Clinical evaluation of the quality of restorative materials carried out taking into account indicators hygiene index, some criteria: absence of side effects on the tissue surrounding the tooth. While taking into account the almost constant contact with the seal gums determined the importance of assessing the extent of its impact on their functional status. Have been revealed the direct dependence of the state of oral hygiene, pathological changes in the soft tissues and the seal quality. One of the objectives of the study was to determine the quality of the restoration carried out in the cervical area, depending on the depth of gingival sulcus in the presence of inflammation in periodontal tissues. The maximum depth of the gingival pocket, which is permissible under the direct defect filling, was 3 mm clinical studies found a direct relationship between the restoration qualities and cavity extent below the gingival margin. Maximum positive results on the quality of sealing recorded in the first group, an intact periodontal pocket depth. Analysis of the results of clinical studies of the oral cavity examined patients before and after treatment and preventive measures are revealed a number of features. Carrying out preventive and curative action using both traditional means and biologically neutral drugs has reduced the value of the index OHI-S GI and achieve significant improvements in oral hygiene status in patients in all stages of clinical trials.

ABSTRACT Objective: Chronic periodontitis (CP) is a common inflammatory disease that causes destruction to the supporting tissues of the teeth. Many treatment modalities tried to stop the disease progression. Platelet-rich plasma (PRP) is one of the regenerative methods that used in adjunct to conventional periodontal treatment. The aim of this study was to evaluate the anti-inflammatory effect of PRP by monitoring the lymphocyte count before and after its application to the periodontal pocket. Materials and Methods: Twenty patients, with CP and a pocket depth equal to or deeper than 4 mm, subjected to scaling, root planing, and PRP injection into the pocket. The lymphocyte count measured before and after 1 month from PRP application. Clinical periodontal parameters were taken during two visits (1 month apart), with customized stent fabrication. Results: All clinical periodontal parameters showed a reduction in their value following 1 month of PRP application. . There was a noticeable reduction in lymphocyte count from (mean 2.47 ± 0.91) to (mean 1.94 ± 0.77). Conclusion: In addition to its traditional uses, PRP has a great role in the periodontal treatment by its anti-inflammatory effect.

Subject. Gingival recession refers to a form of periodontal disease, which is characterized by a decrease in the free gum without signs of chronic inflammation in the apical direction of the gum and periodontal tissues. The goal is to determine the clinical and radiological features of periodontal tissues in patients with gum recession. Methodology. A comprehensive diagnostic examination was conducted according to a specially developed algorithm of 47 patients aged 20 to 45 years old with a diagnosis of gum recession, which included the collection of anamnestic data, special questionnaires, clinical, radiological and ultrasound examinations of periodontal tissues. Results. According to clinical and radiological data, the diagnosis of gum recession was confirmed in all patients: in 31 (62 %) ― Miller's class I gum recession, in 11 (25 %) ― II, in 5 (11 %) ― III and in 1 (2 %) ― IV. The dependence of the gum recession on the quality of oral hygiene was determined: with satisfactory oral hygiene (OHI-S <2.6), the depth of the gum recession was 1-1.5 mm, with poor (OHI-S> 2.6) ― more than 1,5 mm. In 14 (42.8 %) patients, a concomitant diagnosis of chronic generalized catarrhal gingivitis was established, in 57.2 % of patients with chronic generalized periodontitis. Of the total number of patients, a thin biotype was determined in 30 (63.8 %) patients, and a thick biotype was identified in 17 (36.2 %). A combination of a thin biotype with a small vestibule of the oral cavity, the presence of anomalies of the frenum and strands of the mucous membrane in 23.4 % was revealed. Conclusions. Miller class I gingival recessions have the highest frequency of occurrence (62 %). The ultrasonic research method allowed non-invasive determination of the gingival biotype and gum thickness. A thin biotype of the gum was diagnosed in 63.8 % of patients, which must be taken into account when planning surgical treatment.

Honey is a sweet, viscous natural substance made from flower nectar by bees. Honey has been used not only as a nutritional source but also for wound healing and to reduce tissue inflammation. Nevertheless, the use of honey in the treatment of periodontitis is not well established. This study aimed to evaluate the effect of locally delivered Tualang honey on periodontal tissue healing by a randomized controlled split-mouth clinical trial involving 20 chronic periodontitis patients with a periodontal pocket depth of ? 5 mm. Each site was randomly treated either by scaling and root debridement alone (Control Group) or scaling and root debridement with locally delivered Tualang honey (Test Group). Assessment of probing pocket depth (PPD) and clinical attachment level (CAL) was recorded at baseline and after 6 weeks interval. Gingival crevicular fluid samples were collected from treated pockets at baseline and along with periodontal reassessment to evaluate the level of Matrix Metalloproteinase 8 (MMP-8) and Osteoprotegerin (OPG). Data were analysed by using Wilcoxon Signed Rank Test and Paired Sample t-Test. PPD and CAL were significantly improved after the 6 weeks review (P=0.001) in both groups. However, there was no significant difference in the changes of the PPD, CAL, MMP-8 and OPG levels after the 6 weeks review and in between the groups. In conclusion, within the limitations of this study, the effect of locally delivered Tualang honey on periodontal tissue healing is not evident. Nevertheless, all pockets achieved good periodontal healing.

Relevance. The necessity of somatic health evaluation in patients with local inflammatory periodontal processes is determined by frequent relapses of the periodontal diseases. Low effectiveness of the widespread treatment methods and the lack of long-term remission of the disease is caused by the insufficient assessment Based on the clinical examination, diagnosis and laboratory tests, to determine the correlation between the somatic pathology and adaptability of the adult body in patients with localized periodontitis due to trauma. Materials and methods. Advanced examination was carried out in 169 subjects, aged 45-75 y.o., with mild to moderate localized periodontitis due to trauma. The somatic condition was assessed by laboratory and functional tests. Results. 1-2 somatic diseases were revealed in 39.3% of patients with mild localized periodontitis caused by trauma (periodontal pocket depth up to 4 mm). 3 and more comorbidities were detected in 35.3% of patients. Gastrointestinal problems, cardiovascular diseases, allergic conditions, anemiа and respiratory diseases were most frequently encountered. In moderate localized periodontitis caused by trauma and periodontal depth of 4-6 mm, 23.9% of patients had 1-2 comorbidities, while 62.7% of patients had 3 and more diseases. Gastrointestinal problems, cardiovascular diseases, anemia, allergic conditions and urogenital diseases were more frequent. A strong correlation was revealed between the type of adaptive reaction and the degree of localized periodontitis caused by trauma. Thus, the risk of moderate localized traumatic periodontitis development increases by 1.8 times in tense adaptive reaction, and by 3.45 times - in poor adaptation reaction. Conclusion. Functional condition of the body adaptability, which is essential for the development of the treatment strategy, could be determined by the evaluation of the physical condition, peripheral blood tests (entropy of WBC differential, C-reactive protein) in patients with localized periodontitis caused by trauma. The adaptive reaction tension and decrease were detected in patients with 3 or more comorbidities.

Objective: The objective of the present study was to determine the effect of non-surgical periodontal therapy on the disease severity in rheumatoid arthritis patients. Methods: Five hundred and twenty arthritis patients were included in the study, using consecutive non-probability sampling from the outpatient department of various public and private hospitals of Pakistan. Patients with periodontitis along with systemic Rheumatoid Arthritis (RA) were included in the study. Clinical parameters taken for the severity of periodontitis were the Gingival recession (GV), Periodontal Pocket Depth (PPD) and Clinical Attachment Loss (CAL). Independent sample t-test was used for significance before and after non-surgical periodontal therapy. Data was presented with mean (standard deviation). Results: The PPD and CAL of Rheumatoid Arthritis patients were found to be decreased significantly following non-surgical periodontal treatment (p value < 0.05), unlike the gingival recession, which did not decrease significantly (p value > 0.05). Conclusion: The PPD and CAL values of periodontal patients with rheumatoid arthritis were significantly reduced after giving non-surgical Periotherapy along with oral hygiene instruction. Stabilizing or reducing periodontal pocket depth can have a positive influence on the retention of teeth in dogs. A topical 2 % clindamycin hydrochloride gel (CHgel) was evaluated for the treatment of periodontal disease in dogs. The CHgel formulation provides for the sustained erosion of the matrix, but also flows into the periodontal pocket as a viscous liquid, and then rapidly forms a gel that has mucoadhesive properties and also may function as a physical barrier to the introduction of bacteria. A professional teeth cleaning procedure including scaling and root planing was done in dogs with one group receiving CHgel following treatment. Periodontal health was determined before and after the procedure including measurement of periodontal pocket depth, gingival index, gingival bleeding sites, and number of suppurating sites. There was a statistically significant decrease in periodontal pocket depth (19 %), gingival index (16 %), and the number of bleeding sites (64 %) at 90-days in dogs receiving CHgel. Additionally, the number of suppurating sites was lower (93 %) at 90-days for the group receiving CHgel. The addition of CHgel effectively controlled the bacterial burden (e.g., Fusobacterium nucleatum) at both day 14 and 90. Gingival cells in culture were shown to rapidly incorporate clindamycin and attain saturation in approximately 20-minutes. In summary, a professional teeth cleaning procedure including root planning and the addition of CHgel improves the gingival index and reduces periodontal pocket depth.

Background: Ischemia-modified albumin (IMA) is a systemic indicator of inflammatory diseases and is suggested as an oxidative stress marker. Objective: To determine the IMA and high-sensitivity C-reactive protein (hsCRP) serum levels for patients with chronic periodontitis (CP) and to evaluate the impact of non-surgical periodontal therapy on serum IMA and hsCRP levels. Methods: Twenty one systemically healthy patients with CP and 15 systemically and periodontally healthy controls (C) were enrolled in the study. Periodontal pocket depth (PPD), bleeding on probing (BOP) and attachment loss (AL) were recorded at the time of diagnosis and 6 weeks after the nonsurgical periodontal therapy. Blood samples were obtained before and after treatment from all groups, and serum IMA and hsCRP levels were evaluated by ELISA method. Results: All of the clinical findings were found to be elevated in the CP group in comparison to C group (p<0.05). Levels of IMA and hsCRP were higher in the CP group (p<0.05) and decreased after non-surgical periodontal therapy (p<0.05). Positive correlations were determined between PPD, BOP and hsCRP (p<0.05) as well as between PPD, AL, BOP and IMA levels (p<0.01) before treatment. A significant positive correlation was also observed between hsCRP and IMA (p<0.01) before and after treatment. Conclusion: IMA is a marker indicating systemic inflammation during periodontal disease, and is significantly reduced as a result of non-surgical periodontal therapy. Therefore, IMA might be suggested as a useful indicator of periodontal disease.

Latar Belakang: Pada kondisi fisiologis, gigi geligi berada pada posisi stabil dalam lengkung rahang, mempunyai kontak oklusal dengan gigi lawan, dan kontak proksimal dengan gigi disebelahnya. Adanya sedikit pembukaan kontak proksimal akan menyebabbkan terjadinya impaksi makanan, yang merupakan etiologi kelainan periodontal. Pembuatan restorasi permanen dengan menggunakan bahan komposit yang memperhatikan kontak proksimal, kontur permukaan oklusal,  kontur fasial dan lingual dapat melindungi jaringan periodontal. Tujuan: menunjukkan bahwa restorasi komposit untuk perbaikan titik kontak dapat menunjang kesehatan jaringan periodontal. Laporan kusus: pada kasusu pertama ditemukan gigi 2.4 dan 3.5 terdapat karies di sisi distal, dan pada kasus kedua gigi 2.8 terdapat karies di sisi mesial sehingga kehilangan titik kontak yang mengakibatkan terjadi poket periodontal. Penanganan: terapi awal yang dilakukan berupa skeling dan penghalusan akar, serta kuretase. Pembuatan restorasi komposit untuk memperbaiki titi kontak dengan menggunakan matriks dan baji di area proksimal. Maladaptasi pita matriks atau baji menyebabkan kegagalan klinis seperti batas restorasi mengemper, kontur didnding proksimal serta titik kontak yang tidak baik. Pemilihan baji yang benar, harus mempertimbangkan : sudut konvergensi dasar, lebar mesiodistal dasar baji, ketinggian gingivooklusa dari arah transversal, dan kecembungan dinding  baji. Kesimpulan: secara klinis terjadi penurunan kedalaman poket setelah perbaikan restorasi, namun gambaran radiografis belum memperlihatkan hasil signifikan dalam waktu tiga bulan.  Blackground: under physiological conditions, teeth are stabilized in the dental arch by making occlusal contacts with opposing teeth and proximal contacts with adjacent teeth. A weak or slightly open proximal tooth contact would permit food impaction, one of the etiology of periodontal disease. The common therapy is to make a permanent restoration using composite restoratioan having regard to proximal contacts, occlusal contour, and facial and lingual contaour to protect periodontal tissues. Objective: to show that it is important to restore contacts points in maintaining  periodontal tissues using composite restoration. Case: on the first case, we found caries on the distal site of 2.4 and 3.5 and mesial site of 2.8 on the second case, and open contacts, resulting a periodontal pocket. Therapy: the initial treatment are scaling and root planning and curretage. Composite restoration to restore proximal contact points using matrix band and wedge. Maladaptation of either the matrix band or the wedge result in clinical failure such as overhanging margins, faulty contouring of the proximal wall and inadequate contact points. To select a correct wedge, four variables should lie considered the convergence angle of the base, the mesiodistal width of the base, the gingivo-occlusal height of the transverse section and the concavity of the side walls. Conclusion: Clinically, there are reduction in pocket depth after the restoration, but radiograpgically, there are no significant result in three months.

The interdental pressure index (IPI) is introduced to specifically evaluate clinical interproximal-tissue conditions and assess the effect of interproximal hygiene stimulation. This index scores clinical responses of periodontal tissues to the apical pressure of a horizontally placed periodontal probe. It is negative when gingival tissues are firm, bleeding-free, and slightly ischemic by the stimulation; otherwise it is positive. The clinical validation showed high intraoperator agreement (0.92; 95% CI: 0.82–0.96;P=0.0001) and excellent interoperator agreement (0.76; 95% CI: 0.14–1.38;P=0.02). High internal consistency with bleeding on probing (κ=0.88) and gingival index (Cronbach’sα=0.81) was obtained. Histological validation obtained high sensitivity (100%) and specificity (80%) for IPI+ toward inflammatory active form. The same results were recorded for IPI− toward chronic inactive form. IPI results as a simple and noninvasive method with low error probability and good reflection of histological condition that can be applied for oral hygiene motivation. Patient compliance to oral hygiene instructions is essential in periodontal therapy and IPI index can be a practical and intuitive tool to check and reinforce this important aspect.

Introduction: Patients with visual impairment present greater difficulty in achieving adequate oral hygiene due to restricted eyesight. Therefore, oral hygiene motivation and instruction methods should be adapted to these patients to understand the importance of bacterial plaque control, both for the health of dental tissues and periodontal tissues Objective: The objective of the present study was to compare the plaque index (PI) of patients with and without visual impairment before and after the institution of oral hygiene instructions through a Randomized Clinical Trial. Twenty patients with visual impairment (Group 1) from the Instituto dos Cegos and 20 without visual impairment (Group 2) from the ASCES Periodontia Clinic were selected. Data were collected through the IP of O'Leary and reevaluated every 21 days for four sessions. Results: in both groups, the mean of PI decreased with the time of evaluation, with a significant difference between the evaluations (p <0.001). Group 1 had a mean PI higher than group 2 only in the fourth evaluation, but there was no significant difference (p> 0.05). As for the mean of the PI assessments, group 2 presented a mean higher than group 1, but without significant difference (p> 0.05). Conclusion: The study therefore suggests that there is no difference in PI in patients with or without visual impairment, and that the guidelines stimulate and motivate an improvement in oral hygiene conditions.
