Background: The treatment of methicillin-resistant staphylococcal infections has been a growing problem both in and out of hospitals for the past 30 years. Therefore, there is a need for other antibiotics as an alternative to glycopeptides in the treatment of methicillin-resistant staphylococcal infections. This study investigated the in vitro susceptibility of 49 methicillin-resistant Staphylococcus aureus (MRSA) and 59 methicillin-resistant coagulase negative staphylococci (MRCNS) clinical isolates to daptomiycin, telithromycin, tigecyclin, quinupristin/dalfopristin, and linezolid. Methodology: The identification of the strains was made by conventional methods. Antibiotic susceptibility tests were performed according to CLSI. Methicillin resistance was determined by cefoxitin disk. Susceptibilities of the strains to daptomycin, quinupristin/dalfopristin, tigecycline, and vancomycin were performed using the E-test according to the recommendations of CLSI 2011 and the manufacturer. Results: Two strains of MRCNS were resistant, and one was teicoplanin intermediate. It was found that one (2%) strain of MRSA and two (3%) strains of MRCNS were resistant to tigecyclin. Telithromycin resistance was detected in 33% of MRSA strains and 37% of MRCNS strains. Inducible clindamycin resistance was found in nine (18.4%) strains of MRSA and eighteen (30.5%) strains of MRCNS. All strains were susceptible to daptomiycin, quinupristin/dalfopristin, and linezolid. Conclusions:Although it has recently been used, telithromycin has a high percentage of resistance; its use for methicillin-resistant staphylococcal strains, therefore, should be limited. Daptomycin and quinupristin/dalfopristin were found to be effective against MRSA and MRCNS strains and were concluded to be a good choice in the treatment of methicillin-resistant staphylococci.

Introduction: Erythromycin resistant Staphylococcus isolates with inducible resistance appear sensitive to clindamycin in in?vitro sensitivity testing. If clindamycin is used for treatment of such isolates, selection for constitutive mutants may lead to clinical failure. Current study was conducted to detect the presence of inducible clindamycin resistance in erythromycin resistant Staphylococcus isolates by disk diffusion method (D test). To correlate clindamycin resistance phenotypes with minimum inhibitory concentrations (MICs) of clindamycin, erythromycin, oxacillin and vancomycin among the isolates. To correlate various resistance phenotypes with methicillin resistance. Material and Methods: 150 non duplicate isolates of Staphylococcus species were identified and antibiotic susceptibility testing was done using Kirby Bauer’s disc diffusion method. MICs were determined using E?test for oxacillin, vancomycin, clindamycin and erythromycin using E?test strips (Himedia) Results: Among 150 staphylococcus clinical isolates, 96 were of S. aureus and 54 were coagulase negative Staphylococci (CONS). About 81.2% of the S.aureus isolates and 72.2% of the CONS were found to be methicillin resistant. Inducible clindamycin resistance was found in 39.3% of the isolates, constitutive resistance phenotype in 48% while 12.7% demonstrated MS phenotype. 18% and 11.3% of all the isolates had MICs for clindamycin between 0.01?0.06 μg/ml and 0.06?0.1 respectively. 12.5% had MIC ranging from 4?8 μg/ml while 58% had MIC > 8 μg/ml. Constitutive resistant phenotype (cMLS) was the predominant phenotype in methicillin resistant isolates. MS phenotype was the predominant among MSSA (methicillin sensitive S. aureus) while MSCNS (methicillin sensitive CONS) cMLS (46.7%) predominated. MIC of all erythromycin resistant isolates were ≥ 240 μg/ml. Nearly 16.7% of the cMLS and 57.9% of MS isolates were found to be oxacillin sensitive and 83% of iMLS and 83.3% of MS phenotype isolates were oxacillin resistant on MIC testing. 47.2% of cMLS and 73.6% of MS isolates had MIC ≤ 2 μg/ml for vancomycin and 52.7% of cMLS and 26.3% of MS isolates had MICs in intermediate range for vancomycin. Conclusions: D?testing might help clinicians to decide whether to use clindamycin in Staphylococcal infections when erythromycin resistance is present. This study was carried out to evaluate the antimicrobial resistance (AMR) as a risk factor associated with some microorganisms isolated from subclinical mastitis (SCM) milk samples from Holstein Friesian dairy animals in Fayoum area, Egypt. The percentage of the SCM in the farm was found to be 41.18% and 63.88% at quarter and cows level respectively, with mean somatic cell count (SCC) of 8.8×105 ± 9.2×103 cells/mL and electrical conductivity (EC) 6.27 ± 0.066 mS/cm for SCM quarter milk samples. Out of the total 444 SCM cow milk samples, the most often isolated microorganisms were Staphylococcus aureus: 296 (66.6%), Enterococcus spp.: 230 (51.80%), Escherichia coli: 210 (47.29%) and Streptococcus agalactiae: 106 (23.87%). AMR was determined by disc diffusion test and the corresponding resistance genes were detected by PCR. Results of the in vitro susceptibility tests performed and the phenotypes indicated that the highest resistance to antibiotics for isolated microorganisms was against penicillin followed by amoxicillin + clavulanic acid, oxacillin and tetracycline, whereas moderate resistance was exhibited to oxytetracycline, ampicillin, sulfamethazole/trimethoprim, cefotaxime and erythromycin. However the most effective antibiotics against most isolates were nitrofurantoin and gentamicin followed by enrofloxacin, norfloxacin and cefoxitin. It was shown that the resistance to tetracyclines was due to the tetK or tetA(A) genes, the resistance to β-lactams (penicillins) – to blaZ and blaTEM genes, to macrolides (erythromycin): to ermB and ermC genes. Methicillin resistance genes were mecA, mec1 and mecC, glycopeptides (vancomycin) resistance gene was vanA, and norfloxacin resistance was attributed to norA gene.

ABSTRACT To understand quinupristin-dalfopristin resistance among clinical isolates of gram-positive bacteria in Taiwan, where this agent is not yet available for clinical use, we evaluated 1,287 nonduplicate isolates recovered from January 1996 to December 1999 for in vitro susceptibility to quinupristin-dalfopristin and other newer antimicrobial agents. All methicillin-susceptible Staphylococcus aureus (MSSA) isolates were susceptible to quinupristin-dalfopristin. High rates of nonsusceptibility to quinupristin-dalfopristin (MICs, ≥2 μg/ml) were demonstrated for the following organisms: methicillin-resistant S. aureus (MRSA) (31%), coagulase-negative staphylococci (CoNS) (16%),Streptococcus pneumoniae (8%), viridans group streptococci (51%), vancomycin-susceptible enterococci (85%), vancomycin-resistantEnterococcus faecalis (100%), vancomycin-resistantEnterococcus faecium (66%), Leuconostoc spp. (100%), Lactobacillus spp. (50%), andPediococcus spp. (87%). All isolates of MSSA, MRSA,S. pneumoniae, and viridans group streptococci were susceptible to vancomycin and teicoplanin. The rates of nonsusceptibility to vancomycin and teicoplanin were 5 and 7%, respectively, for CoNS, ranging from 12 and 18% for S. simulans to 0 and 0% for S. cohnii and S. auricularis. Moxifloxacin and trovafloxacin had good activities against these isolates except for ciprofloxacin-resistant vancomycin-resistant enterococci and methicillin-resistant staphylococci. In Taiwan, virginiamycin has been used in animal husbandry for more than 20 years, which may contribute to the high rates of quinupristin-dalfopristin resistance.

ABSTRACT In vitro susceptibility tests were performed to document the inhibitory activities of three nonfluorinated quinolone (NFQ) compounds (PGE 9262932, PGE 9509924, and PGE 4175997) compared to those of ciprofloxacin, levofloxacin, and trovafloxacin against 3,030 bacterial isolates. The spectra of the NFQ agents included most gram-positive species as well as quinolone-susceptibleEnterobacteriaceae. Ciprofloxacin-resistant, methicillin-resistant Staphylococcus aureus strains were inhibited by the NFQ series at ≤1.0 μg/ml. The NFQ compounds were not very active against Pseudomonas aeruginosa and most other nonfermentative gram-negative bacilli. Against other species, the potency of the NFQ agents was similar to that of trovafloxacin. Continued investigation of the NFQ compounds seems to be warranted.

Abstract Background Blood stream infections (BSIs) due to Gram-positive pathogens such as methicillin-resistant Staphylococcus aureus (MRSA) are associated with high mortality ranging from 10 to 60%. The current anti-MRSA agents have limitations with regards to safety and tolerability profile which limits their prolonged usage. Levonadifloxacin and its oral prodrug alalevonadifloxacin, a novel benzoquinolizine antibiotic, have recently been approved for acute bacterial skin and skin structure infections including diabetic foot infections and concurrent bacteremia in India. Methods The present study assessed the potency of levonadifloxacin, a novel benzoquinolizine antibiotic, against Gram-positive blood stream clinical isolates (n = 31) collected from January to June 2019 at a tertiary care hospital in Mumbai, India. The susceptibility of isolates to antibacterial agents was defined following the Clinical and Laboratory Standard Institute interpretive criteria (M100 E29). Results High prevalence of MRSA (62.5%), quinolone-resistant Staphylococcus aureus (QRSA) (87.5%), and methicillin-resistant coagulase-negative staphylococci (MR-CoNS) (82.35%) were observed among bacteremic isolates. Levonadifloxacin demonstrated potent activity against MRSA, QRSA, and MR-CoNS strains with significantly lower minimum inhibitory concentration MIC50/90 values of 0.5/1 mg/L as compared with levofloxacin (8/32 mg/L) and moxifloxacin (2/8 mg/L). Conclusion Potent bactericidal activity coupled with low MICs support usage of levonadifloxacin for the management of BSIs caused by multidrug resistant Gram-positive bacteria.

For more than 30 years, penicillin has been the agent of choice for pneumococcal infections. During this time the majority of strains of Streptococcus pneumoniae have been highly susceptible to penicillin. However, during the last ten years there have been sporadic reports of pneumococci with increased resistance to penicillin. The case report of an 18-month-old white boy with meningitis due to a strain of S. pneumoniae with increased resistance to penicillin is presented. The MIC of the organism to penicillin was 0.2µg/ml and the MBC 0.39µg/ml. The patient had normal immunity and no demonstrable sequestered focus of infection but failed to respond to appropriate doses of intravenous penicillin. Treatment with chloramphenicol caused a dramatic bacteriologic and clinical response. This experience reemphasizes the existence of pneumococcal strains of intermediate penicillin sensitivity and the importance of in vitro susceptibility tests.

Abstract Objective.—To assess the performance of clinical microbiology laboratories in the United States when conducting in vitro susceptibility tests with Streptococcus pneumoniae. Methods.—The results of a nationwide College of American Pathologists Proficiency Survey test sample, in which susceptibility testing of an isolate of S pneumoniae was performed, were assessed with respect to precision and accuracy. Results.—Wide variability was noted among participating laboratories with both minimum inhibitory concentration procedures and disk diffusion susceptibility tests when both methods were applied to S pneumoniae. Despite this high degree of variation, categorical interpretive errors were uncommon. Numerous laboratories reported results for antimicrobial agents that are not recommended by the National Committee for Clinical Laboratory Standards for tests with S pneumoniae. Conclusions.—Current susceptibility testing practices with S pneumoniae in the United States indicate limited precision and a tendency for laboratories to test and report results obtained with antimicrobial agents of questionable therapeutic value against this organism. Continued efforts to standardize susceptibility testing of S pneumoniae in the United States are warranted. In addition, modifications of existing interpretive criteria may be necessary.

ABSTRACT: In order to assess the prevalence of Methicillin-resistant Staphylococcus pseudintermedius from skin and nostrils of dogs with pyoderma, to determine its in vitro susceptibility, and to correlate these data with the presence of the mecA gene, 43 dogs were selected. Samples were collected from secretion of their skin lesions and right nostril, cultured, and analyzed for phenotypic and genotypic characteristics of the bacteria studied. In 62 samples (91%) the microorganism was classified as S. pseudintermedius. The rate of resistance against antibiotics ranged from 7% (amikacin; 4/62) to 77% (sulfamethoxazole + trimethoprim; 48/62). Resistance against oxacillin was found in 34% of the samples (21/62). Twenty-five samples (37%) were strains that carried the mecA gene. A significant correlation (P<0.01) was found between presence of the mecA gene and oxacillin resistance. Seventeen dogs were mecA gene carriers, and 8 (47%) of them had the gene in the skin lesions and nostril. A significant correlation (P<0.01) was also observed between the presence of mecA gene in the skin lesions and nostrils. Oxacillin resistance in vitro can be safely used to indicate the presence of mecA gene in MRSP samples. The nostrils can be a reservoir of MRSP in dogs.
