Dental rehabilitation for periodontal and oral mucosa diseases in type 2 diabetes

Relevance. Medical and dental personnel suffering from type 2 diabetes mellitus are susceptible to changes in tissues, periodontal, oral mucosa, and also make oral hygiene, which makes treatment of major dental diseases ineffective, as well as worsening of ongoing periodontal and oral mucosa diseases.Purpose. Study the condition of periodontal tissues and oral mucosa of adults suffering from T2DM, including during dynamic monitoring by a dentist in the conditions of centralized treatment of patients with diabetes mellitus at the “Stomatological polyclinic № 29” of Frunzensky district, St. Petersburg.Materials and methods. 362 middle-aged women who were divided into 3 groups were examined. The 1st (control) group included 127 people who were practically healthy in their psychosomatic status. The 2nd group included 103 people suffering from type 2 diabetes mellitus (T2DM), who were visited by a dentist for treatment. The 3rd group consisted of 132 people, which took place over 2.5-3 years, were under dynamic observation by a dentist who treats patients with diabetes mellitus. During clinical trials, patients studied periodontology using methods of generally accepted methods of periodontology.Results. It was found that people with diabetes suffer from diabetes at the stomatological polyclinic number 29 of the Frunze district. St. Petersburg, with the usually observed 100% prevalence of periodontal inflammatory pathology and the high severity of its ducts, was overwhelmingly sanitized (95.45%), there are good individual oral hygiene (0.58 ± 0.18) and low need for in addition to gingivitis and periodontitis (17.42%), unlike people suffering from type 2 diabetes and visiting a dentist for treatment, where the need for inflammatory periodontal pathology is 100%.Conclusion. Created on the basis of dental clinic number 29 St. Petersburg. The dental care center for people suffering from T2DM syndrome showed that with early access to dental care and with their dynamic observation, it is possible to achieve a good level of dental health with a satisfactory aesthetic nature of the dentition and chewing functions.

Background: Self-care is essential for the prevention of complications in patients with diabetes, but several authors report that even with health education programs, the incidence of complications in patients with diabetes continues to increase. Objective: We aimed to examine adherence to self-care strategies and the repercussions of adherence on the clinical profiles of individuals with type 2 diabetes. Methods: We performed a systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. We searched for related studies in 4 databases: PubMed, Web of Science, Scopus, and Latin American and Caribbean Health Science Literature (LILACS). We included observational studies in English and Portuguese that evaluated the effects of selfcare in individuals with type 2 diabetes. Results: The search resulted in the identification of 615 articles, of which 34 met all the inclusion criteria. General self-care was considered unsatisfactory. Physical exercise was classified as the self-care activity that was performed less frequently by individuals with type 2 diabetes mellitus; adherence to medication was the most frequent behavior among volunteers. Conclusion: The studies indicated poor adherence of the population to good self-care practices, reflected by the increase in complications related to DM.

Background: We sought to determine the frequency of toenail onychomycosis in diabetic patients, to identify the causative agents, and to evaluate the epidemiologic risk factors. Methods: Data regarding patients’ diabetic characteristics were recorded by the attending internal medicine clinician. Clinical examinations of patients’ toenails were performed by a dermatologist, and specimens were collected from the nails to establish the onycomycotic abnormality. All of the specimens were analyzed by direct microscopy and culture. Results: Of 321 patients with type 2 diabetes mellitus, clinical onychomycosis was diagnosed in 162; 41 of those diagnoses were confirmed mycologically. Of the isolated fungi, 23 were yeasts and 18 were dermatophytes. Significant correlations were found between the frequency of onychomycosis and retinopathy, neuropathy, obesity, family history, and duration of diabetes. However, no correlation was found with sex, age, educational level, occupation, area of residence, levels of hemoglobin A1c and fasting blood glucose, and nephropathy. The most frequently isolated agents from clinical specimens were yeasts. Conclusions: Long-term control of glycemia to prevent chronic complications and obesity and to promote education about the importance of foot and nail care should be essential components in preventing onychomycosis and its potential complications, such as secondary foot lesions, in patients with diabetes mellitus. Background. Anaemia is one of the commonest blood disorders seen in patients with diabetes. In Ethiopia, chronic illnesses are tremendously raising with their complications. But very little research has been conducted, particularly on anaemia among diabetes mellitus (DM) patients. Therefore, this study aimed at assessing the prevalence of anaemia and associated factors among type 2 diabetes mellitus patients in Northwest Ethiopia. Methods. A cross-sectional study design was employed at University of Gondar Comprehensive Specialized Hospital from March 1 to April 15, 2019, among 372 type 2 diabetes mellitus patients (T2DM). Multivariable logistic regression analysis was fitted, and the corresponding adjusted odds ratio (AOR) and 95% CI were used to identify factors associated with anaemia. Level of significance was declared at the p value less than 0.05. Results. The study revealed 8.06% (95% CI: 5.68–11.31%) of the participants were anaemic. Being male (AOR = 2.74, CI: 1.02, 7.38), combined type of treatment (AOR = 8.38, CI: 1.66, 42.25), having diabetes-related microvascular complications (AOR = 3.24, CI: 1.14, 9.26), and hypertension (AOR = 0.01, CI: 0.002, 0.06) were the significant factors associated with anaemia. Conclusions. The finding of the current study revealed low prevalence of anaemia among T2DM patients. Sex, type of treatment, diabetes-related microvascular complications, and hypertension were factors associated with anaemia. Assessment of haemoglobin levels among T2DM patients may help to prevent ensuing microvascular complications. Incorporate anaemia screening into the routine assessment of diabetic complication particularly for those who are hypertensive and took combined treatment to allow early appreciation and treatment of anaemia and later improve the overall care of patients with diabetes.

Summary Long-acting pasireotide is an effective treatment option for acromegaly, but it is associated with hyperglycemia, which could impact its use in patients with diabetes. We present a case of a 53-year-old man with acromegaly and type 2 diabetes mellitus (glycated hemoglobin (HbA1c): 7.5%), who refused surgery to remove a pituitary macroadenoma and enrolled in a Phase 3 clinical trial comparing long-acting pasireotide and long-acting octreotide in acromegalic patients. The patient initially received octreotide, but insulin-like growth factor 1 (IGF-1) levels remained elevated after 12 months (383.9 ng/mL; 193.0 ng/mL; reference range: 86.5–223.8 ng/mL), indicating uncontrolled acromegaly. He switched to pasireotide 40 mg and subsequently increased to 60 mg. Within 6 months, IGF-1 levels normalized (193.0 ng/mL), and they were mostly normal for the next 62 months of treatment with pasireotide (median IGF-1: 190.7 ng/mL). Additionally, HbA1c levels remained similar to or lower than baseline levels (range, 6.7% to 7.8%) during treatment with pasireotide despite major changes to the patient’s antidiabetic regimen, which included insulin and metformin. Uncontrolled acromegaly can result in hyperglycemia due to an increase in insulin resistance. Despite having insulin-requiring type 2 diabetes, the patient presented here did not experience a long-term increase in HbA1c levels upon initiating pasireotide, likely because long-term control of acromegaly resulted in increased insulin sensitivity. This case highlights the utility of long-acting pasireotide to treat acromegaly in patients whose levels were uncontrolled after long-acting octreotide and who manage diabetes with insulin. Learning points Long-acting pasireotide provided adequate, long-term biochemical control of acromegaly in a patient with insulin-requiring type 2 diabetes mellitus who was unresponsive to long-acting octreotide. Glycemic levels initially increased after starting treatment with pasireotide but quickly stabilized as acromegaly became controlled. Long-acting pasireotide, along with an appropriate antidiabetic regimen, may be a suitable therapy for patients with acromegaly who also have insulin-requiring type 2 diabetes mellitus.

Currently, diabetes mellitus (DM) is a complex global problem, which is increasing every year. So, in 2019, diabetes mellitus was detected in 463 million adults (from 20 to 79 years old) in the world. And the main cause of death in patients with diabetes mellitus is cardiovascular complications. The study was aimed investigate the features of functional and structural changes in the heart in patients with type 2 diabetes mellitus and nephropathy. A total of 98 patients with type 2 diabetes mellitus were examined, out of which 78 patients had diabetic nephropathy (DN) of varying severity. The control group consisted of 20 healthy patients. After a clinical examination, depending on the state of renal function, all patients were divided into the following groups: group I — patients with type 2 diabetes mellitus without signs of nephropathy (n = 36), group II — patients with type 2 diabetes mellitus with normal glomerular filtration rate (GFR) and albuminuria (n = 33), group III — patients with type 2 diabetes mellitus with decreased GFR and albuminuria (n = 29). To study changes in hemodynamics and structural parameters of the heart, patients underwent transthoracic echocardiography on an ULTIMA PA ultrasound machine (Radmir, Ukraine) using a sectoral phased transducer with a frequency range of 2–3 MHz according to the standard technique based on the recommendations of the American Echocardiographic Society. To determine indexing indicators, the patients underwent anthropometric measurements. Patients with DN and albuminuria and decreased GFR showed an increase in the linear dimensions of the heart in comparison with controls and patients without signs of nephropathy. With DN, patients have a significant increase in left ventricular mass and a significant increase in the detection rate of left ventricular hypertrophy up to 91.3 % in patients with albuminuria and preserved renal function and up to 100 % with a decrease in GFR.

Abstract Introduction and aim Cardiovascular diseases remain the most common cause of morbidity and mortality in patients with diabetes. Epicardial adipose tissue (EAT), visceral fat depot of the heart, was found to be associated with coronary artery disease in cardiac and non-cardiac patients. Increased visceral adiposity is associated with proinflammatory activity, impaired insulin sensitivity, increased risk of atherosclerosis and high mortality. In the present study we aimed to investigate the relationship between EAT and visceral adiposity index (VAI) in patients with diabetes. Methods This was a cross-sectional study involving 128 patients with type 2 diabetes mellitus (73 females, 55 males; mean age, 54.09+±+9.17 years) and 32 control subjects (23 females, 9 males; mean age, 50.09+±+7.81 years). EAT was measured by using a trans-thoracic echocardiograph. Parameters such as waist circumference (WC), body mass index (BMI), triglyceride and high density lipoprotein (HDL) cholesterol were used to calculate VAI. Result EAT and VAI measurements were significantly higher in patients with diabetes when compared to control subjects. In the bivariate correlation analysis, VAI was positively correlated with uric acid level (r=0.214, p=0.015), white blood cell count (r= 0.262, p=0.003), platelet count (r=0.223, p=0.011) and total cholesterol levels (r= 0.363, p<0.001). Also, VAI was found to be the independent predictor of EAT. Conclusion Simple calculation of VAI was found to be associated with increased EAT in patients with type 2 diabetes.

Introduction. Pathological changes in the periodontium in people with diabetes mellitus, according to some data, are aggravated after orthodontic intervention, which dictates the need to carry out preventive measures, the effectiveness of which will determine the quality of orthodontic treatment. The aim of this study was to study the periodontal protective effect of two domestic antidisbiotic drugs containing plant extracts, vitamins, calcium and phosphorus salts. Methodology. The experiments were carried out on white Wistar rats, divided into 5 groups. The gums and periodontal bone tissue were isolated. The activity of urease, lysozyme and the content of hyaluronic acid was determined in the gingival homogenate, and the degree of dysbiosis was calculated. In the homogenate of the periodontal bone tissue, the activity of alkaline and acid phosphatases was determined, the mineralization index and the degree of mineralization, as well as the content of calcium and protein, were calculated. Results. The obtained results of our study have convincingly proved the pronounced periodontal protection efficacy of antidisbiotic drugs for oral applications of the plates "CM-2" and the drug Enoant polyphenols. The proposed therapeutic and prophylactic complex inhibits inflammation, destruction of bone tissue and the development of dysbiosis, along with the manifestation of pronounced adaptogenic, antimicrobial and periodontal protective effects in experimental animals against the background of the development of type 2 diabetes mellitus and orthodontic tooth movement. Conclusion. The proposed therapeutic and prophylactic complex inhibits inflammation, destruction of bone tissue and the development of dysbiosis in experimental animals against the background of the development of type 2 diabetes mellitus and orthodontic tooth movement. The obtained results of the experimental study serve as the basis for the recommendation of the investigated complex, including the local application of plates with herbal extracts, vitamins, calcium and microelements, along with the systemic use of grape polyphenol concentrate, in an orthodontic clinic for patients with diabetes mellitus.

Lockdown during COVID-19 have impact in type 2 diabetes mellitus patients requiring medication and routine physical activity. The stress, development of complications of chronic diseases, locked in experience, fear of dying and loneliness in hospital. All these issues suggest that mental health of the diabetes patient is being affected enormously. In this report, we discussed the experience of three patients with diabetes mellitus and among them two acquired COVID-19 admitted to the COVID ward.
