This article discusses the dynamics of glaucoma prevalence in the Kyrgyz Republic. At present glaucoma is one of the pressing problems of modern health care, as it occupies one of the leading places among the causes of irreversible loss of vision. According to WHO glaucoma accounts for up to 5% of all eye pathology. By 2020, the number of patients will increase to 5.9 million and 5.3 million will lose sight due to primary glaucoma. The proportion of primary glaucoma among the causes of primary disability has been stable at a fairly high level from 20 to 28% over the years. The problem of glaucoma control requires improvement of the system of organization of early detection and dispensary observation of patients, which will contribute to control of the level of morbidity and quality of medical care. The article used data from the Republican Medical and Information Center of the Ministry of Health of the Kyrgyz Republic from 2014 to 2018. On the basis of a comprehensive analysis, the authors identified trends in the incidence of diseases of the eye and its appendages, glaucoma among adults, adolescents and children of the Kyrgyz Republic. The incidence of diseases of the eye and its appendages, glaucoma among adults, teenagers and children in dynamics from 2014 to 2018 has been studied. The share of glaucoma in the total number of diseases of the eye and its appendages among adults, teenagers and children is considered. Analysis of the dynamics of disease prevalence of the eye, its appendages, glaucoma, both among adults and teenagers, and among children under 14 years of age showed an increase in the indicator in 2015, 2017 and 2018. The proportion of glaucoma from the total morbidity of the eye and its appendages among adults and adolescents in the years under study averaged 6.4%, among children under 14 — 0.2%. It is necessary to strengthen measures for early detection, dispensarization of glaucoma patients, as well as to carry out information work among the population.

The article examines the indicators of disability in diseases of the endocrine system of the population of Osh city (Kyrgyzstan). Health promotion, disease prevention, rehabilitation of disabled people are the main tasks in modern health care and social protection of the population. The number of people with disabilities is increasing from year to year in Kyrgyzstan. This growth is associated with a high level of morbidity and traumatism of the population, inadequate quality of medical care, and unfavorable environmental conditions. The study compares the five-year rate of disability in diseases of the endocrine system of the population of Osh. In the periods studied (2016, 2018, 2020), an increase in the number of primary disabilities in diseases of the endocrine system among residents of the city of Osh was revealed.

Background and Objectives: biomarker-based studies are the cornerstone of precision medicine, providing key data for tailored medical care. Enrollment of the planned number of patients is a critical determinant of a successful clinical trial. Moreover, for inclusive medical care, patients from different socio-demographic backgrounds must be recruited. Still, a significant number of trials fail to reach these prerequisites. Designing the informed consent forms based on the patients’ feedback could optimize accrual. We aimed to explore the attitudes of patients from a Romanian tertiary cardiology center towards participation in biomarker-based clinical trials. Materials and Methods: three hundred forty inpatients were interviewed based on a semi-structured questionnaire which included four sections: demographics, personal medical history, attitudes and trust. Results: Roughly, 62.5% of the respondents were interested in enrolling, while altruistic reasons were the most frequently expressed. Clear exposure of the possible risks was most valued (37.78%), followed by the possibility of directly communicating with the research team (23.78%). The most frequently chosen answer by acutely ill patients was improvement of their health, whereas chronically ill individuals indicated the possibility of withdrawal without affecting the quality of medical care. Importantly, the participation rate could be improved if the invitation to enrollment were made by both the current physician and the study coordinator (p = 0.0001). The level of trust in researchers was high in more than 50% of the respondents, and was correlated with therapeutic compliance and with the desire to join a biomarker study. Conclusions: the information gained will facilitate a tailored approach to patient enrollment in future biomarker-based studies in our clinic.

Due to the collapse of the Union, there was a reduction in funding for health care costs, as well as deterioration in the infrastructure and quality of medical services. The transitional economy in the Kyrgyz Republic has identified additional features in the health system. The main ones are the low level of funding, the presence of the shadow market of medical services, inefficient structure and the prevalence of high-cost hospital treatment. The market mechanism is developing, however, without state regulation. The Kyrgyz Republic continues to reform its health-care system. The task was to improve the methods of their financing. New mechanisms for financing medical services have been introduced. Despite the reduction in the number of hospitals, the number of patients treated in hospitals has increased.

Article is devoted to the analysis of material on the relevance of congenital heart defects in the Kyrgyz Republic. The main groups of factors affecting the aetiology of congenital heart defects have been identified. Assessment of social, medical and legal provisions that have developed in cardiac surgery at the present stage dictates the need for monitoring before and after surgical correction, which can provide an improvement in the mechanism for providing medical care to patients. Based on the analysis, measures are proposed to improve the quality of medical care for this category of patients.

The article analyzes the model of doctor-patient relationships in the context of a new type of coronavirus pandemic. The study was conducted by interviewing patients of an infectious diseases hospital organized on the basis of the Federal medical center. According to the data obtained, all patients were satisfied with the quality of medical care. Respondents noted the high level of attention and care of doctors about their health. The survey also showed the proper level of implementation of the patient’s rights to receive information about the state of health. Patients noted the difficult working conditions of medical workers and the validity of state financial support measures. In conclusion, the author notes the formation of a model of relationships between medical professionals and patients, combining paternalistic and cooperative.

The article presents the analysis of results of complex medical sociological survey of effectiveness and efficiency of medical care as main criteria of its quality and accessibility. The survey comprised 1,737 patients of various social groups of population of Moscow and 203 physicians of polyclinics. At self-rating of health, 13.2% of respondents determined it as unsatisfactory, 45.1% as satisfactory and 31.3% as good. In all population groups, the most called-for proved to be polyclinic institutions. The high level of satisfaction of patients with quality of emergency medical care was established in all groups. The quality of medical care in polyclinics and hospitals was assessed significantly lower. The most negatively assessed criteria turned out transition to fee-for-service forms of medical support and time limits of waiting for all modes of medical care, except emergency medical care and district therapist. The evaluation by physicians of their own activity and present problems is characterized by their unanimity in need of increasing of salary (100%) and increasing of typical sectoral standards of time of reception of a patient (96.5%), decreasing of intensity of work of medical personal (92.3%) and reduction of some forms of record cards with the purpose of releasing of time and attention of physician for a patient (88.4%). The disrespectful attitude of patients to medical personnel was mentioned by 77.3% of physicians which is a new phenomenon testifying loss of confidence in patient-physician relationship. The quality of implemented work was assessed equally and rather high by both young and experienced physicians. The positive and negative aspects of reorganization of health care were analyzed. Two alternatives of problems were established to focus plans of development of system of Moscow health care to support accessible and qualitative medical care of population.

According to the Ministry of Health of the Russian Federation, the number of patients with non-insulin-dependent diabetes mellitus (NIDDM) in Russia is currently 1.6 million. Providing effective treatment for NIDDM is a significant problem for domestic health in terms of both quantitative indicators and the quality of medical care. Successful therapy of this disease is impossible without changing the patient’s lifestyle. The treatment process affects such essential aspects of lifestyle as nutrition, physical activity, control of body weight, and continues throughout life. In this regard, the effectiveness of the traditional therapeutic approach to NIDDM is not high enough and, according to experts, the motivation to control and treat diabetes cannot be achieved without training.

This paper summarizes the arguments and counterarguments within the scientific discussion on the issue of quality of medical care. The main purpose of the research is to analyze the quality of medical care in dispensary №1 of municipal non-profit enterprise «Shostka city center of primary health care» and determine recommendations for its improvement in the context of «MEDSTAR» medical information system implementation. The research methods authors used in the article were systematic analysis, comparative research, and patients’ survey. As the information sources, the authors used internal documentation of the dispensary № 1 (data for September 2018) and its electronic documentation from the MEDSTAR medical information system (data for September 2020). First, the authors analyzed and compared the number of patients who visited the dispensary №1, and the number of patients visited by doctors at home in September 2020 and September 2018. Second, the authors determined the number of referrals for examination issued to patients in September 2020 and September 2018. Third, the authors surveyed the patients on their satisfaction with the quality of medical care. The results of the research showed that the total number of patients’ visits at the dispensary №1 decreased by 32.4%, and the rate of home visits decreased by 5.12% in September 2020 compared to September 2018. The same situation is with the number of referrals for examination. In September 2018 763 patients got referrals for further examinations, and in September 2020 the number of referrals was 169. The survey showed that patients are dissatisfied with some aspects of the quality of medical care, particularly, with the automation of medicine, focus of medicine, as well as with the conditions of appointment and accessibility of conventional medical services. In total, the results of the research helped to highlight problems in the work of family doctors caused by healthcare reform and medical information systems implementation which reduce the quality of medical care. The recommendations for problems solving were suggested. The authors’ research will be useful for further research in the quality of medical care.

Subject. Staff capacity of healthcare institutions of the region. Topic. More efficient use of staff capacity of healthcare institutions of the region. Purpose. Delineate the main trends in the formation of staff capacity of the industry and determine the degree of its influence on the efficiency of the system itself as a whole. Methodology. Methods of logical and comparative analysis of intergovernmental regulation practices in the format of interaction «department - healthcare institutions». Results. After analyzing the staffing policy of the Voronezh Region Health Department and the mortality rate in the region for 2020, we identified an inadequate staffing of health care institutions, which affects the improvement in the quality of medical services and the mortality rate. Application area. Sphere of staffing of the Voronezh Region. Conclusions. In 2020, healthcare institutions of the Russian Federation faced a serious threat to public health, namely the COVID-19 pandemic, which in turn determined the parameters of the roadmap for the development of the industry for the coming years and identified the main problem areas. There is traced the dependence of the decrease in the staffing level, depending on the distance of the MO from the regional center. The number of certified specialists, both doctors and paramedics, is at an insufficiently high level, which ultimately affects the quality of medical care and the mortality rate in the Voronezh region. The article presents the results of studying the organization of work of heads of departments of central district hospitals (CRH) and the quality of therapeutic care for the rural population. The study was conducted in 2017–2019. on the basis of 26 Central District Hospital of the Republic of Dagestan. The quality of therapeutic care was studied through an expert assessment of 1,085 inpatient and 748 outpatient patient records. The timing of the working time spent by the heads of the departments of the Central District Hospital for 845 hours. It was found that under the current conditions the heads of departments devote up to 50.7% of their working time to the fulfillment of the main duties. Waste of time is 15.1%. In the therapeutic departments of the Central District Hospital, an untimely and complete examination of a significant part of the patients took place; treatment is not always complete and complete, there is no continuity in the examination and treatment of the patient between the clinic and the hospital. On the basis of the research materials, a set of recommendations was developed to improve the technology of work of heads of departments of the Central District Hospital and improve the quality of medical care. Experimental verification of the recommendations confirmed their effectiveness: the time spent on the main work increased from 51.2% to 72.4%, the share of time spent on organizing and managing the treatment and diagnostic process increased by 15.5%. Improving the organization of work of heads of departments during the experiment had a positive effect on the quality of medical care: the proportion of patients examined at the prehospital stage increased by 25.4%; the number of those fully surveyed increased from 58.4% to 90.3%; the number of patients who received full treatment increased by 14.7%.
