Changing the Face of War through Telemedicine and Mobile E-Commerce

The general perspective of this chapter is designed to relate the rationale used by the Department of Defense (DoD) for the military to adapt the principles of e-commerce to Telemedicine to meet increasing global crises, and to find ways to more effectively manage manpower and time. A mobile telemedicine package has been developed by the Department of Defense to collect and transmit near-real-time, far-forward medical data and to assess how this Web-based capability enhances medical management of the battlespace. Telemedicine has been successful in resolving uncertain organizational and technological military deficiencies and in improving medical communications and information management. The deployable, mobile teams are the centerpieces of this telemedicine package. These teams have the capability of inserting essential networking and communications capabilities into austere theaters and establishing an immediate means for enhancing health protection, collaborative planning, situational awareness, and strategic decision-making through Web-based Internet applications.

The paper also develops Critical Operational Issues (COI) and Measures of Effectiveness (MOE) as methodologies for investigating military utility of telemedicine. In order to meet increasing global crises, the U.S. military must find ways to more effectively manage manpower and time. A mobile Telemedicine package has been developed by the Department of Defense to collect and transmit near-real-time, far-forward medical data and to assess how this Web-based capability enhances management of the battlespace. Telemedicine has been successful in resolving uncertain organizational and technological military deficiencies and in improving medical communications and information management. The deployable, mobile teams are the centerpieces of this telemedicine package. These teams have the capability of inserting essential networking and communications capabilities into austere theaters and establishing an immediate means for enhancing health protection, collaborative planning, situational awareness, and strategic decision making through Web-based internet applications. In order to supplement this mobile commerce aspect of telemedicine, U.S. Navy ships have been utilized to integrate voice commerce interactive technologies to improve medical readiness and mobility. An experimental group was tasked to investigate reporting methods in health and environmental surveillance inspections to develop criteria for designing a lightweight, wearable computing device with voice interactive capability. This chapter is also designed to relate the rationale used by the Department of Defense and the Test and Evaluation (T&E) Integrated Product Team, in order to determine the military utility of the Joint Medical Operations—Telemedicine Advanced Concept Technology Demonstration (JMO-T ACTD) and continuous voice activation applications. Voice interactive computing devices are used to enhance problem solving, mobility and effectiveness in the battlespace. It improves efficiency through automated user prompts, enhanced data analysis, presentation, and dissemination tools in support of preventive medicine. The device is capable of storing, processing, and forwarding data to a server. The prototype devices have enabled quick, efficient, and accurate environmental surveillance. In addition to reducing the time needed to complete inspections, the device supported local reporting requirements and enhanced command-level intelligence. This chapter further focuses on developing a holistic model of implementing a strategy for mobile telemedicine. The model synthesizes current thinking on transformation into a holistic model and also explains the integrative influence of vision on the other four model components: environment, people, methodology, and IT perspective. The model was tested by Testing and Evaluating (T&E) the JMO-T ACTD. JMO-T ACTD has developed a very successful training program and is very aware of the importance of planned change. Top military officials, such as the Commander in Chief (CINC), are actively involved in change and are committed to people development through learning. The model served an applied purpose by allowing insights into how well the military organization fit current theory. The model also fit a theoretical purpose by organizing a holistic, comprehensive framework. Accordingly, we have organized and synthesized the literature into five interrelated components that act as a fundamental guide for research. The model also helped to identify a theoretical link and apply it to the internal operations of the military and its adaptation of mobile e-commerce principles to more effectively deliver telemedicine benefits to military personnel.

One objective of this chapter is to relate the rationale used by the DOD to determine the military utility of the Joint Medical Operations–Telemedicine Advanced Concept Technology Demonstration (JMO-T ACTD) or ACTD for short. ACTD is a JMO-T initiative developed for the purpose of improving joint medical planning capabilities, access and timeliness of medical care, and integration of medical situational awareness. The chapter discusses in detail the complexities involved in the ACTD initiative. A second objective of the chapter is to articulate the development of Critical Operational Issues (COIS) and Measures of Effectiveness (MOE) as methodologies for investigating the military utility of telemedicine.

The purpose of this chapter is to provide an overview of the operations research techniques employed across the Department of Defense, focusing on how the operations research officers and government civilians contribute to senior leader decisions. There are four general areas that the Department of Defense conducts operations research: assessing strategic goals through a gap determination process; assessing the effectiveness of operations and exercises, wargaming, modeling, and simulation of combat for strategic analysis and acquisition; and personnel analysis across the Joint Force. The authors will describe the management processes and components of military only so far as needed to provide the context and background required to adequately describe the operations research techniques. This chapter will contribute to the overall mission of this book by generalizing the techniques used by the military to demonstrate how analysts can better evaluate proposed and existing programs throughout the government and the public sector.

In the era of the Industrial Revolution 4.0. this is where technology and technology have been very advanced demanding competent human resources who have high morale and discipline in carrying out their roles and functions for both the individual and the objectives of the organization. In the role and contribution of STTAL in the Industrial Revolution 4.0, the current conditions, where the quality of human resources both students, lecturers, and Antap STTAL are considered unable to answer the challenges in the cyber field so that strategic decision-making and steps are needed to be able to accompany the development of science and technology while carrying out missions STTAL is to hold a higher education program to produce human resources with Indonesian character and personality, quality and relevant science and technology based, with defense needs in the military, maritime and maritime fields. PurposeFirearms account for the majority of suicide deaths in the US military and general population. The percentage of suicides resulting from firearms is higher in the military, however, and as such, the ratio of non-lethal to lethal suicide attempts is lower in the military than in the general population. In 2013, Congress passed the National Defense Authorization Act, which facilitated a Department of Defense (DoD) shift toward allowing commanding officers and clinicians to inquire about personal firearms with service members perceived as being at risk and also began giving free cable locks to firearm-owning military personnel. The purpose of this paper is to provide a preliminary understanding of the effectiveness of this change, the authors examined trends in firearm suicide attempts within the US military and general population from 2010 to 2015.Design/methodology/approachData on non-lethal and lethal suicide attempts overall and within specific methods were extracted from the Department of Defense Suicide Event Report and the Centers for Disease Control and Prevention’s Web-based Injury Statistics Query and Reporting System (2011–2015).FindingsContrary to expectations, firearms were not utilized in a smaller proportion of suicide attempts within the military post-law change. Consistent with expectations, however, the ratio of non-lethal to lethal suicide attempts increased, particularly after the change in law, with the ratio in the military converging somewhat with that of the general population.Originality/valueOverall, results were mixed, with only limited and tangential evidence that the change in law has proven effective. More precise data collection will be required in order to fully evaluate such laws.
