Problem setting. The active development of the service sector contributes to the growth of scientists’ attention to the concept of marketing relations, the center of which is a customer-oriented approach. This concept is especially relevant for the field of medical services, as the quality of these services not only forms aspects of patient satisfaction, but also determines their quality of life and longevity. The Ukrainian model of health care and the provision of medical services has recently been in the process of reform. The changes affect key elements of the functioning of the medical sector: transformation of the structure of the medical services market, aimed at increasing the share of the private sector, changing the principles and conditions of financing, improving the system of medical education and quality of health care. The insurance mechanism in the system of medical services is being reformed and the system of legal regulation of state medical institutions is being changed. One of the key tasks of the reform is to ensure the orientation of the health care system to meet the needs of patients in treatment. This necessitates scientific research in the interaction of two key aspects of medical service – the patient (recipient of medical services) and the medical worker (institution of medical services). Recent research and publications analysis. Ukrainian researchers have devoted their work to analyzing the needs of patients in the system of patient-medical institution and finding effective mechanisms for managing medical institutions to meet these needs. The analysis of patients’ opinions on the availability, satisfaction with the provision of medical services and health care reform was conducted by scientists Shkrobanets, V. Galkin researched the issue of improving outpatient therapeutic care as an urgent task of health care. English scientists S. Cavanagh and K. Chadwick identified the basic needs of public health. They analyzed the needs, aspirations and goals: welfare, quality of life and public policy in the field of medicine, and conducted applied research on the quality of life. However, given the realities of the development of the medical services market in Ukraine, patient orientation issues need to be explored in more depth. Highlighting previously unsettled parts of the general problem. Ukrainian patients face significant difficulties in obtaining medical services. In addition to physiological problems, there are psychological problems associated with the implementation of the choice among the complex processes of diagnosis and treatment. The greatest responsibility for health care processes lies with physicians, heads of health care facilities, and other health care providers who are aware of patients’ needs and are responsible for the functioning of the system. Their ability to identify problems and respond appropriately to public demand is fundamental to working in terms of their competencies. Thus, the competence of health professionals is a systemic factor that contributes to the patient’s receipt of high-level medical services. Paper main body. Modern market relations are subject to economic laws that are specific to management and are in conflict with the methods of administration. Medical care within market relations is a specific type of intangible production, filled with economic characteristics. In recent years, Ukraine’s medical sector has achieved significant results in overcoming the crisis that arose during the establishment of independence in all spheres of society, and, as a result of global social, political and economic changes, continues to be reformed. The essence of the current stage of health care reform in Ukraine is characterized by objective conditions under which medical services become a commodity, and the relationship between doctor and patient in its content begins to be defined as marketing with the predominant role of the patient. The introduction of economic criteria in the activities of the medical sector in modern market relations is determined by the need to develop the health care system. At the same time, the reform of health care delivery is being transformed into a system that regulates supply and demand in the health care market. Conclusions of the research and prospects for further studies. According to the results of the research, it is established that Ukrainian consumers average the patient’s focus on medical services. Among the key, unresolved issues that hinder the introduction of patient orientation are the insufficiently comfortable conditions of patients’ stay in inpatient facilities and treatment, insufficient effectiveness of treatment in general and the results obtained. Weaknesses include low awareness of health services in public health facilities, lack of focus on patient health, lack of psychological support for patients during treatment and rehabilitation, late delivery of services, and problems with non-compliance. This creates a field for new research to address the identified problems.

The purpose of this article is to study the factors that have a direct impact on improving the quality of life through improving public administration in the field of health care. In the course of our research are considered the following aspects: significant economic and social aspects of the medical sphere that affect the quality of life; a list of public services that are of significant public interest and have the greatest impact on quality of life, methodological approaches to assessing the quality of life, the main problems and possible ways to improve public health policy to improve the quality of life. To achieve the objectives of the study, several statistical indicators were selected and a number of statistical methods of analysis were used: pairwise correlation, regression analysis, methods of comparison, synthesis and comparison. Statistical analysis was conducted according to the countries of the European continent, which belong to different social models. An economic-mathematical model of the dependence of the Quality of Life Index on the factors of public management of medical services has been built, which shows that the increase in the Quality of Life Index by 70.2% is due to the level of state funding, insurance and the number of hospital beds. One of the main conclusions is the fact that the Health Index, along with the Purchasing Power Index, have the greatest positive impact on the quality of life of the population. According to the results of the regression analysis, it was found that that the most statistically and practically significant factor of the linear dependence of the studied indicators of public health services management on the Quality of Life Index is public expenditures in the field of health care. In this context, public policy should be mainly aimed at addressing the problems of efficient allocation of resources and fragmentation of policies and strategies for the development of effective socio-economic systems for providing quality health services to ensure a high level of quality of life.

Topicality. Reforming the modern health care system requires comprehensive strategic measures that will help improve the management of modern health care facilities, including dental health facilities. The market of dental medical services is an important component in the structure of the market of medical services, as evidenced by the intensive development of dental services, expansion of not only medical but also cosmetic services, improving their quality and level of service. Therefore, there is an urgent need to determine the essence of the management of strategic development of dental institutions. Aim and tasks. Purpose and objectives: to determine the fundamental theoretical approaches to understanding the essence of management of strategic development of dental institutions. Research results. The level and quality of medical services characterize the sustainable development of the country, as other areas cannot be fully developed if the health care system is low. Formation and development of strategic management is an important part of corporate management not only of the production enterprise, but also of health care institutions, and it acquires especially important participation in the conditions of medical reform with the purpose of further social, medical, financial and economic competitive advantages in the medical services market. Without effective medical care, citizens are not interested in solving secondary problems, because the issue of preserving and maintaining health is always in the first place. With the spread of market development priorities, the management of medical institutions in general and dental medical institutions in particular is becoming important. Conclusion. Management of strategic development of dental medical institution is aimed at introducing new dental medical technologies, improving the level of professional training of medical staff of dental medical institutions and improving the quality of dental care using new standards in dentistry. Management of strategic development of dental medical institutions is possible only with the support of the dental care system by the state, through standardization, certification and financial support, observance of human rights in the health care system and state support of dental care to socially vulnerable groups.

Objectives: Interstitial lung disease (ILD) is a debilitating and life-limiting condition, requiring multi-disciplinary care. While guidelines recommend early specialist palliative care referral to improve symptoms and quality of life, few patients access such care towards the end-of-life. This study aimed to explore clinicians’ perspectives regarding specialist palliative care and opioids to understand barriers to optimal care and guide clinical practice improvement initiatives. Methods: A cross-sectional, exploratory, qualitative study was undertaken with Australian respiratory clinicians caring for people with ILD (n = 17). In-depth, semi-structured interviews were audio-recorded, transcribed verbatim and coded. Thematic analysis was undertaken to extrapolate recurring ideas from the data. Results: Four themes were identified: 1) understanding how to improve patient care and support, 2) the need to dispel stigmatized beliefs and misconceptions, 3) the importance of trusted relationships and good communication and 4) the challenges of navigating the health-care system. Participants discussed the need to implement early specialist palliative care and symptom palliation to alleviate symptoms, provide emotional support and augment quality of life. Participants described challenges accessing palliative care and opioids due to stigmatized beliefs amongst patients and clinicians and difficulties navigating the health-care system. Trusted therapeutic relationships with patients and strong inter-disciplinary partnerships with collaborative education and communication were perceived to improve patients’ access to symptom palliation. Conclusion: Specialist palliative care and opioids were believed to improve patients’ quality of life, however, many barriers can make accessing such care challenging. To address these issues, multi-disciplinary collaboration, high-quality communication and trusted therapeutic relationships are crucial throughout the ILD illness journey.

Purpose Seniors are nowadays at the core of important reflections to understand both how to ensure them a proper quality of life and better recognize their social role, providing them services and proper health care to value them as persons and resources. This paper aims to find a through definition about who is a senior, in the author’s opinion the starting point to help them flourishing. Design/methodology/approach As an example of definitions, an online dictionary and two geriatric text-books are quoted, highlighting qualities and rights referred to seniors especially in the delicate context of the health-care system. Findings The lack of a commonly shared perspective on this delicate kind of patient entails the difficulty to reach a coherent and satisfying definition about who a senior is. Originality/value The lack of a commonly shared definition leads to inevitable misunderstandings and could explain the arduousness of considering seniors in all their aspects. Further investigations are suggested.

Purpose The purpose of this study is to investigate the role of the COVID-19 health-care system quality (HSQ) and its impact on the individual (satisfaction) and social (quality of life [QOL]) outcomes in the context of a transformative health-care delivery system using service-dominant logic (SDL). Design/methodology/approach A sample consisting of 1,008 individuals who have experienced the COVID-19 health-care system was drawn from four different regions of Saudi Arabia using the simple random sampling technique. The survey was conducted using an online survey and 1,008 respondents answered, based on their experience and knowledge of the COVID-19 health-care system. Partial least squares structural equation modeling was applied to test the proposed research model. Findings The study findings suggest that service system satisfaction (SAT) significantly mediates the role of the HSQ in delivering and enhancing the QOL. HSQ also has a significant role to play on the SAT as well as the QOL. These findings contribute to the body of knowledge on SDL in the context of HSQ in understanding the significant role of technologies can play in enhancing service satisfaction and better QOL during a crisis such as COVID-19. This study also improves the understanding of the importance of customer-centricity, real-time visibility through tracking and tracing of service flow, agile decision-making, fewer but better-defined service objectives, and finally shaping mindsets and behaviors of all the relevant parties involved in the HSQ service delivery process. Research limitations/implications One of the major limitations of this study is that, although COVID-19 is an ongoing global pandemic, cross-sectional data were collected in only one country. The findings may not be generalizable across subsequent waves of the pandemic. The best practices of HSQ could be studied around the globe and the results used to support continuous improvement. Originality/value This study advances the understanding of the SDL in the context of a transformative health-care system for a transitional economy by focusing on individual and social well-being during an unexpected crisis such as the COVID-19 pandemic. This study also contributes toward the understanding of the roles of enabling technologies to improve the service delivery system which results in an improved SAT, as well as better QOL for the society at large. Based on SDL this research validates the HSQ model, relevant measures and its overall impact on SAT and QOL in the context of a transformative health-care service system in Saudi Arabia.

An important part in this article is allocated to the structure and main principles of total quality management in the health care system. The work shows the importance of rational and efficient assessment of control and supervision in health care for society as a whole and for its individual members. Health care is part of the system that shapes the quality of life of the population, influences the development of human potential, economic performance, on the adoption and diffusion of innovations in Russia. Control and supervision in public administration reform received much attention. The Ministry of Economic Development of the Russian Federation has developed a methodology for assessing and comparing the activities of Supervisory authorities. However, there is a need for not only implementation of the standardized approach in assessing but taking into account the specific features of activity of Supervisory authorities. The article presents the system of socially significant indicators for assessing the activities of Roszdravnadzor and its territorial offices in the RF entities. Currently the authority spends a lot of tests (about 10 thousand per year), however, the effectiveness of control and supervision is low. The health of the population remains low, satisfaction with public health services is reduced. Researching foreign experience of health systems development in countries with socially oriented economy allows to draw a conclusion about the necessity of building a qualitatively new assessment system of control and supervision, which should be based on the interests of society. The author proposes to depart from the principle of "check for validation" and change over to socially significant indicators for the government activities. The article highlights such indicators for healthcare and proposes their decomposition for fractional quantitative and qualitative assessment of activities. This system of indicators is aimed at improving the efficiency of public health care management and growth of the citizens’ health and their quality of life in a whole. The article defines the scientific and terminological problems of researching services in the field of services, the basic problems of the evaluation of services in health care organizations. On the basis of a systematic approach to the category of quality of medical services the article investigates patient satisfaction with the perceived quality of service, shows the role of consumer expectations in the evaluation of the perceived quality of services and finds that the requirements for the service in medical institutions are underestimated. On the basis of studies the authors identify behaviors of consumers of budgetary medical services organizations, develop a model of consumer activities, which differs from the existing ones that along with the economic component the model is complemented with communication components. The approbation of the authors´ model for health care services shows a characteristic pattern of consumer activity of budgetary organizations. The article highlights the factors of subjective judgment of health care consumers in assessing perceived quality. Based on the study of patient satisfaction the authors develop a hierarchical model of the perceived quality of health services, as well as the place of services defined in the model.
