories in Duhok city/Kurdistan region in Iraq, for two years (2014 and 2015). A retrospective survey study was conducted and 1003 data of urine culture sensitivity test were collected from recorded archives of the Azadi hospital of two years; 2014 and 2015.  The study included all recorded positive urine samples cultures isolated from inpatients and outpatients. Results revealed that the female to male ratio for urinary tract infection was 2.7:1 and the most common microorganisms isolated were Escherichia coli (52%), Staphylococcus aureus (11%), Staphylococcus haemolyticus (9.6%) and Klebsiella pneumoniae (8.1%). In general maximum sensitivity was seen for Meropenem (94.9%), followed by Imipenem (89.7%) and Ertapenem (88.7%). The maximum resistance was seen against Cefazolin (79.7%) and Amoxicillin/clavulanic acid (77.5%). The antibiotic sensitivity pattern of E. coli reveals that the maximum sensitivity was seen for Meropenem (97.3%) and Imipenem (95.6%). The maximum resistance was seen against Amoxicillin/clavlanic acid (83.1%). The resistance against many commonly used antibiotics have been increased which limit the options for treating urinary tract infection. This is resulted from uncontrolled use of the antibiotics and lack of policies and guidelines for their use in public health.
