Standardization is defined as an activity that regulates the general and repetitive application of a system of prescriptions on different (including specific) issues in order to achieve optimal order in certain circumstances. It involves the creation, dissemination and implementation of standards aimed at improving the common properties (quality, suitability, etc.) of products, processes and services. Standards are documents that contain rules, norms, and requirements for a product or service, in order to achieve better efficiency and meet the expectations of consumers and customers. Standards are means of information, serving to improve quality, to support the economic development and contributing to the benefit of the whole Society. The term ‘Standard’ is at the core of the standardization process. Applied to the health system, the standard is a regulatory document developed on the basis of different consensus and validated by a relevant body. This regulatory document establishes a set of rules and requirements concerning different types of medical activities and the results of their implementation. An important objective of each healthcare system is to provide the population with accessible, quality and safe medical care. The introduction of a system of standards in healthcare and in medicine, in particular, is a powerful tool for its achievement. This ensures high quality of the medical assistance offered, a rational use of financial, material and personnel resources as well as an optimization of the diagnostic and healing process. Standards set a minimum/optimal level of Quality Assurance through mandatory requirements that are the basis of State guarantees for affordable medical care, range and types of health services, lists of specific technologies with proven effectiveness, as well as guidelines to further improve the level of quality by formulating further scientifically substantiated recommendations. Standards allow for aggregation of information, which leads to optimization of solutions at all levels in the healthcare system. Standardization is one of the main means of achieving and maintaining the necessary level and one of the main tools in Quality Management. In laboratory medicine, standardization is an approach for the optimization of clinical and laboratory activities. The provision of high quality services is the responsibility of every professional working in the respective diagnostic structure. The quality and standardization in the Specialization of “Clinical Laboratory” are focused not only on the accuracy of laboratory results, but also they step on a broader basis, including each stage of the clinical and laboratory analysis, in order to make important medical decisions subject to patient care.

В условиях социального медицинского страхования удовлетворенность пациентов полученной медицинской помощью является одним из основных критериев оценки её качества. Цель исследования. Оценить уровень удовлетворенности пациентов, получающих лечение в стоматологическом кабинете. Материал и методы. Был проведен социологический опрос 83 пациентов, получающих лечение в стоматологическом кабинете. Выводы. На основании проведенного нами опроса и полученных результатов выявлено, что в целом респонденты отмечают удовлетворенность стоматологической услугой. Также отмечают высокое качество оказываемых услуг. In the context of social health insurance, the satisfaction of patients with the received medical care is one of the main criteria for assessing its quality. Purpose of the study. Assess the level of satisfaction of patients receiving treatment in the dental office. Material and methods. A sociological survey was conducted of 83 patients receiving treatment in the dental office. Conclusions. Based on our survey and the results obtained, it was revealed that, in general, the respondents note satisfaction with the dental service. They also note the high quality of the services provided.

Chronic nonhealing wounds pose a significant challenge to healthcare system because of its tremendous utilization of resources and time to heal. It has a well-deserved reputation for reducing the quality of life for those affected and represent a substantial economic burden to the healthcare system overall. Earthworms are used as a traditional Chinese medicine, and have been applied pharmacologically and clinically since a long time in China. However, there is paucity in data regarding its wound healing effects. Therefore, we investigated the effect of earthworm extract (EE) on skin wound healing process. The obtained data showed that EE has healing effects on local wound of mice. It decreased the wound healing time and reduced the ill-effects of inflammation as determined by macroscopic, histopathologic, hematologic, and immunohistochemistry parameters. The potential mechanism could be accelerated hydroxyproline and transforming growth factor-β secretion—thus increasing the synthesis of collagen, promoting blood capillary, and fibroblast proliferation. It could accelerate the removal of necrotic tissue and foreign bodies by speeding up the generation of interleukin-6, white blood cells, and platelets. It thus enhances immunity, reduces the risk of infection, and promotes wound healing. All in all, the obtained data demonstrated that EE improves quality of healing and could be used as a propitious wound healing agent.

Abstract Background: The French healthcare system is evolving to meet the challenges of an aging population, the growing prevalence of chronic diseases, the development of new technologies and the increasing involvement of patients in the management of their disease. The aim of this study is to assess the satisfaction and priorities of chronic patients regarding medical care, information and services and their quality of life.Methods: A cross-sectional study was conducted from February to March 2018 via the French Carenity platform. Adult patients enrolled in type 1 diabetes, heart failure or obesity communities were invited to answer an online questionnaire. A numeric scale from 0 (meaning not satisfied) to 5 was used to evaluate patients’ satisfaction. Patients’ priorities were assessed using a ranking question. Patients’ satisfaction and priorities have been combined in a matrix to identify patients’ expectations.Results: 67 respondents of each condition answered a questionnaire. The most important and least satisfactory items about medical care are availability and active listening from healthcare providers, as well as access to coordinated and multidisciplinary care. Regarding information and services, respondents mostly expect connected medical devices, in addition to lifestyle and dietary measures. As for the quality of life, respondents fear that their chronic condition will keep impacting their daily mood and ability to do physical activities.Conclusions: This study shows that chronic patients want to be more actively involved in their care pathway. Patient training and therapeutic patient education programs could help them manage their chronic conditions within a patient-centred healthcare system.

Objective. To determine frequency of retinopathy cases in profoundly preterm newborns treated at Gorodkov Ivanovo Scientific Research Institute of Maternity and Childhood in 2016-2017, as well as to evaluate impact of the risk factors on disease severity in the conditions of specialized medical care. We examined 344 profoundly preterm newborns with gestational age of less than 32 weeks. 83 children (24.1%) were diagnosed with retinopathy of preterm newborns. A high quality of medical care was proved by a low percentage of children with severe forms of retinopathy in preterm newborns and absence of blindness in the children. A low gestational age, low Apgar score at the end of the first minute of life and traditional artificial lung ventilation are the most significant factors of severe retinopathy of preterm newborns.

One of the most urgent areas of Russian health care is the organization of medical care in the new socio-economic conditions, aimed at providing affordable high-quality and safe medical care to the population, therefore, a new approach to the formation of control over both the quality of medical care and its safety is of particular interest. At all stages of the treatment and diagnostic process, serious mistakes can be made that can have a negative impact on the life and health of patients.The article discusses the methodological aspects of assessing the quality of medical care at all stages of the treatment and diagnostic process. Requirements for the formation of an expert opinion, which is prepared based on the results of the examination of the quality of medical care, have been formulated. In order to effectively manage the quality of medical care, the author proposes the creation of a digital “smart” system for monitoring defects in medical care.

“Clinical Laboratory” is an independent medical specialty, a dynamic clinical discipline of a scientific and applied nature. Laboratory researches, the results of which are objective indicators of the state of the organism, are of great importance in terms of modern medicine. Laboratory analyses help to achieve, within certain limits, such a result that can be used for the purpose of making the necessary medical decisions. “Laboratory Medicine” is a complex specialty, covering different directions: clinical chemistry, laboratory haematology, immunology, immunohematology, biology, drug monitoring and laboratory toxicology. A wide varietyof techniques is used for the analysis of biological fluids, the composition and properties of different types of cells, tissues, organs and systems.According to World Health Organization (WHO)data, in the past 20 years, the rate of morbidity within the population of nosologic units requiring high technologies and accuracy of diagnostic tests has increased. Modern revolution in clinical medicine complicates research methods and, as a result, increases significantly their value and cost.Laboratory units are widely considered to determine the status of each national healthcare system. Effectiveness of laboratory diagnostic process can be ensured by two key elements – high-tech equipment and professional staff. A need exists for identifying and using internal reserves with a view to the optimization of work volume and structures of laboratory and diagnostic units, being a promising and the most realisticdirection in the contemporary socio-economic situation.The mission of Clinical Laboratory departments is focused both on the provision of fast, adequate and accurate medical-diagnostic assistance, as well as on making effective use of any available material and technical resources and personnel potential. The problem being studied, its importance and the analysis of Clinical Laboratory activity are an essential component of the vision for this activity: high quality of clinical and laboratory results, timely and accurate diagnosis, adequate, timely and effective medical decisions.Main activities such as management; documentation of researches assigned and carried out; financial and economic aspects related to the valuation of laboratory analyses; tracking cost and available options for their optimization; reasonable time reduction for the production of laboratory analyses; introducing new high-performance technological equipment to improve the quality of laboratory activities and optimize the healing process have been researched and analysed in the Clinical Laboratory of “Kanev” University Multi-Profile Hospital for Active Treatment, Ruse.

The increase in the number of patients with pathology of the maxillofacial region necessitates improving the quality of medical care provided to the population in the field of "Maxillofacial surgery". Anatomical and physiological features of the maxillofacial region, the prevalence of pathology determine the implementation of organizational measures, the involvement of narrow specialists, the need for medical organizations in modern material and technical equipment. The indicators of medical care mainly depend on a full-fledged diagnosis, and are also determined by the availability, quality and speed of specialized medical care. In modern conditions of the development of the health care system, an integrated approach to the organization, the introduction of new methods of providing medical care and rehabilitation of patients is required. Optimization of the maxillofacial surgery service within a multidisciplinary medical organization, the introduction and development of an outpatient polyclinic link, the training of highly qualified personnel contribute to improving the quality of diagnosis, treatment and timely detection of pathology. This article presents the results of optimizing the service for providing medical care to patients with pathology of the maxillofacial region. In the Penza region, the full-fledged, high-quality functioning of the service is realized through the organization of the Center for Maxillofacial Surgery on the basis of the State Budgetary Healthcare Institution «Penza Regional Clinical Hospital named after N.N. Burdenko». The structure of the Center for Maxillofacial Surgery ensures the continuity and complementarity of specialists of a medical organization in the treatment and diagnostic process. Optimal conditions have been created for the diagnosis, treatment and rehabilitation of patients with pathology of the maxillofacial region, in order to provide the population with affordable and high-quality medical care, regardless of the category of citizens. The creation of the Center for Oral and Maxillofacial Surgery contributes to an increase in the structural efficiency and medical efficiency of the health care system of the Penza region. The results of the measures and innovations carried out can be used to eliminate the existing shortcomings in the organization of medical care for patients with pathology of the maxillofacial region in other regions.

The article deals with changes in sociological models of the professional role of a physician in a market economy. The article analyzes the importance of marketing in healthcare and models of providing medical services in Russian and foreign healthcare systems. Based on the accumulated literature data, it is possible to trace the dynamics of the relationship between the patient and the healthcare system towards the formation of consumer ideology and the importance of sociological research of the status and role parameters of the physician in order to improve the eff ectiveness and quality of provided medical care.

The purpose of work was the clinical laboratory research, identification of reaction of local immunity mucous a mouth, indicators of globulins fraction on carrying within 6 months of the removable orthodontic device from domestic light-cured polymer at children. Immunoglobulins of three classes IgM, IgG, IGA and calculated values of sIgA and sc (free secretory component) were investigated. Contents sekretorny IgA (sIgA) and a free secretory component (sc) is especially important. Results were estimated by V.V. Zverev’s technique et al. (2011) [6]. Results showed that immunoglobulins changed individually at each patient within one extent of dysbiotic disorders that is connected with an initial situation of a condition of oropharynx of the child which had no considerable fluctuations within one degree of a condition of a microbiota of a mucous oropharynx of the child. The bioinertness of Nolatek photopolymer in the form of basic material of the orthodontic device is established. Light-cured domestic nanostructural polymer has high quality of the operational properties in children’s practice of clinic of orthodontics conforming to modern requirements for bioinertness, an esthetics and functionality.
