Evaluation of hemomicrocirculation of periodontal tissues in localized periodontitis of traumatic etiology

Relevance. The necessity of somatic health evaluation in patients with local inflammatory periodontal processes is determined by frequent relapses of the periodontal diseases. Low effectiveness of the widespread treatment methods and the lack of long-term remission of the disease is caused by the insufficient assessment Based on the clinical examination, diagnosis and laboratory tests, to determine the correlation between the somatic pathology and adaptability of the adult body in patients with localized periodontitis due to trauma. Materials and methods. Advanced examination was carried out in 169 subjects, aged 45-75 y.o., with mild to moderate localized periodontitis due to trauma. The somatic condition was assessed by laboratory and functional tests. Results. 1-2 somatic diseases were revealed in 39.3% of patients with mild localized periodontitis caused by trauma (periodontal pocket depth up to 4 mm). 3 and more comorbidities were detected in 35.3% of patients. Gastrointestinal problems, cardiovascular diseases, allergic conditions, anemiа and respiratory diseases were most frequently encountered. In moderate localized periodontitis caused by trauma and periodontal depth of 4-6 mm, 23.9% of patients had 1-2 comorbidities, while 62.7% of patients had 3 and more diseases. Gastrointestinal problems, cardiovascular diseases, anemia, allergic conditions and urogenital diseases were more frequent. A strong correlation was revealed between the type of adaptive reaction and the degree of localized periodontitis caused by trauma. Thus, the risk of moderate localized traumatic periodontitis development increases by 1.8 times in tense adaptive reaction, and by 3.45 times - in poor adaptation reaction. Conclusion. Functional condition of the body adaptability, which is essential for the development of the treatment strategy, could be determined by the evaluation of the physical condition, peripheral blood tests (entropy of WBC differential, C-reactive protein) in patients with localized periodontitis caused by trauma. The adaptive reaction tension and decrease were detected in patients with 3 or more comorbidities.

Микрофлора человека - сложная саморегулирующая система, способная восстанавливаться при грамотной коррекции. Исследования последних лет показали, что нарушение нормального микробиоценоза полости рта растет из года в год и у жителей Российской Федерации превышает 90 %, вызывая при этом воспалительные заболевания в тканях пародонта, и, как правило, сопровождается дисбиозом полости рта, выраженность которого соответствует степени поражения пародонта. Одно из ведущих мест в развитии данной патологии занимает резидентная облигатно-анаэробная и микроаэрофильная микрофлора полости рта. Изучение микробиологического состава пародонтальных карманов (ПК) при воспалительных заболеваниях тканей пародонта (ВЗТП) имеет большое значение непосредственно для эффективного лечения и понимания патогенетических этапов этих заболеваний. Достаточно серьезной проблемой современной стоматологии как в нашей стране, так и за рубежом являются заболевания пародонта. Указанная патология снижает принятый ВОЗ критерий качества жизни и приводит к преждевременной потере зубов и представляет серьезную медицинскую, социальную и экономическую проблему. Первичным фактором, вызывающим поражение пародонта, являются бактерии зубного налета. Этиологическая структура инфекционных процессов в последнее десятилетие значительно изменилась, что связано с постоянной эволюцией микробов и вовлечением в патологический процесс условно-патогенных микробов, которые могут выступать в качестве комменсалов в составе нормальной микрофлоры и проявлять свою патогенность при снижении иммунного статуса организма The human microflora is a complex self-regulating system that can be restored with proper correction. Recent studies have shown that the violation of the normal microbiocenosis of the oral cavity increases from year to year and in the residents of the Russian Federation exceeds 90 %, causing inflammatory diseases in the periodontal tissues, and, as a rule, is accompanied by dysbiosis of the oral cavity, the severity of which corresponds to the degree of periodontal damage. One of the leading places in the development of this pathology is occupied by the resident obligate-anaerobic and microaerophilic microflora of the oral cavity. The study of the microbiological composition of periodontal pockets (PC) in inflammatory diseases of periodontal tissues (VSTP) is of great importance directly for the effective treatment and understanding of the pathogenetic stages of these diseases. Quite a serious problem of modern dentistry both in our country and abroad are periodontal diseases. This pathology reduces the WHO-accepted quality of life criterion and leads to premature tooth loss, and is a serious medical, social and economic problem. The primary factor that causes periodontal disease is plaque bacteria. The etiological structure of infectious processes has changed significantly in the last decade, which is due to the constant evolution of microbes and the involvement of conditionally pathogenic microbes in the pathological process, which can act as commensals in the normal microflora and show their pathogenicity with a decrease in the immune status of the body

Background: Periodontitis is a common chronic inflammatory dental disease, which occurs due to the existence of pathogenic microorganisms within the gingival plaque and lead to the formation of periodontal pocket. This study was aimed to evaluate the effectiveness of satranidazole and ornidazole in the treatment of chronic periodontal diseases along with mechanical debridement.Methods: Forty subjects were randomly selected to access the effectiveness of selected drugs on the basis of clinical and microbiological investigations over a period of 14 days. Six Ramfjord teeth (i.e. 16, 21, 24, 36, 41 and 44) were examined for investigating clinical parameters such as gingival inflammation, pocket depth and bleeding on probing. Microbiological investigations were carried out to examine the presence of gram positive (cocci and bacilli), gram negative (cocci and bacilli) and spirochaetes.Results: A substantial progress was recorded in treating gingival inflammation, pocket depth and bleeding on probing. The results of microbiological investigations suggest that the satranidazole and ornidazole were equally effective when used alone and with scaling and root planning in reducing microbial infections. The results indicated a significant (p <0.0001) effect of model drugs on clinical and microbiological parameters in different study subjects at baseline (pre-treatment), and 7 days and 14 days post treatment.Conclusions: The results concluded that ornidazole is better than satranidazole in treating periodontal diseases.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

Abstract Background HIV infection is associated to different oral manifestations (including periodontal diseases), which have decreased with the advent of antiretroviral therapy (ART). Yet, the occurrence of periodontitis is still consistent among patients with HIV living in sub Saharan-Africa, with limited evidence on the driven factors and mitigating measures in these settings. We aimed at evaluating the occurrence of periodontitis and its associated immunological and virological factors in patients with HIV living in Yaoundé, Cameroon. Methods We included 165 (44 ART-naïve and 121 ART-experienced) patients > 18 years old attending the Yaoundé Central Hospital and the Chantal BIYA International Reference Centre, from January-April 2018. The periodontal status was assessed by measuring the clinical attachment loss, periodontal pocket depth, plaques index and gingival bleeding index. CD4+/CD8+ cells and viremia were measured using the fluorescence-activated cell sorting method (FACS Calibur) and the Abbott m2000 RT HIV-1 RNA kit respectively. A standard-questionnaire concerning participants’ medical records and oral hygiene methods was filled. Data was analyzed and p < 0.05 considered statistically significant. Results There was a significantly high prevalence of periodontitis in the ART-naïve (53.2%) compared to the ART-experienced group (37.3%), with a twofold increased risk of the ART-naïve population presenting with periodontitis than the ART-experienced population (OR 2.06, p = 0.03). More importantly, ART-naïve, patients with CD4 < 200 cells presented with higher risk of having periodontitis compared to those with higher CD4-values, with a threefold difference (OR 3.21). Worth noting, males presented with a higher risk of having clinical attachment loss (OR 6.07). There was no significant association between the occurrence of periodontitis and the CD8 (p = 0.45) or viremia (p = 0.10). Conclusion In the Cameroonian context, a considerable number of adults infected with HIV suffer from periodontitis regardless of their treatment profile. Nonetheless, ART-naïve patients have a higher risk, indicating the protective role of ART. Interestingly, severely immune-compromised patients and men are vulnerable to periodontitis, thereby highlighting the need for clinicians to refer patients for regular periodontal screening especially male patients and those with low CD4. Such measures could greatly improve the quality of life of the population living with HIV in Cameroon.

Background: Stress is playing a major role as an aetiologic factor in the initiation of many systemic diseases. Similarly, it is also thought to be an aetiologic factor in the progression of periodontal diseases. Aim: The production of free radical species is associated with various inflammatory diseases and also during stressful conditions. Hence, an effort in the present study is done to correlate psychosocial stress and periodontitis using serum MDA as an oxidative stress biomarker. Materials and Methods: A current cross-sectional study included 201 individuals between 20 to 60 years of age. A periodontal examination included Probing Pocket Depth, CAL, Plaque Index, and Gingival Index. According to CAL individuals were divided into four groups i.e. healthy, slight (CAL = 1-2 mm), moderate (CAL = 3-4 mm) and severe (CAL = ≥5 mm) periodontitis. Psychosocial stress of the individuals was evaluated using occupational stress index. Serum MDA level was evaluated using spectrochromatometer. Results: Patients with stress demonstrated increased levels of MDA along with a higher loss of attachment. Conclusion: The results revealed statistically significant association between psychosocial stress and periodontitis indicating psychosocial stress as a risk factor for developing periodontitis.

The article presents a retrospective review of professional domestic and foreign literature on modern aspects of treatment of inflammatory periodontal diseases, including vector therapy, plasmalifting, which can be used in various forms of this pathology. Periodontal diseases are one of the most pressing problems of dentistry, which has not only general but also great social significance, due to the high prevalence, intensity of the process, the subsequent formation of chronic odontogenic foci of infection, tooth loss at a young age and adverse effects on the body. Treatment of periodontal disease is a complex and lengthy process that includes general methods of influencing the pathophysiological processes occurring in the body, both local - therapeutic, orthopedic and surgical measures. Before planning treatment, the dentist should determine the patient's medical and dental history, conduct clinical and radiological examinations, determine the degree of loss of the alveolar ridge, functional disorders of the dental system and the possibility of their correction. Depending on this, determine the plan of treatment and rehabilitation measures, the sequence of medical interventions. The main task of periodontal treatment is to optimize the conditions for maintaining a healthy dentition and restoring its physiological functions. The difficulty of treating inflammatory periodontal diseases is that known and standard methods (antibacterial therapy and courses of local therapy) do not always give a positive result and a long period of remission. Therefore, to improve the quality of medical care for patients with periodontal pathology, it is necessary to introduce new innovative methods of treatment in dental practice. Given the patterns of pathophysiological processes occurring in periodontal tissues and bone tissue of the alveolar process, to date, several stages of treatment and rehabilitation measures have been proposed. The first stage is the elimination of the pathogenic action of microbial factors and inflammatory processes in periodontal tissues. Microbial invasion and the inflammatory reaction of periodontal tissues caused by it activate the processes of osteoclastical bone resorption. The task of the second stage of treatment and rehabilitation measures is to restore the physiological structure of gums and bone by normalizing metabolic processes in periodontal tissues, reducing the activity of resorption processes and stimulating reparative osteogenesis in the bone tissue of the alveolar process and the body in general, surgical treatment. At the third stage of treatment and rehabilitation measures, teeth with a significant degree of atrophy of the alveolar process are removed, which cannot be used for prosthetics. To restore the integrity of the dentition, even distribution of masticatory load, appropriate prosthetics are performed. The use of the latest methods contributes to the effective treatment of periodontal disease in the early stages, stabilization of the process at deeper stages, in some cases helps to avoid surgery.

Honey is a sweet, viscous natural substance made from flower nectar by bees. Honey has been used not only as a nutritional source but also for wound healing and to reduce tissue inflammation. Nevertheless, the use of honey in the treatment of periodontitis is not well established. This study aimed to evaluate the effect of locally delivered Tualang honey on periodontal tissue healing by a randomized controlled split-mouth clinical trial involving 20 chronic periodontitis patients with a periodontal pocket depth of ? 5 mm. Each site was randomly treated either by scaling and root debridement alone (Control Group) or scaling and root debridement with locally delivered Tualang honey (Test Group). Assessment of probing pocket depth (PPD) and clinical attachment level (CAL) was recorded at baseline and after 6 weeks interval. Gingival crevicular fluid samples were collected from treated pockets at baseline and along with periodontal reassessment to evaluate the level of Matrix Metalloproteinase 8 (MMP-8) and Osteoprotegerin (OPG). Data were analysed by using Wilcoxon Signed Rank Test and Paired Sample t-Test. PPD and CAL were significantly improved after the 6 weeks review (P=0.001) in both groups. However, there was no significant difference in the changes of the PPD, CAL, MMP-8 and OPG levels after the 6 weeks review and in between the groups. In conclusion, within the limitations of this study, the effect of locally delivered Tualang honey on periodontal tissue healing is not evident. Nevertheless, all pockets achieved good periodontal healing.

The aim of this study was to assess the periodontal status of cystic fibrosis (CF) adult patients and to evaluate whether there is a correlation between the bacterial population of the subgingival biofilm and the health status of the periodontal tissues in this group of adults. The study involved 22 cystic fibrosis adult patients. The periodontal condition was assessed using Plaque Index (PLI), Gingival Index (GI), and Probing Pocket Depth (PPD). The gingival sulcus samples were analyzed by the Real-Time PCR assay (RT-PCR). Majority of patients showed moderate or severe bacterial dental plaque accumulation, but none of them had clinical symptoms of periodontal diseases. RT-PCR showed the presence of periopathogens in 50% of patients. Red complex microorganisms were detected in 9.09%, orange complex in 27.27%, and green complex in 31.82% of the samples analyzed. In cystic fibrosis patients colonized by periopathogens, the periodontal markers were significantly higher in comparison to not colonized by periopathogens patients. Despite the widespread presence of bacterial dental deposits in the cystic fibrosis adult patients examined, none of them has clinical symptoms of periodontal disease; however, the presence of periodontal pathogens in subgingival biofilm may represent a possible risk factor of this disease in the future. An unsatisfactory level of oral hygiene in any patient with cystic fibrosis indicates a need to focus on standards of dental care for such patients.

Subject. The article presents the data of a comparative analysis of the traditional method of treating chronic generalized periodontitis of mild to moderate severity and with the use of periodontal dressings in combination with an antibacterial photodynamic system. The goal is to study the effectiveness of the methodology of complex therapy for the treatment of periodontal diseases using periodontal dressings and antibacterial photodynamic therapy. Methodology. We observed 120 patients with inflammatory periodontal diseases aged 18 to 55 years (men ― 52, women ― 68), who were divided into the control and main groups. In the control group, treatment was carried out according to the traditional scheme of drug therapy, in the main ― according to the method of complex therapy. The treatment regimen according to this technique included the use of periodontal dressings and an antibacterial photodynamic therapy system: photosynthesis was applied to the affected area of the periodontal pocket, after which the pathogenic microorganisms were stained for 1-3 minutes, then the liquid was thoroughly washed from the periodontal pockets and the laser affected the affected area for 60 seconds. Results. The proposed treatment regimen using an antibacterial photodynamic system in combination with periodontal dressings allows to achieve a pronounced and prolonged anti-inflammatory effect in periodontal tissues without local and systemic use of antibiotics, reduces treatment time for mild periodontitis by 29, average ― by 57 %, reduces the likelihood recurrence, lengthens the duration of remission. Findings. The use of complex treatment of periodontitis of mild to moderate severity allows the method to accelerate the restoration of periodontal tissues, relieve inflammation, saturate the tissues with oxygen, reduce the rate of relapse by 2.6 times and reduce the duration of relapse of this disease within 24 months. Our data allow us to implement this treatment regimen in practical healthcare.

As a result of the continuing changes in the condition of tissues and organs of the dental cavity and the body as a whole, there is a rise in the intensity of tooth decay and periodontal diseases with the onset of pregnancy. The aim of our research was to look into the prevalence and severity of pathology in the hard tissues of teeth and periodontal tissues during pregnancy, as well as to figure out how to better organize therapeutic and prophylactic steps for pregnant women. A comprehensive epidemiological study of 170 pregnant women aged 18 to 40 living in Tbilisi was carried out according to the WHO method. The findings show a high prevalence of dental cavity (from 61 percent to 98.8%) and a high severity of dental caries among pregnant women (from 2.55 1.03 to 14.31 5.44). The intensity of caries indicators increased as the gestational stage lengthened, but the greatest increase was observed in the second trimester of pregnancy. The results indicate a substantial prevalence of periodontal pathology (according to the PMA index) among pregnant women, with the level of pathology increasing with their age and gestational age (from 41.7 percent to 78.8 percent). By the end of the second trimester of pregnancy, the most noticeable improvements in periodontal tissues had been observed. Oral hygiene played a big role in determining the presence and incidence of periodontal disease. Low dental health indicators and worsening dental status during pregnancy point to the need for a more comprehensive diagnosis of all risk factors for major dental diseases, as well as the implementation of all stages of therapeutic and prophylactic interventions during pregnancy, based on the findings of a complete dental examination.
