When a study population is relatively healthy, such as an occupational population, epidemiological studies are likely to underestimate risk. We used a case study on the cancer risk of workers with exposure to acid mists, a well-documented carcinogen, to demonstrate that using proportional mortality ratios (PMRs) is more appropriate than mortality ratios in assessing risk in terms of mortality. The study included 10,229 employees of a telecommunication company who worked in buildings with battery rooms. In these buildings, the battery rooms had the highest levels of sulfuric acid in the air (geometric mean = 10.7 μg/m3). With the general population in Taiwan as a reference, a decreased standardized mortality ratio (0.42, p < 0.01) from all causes combined, between 1 January 1985 and 31 December 1996, was observed, indicating a healthy worker effect. When we reanalyzed the data using standardized PMR, elevated risks were observed for all cancers combined (1.46, p = 0.01) and cancers of the digestive organs and peritoneum (1.61, p = 0.02), especially stomach cancer (2.94, p = 0.01). The results showed that PMR can detect increases in mortality when a study population is generally healthier than the comparison population and call for further studies on the possible carcinogenic effects of low-level acid mist exposures on the stomach.

We provide an analysis of COVID-19 mortality data to assess the potential magnitude of COVID-19 among prison residents. Data were pooled from Covid Prison Project and multiple publicly available national and state level sources. Data analyses consisted of standard epidemiologic and demographic estimates. A single case study was included to generate a more in-depth and multi-faceted understanding of COVID-19 mortality in prisons. The increase in crude COVID-19 mortality rates for the prison population has outpaced the rates for the general population. People in prison experienced a significantly higher mortality burden compared to the general population (standardized mortality ratio (SMR) = 2.75; 95% confidence interval = 2.54, 2.96). For a handful of states (n = 5), these disparities were more extreme, with SMRs ranging from 5.55 to 10.56. Four states reported COVID-19 related death counts that are more than 50% of expected deaths from all-causes in a calendar year. The case study suggested there was also variation in mortality among units within prison systems, with geriatric facilities potentially at highest risk. Understanding the dynamic trends in COVID-19 mortality in prisons as they move in and out of hotspot status is critical.

Malignant mesothelioma is one of the appropriate indicators for assessing the carcinogenic effects of asbestos. This study compared the risk ratio of mesothelioma according to the industry in the worker cohort. A cohort was constructed using the Korean employment insurance system during 1995–2017, enrolling 13,285,895 men and 10,452,705 women. The standardized mortality ratio (SMR) and standardized incidence ratio (SIR) were calculated using the indirect standardization method. There were 641 malignant mesotheliomas that occurred; the SIR was significantly higher than the general population (men 1.36, 95% confidence interval(CI) 1.24–1.48, women 1.44, 95% CI: 1.23–1.7). More than half (52.8%) of malignant mesothelioma cases occurred in the manufacturing (n = 240, 38.6%, SIR: men, 1.72, 95% CI: 1.37–2.15, women, 3.31, 95% CI: 1.71–5.79) and construction industries (n = 88, 14.2%, SIR: men, 1.54 95% CI: 1.33–1.78, women, 1.62 95% CI: 1.25–2.11). The accommodation and food service (men, 2.56 95% CI: 1.28–4.58, women 1.35, 95% CI: 0.65–2.48) and real estate (men 1.34, 95% CI: 0.98–1.83, women 1.95, 95% CI: 0.78–4.02) also showed a high SIR, indicating the risk of asbestos-containing materials in old buildings. The incidence of malignant mesothelioma is likely to increase in the future, given the long latency of this disease. Moreover, long-term follow-up studies will be needed.

This study was conducted to investigate the possible long-term health effects, in particular carcinogenic effects, of occupational exposure to the organochlorine insecticides dieldrin and aldrin. We updated an earlier cohort mortality study of 570 employees involved in the production of these insecticides. All of the employees had worked in the production plants between 1 January 1954 and 1 January 1970 and were followed for cause-specific mortality until 1 January 2001. Based on dieldrin levels in blood samples taken during the exposure period, available for 343 workers, individual estimates of the total intake of dieldrin were estimated for all individual subjects in the cohort. The estimated total intake ranged from 11 to 7755 mg of dieldrin, with an average of 737 mg. One hundred and seventy-one workers had died before 1 January 2001, compared with an expected number of 226.6, giving a standardized mortality ratio (SM R) of 75.6 [95% confidence interval (CI): 64.6-87.7]. This deficit in total mortality was mainly attributable to a deficit in cardiovascular disease mortality, but cancer mortality was also lower than expected. The observed number of deaths from rectal cancer was significantly higher than expected (SM R 3/4-300.0; 95% CI: 109.5-649.3), but was most pronounced in the low-intake subgroup and appears to be unrelated to exposure to dieldrin and aldrin. This study reinforces the earlier findings that occupational exposure of workers to significant amounts of dieldrin and aldrin has not led to a higher cancer mortality than would be found in an unexposed population.

Fumonisins are mycotoxins that frequently contaminate maize, a dietary staple in Burkina Faso. Fumonisins have been linked with both the incidence of oesophageal cancer, and to neural tube defects. However, epidemiological studies of these associations are hampered by lack of validated exposure biomarkers. One candidate biomarker is the sphinganine/sphingosine (Sa/So) ratio in biological samples. Twenty husband-wife pairs aged 20-40 were recruited from Dingasso-1, western Burkina Faso. Plate ready food was sampled over three consecutive days (day one to three) for fumonisin B1 and B2, to provide a measure of total fumonisin intake. The Sa/So ratio was determined in (1) first morning urines taken on days two to four, (2) buccal cells and (3) serum on days one and four only. Fumonisin intake was moderate (mean 0.81 µg/kg bw/day (range 0.01-2.40 µg/kg bw/day) compared to exposures reported in China and South Africa. For each person the mean fumonisin intake over three days was compared to the mean Sa, So and Sa/So ratio in urine, buccal cells and serum. A modest positive trend between mean fumonisin intake and mean serum Sa/So was observed (P=0.067). When individuals were dichotomised based on the median fumonisin intake (<0.75 µg/kg bw/day), the serum Sa/So ratio was moderately higher (P=0.044) in the high intake group (geometric mean 0.64; 95% CI: 0.54, 0.75) compared to the low intake group (0.49; 95% CI: 0.41, 0.59). Neither urinary nor buccal cell Sa/So ratio was associated with mean fumonisin intake. Neither mean individual Sa nor So level for urine, buccal cells or urine was associated with mean individual fumonisin intake. The study population was exposed to moderate levels of fumonisin with limited evidence of altered Sa/So ratio. Further work on serum Sa/So ratio is merited in comparison with other biomarker approaches such as urinary fumonisin B1 or sphingolipid 1-phosphate metabolites.

Epilepsy is a chronic neurological disease affecting both sexes and all ages with worldwide distribution and peaks in children and the elderly. The pooled incidence of epilepsy is 61.4/100,000/year, with peaks in LMICs; the point prevalence is 6.4/1000 and the lifetime prevalence 7.6/1000. A documented aetiology is present in about one-half of cases. The overall prognosis is mostly favourable, with 55–68% of cases achieving prolonged seizure remission. Mortality ranges from 1 to 8 per 100,000/year, with a standardized mortality ratio of 1.3–6.0. Since 1990, a significant decrease of persisting disability and premature mortality has been observed, while age-standardized prevalence remained stable. Despite the decrease in the burden, epilepsy is still an important cause of disability and mortality. Future epidemiological studies will be affected by the new definition (now including people with a single seizure at high risk of recurrence) and the classification of seizures and epilepsy.

Dalam penelitian ini menggunkan suatu pen dekatan dengan analisis SWOT yang merupakan langkah awal dari suatu perencanaan strategi pengembangan yang dimulai dengan identifikasi masalah, tujuan organisasi sampai pada menimbang kekuatan dan kelemahan sendiri serta peluang dan ancaman dari luar dan juga melakukan beberapa langkah penting yang menunjang pemasaran atau pengembangan. Jenis penelitian ini adalah penelitian studi kasus dengan menggunakan rancangan penelitian deskriptif kualitatif dan Populasi dalam penelitian ini populasinya adalah pasien rawatinap dan pasien rawat jalan sebanyak 200 orang responden Sampel merupakan sebagian atau wakil populasi yang diteliti. Dalam penelitian ini mengunakan sampling pertimbangan (Judgement Sampling) Dari hasil pendekatan dengan analisis SWOT perlunya pelaksanaan atau realisasi dari struktur organisasi yang menempatkan farmasis dalam farmasi klinik, peningkatan kualitas dan kuantitas sumber daya manusia, serta perlunya penambahan fasilitas berhubungan dengan IPTEK untuk kegiatan pelayanan farmasi baik secara manajerial maupun ke arah farmasi klinik. This research is a case study using qualitative descriptive study design and population in this study population was rawatinap patients and outpatients as many as 200 people respondent sample is partially or representative of the population studied. In 2020, the IOC released a consensus statement that provides overall guidelines for the recording and reporting of epidemiological data on injury and illness in sport. Some aspects of this statement need to be further specified on a sport-by-sport basis. To extend the IOC consensus statement on methods for recording and reporting of epidemiological data on injury and illness in sports and to meet the sport-specific requirements of all cycling disciplines regulated by the Union Cycliste Internationale (UCI). A panel of 20 experts, all with experience in cycling or cycling medicine, participated in the drafting of this cycling-specific extension of the IOC consensus statement. In preparation, panel members were sent the IOC consensus statement, the first draft of this manuscript and a list of topics to be discussed. The expert panel met in July 2020 for a 1-day video conference to discuss the manuscript and specific topics. The final manuscript was developed in an iterative process involving all panel members. This paper extends the IOC consensus statement to provide cycling-specific recommendations on health problem definitions, mode of onset, injury mechanisms and circumstances, diagnosis classifications, exposure, study population characteristics and data collection methods. Recommendations apply to all UCI cycling disciplines, for both able-bodied cyclists and para-cyclists. The recommendations presented in this consensus statement will improve the consistency and accuracy of future epidemiological studies of injury and illness in cycling.

Abstract Introduction Exercise is widely accepted to improve health, reducing the risk of premature mortality, cardiovascular disease (CVD) and cancer. However, several epidemiological studies suggest that the exercise-longevity relationship may be ‘J’ shaped; with elite athlete’s likely training above these intensity and volume thresholds. Therefore, the aim of this meta-analysis was to examine this relationship in former elite athletes. Methods 38,047 English language articles were retrieved from Web of Science, PubMed and SportDiscus databases published after 1970, of which 44 and 24 were included in the systematic review and meta-analysis, respectively. Athletes were split into three groups depending on primary sport: Endurance (END), Mixed/Team, or power (POW). Standard mortality ratio’s (SMR) and standard proportionate mortality ratio (SPMR) were obtained, or calculated, and combined for the meta-analysis. Results Athletes lived significantly longer than the general population (male SMR 0.69 [95% CI 0.61–0.78]; female SMR 0.51 [95% CI 0.40–0.65]; both p < 0.01). There was no survival benefit for male POW athletes compared to the general population (SMR 1.04 [95% CI 0.91–1.12]). Although male athlete’s CVD (SMR 0.73 [95% CI 0.62–0.85]) and cancer mortality (SMR 0.75 [95% CI 0.63–0.89]), were significantly reduced compared to the general population, there was no risk-reduction for POW athletes CVD mortality (SMR 1.10 [0.86–1.40]) or END athletes cancer mortality (SMR 0.73 [0.50–1.07]). There was insufficient data to calculate female sport-specific SMR’s. Discussion Overall, athletes live longer and have a reduced incidence of both CVD and cancer mortality compared to the general population, refuting the ‘J’ shape hypothesis. However, different health risks may be apparent according to sports classification, and between sexes, warranting further investigation.
