Simultaneous Implantation of Dental Implants and Autogenous Human Dentin

In our previous clinical study, autogenous demineralized dentin matrices (DDM) prepared from the functional vital teeth (#38, #41) of thirty-five-year-old female were grafted on the bone defect, using newly developed mill, and then received to the host without troubles. In this study, we implanted the human tooth dentin adjusted previously and the dental implants into the regions of missing tooth simultaneously. Fifty-seven-year-old female presented with missing teeth (#35-#37, #45-#47). First, a non-functional vital tooth (#18) were extracted and cryopreserved immediately. 11 months after extraction, the tooth was crushed by newly developed auto-crash mill using ZrO2 vessel and ZrO2 blade for 1 minute. The crushed granules were demineralized completely in 2% HNO3 solution, rinsed in cold distilled water and lyophilized (granule size: 0.5-2.0mm). The bacteria-free of the DDM were confirmed by the bacteriological examination before use. Drilling of the prospective implant beds were then performed according to the manufacture’s protocol and a screw-type rough surface implants (Nobel Biocare® Mk III) were placed. The adjusted DDM granules were implanted into the bone defect (#45). There are no postoperative complications at 3 years after implantation. This case indicates that the preserved autogenous DDM can be used as collagenous biomaterials with osteoinductive potency.

1. Background: Loss of permanent teeth is very common, affected chewing function, speech and aesthetics; restoration of missing teeth with dental implant has several advantages, but we need thoroughly study the clinical and X ray features at the position at missing teeth, then to select the type of implant and make the best plan for the dental implant patients. 2. Materials and method: Cross-section descriptive study. From January 2009 to November 2010, study with 56 patients with 102 implants of MIS and Megagen systems at the Faculty of Odonto-Stomatology, Hue College of Medicine and Pharmacy and Vietnam-Cuba Hospital in Ha Noi. 3. Results: Distributed equally in male and female, common ages 40 – 59 (55,4%), the majority of missing teeth occurs in the lower jaw (63,8%) and especially, the teeth 36 and 46 (25,4%). The majority of missing teeth due to dental caries, dental pulp and apical diseases (64,7%) of the molar teeth (51,9%); the most position of missing tooth have enough bone for dental implants (87,3%), time of tooth loss and bone status in the position of tooth loss are related to each other (p < 0,01). Diameter and length of implant usually used 4.0 – 6.0 mm (63,7%) and 8.5 – 13.0 mm (83,3%). 4. Conclusion: Clinical and X ray features of edentulous patients has an important role in determining the type of implants and treatment planning of dental implants. AbstractDental implants are commonly used to repair missing teeth. The implant surface plays a critical role in promoting osseointegration and implant success. However, little information is available about which implant surface treatment technology best promotes osseointegration and implant stability. The aim of this network meta-analysis was to evaluate the osseointegration and stability of four commonly used dental implants (SLA, SLActive, TiUnite, and Osseotite). The protocol of the current meta-analysis is registered in PROSPERO (International Prospective Register of Systematic Reviews) under the code CRD42020190907 (https://www.crd.york.ac.uk). We conducted a systematic review following PRISMA and Cochrane Recommendations. Medline (PubMed), Cochrane Library, Embase, and the Web of Science databases were searched. Only randomized controlled trials were considered. Twelve studies were included in the current network meta-analysis, eleven studies were included concerning the osseointegration effect and five studies were included for stability analysis (four studies were used to assess both stability and osseointegration). Rank possibility shows that the SLActive surface best promoted bone formation at an early healing stage and TiUnite seemed to be the best surface for overall osseointegration. For stability, TiUnite seemed to be the best surface. The present network meta-analysis showed that the SLActive surface has the potential to promote osseointegration at an early stage. The TiUnite surface had the best effect on osseointegration regarding the overall healing period. The TiUnite surface also had the best effect in stability.

Abstract Dental implants are the new era in the field of dentistry providing the new opportunities to the clinicians to manage their patients with missing teeth. The procedure is more technique sensitive in maxilla than mandible. The scenario becomes even more challenging with severely resorbed maxillary arches. The idea of zygomatic bone implants put forward the new approach to manage such patients. Introduction: Non-syndromic tooth agenesis defined as developmental absence of more than one tooth that appears as independent congenital oral trait. Its prevalence, pattern and distribution rates vary by populations. The aim of this study was to identify the pattern and distribution of tooth agenesis in permanent dentition among IIUM dental polyclinic patients. Materials and Methods: 3481 patients aged 12 to 60 years who attended IIUM Dental Polyclinic from April 2016 until April 2017 was evaluated. 33 hypodontia and 2 oligodontia were ruled out. Patients were required to undergo clinical and radiographic examinations. Results: On an average, two teeth were missing per patients. Excluding third molar, the prevalence of population under study was 1.005%. Half of the total cases showed missing a single tooth (45.7%), whereas the others ranged from two until nine. The most common missing tooth in maxillary and/or mandibular are lateral incisors (28%) followed by second premolars (26.67%) and first premolars (25.33%). There was no significance difference in missing tooth between male and female (p=0.48). More missing teeth identified at maxilla (54.67%) compared to mandible (45.33%). More teeth were found missing on the right side (54.67%), on the posterior region (53.33%) and also unilaterally (71.42%). Tooth anomalies such as peg-shaped tooth (8.57%) and microdontia (5.71%) were found in some cases. Conclusion(s): In conclusion, mild hypodontia is considered as the most common tooth agenesis. The prevalence studied population was less than the previous studies in Malaysia and lateral incisors were identified as the most common missing tooth.
