This paper analyzed the factors determining participation in irrigation project on agro pastoral household and their perception towards the scheme. The study result depends on cross-sectional data collected from a sample of 144 households of which 72 irrigation users and 72 non-users using a combination of multistage, stratified and random sampling. The data were analyzed using descriptive statistics and logistic regression to assess factors that affect participation in irrigation. Result revealed that agro pastoralists do have medium to very strong perception towards different aspects of irrigation performance indicator variable. It is observed that among the variables in logistic regression age, sex, income, input use and participation in cooperative organization have affected participation significantly and positively, while, farm experience, distance to the district market, and total livestock unit, affected participation in irrigation significantly and negatively. The study has also substantiated that irrigation in the study area has significant role on income and recommend that it shall be great and rewarding if policy makers, designers, implementers, and any funding agencies with similar interest. Abstract Background: The knowledge of the key determinants of fertility desire among people living with HIV/AIDS is crucial for the design of efficient maternal and child health care programs. However, such determinants are not well understood in the context of a successful scale-up of antiretroviral therapy in Rwanda. The present study aim was to assess fertility desire among HIV- positive women and its determinants in Rwanda.Methods: Data were extracted from the 2015 Rwanda demographic health survey (RDHS) for 243 HIV-positive women of reproductive age. Univariate and multivariable based variable selection as well as multivariable logistic regression analysis were conducted. Results: The prevalence of desire to have another child in HIV-positive women was as high as 40.7%. Multivariable logistic regression analyses showed that the woman’s age, number of her living children, woman’s employment status, and having a co-wife are significant determinants of fertility desire among HIV- positive women in Rwanda. Women younger than 25 years, with no living child, who were unemployed or who had at least a co-wife were associated with greater odds of desire to have another child in the future than other HIV- positive women.. Conclusion: There is a need to devise integrated programs and services that are tailored to support HIV-positive women in planning their fertility, and controlling HIV transmission to their prospective children. Efficient policy and fertility interventions among WLHA in Rwanda should target young women, especially those living with partners and have fewer children than they prefer.

Background. Globally, electronic information and communication technology has been applied and much expanded in the healthcare industry. However, in developing counties including Ethiopia, EMR system adoption and utilization for medical practice are still inconsistent, and healthcare institutions which started utilization currently have also failed to sustain. A desirable readiness of healthcare experts is mandatory to expand digital health service delivery. Thus, this study is aimed at estimating the proportion of the willingness of professionals in Bahir Dar city to use EMR and at identifying factors associated with this proportion. Methods. An institution-based cross-sectional study was conducted from September 1 to October 30, 2019, among 634 health professionals. Respondents were selected using a simple random sampling method. Data were entered into EpiData version 3.1 and exported to SPSS version 23 for further analysis. Descriptive statistics were computed to describe study variables and presented using tables. Willingness to use the EMR system was computed. Bivariable and multivariable binary logistic regression models were fitted to identify the associated factors. The odds ratio with 95% confidence interval was used to measure the strength of association. Results. A total of 616 health professionals participated in the study with a response rate of 97%. The proportion of willingness to use the EMR system was 85.9%. Among health professionals who were not willing to use EMR, lack of access to EMR training (73.4%) was a major barrier to the willingness to use EMR. A multivariable logistic regression analysis showed that those health professionals who had good computer skill (AOR=2.5; 95% CI: 1.3-4.6), good knowledge on EMR (AOR=2.1; 95% CI: 1-4.4), gotten EMR training (AOR=3.8; 95% CI: 1.7-8.1), EMR guideline access (AOR=2.8; 95% CI: 1.4-5.6), and management support (AOR=2.6; 95% CI: 1.4-4.8) were more likely willing to use the EMR system. Conclusions. Majority of the professionals were willing to use the EMR system. EMR program should involve computer illiterate, less knowledgeable, those unable to access EMR guidelines, and managerially unsupported professionals. Enhancing health professionals’ attitude and contextualizing EMR training in the healthcare curricula are highly recommended to scale up EMR use.

Abstract Background: Physician burnout is a serious issue associated with physician attrition, mental and physical problems, and medical errors. This study aimed to investigate prevalence of overall burnout among Interns-Residents and general physician (GPs)-Specialist during the COVID-19 pandemic in Iran.Methods: This cross-sectional study was conducted among 220 Interns-Residents and 212 GPs-Specialists in university affiliated hospitals and clinics in Shiraz, Iran. A random sampling method used to recruit the samples and the data gathered using a valid questionnaire. We used covariance analysis and multiple logistic regression to analyze the data.Results: Multiple logistic regression for GPs-Specialists showed that working in COVID-19 wards during June-July (OR = 13.93) was associated with increasing odds of overall burnout. However, older age was associated with decrease odds overall burnout (OR = 0.94). Multiple logistic regression for Interns-Residents revealed that age (OR = 1.24) and being single (OR = 1.66) were positively associated with overall burnout.Conclusions: The present finding could remind policy makers the importance of burnout issue among physicians during the pandemics and could persuade them for taking appropriate action in order to prevent this phenomenon. It is suggested that focusing on strategies such as improving organizational resilience, improvement of healthcare working environment, and development of coping skills among physicians could be helpful in this regard.

Depot-Medroxyl-Progesterone-Acetate-Subcutaneous/Self Injection (DMPA-SC/SI) is an intentional approach used by the Nigerian government to improve the coverage rate of contraceptives method-mix. In this study, the knowledge, practice, and determinants of use of DMPA-SC/SI among women in North-Central and South-West, Nigeria, was assessed. A cross-sectional health facility study was conducted across four Nigerian states. Structured pretested questionnaires were used to elicit information from 844 women within reproductive age. Twenty focused group discussions and 40 In-depth interviews (IDIs). Descriptive statistics were used to analyze the characteristics of the women. Multivariate logistic regression was used to examine the determinants of the current use of DMPA-SC /SI. Statistical analyses were set at 5%. Recorded voices were transcribed in full and analyzed using literary methods. About 89.5% of respondents reported ever heard family planning; 97.6% reported heard DMPA-SC; 83.2% reported ever use family planning; 95.7% reported current use of family planning; 94.3% reported ever use DMPA-SC, and 46.5% reported ever use DMPA-SC/SI. The reason for the use of DMPA-SC/SI was because it was easy to use, effective and self-administered. The factors that discourage women from using DMPA-SC/SI were mainly fears associated with side effects, for and fear of SI-self-injury. The logistic regression results showed that those residing in a rural part of the country were less likely to use DMPA-SC/SI. The government should scale up this model nationally, including rural and hard-to-reach communities with public awareness programmes, public sensitization, and refresher training to improve on Provider’s counselling skills. Background Physicians have a crucial role in the implementation and scale up of pre-exposure prophylaxis (PrEP). The objective of this study is to examine Belgian physicians’ PrEP knowledge, concerns, acceptance and their willingness to prescribe PrEP. Methods: A cross-sectional online survey was conducted between March and June 2016. Dissemination targeted Belgian primary care physicians (PCPs) and HIV specialists. Sociodemographic characteristics, experience with HIV and PrEP, self-assessed PrEP knowledge, concerns about PrEP, and PrEP acceptance were stratified according to professional background. Associations with willingness to prescribe PrEP were examined using univariable and multivariable binary logistic regression analyses. Results: In total, 333 completed surveys were included in the analysis. Sixty-two physicians (18.6%) scored their knowledge of PrEP to be good, 263 (79.0%) had an accepting attitude towards PrEP and 198 (59.5%) were willing to prescribe PrEP if approved in Belgium. HIV specialists consistently reported having better knowledge of PrEP, less concerns and a more accepting attitude towards PrEP than PCPs. In multivariable logistic regression analysis, higher PrEP knowledge (OR 2.4; 95%CI: 1.0–5.7) and higher PrEP acceptance (OR: 3.8; 95%CI: 2.1–6.8) remained significantly associated with the willingness to prescribe PrEP. Conclusions: HIV specialists are better prepared to provide PrEP than PCPs in Belgium. Interventions to improve PrEP knowledge and acceptance among all providers are needed. The role of PCPs could be very important in optimising the rollout of PrEP, but additional training and guidelines will be needed.

This paper determines the probability of women in Bangladesh taking prenatal care based on changes in socioeconomic and health-related variables. Insight into factors affecting prenatal care usage will help policy-makers redirect health-related strategies and policies in more equitable directions. We used a total of 1,099 cross-sectional observations from Bangladesh Bureau of Statistics (2000) to estimate a logistic regression model. Our results show that education and income is positively associated with odds of women taking prenatal care while increase in age reduces odds of taking this service. We end by making several policy-relevant recommendations.

Antenatal care is a service provided to pregnant women to monitor, support maternal health and detect mothers whether normal or problematic pregnant women. According to the WHO, globally more than 70% of maternal deaths are caused by complications of pregnancy and childbirth such as hemorrhage, hypertension, sepsis, and abortion. Based on data obtained from the profile of the North Sumatra provincial health office in 2017, in the city of Padangsidimpuan in 2017 the coverage of ANC visits reached (76.58%) and had not reached the target in accordance with the 2017 Provincial Health Office strategy plan (95%). This type of research was an observational analytic study with a cross sectional design. The population in this study were independent practice midwives who were in the Padangsidimpuan, the sample in this study amounted to 102 respondents. The technique of collecting data used questionnaires and data analysis used univariate, bivariate and multivariate analysis with logistic regression analysis. Based on bivariate analysis showed that there was a relationship between facilities, knowledge and attitudes of independent midwives with compliance with the standards of antenatal care services with a value of p <0.05. The results of the study with multivariate logistic regression analysis showed that the factors associated with the compliance of independent midwives in carrying out antenatal care service standards were attitudes with values (p = 0.026).

Background: The prevalence of gastroesophageal reflux disease (GERD) is increasing worldwide and the related chronic symptoms can be associated with morbidity and poor quality of life. Objective: The objective of this study was to identify foods and beverages consumed by the Lebanese population, dietary habits, socio-demographic and lifestyle factors, health parameters and perceived stress, implicated in increasing GERD symptoms. Methods: This observational cross-sectional study was carried among Lebanese adults in 2016. A convenient sample of 264 participants was equally divided into a GERD group and a control group. Data on socio-demographic characteristics, lifestyle, health status and dietary habits including Lebanese traditional dishes were collected. The perceived stress scale (PSS) was also used to assess the participants’ perception of stress. Logistic regression analyses were conducted with GERD symptoms (presence or absence) being the dependent variable. Results: The GERD symptoms were significantly associated with age (-p-value=0.017), family history of GERD symptoms (-p-value<0.001), smoking (-p-value=0.003) and chronic medical conditions (-p-value<.001). Regarding the dietary factors, participants who ate three meals or less/day, between meals and outside homes were 2.5, 2.9 and 2.4 times at a higher risk of experiencing GERD symptoms than others, respectively. Moreover, the logistic regression model showed that the GERD symptoms were significantly associated with the consumption of coffee (-p-value=0.037), Lebanese sweets (-p-value=0.027), fried foods (-p-value=0.031), ‘Labneh’ with garlic (-p-value<0.001), pomegranate molasses (-p-value=0.011), and tomatoes (-p-value=0.007). Conclusion: Some specific lifestyle factors and components of the Lebanese Mediterranean diet could be associated with GERD symptoms.

<p><em>Perawatan pada lansia di panti tresna werdha X dilakukan berdasarkan rutinitas kerja dan petugas belum mendapat pelatihan mengenai kebutuhan dasar lansia yang harus dipenuhi. Berdasarkan hal tersebut, peneliti ingin mengetahui hubungan budaya kerja petugas panti menurut persepsi lansia dengan kejadian pengabaian di PSTW X Sumatera Barat. Desain penelitian deskriptif korelasi, menggunakan pendekatan cross sectional. Pengambilan sampel dengan total sampling, berjumlah 75. Uji statistik digunakan chi square dan regresi logistik. Hasil yang diperoleh seluruh lansia mengalami pengabaian; ada hubungan antara sikap terhadap apa yang dikerjakan, lingkungan pekerjaan; waktu dengan pengabaian lansia; Tidak ada hubungan antara perilaku ketika mengambil keputusan dengan pengabaian lansia. Sikap terhadap apa yang dikerjakan memiliki pengaruh paling besar terhadap kejadian pengabaian lansia. Disimpulkan bahwa petugas harus lebih memahami tentang peran dan fungsinya sebagai pemberi pelayanan pada lansia. Direkomendasikan agar pejabat yang berwenang dapat menempatkan petugas panti sesuai dengan peran dan fungsi serta kewenangannya, dan merekrut perawat sesuai dengan kebutuhan panti.</em></p><p> </p><p><em>The nursing of elderly in </em><em>Tresna Werda X nursing home is mainly based on the workrout in and that the caregiver have not yet received training on the basic needs of the elderly that they should meet. Therefore, the researcher wanted to know the relations of the work culture of caregiver according to the elderly’s perception with the incidence of neglect in PSTW X West Sumatera. This study used Descriptive correlation design with cross sectional approach. The method of sampling used total sampling, which amounts to 75. The statistical test used chi square and logistic regression. The results of the study show that all of the elderly experienced neglect; there is a relation between attitude towards what is conducted, work environment, time with elderly neglect; no relation between behavior in making decisions with elderly neglect. The attitude towards what is conducted has the most impacton the incidence of elderly neglect. It is concluded that the caregiver should have a better understanding on their roles and functions as the caregivers to the elderly. Moreover, it is recommended that the authorities should put nursing personnel in accordance with the roles, functions and competence, and recruit nurses in accordance with the needs of the nursing home.</em></p>

Background:Acromegaly is a rare disease with a remarkable impact on patients, both in terms of life expectancy and quality of life. Osteo-articular complications are one of the most frequently reported bothers. The “acromegaly artropathy” characterizes more than 70% of patients at diagnosis. Artropathy affects both spine and peripheral joints. A recent prospective study documented progression of acromegalic arthropathy identified as a worsening of osteophytes and joint space narrowing in 72–74% of patients despite long-term biochemical control. In addiction the Literature has occasionally reported cases of simultaneous presence of rheumatic diseases (rheumatoid arthritis, polymyalgia rheumatica, undifferentiated connective tissue diseases) and acromegaly and in all these cases the treatment has been delayed, because of wrong symptoms attribution to acromegaly artropathy.Objectives:The primary goal of the study is to better characterize joint pain in acromegaly patients and to evaluate the prevalence of rheumatic disease in growth hormone (GH) secreting pituitary tumor patients.Methods:We enrolled 20 acromegaly subjects (AS) and 20 control subjects (CS). In each subject immunological pattern (rheumatoid factor – RF; antinuclear antibodies - ANA, ENA; anti-citrullinated protein antibodies - ACPA; erythrocyte sedimentation rate – ESR) has been evaluated; they, also, underwent bilateral joint ultrasound of hands and wrists and nail capillaroscopy. The Chi square test and the Fisher’s exact test were used to evaluate the association between binary variables, while the Spearman’s test to evaluate the correlation of continuous ones. A multiple or logistic regression model was calculated in order to define the association between the capillaroscopic alterations and other detected variables.Results:Articular pain emerged as significantly more frequent in AS (p = 0.0269). No statistically significant differences are detected regarding immunological pattern. ANA and ENA screening resulted positive in 10% in AS and in 5% in CS. No IgA ACPA were detected in AS or CS, while IgG ACPA were positive only in one AS subject. No significant differences were detected between IgM and IgG RFin the two groups (AS 5% and CS 0%). Three fold higher IgG FR in AS compared to CS were detected. ESR levels were significantly higher than CS (p = 0.0405), as well as increased power doppler (PWD) articular uptake (AS 30% vs CS 5% p 0.081). The capillaroscopic evaluation showed a significant difference in almost each parameter that has been evaluated (logistic regression: number of enlargement p 0.004, hemorragies p 0.01 and capillaries p 0.001), showing a moderate-severe microangiopathy in AS. Interestingly, analyzing only the acromegaly cohort, we noticed higher GH levels at the enrollment in patients which developed capillary enlargements (GH: 0.95 ng/ml IQ 0,6-1,6) compared to other ones (GH: 0.55 ng/ml IQ 0.4-0.7; p = 0.08) and a significant lower number of hemorrhages (p = 0.02) in patients treated with GH antagonist pegvisomant.Conclusion:Our results demonstrated that joint damage in acromegaly does not seem to have an autoimmune etiology. Therefore, articular damage is mechanical and increased ESR and PWD alterations seems to confirm the presence of an inflammatory component. In addition, acromegaly is characterized by a microvascular pattern of moderate-severe microangiopathy, without correlation to IGF-I, but GH levels. Although requiring further confirmatory studies, our preliminary results seem to indicate how the capillaroscopic examination could be useful to detect earlier microangiopathy and to identify patients with a greater risk of macroangiopathy development..References:[1]Claessen KMJA et al.
