Background: Few studies have addressed patient preferences in emergent surgical decision making. Aim of the study: Analyzing patient preferences for hand trauma reconstruction to propose a decision-making model. Methods: A conjoint analysis survey was developed with Sawtooth Software. Three common flaps—i.e., a cross-finger flap (CFF), a dorsal metacarpal artery perforator flap (DMAPF), and an arterialized venous flap (AVF)—were listed as treatment alternatives. Five attributes corresponding to these flaps were recovery time, total procedure, postoperative care methods, postoperative scar condition, and complication rate. Utility and importance scores were generated from the software, and preference characteristics were evaluated using cluster analysis. Results: The survey was completed by 197 participants with hand trauma. Complication risk received the highest importance score (42.87%), followed by scar condition (21.55%). Cluster analysis classified the participants as “conservative,” “practical,” and “dual-concern.”. The dual-concern and conservative groups had more foreign laborers and highly educated participants, respectively, than the other groups. Most participants in the conservative and practical groups preferred DMAPF, whereas those in the dual-concern group favored CFF. Our proposed model consisted of shared decision-making and treatment recommendation pathways. Conclusion: Incorporating patient preferences into the decision-making model can strengthen patient-centered care. Further research on the applications of the proposed model is warranted.

This paper develops a decision-making model to assist the improvement of the carrying capacity of ship locks by combing fuzzy logic, the analytic hierarchy process (AHP) method, and the technique for order preference by similarity to an ideal solution (TOPSIS). A three-level hierarchical structure is constructed to identify the key factors influencing the carrying capacity of ship locks from the aspects of ship locks, vessels, environment, and administration. On this basis, a series of targeted strategies have been put forward to improve the carrying capacity of ship locks, and the TOPSIS method is applied to rank these strategies in terms of their performance. A case study of the five-stage dual-track ship lock of the Three Gorges Dam in China has been conducted to demonstrate the feasibility and rationality of the proposed model, and correlation analysis is conducted to verify the identified influencing factors in order to eliminate potential bias which may be generated from using AHP. The results obtained from the proposed methods are consistent with the real-life situation to a certain extent, indicating that the proposed method can provide a useful reference for improving the carrying capacity of ship locks.

Purpose The purpose of this paper is to propose a grey target decision model based on cobweb area in order to overcome the effect and influence from the extreme value of the index on the decision result. However, it does not take into account the impact of the correlation between indicators on the angle of the index, and produce a certain degree decision information distortion as a result of the equal angle between the indicators. In order to solve the above problems, a novel grey decision-making model based on cone volume is proposed. Design/methodology/approach In this paper, the model uses the whitening weight function to whiten the interval grey number, and the Delphi method and the maximal entropy method are exploited to integrate the weight of the index. On the basis of this, the center of the bull’s eye, the weight and the index value are constructed as the center circle, the radius, and the high cone, respectively. The scheme is selected by the volume of the cone, the decision is made according to the order relation, and the example is utilized to prove and analyze the validity of the proposed model. Findings The results show that the proposed model can well improve the traditional grey target decision-making model from the modeling object and modeling method. Practical implications The method exposed in the paper can be used to deal with the grey target decision-making problems which characteristics are multi-indexes, and the attribute values are interval grey numbers. Originality/value The paper succeeds in overcoming the disadvantages of grey target decision making based on the target center distance and the cobweb area.

The paper provides a multiple-experts Fuzzy-TOPSIS decision-making model for the selection among maintenance contractors based on the quality of tendering documents. The study introduces a set of selection criteria utilizing benefit and cost criteria from literature. The proposed model aggregates subjective linguistic assessments of multiple experts that express their opinions on the degree of importance of criteria and allows multiple decisionmakers to evaluate the compliance of contractors’ documents. For a case study, the model is applied to select among contractors tendering to maintain the heavy-duty cranes of an international steel company from literature. Several decision-making scenarios are investigated, and major changes in the final decision are observed. The changes in obtained results illustrate the need to better address uncertainties in rating and tendering an overqualified contractor at a higher cost.

Background: Treatment of a first-time anterior shoulder dislocation (FTASD) is sensitive to patient preferences. The operative or nonoperative management debate provides an excellent opportunity to learn how surgeons apply patient preferences in treatment decisions. Purpose: To determine how patient preferences (repeat dislocation risk, recovery difficulties, fear of surgery, treatment costs) and surgeon factors influence a surgeon’s treatment plan for FTASD. Study Design: Cross-sectional study. Methods: Eight clinical vignettes of hypothetical patients with FTASD (including age, sex, and activity level) were presented to members of the Magellan Society. A second set of matched vignettes with patient preferences and clinical variables were also presented. The vignettes represented scenarios in which evidence does not favor one treatment over another. Respondents were asked how they would manage each hypothetical case. Respondents also estimated the risk of redislocation for the nonoperative cases for comparison with the published rates. Finally, respondents completed a Likert-scale questionnaire to determine their perceptions on factors influencing their decisions. Results: A total of 103 orthopaedic surgeons completed the survey; 48% practiced in an academic hospital; 79% were in practice for 10 years or longer; and 75% had completed a sports medicine fellowship. Patient preferences were the single most important factor influencing treatment recommendation, with activity type and age also important. Just 62% of the surgeon estimates of the risk of redislocation were consistent with the published rates. The inclusion of patient preferences to clinical variables changed treatment recommendations in 62.5% of our hypothetical cases. Respondents rated patient treatment preference as the leading factor in their treatment decision making. Conclusion: Patient preferences were important when deciding the appropriate treatment for FTASD. Respondents were inconsistent when applying evidence in their decision making and estimates of recurrent instability. Decision support tools that deliver patient preferences and personalized evidence-based outcome estimates improve the quality of decision making at the point of care.

e15034 Background: Optimal management of CSI seminomatous (S) and nonseminomatous (NS) TC is an open question. Quality of life and patient preferences are the principal end-points for the best treatment choice. Multidisciplinary approach is required for CSI TC patients to allow an individualized treatment with a patient-centered approach and an active participation in decisions. The “shared decision making” (SDM) is the prevailing approach because patients together with clinicians arrive at a mutually agreed-on choice. We have a project concerning the involvement of patients with CSI TC in the SDM. Methods: We used some key points of SDM: define the problem; treatment options; discuss pros/cons; clarify patient preferences; clarify the patient’s understanding. Educational material are offered to patients. From 2005 to 2012, 32 patients with CSI TC were evaluated. For each patient some clinical and socio-demographic features, type of treatment chosen and some sequel related to treatment were analyzed. Results: Patients characteristics are reported in the table. 4 S patients chose surveillance, 10 chemotherapy and 6 radiotherapy. 2 NS patients chose surveillance, 9 chemotherapy. 4 relapsed: 1 S patient after 13 months from chemotherapy and 1 during the surveillance after 24 months. 2 NS patients relapsed after 12 and 18 months both during the surveillance. All relapsed patients received chemotherapy. Metabolic alterations were analyzed. 1 S patient treated with chemotherapy had a cardiovascular accident. Conclusions: We observed an high gap between surveillance and the active treatments: there is no best choice for every one so we should improve decision-making process and decision aids into the management of CSI patients with TC. Abstract A new agent-based model is proposed to support designers in assessing the evacuation capabilities of passenger ships and in improving ship safety. It comprises models for goal-driven decision-making, path planning, and movement. The goal-driven decision-making model determines an agent’s target by decomposing abstract goals into subgoals. The path-planning model plans the shortest path from the agent’s current position to its target. The movement model is a combination of social-force and steering models to control the agent in moving along its path. The utility of the proposed model is verified using 11 tests for passenger ships proposed by the Maritime Safety Committee of the International Maritime Organization.

The commercial banks (CBs) performance evaluating has been a necessary problem in currently integration trend and usually implemented by a committee of experts under criteria selected. Therefore, it is considered as a Multi - Criteria Decision - Making model (MCDM). Nowadays, there have been many researches proposing various standards and models to evaluate and rank CBs. But in Vietnam, the number of studies related to the Vietnamese banking evaluation model have still been limited. As a result, this study develops a multi-criteria decision model integrating Fuzzy Analytical Hierarchy Process (FAHP) and Fuzzy the Technique for Order of Preference by Similarity to Ideal Solution (FTOPSIS). The proposed model has evaluated and ranked five Vietnamese commercial banks including CTG, VCB, BIDV, TCB and MB. The paper revealed their ranks. Besides, the results of the research show that the Analytical Hierarchy Process (AHP) model is suitable for applying it to the process evaluating bank performance.

This paper attempts to propose a discretionary lane-changing decision-making model based on signalling game in the context of mixed traffic flow of autonomous and regular vehicles. The effects of the heterogeneity among different drivers and the endogeneity of same drivers in lane-changing behaviours, e.g., aggressive or conservative, are incorporated through the specification of different payoff functions under different scenarios. The model is calibrated and validated using the NGSIM dataset with a bilevel calibration framework, including two kinds of methods, genetic algorithm and perfect Bayesian equilibrium. Comparative results based on simulation show that the signalling game-based model outperforms the traditional space-based lane-changing model in the sense that the proposed model yields relatively stable reciprocal of time to collision and higher success rate of lane-changing under different traffic densities. Finally, a sensitivity analysis is performed to test the robustness of the proposed model, which indicates that the signalling game-based model is stable to the varying ratios of driver type.
