Background: Sero-molecular methods have been used for the study on the prevalence and economic impact of brucellosis in Bangladesh. The physio-pathological effects and therapeutic trials against brucellosis in animals or humans could not be traced in the available inland literature. Objectives: This study was conducted to determine the sero-prevalence and haemato-biochemical and therapeutic responses in Brucella-infected dairy crossbred cows. Materials and Methods: Sera samples of 552 cross-bred dairy cows of Military Dairy Farm, Jessore and smallholder dairy farms of Sirajgonj and Dhaka were initially screened for Brucella infection with Rapid kit test (RKT) and Rose Bengal test (RBT) and positive samples were tested further with ELISA and PCR for confirmatory diagnosis during the period from January 2018 to June 2019. Out of 11 all tests positive cows, of which four had history of abortion were selected for therapeutic trials with combined long acting oxytetracycline @ 25 mg/kg BW 16 doses at 72 hours intervals and streptomycin @ 25 mg / kg BW 10 doses at 24 hours interval injections. Blood samples of all the Brucella negative control and pre- and post-treatment stages of all the Brucella-infected cows were tested for haemato-biochemical changes and Brucella antibody responses by using ELISA and PCR. Results: Of the 552 sera screened for Brucella infection, of which 18 (3.26%) cows were found positive with the RKT, RBPT and ELISA, whereas only 11 (1.99%) samples showed positive with PCR. The haemato-biochemical values between Brucella-negative and positive cows and antibiotic pre-treated and post-treated values of Brucella-infected cows were compared and discussed. The antibody titer decreased with antibiotic treatment and increased on with-drawl of the antibiotic at 180 days which indicates that antibiotics only effective against bacteremic form not intracellular stage that caused to relapse. However, of the four treated cows, one became pregnant on artificial insemination with normal reproductive cycle which needs to explore its status in further research. Conclusions: The haemato-biochemical values in cows affected with sub-clinical brucellosis can determine the extent of harmful effects on the health of cows. Therapy with multiple antibiotics for long period, absence of effective vaccines and the most expensive quarantine and ‘test and slaughter’ methods, brucellosis remains as a challenge for its control and eradication in developing world. Human patients affected with brucellosis is treated with antibiotics with overall neglecting the animal reservoir of Brucella infection, therefore ‘One Health’ approach would be required to control this disease.

This report characterizes the digital dermatitis (DD) lesions in the accessory digits of dairy cows and presents data on the applied therapy. Fifteen Holstein cattle with DD affecting the accessory digits of the hindlimbs from four dairy farms with previous history of DD were evaluated. Lesions were excised, the wounds were sutured, and a topical application of oxytetracycline powder covered by bandaging was associated with a single parenteral administration of long acting oxytetracycline IM (20mg/kg). Tissue samples were obtained for histopathology and transmission electronic microscopy (TEM). Lesions from all the animals were recuperated 15 days after surgical procedure. Overal, most DD lesions were papillomatous epidermal projections or wartlike verrucous lesions. Histopathologically, samples revealed hyperplasia of epidermis with hyperkeratosis, several mitoses in the stratum basale and elongated rete ridges in the superficial and middle dermis. TEM revealed long, thin spirochete-like bacteria. Morphologic features of lesions and its response to therapy were comparable to those described for DD.

Abstract Background Recently, the addition of inhaled corticosteroid (ICS) to long-acting muscarinic antagonist (LAMA) and long-acting beta-agonist (LABA) combination therapy has been recommended for patients with COPD who have severe symptoms and a history of exacerbations because it reduces the exacerbations. In addition, a reducing effect on mortality has been shown by this treatment. However, the evidence is mainly based on one large randomized controlled trial IMPACT study, and it remains unclear whether the ICS add-on treatment is beneficial or not. Recently, a large new ETHOS trial has been performed to clarify the ICS add-on effects. Therefore, we conducted a systematic review and meta-analysis to evaluate the efficacy and safety including ETHOS trial. Methods We searched relevant randomized control trials (RCTs) and analyzed the exacerbations, quality of life (QOL), dyspnea symptom, lung function and adverse events including pneumonia and mortality, as the outcomes of interest. Results We identified a total of 6 RCTs in ICS add-on protocol (N = 13,579). ICS/LAMA/LABA treatment (triple therapy) significantly decreased the incidence of exacerbations (rate ratio 0.73, 95% CI 0.64–0.83) and improved the QOL score and trough FEV1 compared to LAMA/LABA. In addition, triple therapy significantly improved the dyspnea score (mean difference 0.33, 95% CI 0.18–0.48) and mortality (odds ratio 0.66, 95% CI 0.50–0.87). However, triple therapy showed a significantly higher incidence of pneumonia (odds ratio 1.52, 95% CI 1.16–2.00). In the ICS-withdrawal protocol including 2 RCTs, triple therapy also showed a significantly better QOL score and higher trough FEV1 than LAMA/LABA. Concerning the trough FEV1, QOL score and dyspnea score in both protocols, the differences were less than the minimal clinically important difference. Conclusion Triple therapy causes a higher incidence of pneumonia but is a more preferable treatment than LAMA/LABA due to the lower incidence of exacerbations, higher trough FEV1 and better QOL score. In addition, triple therapy is also superior to LABA/LAMA due to the lower mortality and better dyspnea score. However, these results should be only applied to patients with symptomatic moderate to severe COPD and a history of exacerbations. Clinical Trial Registration: PROSPERO; CRD42020191978.

Introduction: Treating chronic macular edema (CME) post endophthalmitis is a challenge. Use of steroids may reactivate the infection and repeated intravitreal therapy with anti-vascular growth factor inhibitors (Anti-VEGF) puts the patient again at the risk of exacerbation of inflammation or endophthalmitis. We describe a case of CME post traumatic endophthalmitis successfully treated with topical interferon therapy. Case description: A 34-year-old Asian Indian lady with a history of cat bite to her right eye and treated elsewhere as traumatic endophthalmitis with recurrent macular edema, presented to us 1 year after the injury. She had received anti-VEGF injection for same. Her medical history was non-contributory except for close contact with her cat. Therapeutic trials with oral doxycycline followed by oral albendazole with steroids, as well as repeated anti-VEGF therapy failed to prevent recurrence of CME. Patient’s steroid responsiveness and reluctance for injections, made us to opt for a novel topical Interferon therapy. Macular edema resolved in 2 months. Interruption of interferon therapy due to COVID-lock down resulted in recurrence of the CME, which again responded well to interferon monotherapy. Conclusion: Topical interferon may have a role in the treatment of inflammatory macular edema and can serve as a, safer, economical and non-invasive treatment option compared to intravitreal steroids and anti-VEGFs.

A 10-year-old girl with a 1-year history of lower genitourinary tract symptoms suggestive of bacterial infection but with numerous negative urine cultures was referred to the University of Alabama urology clinic after empirical treatment with multiple antibiotics failed to resolve her symptoms. An extensive urologic evaluation revealed no structural or physiologic abnormalities, but an exudative vaginitis was noted and large numbers of Ureaplasma urealyticum and Mycoplasma hominis were isolated from the lower genital tract. Cultures for Chlamydia, viruses, and routine bacterial pathogens were negative. After initiation of tetracycline therapy, symptoms resolved and subsequent cultures for mycoplasmas were negative. In addition, a seroconversion was noted for M hominis but not for U urealyticum. Chlamydia serology was negative. It was later learned that the patient had been sexually molested just prior to the onset of symptoms. This case illustrates the necessity of early consideration of a mycoplasmal etiology in the patient with persistent genitourinary symptoms and no obvious bacterial pathogen, or in the patient whose condition is refractory to routine antibiotic therapy.

This study aimed to evaluate the effects of rectal temperature (RT) on conception rate (CR), as well as the effects of seasonality (spring-summer vs. autumn-winter) and timing of artificial insemination (AI) (morning vs. afternoon) on RT and CR in crossbred dairy cows (Holstein x Gyr). The experiment was conducted on a dairy farm in Centralina, MG, where 1,219 conventional and fixed-time inseminations were analyzed. The RT was measured immediately before AI using a digital thermometer. Pregnancy diagnosis was performed using ultrasonography between 28 and 60 days after AI. T The effects of seasonality and timing of AI on RT were analyzed with a Mann-Whitney U test and the effects of RT (above or below the average), seasonality and timing of AI on CR were analyzed with a Chi-squared test, both using the SAS program. The RT average was 39.4°C. Cows with RT ? 39.4°C had lower CR than cows with RT < 39.4°C (25.78% vs. 32.54%; P = 0.0096). During spring-summer, cows had higher RT (39.44°C ± 0.025 vs. 39.27°C ± 0.022; P < 0.0001) and lower CR (25.49% vs. 31.75%; P = 0.0146) compared with autumn-winter. Cows inseminated in the morning had lower RT (38.96°C ± 0.022 vs. 39.60°C ± 0.018; P < 0.0001) and higher CR (32.86% vs. 26.06%; P = 0.0102) than cows inseminated in the afternoon. In conclusion, crossbred dairy cows with rectal temperature equal to or greater than 39.4°C had lower conception rate. Moreover, rectal temperature and conception rate were affected by seasonality and insemination time.

The aim of this research was to observe the effect of heat stress on milk yield and milk compositions of Holstein Friesian crossbred (HF) dairy cows. To fulfill the objectives, a total of 9 Holstein Friesian crossbred cows were selected for this study. Green grasses (German, Para) were supplied adlibitum and concentrate feeds (mixture of wheat bran, rice polish, mustard oil cake, di-calcium phosphate and salt) were supplied at the rate of 2.0 kg/day/cow. Management practices for all the cows were similar following the BAU Dairy farm practices. Data were collected on milk yield (l/h/d), relative humidity (%) and barn temperature (0C). The obtained temperature humidity index (THI) of July, August, September and October were 84.95, 81.99, 81.40 and 79.57, respectively. The highest THI was found in July which indicated higher heat stress during this month. A significant difference (p<0.05) in milk yield of cows was found among different months of July to October. The highest milk yield (6.10±0.50 l/h/d) was found in October among observed months. The compositions of milk such as total solids (TS), solids-not-fat (SNF), fat, protein, lactose, and ash also differed significantly (p<0.01). The highest values (%) of TS, SNF, fat, protein, lactose and ash content of milk were found in October as 12.63, 8.80, 3.83, 3.69, 4.39 and 0.72, respectively and lowest values (%) were in July as 12.20, 8.50, 3.71, 3.50, 4.30 and 0.69, respectively due to the high THI value. From these results, it is concluded that heat stress has strong effect on milk yield and milk composition of HF cows in Bangladesh. Subacute ruminal acidosis is a major issue in dairy cattle and a definite diagnosis is only established by measuring the rumen fluid pH, most credibly collected by rumenocentesis. However, due to its invasive nature, there is still some debate whether it is a safe method or poses risks for cows’ health and welfare. The aim of the study was to retrospectively evaluate the safety of rumenocentesis as a technique to obtain rumen fluid in dairy cows. Results were derived from 2 studies. In study 1, rumenocentesis was performed in 153 Holstein cows from 12 herds, once, between 10 and 90 days in milk (DIM). In study 2, 83 Holstein cows from a dairy farm were repeatedly subjected to rumenocentesisat 30, 90 and 150 DIM. From the 83 cows that were initially enrolled, 8 were culled before the end of the study for reasons irrelative to rumenocentesis; therefore, 236 rumenocenteses were actually performed in study 2 (3 times in 75 cows, twice in 3 cows and once in 5 cows). All cows were monitored for 10 days after rumenocentesis for presence of complications. In addition, daily milk yield was automatically recorded for each cow in study 2 to detect any possiblepost-rumenocentesis short-term reduction of milk yield. Minor only complications were recorded in 7 cases: in 4/153 (2.61%) and 3/236 (1.27%) rumenocenteses in studies 1 and 2, respectively. Small diameter abscesses in 6 cows (3 in study 1 and 3 in study 2) and a larger one in 1 cow in study 1 were observed. The 3 small abscesses in study 2 were all recorded after the 1st rumenocentesis, at DIM 30. All 7 cases were resolved spontaneously within two weeks. Moreover, short-term daily milk yield of study 2 cows was not affected by rumenocentesis. The conclusion is that rumenocentesis is a safe technique to collect small volume of rumen fluid for SARA diagnosis, which does not compromise cows’ health and welfare when appropriately performed.

Background and Aim: Metabolism and environment are closely related. Under high ambient temperature (HTa), dairy cows may have different energy metabolism during summer and winter. The present study was carried out to investigate the effect of HTa on the milk yield and blood concentration of beta-hydroxybutyrate (BHBA) and glucose at the herd level. Materials and Methods: One large dairy farm in Thailand with more than 100 crossbred Holstein cows milked each month was selected. The first experiment was performed on non-lactating cows to determine the normal daily concentrations of blood BHBA and glucose. Under the HTa condition, there was no significant change in blood BHBA and glucose concentrations. The second experiment was performed using a prospective cohort clinical design to demonstrate the seasonal effect on milk yield and blood BHBA as an indication of energy metabolism at the herd level. Results: The temperature and humidity index for the winter (78.1±0.5) and summer (83.4±0.7) periods differ significantly. The average milk yield during the winter period was 17.8% higher than during the summer period. The reduction of body condition score (BCS) during early lactation was significant in the winter cows. Both higher milk yield and lower BCS in the winter cows suggested a state of negative energy balance. However, there was no difference in blood BHBA and glucose concentrations between winter and summer cows. The effect of HTa on insulin signaling appeared to be a counterbalancing factor for the ketogenic status. Based on the present results, it would be interesting to further investigate the incidence of subclinical and clinical ketosis in a dairy farm under tropical conditions. Conclusion: The present experiment revealed that HTa during summer decreased milk yield in dairy cows fed under tropical conditions. Higher milk yield in winter caused a greater reduction of BCS and suggested a greater negative energy balance. However, there was no seasonal effect on blood BHBA and glucose concentrations.

For two decades, the GOLD Initiative has consistently identified the use of bronchodilators as a priority in the pharmacotherapeutic strategy for COPD. The authors of international and national clinical guidelines consider fixed combinations of long-acting beta2-agonists (LABAs) and long-acting muscarinic receptor antagonists (LAMAs) as “first-line” drugs in most patients with COPD. Numerous clinical studies have shown that fixed LABAs/LAMAs combinations provide optimal bronchodilation and play a paramount role in preventing exacerbations of COPD. Outperforming placebo and active controls, LABAs, LAMAs, inhaled glucocorticosteroids (ICS)/LABAs combination bronchodilators may differ in their therapeutic potential. The available evidence base currently does not allow to make an unambiguous choice in favor of one or another fixed LABAs/ LAMAs combination. With the appearance of “triple” combinations (ICS/LABAs/LAMAs) on the pharmaceutical market, the issue of their comparison with “dual” bronchodilators has become particularly acute. Currently available data suggest that the use of “triple” therapy is not considered as a starting treatment option for COPD and is appropriate only in a subgroup of patients with a higher baseline risk of exacerbations: in the presence of a history of exacerbations ≥ 1, which required prescription of systemic antibiotics and/or glucocorticosteroids, or necessitated hospitalization during the previous year. Thus, ICS-containing therapy is justified only in cases of recurrent exacerbations of moderate COPD or single episodes of severe exacerbations, despite the continued administration of LABAs/LAMAs, as well as in certain categories of patients whose inflammatory profile suggests a “response” to ICS.
