Fumonisin exposure and the sphinganine/sphingosine ratio in urine, serum and buccal cells in adults from Burkina Faso, West Africa

Fumonisins are mycotoxins that frequently contaminate maize, a dietary staple in Burkina Faso. Fumonisins have been linked with both the incidence of oesophageal cancer, and to neural tube defects. However, epidemiological studies of these associations are hampered by lack of validated exposure biomarkers. One candidate biomarker is the sphinganine/sphingosine (Sa/So) ratio in biological samples. Twenty husband-wife pairs aged 20-40 were recruited from Dingasso-1, western Burkina Faso. Plate ready food was sampled over three consecutive days (day one to three) for fumonisin B1 and B2, to provide a measure of total fumonisin intake. The Sa/So ratio was determined in (1) first morning urines taken on days two to four, (2) buccal cells and (3) serum on days one and four only. Fumonisin intake was moderate (mean 0.81 µg/kg bw/day (range 0.01-2.40 µg/kg bw/day) compared to exposures reported in China and South Africa. For each person the mean fumonisin intake over three days was compared to the mean Sa, So and Sa/So ratio in urine, buccal cells and serum. A modest positive trend between mean fumonisin intake and mean serum Sa/So was observed (P=0.067). When individuals were dichotomised based on the median fumonisin intake (<0.75 µg/kg bw/day), the serum Sa/So ratio was moderately higher (P=0.044) in the high intake group (geometric mean 0.64; 95% CI: 0.54, 0.75) compared to the low intake group (0.49; 95% CI: 0.41, 0.59). Neither urinary nor buccal cell Sa/So ratio was associated with mean fumonisin intake. Neither mean individual Sa nor So level for urine, buccal cells or urine was associated with mean individual fumonisin intake. The study population was exposed to moderate levels of fumonisin with limited evidence of altered Sa/So ratio. Further work on serum Sa/So ratio is merited in comparison with other biomarker approaches such as urinary fumonisin B1 or sphingolipid 1-phosphate metabolites.

When a study population is relatively healthy, such as an occupational population, epidemiological studies are likely to underestimate risk. We used a case study on the cancer risk of workers with exposure to acid mists, a well-documented carcinogen, to demonstrate that using proportional mortality ratios (PMRs) is more appropriate than mortality ratios in assessing risk in terms of mortality. The study included 10,229 employees of a telecommunication company who worked in buildings with battery rooms. In these buildings, the battery rooms had the highest levels of sulfuric acid in the air (geometric mean = 10.7 μg/m3). With the general population in Taiwan as a reference, a decreased standardized mortality ratio (0.42, p < 0.01) from all causes combined, between 1 January 1985 and 31 December 1996, was observed, indicating a healthy worker effect. When we reanalyzed the data using standardized PMR, elevated risks were observed for all cancers combined (1.46, p = 0.01) and cancers of the digestive organs and peritoneum (1.61, p = 0.02), especially stomach cancer (2.94, p = 0.01). The results showed that PMR can detect increases in mortality when a study population is generally healthier than the comparison population and call for further studies on the possible carcinogenic effects of low-level acid mist exposures on the stomach.

SummaryFor assessment of differential renal function (PF) by means of static renal scintigraphy with Tc-99m-dimer-captosuccinic acid (DMSA) the calculation of the geometric mean of counts from the anterior and posterior view is recommended. Aim of this retrospective study was to find out, if the anterior view is necessary to receive an accurate differential renal function by calculating the geometric mean compared to calculating PF using the counts of the posterior view only. Methods: 164 DMSA-scans of 151 children (86 f, 65 m) aged 16 d to 16 a (4.7 ± 3.9 a) were reviewed. The scans were performed using a dual head gamma camera (Picker Prism 2000 XP, low energy ultra high resolution collimator, matrix 256 x 256,300 kcts/view, Zoom: 1.6-2.0). Background corrected values from both kidneys anterior and posterior were obtained. Using region of interest technique PF was calculated using the counts of the dorsal view and compared with the calculated geometric mean [SQR(Ctsdors x Ctsventr]. Results: The differential function of the right kidney was significantly less when compared to the calculation of the geometric mean (p<0.01). The mean difference between the PFgeom and the PFdors was 1.5 ± 1.4%. A difference > 5% (5.0-9.5%) was obtained in only 6/164 scans (3.7%). Three of 6 patients presented with an underestimated PFdors due to dystopic kidneys on the left side in 2 patients and on the right side in one patient. The other 3 patients with a difference >5% did not show any renal abnormality. Conclusion: The calculation of the PF from the posterior view only will give an underestimated value of the right kidney compared to the calculation of the geometric mean. This effect is not relevant for the calculation of the differntial renal function in orthotopic kidneys, so that in these cases the anterior view is not necesssary. However, geometric mean calculation to obtain reliable values for differential renal function should be applied in cases with an obvious anatomical abnormality.

In 2020, the IOC released a consensus statement that provides overall guidelines for the recording and reporting of epidemiological data on injury and illness in sport. Some aspects of this statement need to be further specified on a sport-by-sport basis. To extend the IOC consensus statement on methods for recording and reporting of epidemiological data on injury and illness in sports and to meet the sport-specific requirements of all cycling disciplines regulated by the Union Cycliste Internationale (UCI). A panel of 20 experts, all with experience in cycling or cycling medicine, participated in the drafting of this cycling-specific extension of the IOC consensus statement. In preparation, panel members were sent the IOC consensus statement, the first draft of this manuscript and a list of topics to be discussed. The expert panel met in July 2020 for a 1-day video conference to discuss the manuscript and specific topics. The final manuscript was developed in an iterative process involving all panel members. This paper extends the IOC consensus statement to provide cycling-specific recommendations on health problem definitions, mode of onset, injury mechanisms and circumstances, diagnosis classifications, exposure, study population characteristics and data collection methods. Recommendations apply to all UCI cycling disciplines, for both able-bodied cyclists and para-cyclists. The recommendations presented in this consensus statement will improve the consistency and accuracy of future epidemiological studies of injury and illness in cycling.

Abstract Background Nasal obstruction is a significant medical problem. This study aimed to examine the effect of nasal obstruction and nasal packing on arterial blood gases and pulmonary function indices, and the impact of the elimination of nasal obstruction on preoperative values. Results The mean age of the study population was 26.6 ± 10.1 years, males represented 50.8%. Spirometric indices showed statistically significant improvement (preoperative forced expiratory volume in 1st second 66.9 ± 13.9 vs 79.6 ± 14.9 postoperative and preoperative forced vital capacity 65.5 ± 12.7 vs 80.4 ± 13.8 postoperative). Oxygen saturation was significantly lower during nasal packing (95.6 ± 1.6 preoperative vs 94.7 ± 2.8 with nasal pack), and significant improvement (97.2 ± 1.4) was observed after removal of the nasal pack. Nasal obstruction scores significantly improved. Conclusion The results of this study indicate that either simple nasal obstruction or nasal packing may cause hypoxemia and abnormalities in lung function tests. Hypoxemia was more evident with nasal packing.

Background: Neurological disorders is becoming a growing concern both for developed and developing countries. Magnitude of the problem is increasing day by day. Among all neurological disorders, stroke is the leading cause of morbidity and mortality globally.Objectives: The purpose of the study was to see the trend of admission of patients with neurological diseases and to study the outcome of patients at referral neurology hospital in Bangladesh.Methodology: This retrospective chart review was conducted in the blue unit of the Department of Neurology at National Institute of Neurosciences and Hospital, Dhaka, Bangladesh from 1st January to 31st December 2016 for a period of one (01) year. All the admitted patients with both sexes were selected as study population. The outcome was observed among the study population.Result: A total number of 1044 patients were admitted during the study period. Majority of the patients were in the age group of the 41 to 50 years which was 417(39.9%) cases. Both male and female were in highest number in the month of May which was 63 and 48 cases respectively. The total death of the study population was 146(14.0%) cases. The mean length of hospital stay was 8.4±2.31 days.Conclusion: Middle aged male is the main bulk of the neurological patients, admitted in a referral neurology hospital in Bangladesh. A sample of 4973 kidneys from sheep stratified by age and shire of origin within the Agricultural Region of Western Australia, was analysed for cadmium (Cd) content during the period August 1989 to April 1991. The geometric mean Cd concentration in the kidney of hogget ewes was 0.9 mg/kg, in 4-tooth ewes 1.47 mg/kg and in adult ewes 3.34 mg/kg on a wet weight basis. The mean Cd concentrations of either ewe or hogget flocks from different Divisions of the Agricultural Region did not differ from each other. The annual increase in Cd concentration of kidney from hogget sheep was estimated to be 0-65 mg/kg. The rate of accumulation of Cd in kidney from cattle and sheep grazing the same properties was similar. Kidneys from a sample of 354 adult cattle from the Kimberley Region and 483 aged sheep from the Pastoral Region, both areas of unimproved rangelands, had geometric mean Cd concentrations of 0.15 mg/kg and 0-31 mg/kg respectively. A higher Cd concentration in flocks from the divisions adjacent to the Agricultural Region may have resulted from the establishment of some volunteer species of winter annual pastures in the rangeland.

Background. The role of matrix metalloproteinase-2 and -9 (MMP-2, MMP-9) in matrix degradation and metastasis has been described in various tumors. Their action is inhibited by their natural tissue inhibitor molecules TIMP-1 and -2.Methods. The study population consisted of 12 squamous cervical carcinoma patients and 27 healthy volunteer control patients. MMP-9, MMP-2-TIMP-2 complex, TIMP-1, and TIMP-2 were analyzed from serum samples using enzyme-linked immunoassay (ELISA).Results. The mean levels of serum TIMP-2 and of MMP-2-TIMP-2 complex were higher in healthy controls compared to patients with a malignant tumor. Serum TIMP-2 values decreased significantly from healthy controls (median 323 g/l, range 305–342 g/l) to malignant (median 136 g/l, range 120–151 g/l) squamous cervical carcinoma patients . Also, serum proMMP2-TIMP2 complex values decreased from control patients to squamous cervical carcinoma patients .Conclusion. This paper shows that the levels of circulating TIMP-2 and that of MMP-2-TIMP-2 complex are lower in squamous cervical carcinoma patients than in healthy women.

Purpose. The Lofoten Basin is one of the most energetic zones of the World Ocean characterized by high activity of mesoscale eddies. The study is aimed at analyzing different components of general energy in the basin, namely the mean kinetic and vortex kinetic energy calculated using the integral of the volume of available potential and kinetic energy of the Lofoten Vortex, as well as variability of these characteristics. Methods and Results. GLORYS12V1 reanalysis data for the period 2010–2018 were used. The mean kinetic energy and the eddy kinetic one were analyzed; and as for the Lofoten Vortex, its volume available potential and kinetic energy were studied. The mesoscale activity of eddies in winter is higher than in summer. Evolution of the available potential energy and kinetic energy of the Lofoten Vortex up to the 1000 m horizon was studied. It is shown that the vortex available potential energy exceeds the kinetic one by an order of magnitude, and there is a positive trend with the coefficient 0,23⋅1015 J/year. It was found that in the Lofoten Basin, the intermediate layer from 600 to 900 m made the largest contribution to the potential energy, whereas the 0–400 m layer – to kinetic energy. The conversion rates of the mean kinetic energy into the vortex kinetic one and the mean available potential energy into the vortex available potential one (barotropic and baroclinic instability) were analyzed. It is shown that the first type of transformation dominates in summer, while the second one is characterized by its increase in winter. Conclusions. The vertical profile shows that the kinetic energy of eddies in winter is higher than in summer. The available potential energy of a vortex is by an order of magnitude greater than the kinetic energy. An increase in the available potential energy is confirmed by a significant positive trend and by a decrease in the vortex Burger number. The graphs of the barotropic instability conversion rate demonstrate the multidirectional flows in the vortex zone with the dipole structure observed in a winter period, and the tripole one – in summer. The barotropic instability highest intensity is observed in summer. The baroclinic instability is characterized by intensification of the regime in winter that is associated with weakening of stratification in this period owing to winter convection.
