Schizophrenia is a serious psychosis that appears in young adult, usually chronic, characterised by signs of mental dissociation and incoherent delirious activity that determines the breaking of the contact with the exterior world. The primary objective of this study is to compare the efficiency of Haloperidol and Risperidone. The study consisted in enrolling, evaluating and the clinical and therapeutic following of 125 patients hospitalised at the Clinic Hospital of Psychiatry..., in Galai, between 2000 and 2015. They were divided into 4 equal sub-lots according to the medication received. The modern treatment consists in the administration of atypical neuroleptics, such as: Risperidone, Olanzapine, Clozapine, etc. These substances proved their efficiency in the negative and affective symptoms together with the increase of therapeutic compliance due to the absent secondary effects. The precocious diagnostic, the modern anti-psychotic treatment from its first manifestations and maintained in order to avoid relapse, improve greatly the prognostic in the long run.

Worldwide, regulatory bodies are applying increasing pressure on pipeline operators to manage their pipeline systems in a safe and reliable manner. To respond to these escalating requirements, operators are developing comprehensive integrity management systems. Integrity management begins with developing strong data management capabilities to host and integrate numerous sources of physical data. A key issue in today’s integrity data management system development is integration of all pipeline data from multiple levels of the organization to one central location for easy and effective utilization of the information. Data are often scattered throughout the organization, which often tends towards data duplication, poor decisions, errors, and inefficiencies. The lack of an effective data management process leads to time and resource constrains. In the long run; development of a comprehensive integrated system has proven to be worth the investment. The primary objective of the data model is to account for a common/standard linear referencing of all pipeline features occurring along the pipeline route, and/or within the pipeline right of way. This paper describes the key factors to consider when developing a full data management system and provides some insights into how these integrated data are used to address the need for integrity management.

Introduction: Unnecessary and rampant use of antibiotic for perioperative prophylaxis are not only increasing cost of treatment but also are hazardous in terms of increasing antibiotic resistance, super infection with resistant pathogens and antibiotic toxicity. Objective: Our primary objective is to assess the efficacy of short course antibiotic prophylaxis in preventing surgical site infection (SSI) in surgery of closed orthopedic fracture. Methodology: Patients fulfilling all the inclusion criteria and willing and able to give informed consent were included in the study. All participants were treated by standard surgical technique following standard surgical protocol. Short course antibiotic prophylaxis was administered in peri-operative period to all patients in given format: Three divided dose of intravenous 1.5 gram cefuroxime during peri-operative period followed by no intravenous or oral antibiotics; 1st dose half an hour before incision; 2nd and 3rd dose repeated at 12 hours interval. Results: Of total 100 patient included, three developed surgical site infection till one year follow-up. All three patients with surgical site infection were treated successfully with intravenous antibiotic based on the culture sensitivity report. The average cost of antibiotic for short course antibiotic prophylaxis was 1500 NPR for each patient. Conclusions: There was no significant increase in rate of surgical site infection with short course antibiotic prophylaxis, rather it was less expensive and may have less adverse effect, in terms of antibiotic resistance and antibiotic side effects on long run.

A rapid change in technology trend and lifestyle around the globe has induced a drastic increase in energy production, delivery, distribution, and consumption. That forced building planners, designers, scholars, researchers, and practitioners to come up with a sustainable solution within constrained economic and environmental dimensions. With a proper definition and usage of efficiency and sustainability dimensions in terms of green building design and construction, global challenges (global warming, climate change, poverty, global health and education, etc.) can be mitigated, leading to long-run sustainability. This chapter presents indicators to define, manage, measure, and enhance efficiency and sustainability phenomena for proposing a green building. A primary objective of this study is to identify influencing factors and set forth viable indicators and framework in terms of energy-efficient green building from different standpoints hiring innovative tangible and non-tangible tools and technique.

Safer, controlled and efficient braking is the primary objective of Anti-lock Braking System wherein an efficient and robust braking system significantly enhances the vehicle performance during both straight line motion and cornering thus resulting in drastic reduction of stopping time and distance especially for a race car in long run. Hence clocking better lap times and a considerable reduction in wear of tires are an obvious outcome apart from the enhanced vehicle stability. This work on Anti-lock Braking System (ABS) prediction and control algorithm deals with technical paradigm for estimation of vehicle velocity using wheel angular velocity from wheel rpm sensors as the sole input and methodology to control the braking torque on each wheel so as to prevent loss of traction. The proposed algorithm is modeled using advanced simulating tools involving theoretical estimation of braking torque on each wheel. This is supposed to reduce tire skid with controlled wheel slip estimated using the tire data and the car vehicle dynamics with formula student vehicle as the subject. The work and hence the control algorithm can potentially be extended into a better traction control strategy with acceleration and yaw inputs from accelerometers and yaw sensors.

AbstractObjectivesThe primary objective of this study was to determine the prevalence of BRAFV600E and TERTpromoter mutations in paediatric and young adult patients with papillary thyroid carcinoma (PTC) and the secondary objective, to assess their association with clinicopathological features.MethodsPatients ≤20 years who underwent surgery for differentiated thyroid cancer (DTC) from 2005 to 2018 were consecutively enrolled for BRAFV600E and TERTpromoter mutations analysis and records analysed for the association of aggressive features. Univariate analysis and multivariate logistic regression were used to identify the independent predictors of BRAFV600E mutations.ResultsAmong 100 patients with DTC, 68 patients were ≤18 years and the remaining 30 patients were >18 years of age with a median age of 17 years (IQR 14–19 years) 98 patients had PTC and 2 had FTC. BRAFV600E mutation was present in 14/98 (14.3%) PTC and TERTpromoter mutation noted in none. Multivariate analysis identified RAI refractoriness (OR:10.57, 95% CI: 2.6 to 41.6, P-0.0008) as an independent factor associated with BRAFV600E mutation. 17 patients with distant metastases were negative for both BRAFV600E or TERTpromoter mutation. No significant association was observed between age, gender, PTC variants, extra-thyroidal extension, lymphovascular invasion, multifocality, RAI administration and event rate with BRAFV600E mutation. Irrespective of BRAFV600E mutation, radioiodine refractory status (p-0.0001) had a reduced EFS probability.ConclusionIn paediatric & young adult PTC, TERTpromoter mutation is absent and BRAFV600E mutation is not associated with distant metastasis. The prevalence rate of the BRAFV600E mutation is much lower compared to adult PTC patients.

              Schizoaffective disorder is a mental disorder accompanied by schizophrenic and affective symptoms that both stand out at one time. Affective symptoms that appear are manic, depressive or both. The prevalence of patients with schizoaffective disorder is about 0.3% of the general population. Women suffer more from schizoaffective disorder and usually suffer from depressive type schizoaffective disorder. The case that will be discussed here is a young adult woman who first suffered from a schizoaffective mixed type disorder. The difficulty faced in handling this case is to provide an understanding of the patient and family about schizoaffective disorders and how patients take medication regularly for a long time. Therefore, a biopsychosocial approach is considered the most suitable to overcome the difficulties in handling this case.

<em>Background</em>. The aim of the study is to assess the long-term secondary effects of personal experience with the H1N1 pandemic of 2009/2010 and the perception of the institutional reaction to it on Italians’ willingness to get vaccinated in case of a novel influenza pandemic. <em><br />Design and Methods</em>. We conducted 140 face-to-face interviews in the Registry Office of the Municipality of Milan, Italy, from October to December 2012. <em><br />Results</em>. Willingness to get vaccinated during a novel influenza pandemic was best predicted by having been vaccinated against the seasonal flu in the past (OR=5.18; 95%CI: 1.40 to 19.13) and fear of losing one’s life in case of an infection with H1N1 (OR=4.09; 95%CI: 1.68 to 9.97). It was unaffected by the assessment of institutional performance. <em><br />Conclusions</em>. The findings of this study do not point to long-term secondary effects of the institutional handling of the H1N1 pandemic. The results highlight the fact that behavioural intention is not the same as behaviour, and that the former cannot simply be taken as an indicator of the latter.

At National Stock Exchange (NSE), the largest by market capitalization and the most liquid stocks, which form the majority of free float market capitalization of the exchange, have traded futures contracts. The equity stock futures segment in India has recorded very high growth in trading volume and turnover for more than one decade where majority trade happens on NSE. The primary objective of this study is to investigate the dynamic linkages between equity stock futures and their underlying spot markets. The article examines market efficiency and the causal relationship between single-stock futures and underlying stocks traded at NSE, by employing Johansen cointegration test, a test for autoregressive (AR) order of basis, vector error correction model (VECM) and impulse response functions. The result shows the existence of long-run equilibrium relationship between equity stock futures and their underlying stocks. The spot market is found to play a lead role in correction of any short-run disequilibrium towards long-run equilibrium, for the majority of stocks. Both spot and future markets contribute in price discovery and neither of the markets display considerably higher information efficiency compared to the other. In contrast, the study also reveals possibilities of arbitrage opportunity between the equity stock futures market and the underlying spot market in absence of transaction costs possibly due to faster correction of short-run disequilibrium by spot prices.
