THE QUALITY OF MEDICAL CARE IN MEDICAL ORGANIZATIONS OF MOSCOW PUBLIC HEALTH CARE SYSTEM IN APPRAISALS OF PATIENTS AND PHYSICIANS

The article presents the analysis of results of complex medical sociological survey of effectiveness and efficiency of medical care as main criteria of its quality and accessibility. The survey comprised 1,737 patients of various social groups of population of Moscow and 203 physicians of polyclinics. At self-rating of health, 13.2% of respondents determined it as unsatisfactory, 45.1% as satisfactory and 31.3% as good. In all population groups, the most called-for proved to be polyclinic institutions. The high level of satisfaction of patients with quality of emergency medical care was established in all groups. The quality of medical care in polyclinics and hospitals was assessed significantly lower. The most negatively assessed criteria turned out transition to fee-for-service forms of medical support and time limits of waiting for all modes of medical care, except emergency medical care and district therapist. The evaluation by physicians of their own activity and present problems is characterized by their unanimity in need of increasing of salary (100%) and increasing of typical sectoral standards of time of reception of a patient (96.5%), decreasing of intensity of work of medical personal (92.3%) and reduction of some forms of record cards with the purpose of releasing of time and attention of physician for a patient (88.4%). The disrespectful attitude of patients to medical personnel was mentioned by 77.3% of physicians which is a new phenomenon testifying loss of confidence in patient-physician relationship. The quality of implemented work was assessed equally and rather high by both young and experienced physicians. The positive and negative aspects of reorganization of health care were analyzed. Two alternatives of problems were established to focus plans of development of system of Moscow health care to support accessible and qualitative medical care of population.

The mission of the emergency medical services is to promote and support a system that provides timely, professional, and state-of-the art emergency medical care, including ambulance services, to anyone who is victim of a sudden injury or illness, at any time or location and at any phase of the emergency incident. These phases include lay people’s prevention and preparedness, occurrence of the problem, its detection, alarming of trained responders, help provided by bystanders and trained pre-hospital providers, transport to the appropriate hospital, and, if necessary, admission or transfer to a more appropriate hospital. In order to meet the goal outlined, emergency medical services must work closely with local and state officials—fire and rescue departments, other ambulance providers, hospitals, and other agencies—to foster a smooth functioning network. The term emergency medical services evolved to reflect a change from a simple system of ambulances, providing only transportation, to a system in which actual medical care is given at the scene and during transport. Medical supervision and/or participation of emergency medicine physicians in the emergency medical services systems contribute to the quality of medical care. This emergency medical services network must be capable of responding instantly and reliably around the clock, with well-trained, well-equipped personnel linked, as needed, through a strong communication system. Research plays an important role in conserving resources and improving the delivery of health care. This chapter gives an overview of the different aspects of emergency medical services and calls for high-quality research in pre-hospital emergency care in a true partnership between cardiologists and emergency physicians.

Subject. Staff capacity of healthcare institutions of the region. Topic. More efficient use of staff capacity of healthcare institutions of the region. Purpose. Delineate the main trends in the formation of staff capacity of the industry and determine the degree of its influence on the efficiency of the system itself as a whole. Methodology. Methods of logical and comparative analysis of intergovernmental regulation practices in the format of interaction «department - healthcare institutions». Results. After analyzing the staffing policy of the Voronezh Region Health Department and the mortality rate in the region for 2020, we identified an inadequate staffing of health care institutions, which affects the improvement in the quality of medical services and the mortality rate. Application area. Sphere of staffing of the Voronezh Region. Conclusions. In 2020, healthcare institutions of the Russian Federation faced a serious threat to public health, namely the COVID-19 pandemic, which in turn determined the parameters of the roadmap for the development of the industry for the coming years and identified the main problem areas. There is traced the dependence of the decrease in the staffing level, depending on the distance of the MO from the regional center. The number of certified specialists, both doctors and paramedics, is at an insufficiently high level, which ultimately affects the quality of medical care and the mortality rate in the Voronezh region.  Emergency medical workers can be at ributed to specialists of extreme prof le of activity, as their work is associated with a variety of stress factors. 44 paramedics of the Arkhangelsk region (mean age 38.1±10.8 years, M±σ) were examined in order to identify the features of occupational stress in the activities of emergency medical personnel (NSR). T e following methods we used: scale of organizational stress, Mak-Lin, the questionnaire «At itude to work and professional burn-out» Vinokur V.A., test Ch. Spielberger, coping test of R. Lazarus and S. Volkman, a method of studying the risk appetite Shmelev A.G., projective test «Nonexistent animal», a method of assessing the psychological atmosphere in the team. It is established that during the performance of professional duties more than 2/3 of paramedics emergency medical care are of en faced with stressful situations; every third employee is threatened with life or danger of injury, injury in the process. More than half of the respondents have a high level of professional stress and burnout, 40.5% — use non-adaptive coping strategies in the f ght against intractable situations, and 1/5 — there is a high rate of situational anxiety and aggressiveness. T e directions of prevention of occupational stress in paramedics ambulance.

Currently, among the priority tasks of the health care system is to improve the quality of medical care provided to the population and the accessibility of services. Incomplete staffing is a serious slowdown to the provision of quality care. The shortage of medical personnel is widespread in medicine, which is especially pronounced in primary health care, and endocrinology is no exception. The shortage of personnel is directly related to the problems of accessibility and quality of medical care. Reorganizing the system of professional training, retraining and continuing medical education in a volume that allows eliminating quantitative and qualitative defects in providing care for people with endocrine diseases, as well as analyzing staffing needs and planning based on the real needs of the population, will help to achieve the solution to the set tasks of the healthcare system.

The article deals with the question of quality assurance of emergency medical care. It is of vital importance in terms of reforming the system of healthcare in the Russian Federation, as the quality and availability of various types of medical services are priority indicators of reform. Quality ensuring of emergency medical care is the most important socially significant task of health care, which lies in maintaining the health of people in extreme conditions and in meeting the challenges of mortality reduction. This is resulted in increased attention to research in this area. Quality ensuring of emergency medical services actualizes the task of improving of its assessment methodology. The methodological problems of services quality assessment in the system of emergency medical care limiting applied research are the subject of the study. The structure of emergency medical care is specified, the separation of concepts of "medical service" and "medical care" of services of emergency medical care is accomplished, and the concept of "medical care" is defined in the article. Based on the results of market research of patients’ satisfaction with quality of medical care in emergency hospital of Krasnoyarsk a structural model of services of emergency medical care has been created. This model reflects the patients’ understanding that the quality of service of emergency medical care is an integrated category, combining the quality of the result of medical services and quality of care. Content analysis of the definition of basic concepts in the field of quality of health care has allowed to formulate the concept of "service quality of emergency medical care”. Its difference is that the definition includes goal-setting of quality of emergency medical care as ensuring the effective medical care, as well as performance indicators of emergency medical care are formulated. The proposed concept and performance indicators of emergency medical care stimulate new directions for research in this area.

The aim of the study. To evaluate satisfaction with the quality of medical care at the outpatient and inpatient stages on the example of the rural population of the Saratov region. Material and methods. The study involved 102 respondents (34 people from each municipal district) of both sexes aged 20 to 70 years, the average age was 46.3 [22.1; 68.3]. The study was conducted using a voluntary patient questionnaire. Results. Respondents are more satisfied with the medical care provided by round-the-clock hospitals and emergency medical teams in 47.1% and 43.1% of cases, respectively, and the least confident with polyclinics (on average, 35.2%). Half of the respondents attribute the decline in the quality of medical care at the outpatient and inpatient stages simultaneously to several factors (long waiting for a specialist consultation, lack of necessary medical equipment, the inattentive attitude of medical personnel to patients). The respondents react to the decrease in the quality of medical care in the following way: 9 (8.8%) of respondents wrote complaints about the medical worker to the chief doctor, 57 (55.9%) - expressed their dissatisfaction with the medical staff, trying to solve the problem at the initial stages of its appearance, 36 (35.3%) - did not show active actions. Discussion. The low level of satisfaction of rural population with quality of medical care, primarily due to understaffing of medical personnel and inadequate funding of health institutions, lack of high-level professional experience young professionals, the subjective perception of patients respondents on how to provide medical assistance based on information obtained from Internet resources. Conclusions. On average, 41.8% of patients are satisfied with the quality of medical care in the Saratov region. Recommendations offered for solving problems that cause a decrease in the quality of medical care in this area.

In the last decade, the health care system has been suffering from an imbalance of medical personnel and low efficiency in the use of nursing personnel in practical health care, which significantly affects the quality of medical care. The article reveals the main goals and objectives of the activity of mid-level medical staff in the system of providing medical care to patients in a urological hospital. The need for nurses to comply with the formulated standards is a central link in the process of organizing their activities. The effectiveness of treatment and recovery of patients without reducing the further quality of their lives depends on timely measures for care, psychological assistance, tact and compliance with ethical standards by nurses. All these factors directly affect the prospects for the treatment and rehabilitation of patients with urological pathology, as well as the performance indicators of the urological hospital.

The article presents the data of international statistics and seven-year experience of the Rehabilitation Center for children born with very low and extremely low body weight, based on a large multidisciplinary children’s hospital. The authors pay particular attention to improvement of medical care of children born with very low and extremely low body weight.

At present, the quality and availability of medical care are the aspects to which close attention is paid in the system of organizing medical care. One of the ways to achieve a high level of quality and accessibility of medical care is building up human resources, i.e. the presence of highly qualified general practitioners, sub-specialties and middle medical personnel in sufficient numbers in a medical organization. Currently, the oncological service of Russia has been assigned tasks to fulfill the instructions from the State Program for the Development of Health Care. The results of the work should lead to a decrease in mortality and an increase in the quality of life of the population. To address the issue of reducing mortality from malignant neoplasms, in particular from prostate cancer, a three-level system of oncological care has been created on the territory of the Nizhny Novgorod Region, including 1 regional center, 2 interdistrict cancer centers, 88 primary oncology rooms and 96 examination rooms. The tasks set to reduce morbidity and mortality from oncological diseases can be realized only with close interaction of the oncological service with the primary health care sector, in which the prevention should be the priority direction of work.

One of the major modern advances in the organization and delivery of health care has been the introduction of multidisciplinary team management. This approach has reduced mortality levels in patients suffering with cancer and other complex multiorgan pathologies. Many centres of excellence and teaching hospitals have established multidisciplinary teams in order to streamline treatment pathways and optimize patient care. This article presents an overview of multidisciplinary teams, their history, their introduction into mainstream medical care and the issues resulting from their introduction to the treating organizations as well as clinicians.
