Self-reported weight gain during the COVID-19 shelter-at-home has raised concerns for weight increases as the pandemic continues. We aimed to investigate the relationship of psychological and health markers with energy balance-related behaviors during the pandemic-related extended home confinement. Ratings for stress, boredom, cravings, sleep, self-control, and beliefs about weight control were collected from 1,609 adults using a questionnaire between April 24th–May 4th, 2020, while COVID-19 associated shelter-in-place guidelines were instituted across the US. We calculated four energy balance behavior scores (physical activity risk index, unhealthy eating risk index, healthy eating risk index, sedentary behavior index), and conducted a latent profile analysis of the risk factors. We examined psychological and health correlates of these risk patterns. Boredom, cravings for sweet/savory foods, and high sleepiness ratings related to high risk of increasing unhealthy eating and sedentary behavior and decreasing physical activity and healthy eating. Having greater self-control, control over cravings, or positive mood was related to lowering all aspects of energy intake and energy expenditure risks. Although individuals in risk pattern classes showed similarity in physical activity and healthy/unhealthy eating habits, they exhibited different patterns of positive mood, craving control, food cravings, boredom, and self-control. Psychological and health variables may have a significant role to play in risk behaviors associated with weight gain during the COVID-19 related home confinement. Emerging behavioral patterns may be meaningful in developing targeted weight management interventions during the current pandemic.

Abstract Background: Electronic (eHealth) and mobile (mHealth) health interventions can provide a large coverage, and are promising tools to change health behavior (i.e. physical activity, sedentary behavior and healthy eating). However, the determinants of intervention effectiveness in primary prevention has not been explored yet. Therefore, the objectives of this umbrella review were to evaluate intervention effectiveness, to explore the impact of pre-defined determinants of effectiveness (i.e. theoretical foundations, behavior change techniques, social contexts or just-in-time adaptive interventions), and to provide recommendations for future research and practice in the field of primary prevention delivered via e/mHealth technology.Methods: PubMed, Scopus, Web of Science and the Cochrane Library were searched for systematic reviews and meta-analyses (reviews) published between January 1990 and May 2020. Reviews reporting on e/mHealth behavior change interventions in physical activity, sedentary behavior and/or healthy eating for healthy subjects (i.e. subjects without physical or physiological morbidities which would influence the realization of behaviors targeted by the respective interventions) were included if they also investigated respective theoretical foundations, behavior change techniques, social contexts or just-in-time adaptive interventions. Included studies were ranked concerning their methodological quality and qualitatively synthesized.Results: The systematic search reveled eleven systematic reviews and meta-analyses of moderate quality. The majority of original research studies within the reviews found e/mHealth interventions to be effective, but the results showed a high heterogeneity concerning assessment methods and outcomes, making them difficult to compare. Whereas theoretical foundation and behavior change techniques were suggested to be potential positive determinants of effective interventions, the impact of social context remains unclear. None of the reviews included just-in-time adaptive interventions.Conclusion: Findings of this umbrella review support the use of e/mHealth to enhance physical activity and healthy eating and reduce sedentary behavior. The general lack of precise reporting and comparison of confounding variables in reviews and original research studies as well as the limited number of reviews for each health behavior constrains the generalization and interpretation of results. Further research is needed on study-level to investigate effects of versatile determinants of e/mHealth efficiency, using a theoretical foundation and additionally explore the impact of social contexts and more sophisticated approaches like just-in-time adaptive interventions.Trial registration: The protocol for this umbrella review was a priori registered with PROSPERO: CRD42020147902.

Abstract Background Although physical activity (PA) in pregnancy benefits most women, not much is known about pregnancy-related changes in PA and its association with gestational diabetes mellitus (GDM) risk. The aim of this study was to identify the trajectory of PA during pregnancy and possible associations with the risk of GDM. Methods This was a prospective cohort study of 452 pregnant women recruited from 3 health clinics in a southern state of Peninsular Malaysia. PA levels at the first, second, and third trimester were assessed using the Pregnancy Physical Activity Questionnaire. GDM was diagnosed at 24–28 weeks of gestation following the Ministry of Health Malaysia criteria. Group-based trajectory modeling was used to identify PA trajectories. Three multivariate logistic models were used to estimate the odds of trajectory group membership and GDM. Results Two distinct PA trajectories were identified: low PA levels in all intensity of PA and sedentary behavior (Group 1: 61.1%, n = 276) and high PA levels in all intensity of PA as well as sedentary behavior (Group 2: 38.9%, n = 176). Moderate and high intensity PA decreased over the course of pregnancy in both groups. Women in group 2 had significantly higher risk of GDM in two of the estimated logistic models. In all models, significant associations between PA trajectories and GDM were only observed among women with excessive gestational weight gain in the second trimester. Conclusions Women with high sedentary behavior were significantly at higher risk of GDM despite high PA levels by intensity and this association was significant only among women with excessive GWG in the second trimester. Participation in high sedentary behavior may outweigh the benefit of engaging in high PA to mitigate the risk of GDM.

AbstractIntroductionHealthy dietary and physical activity behaviours during and after pregnancy are important for optimal maternal health. Unhealthy lifestyle habits promote excessive weight gain during pregnancy or weight retention in the postpartum period, which increase the risk of obesity. The aim of this study was to explore dietary habits and associated lifestyle patterns and barriers to healthy eating in UK postpartum women.MethodsCross-sectional study of 228 females (56.1% were 25–34 years old) with a mean postpartum period of 5.73 ± 3.31 months. Participants completed an online survey exploring eating habits, weight status, sleep duration, breastfeeding, nutrition knowledge, physical activity, provision of advice and barriers to healthy eating during the postpartum period. A sub-sample of 34 women (50% of the sample were between 35–44 years old) completed an optional online dietary intake assessment using a multiple-pass 24-hour recall.ResultsIn total, 73.7% were not meeting the five a day fruit and vegetable UK recommendations, 40.4% of women were skipping breakfast and 44.7% were skipping lunch every day. Average weight gain from pre-pregnancy to postpartum was 5.56 ± 4.61 kg (range = 0.8 to 25 kg) with only 2.2% of women meeting current UK physical activity guidelines during the postpartum period. Consumption of high calorie snacks and meal skipping were significantly higher during the postpartum period compared to pre-pregnancy (p < 0.01). Women who were breastfeeding had significantly lower body mass index than those who were bottle feeding (p < 0.05). Fatigue, lack of time and feeling stressed had the most impact on women's ability to eat healthily. Barriers to consume a balanced healthy diet were significantly greater for women with low combined household income and those having three or more children (p < 0.05). Poor nutrition knowledge was significantly associated with increased meal skipping (p < 0.05). The sub-sample dietary analysis (N = 34) showed that women were not meeting the UK Dietary Reference Values for energy, fibre, iron, and vitamin D intakes. Those breastfeeding were not meeting requirements for calcium and zinc.DiscussionTo our knowledge, this is the first study to explore dietary habits and lifestyle patterns in UK postpartum women. Health care professionals should support women to adopt lifestyle behaviours following childbirth, with a greater focus on those likely to be experiencing more barriers. Advice given to women during the postpartum period needs to be tailored to potential differences in sociodemographic characteristics, pre-pregnancy health status and baseline nutrition knowledge and target multiple dietary and lifestyle behaviours.

Unhealthy lifestyle behaviors may set youths on the path to developing non-communicable diseases. Singapore schooling youths (n = 235, 67.2% female, age 17.5 (mean) ± 1.0 (SD) years) were recruited to examine the dietary, alcohol use, physical activity, and sedentary behaviors of the Singapore schooling youths. Demographic, anthropometric, and sedentary hours information were collected using structured questionnaires. Physical activity levels and dietary intakes, including alcohol use, were assessed using the validated International Physical Activity Questionnaire - long-form and localized food frequency questionnaire, respectively. The surveyed youths consumed significantly larger daily amounts of energy, sugar, saturated fat, and sodium than the levels recommended by the Singapore Health Promotion Board. They did not achieve the daily amounts of dietary fiber, fruits, and vegetables, and derived unhealthy proportions of daily energy from sugar and saturated fat. They did not exceed the recommended maximum daily alcohol allowance. Close to all the youths (91.2%) failed to adhere to more than five of the nine recommended healthy eating habits. Majority of these youths (73.4%) partook less than 600 MET-minute/ week. A significant number of the youths (38.1%) spent more than eight sedentary hours daily. Physical activity correlated significantly with sedentary behavior levels, but not the dietary scores. Most surveyed youths engaged in low physical activity and high sedentary behavior, and did not comply with most of the healthy eating guidelines proposed by the Singapore Health Promotion Board, specifically on daily energy, sugar, dietary fibers, saturated fat, sodium, and fruit and vegetable intakes.

Abstract The COVID-19 outbreak holds public health concerns. Staying at home, increases sedentary behavior, with unintended adverse outcomes. Since organized recreation and sports facilities were closed, we aimed to study how the crisis of closure affected exercise habits and weight gain among the trainee population in Israel, assess the frequency of activity and age-adjusted weight gain, measure and ability to embrace digital media, as an alternative training frameworkA cross-sectional survey consisted of a multiple-choice questionnaire that was obtained using a web-based survey application. 1,202 trainees who exercise on a regular basis, anonymously answered the questionnaire sent by their coaches regarding their activity and weight gain during lockdown times. Results confirmed that 70% of Israelis trained less than their usual routine, 60% used digital media for training, 55% gained weight. Half of the respondents gained more than 2 kg, with an average increase of 1.2 kg. However, those who exhibited a higher physical activity level gained less weight. Using digital media for training was associated with higher physical activity levels. The aged population was less likely to use digital media.Since increased sedentary behavior could increase the risk for potential worsening of health conditions, health agencies should look for strategies, especially in the aged population, including digital media to promote physical activity and subsequently, preventing the increased burden of future comorbidities worsening by a sedentary lifestyle.

Background: Individuals with low level of trait self-control exhibit several negative behaviors such as violence, addictive behaviors (drug addiction, technological addictions), suicide, unhealthy eating, and criminal behaviors. In contrast, high level of self-control thought to be associated with positive psychosocial and physical health related outcomes. Interventions contribute to boost self-control seems essential for individual with risk of low trait self-control. Objectives: The purpose of this study is to systematically evaluate the relationship between physical activity and trait self-control. Methods: For this purpose, full-text articles were searched in research databases including Psych INFO, PubMed, Medline, The Cochrane Library, Wiley Library, Taylor & Francis, Science Direct, Springer, ERIC, JSTOR, Google scholar, and Sage Journals.  Following screening for the study selection criteria and assessment of risk of bias, twenty one studies (13cross-sectional, 5 longitudinal and 3 interventions) with low or moderate risks of bias were finally selected for qualitative synthesis. Results: Findings from all of the included studies consistently showed that there was a significantly positive relationship between physical activity and trait self-control.It is suggested that increasing self-efficacy, decreasing aggression, and improvement in positive and negative emotional states might be underlying mechanisms working for beneficial effects of physical activity on trait self-control. Conclusion: These suggest that physical activity and exercise may serve as a potential means to enhance trait self-control. Practical implementation and recommendations for policy makers are put forward in this connection.

PURPOSE: Weight gain is a common problem among breast cancer patients who receive adjuvant chemotherapy (CT). We undertook a study to determine the causes of this energy imbalance. PATIENTS AND METHODS: Factors related to energy balance were assessed at baseline (within 3 weeks of diagnosis) and throughout 1 year postdiagnosis among 53 premenopausal women with operable breast carcinoma. Thirty-six patients received CT and 17 received only localized treatment (LT). Measures included body composition (dual energy x-ray absorptiometry), resting energy expenditure (REE; indirect calorimetry), dietary intake (2-day dietary recalls and food frequency questionnaires) and physical activity (physical activity records). RESULTS: Mean weight gain in the LT patients was 1.0 kg versus 2.1 kg in the CT group (P = .02). No significant differences between groups in trend over time were observed for REE and energy intake; however, a significant difference was noted for physical activity (P = .01). Several differences between groups in 1-year change scores were detected. The mean change (± SE) in LT versus CT groups and P values for uncontrolled/controlled (age, race, radiation therapy, baseline body mass index, and end point under consideration) analysis are as follows: percentage of body fat (−0.1 ± 0.4 v +2.2 ± 0.6%; P = .001/0.04); fat mass (+0.1 ± 0.3 v +2.3 ± 0.7 kg; P = .002/0.04); lean body mass (+0.8 ± 0.2 v −0.4 ± 0.3 kg; P = .02/0.30); and leg lean mass (+0.5 ± 0.1 v −0.2 ± 0.1 kg; P = .01/0.11). CONCLUSION: These data do not support overeating as a cause of weight gain among breast cancer patients who receive CT. The data suggest, however, that CT-induced weight gain is distinctive and indicative of sarcopenic obesity (weight gain in the presence of lean tissue loss or absence of lean tissue gain). The development of sarcopenic obesity with evidence of reduced physical activity supports the need for interventions focused on exercise, especially resistance training in the lower body, to prevent weight gain.

AbstractObjectiveTo identify family- and school-based correlates of specific energy balance-related behaviours (physical activity, sedentary behaviour, breakfast consumption, soft drink consumption) among 10–12-year-olds, using the EnRG framework (Environmental Research framework for weight Gain prevention).DesignA literature review to identify observational studies exploring at least one family- or school-based correlate of the specific behaviours, resulting in seventy-six articles.SettingEighteen studies were conducted in Europe, forty-one studies in North America and seventeen studies in Australasia.SubjectsHealthy children aged 10–12 years.ResultsParental and maternal physical activity, doing physical activities with parents and parental logistic support were identified as the most important, positive correlates of physical activity. Parental rules was the most important correlate of sedentary behaviour and was inversely related to it. School socio-economic status was positively related to physical activity and inversely related to sedentary behaviour. The available studies suggested a positive relationship between soft drink availability at home and consumption. Soft drink availability and consumption at school were the most important school-based correlates of soft drink consumption. A permissive parenting style was related to more soft drink consumption and less breakfast consumption.ConclusionsAn important role has been awarded to parents, suggesting parents should be involved in obesity prevention programmes. Despite the opportunities a school can offer, little research has been done to identify school-environmental correlates of energy balance-related behaviours in this age group. Obesity prevention programmes can focus on the most important correlates to maximize the effectiveness of the programme. Future research should aim at longitudinal studies.
