Maxillary canine is the most frequently impacted tooth in the dental arch and twice common in females than in males. Treatment of impacted maxillary canine can be difficult and time consuming, depending on its position. Improper direction and magnitude of applied force can lead to increased chances of adjacent tooth resorption. This article describes about a simple cantilever spring that can be fabricated at chair side for extrusion of a bucally impacted canine.

ABSTRACT Treatment of asymptomatic impacted maxillary canines in adults is inevitable when primary canine becomes lost through extraction or exfoliation or when the impacted tooth becomes symptomatic. Treatment alternatives include an orthodontic procedure to bring the unerupted tooth to the dental arch or prosthetic replacement of the missing tooth. This is an alternative treatment that involves simultaneous placement of implants into extraction sockets of the teeth for replacement of maxillary canine. This treatment modality avoids the need for conventional preparation of teeth as part of prosthetic reconstruction or prolonged orthodontic treatment aimed at bringing the impacted canine to the dental arch. Latar belakang: suatu kasus impaksi gigi dapat menyebabkan maloklusi, dan kelainan oklusi akan semakin bertambah dengan bertambahnya usia. Impaksi gigi kaninus merupakan gigi kedua setelah gigi molar ketiga yang berfrekuensi tinggi untuk mengalami impaksi, persentasenya sekitar 12%-15% dari populasi. Gigi supernumerary adalah gigi tambahan/berlebih, sehingga jumlah gigi yang terbentuk dalam rahang lebih banyak dari jumlah normal. Terjadinya impaksi gigi kaninus dan supernumerary secara bersamaan jarang terjadi. Tujuan: Menambah wawasan du bidang bedah mulut minor, terutama dalam menangani suatu kasus impaksi gigi kaninus dan supernumerary untuk perawatan orthodonti. Kasus dan Penanganan: Dilaporkan seorang apsien, wanita, berusia 38 tahun yang baru menyadari kelainan maloklusinya dengan keluhan gigi depannya bertambah maju akibat adanya impaksi gigi kaninus dan impaksi gigi supernumerary. Pasien dikonsulkan dari Bagian Orthodonti ke Bagian Bedah Mulut untuk penanganan impaksi gigi kaninus dan supernumerary dengan kemungkinan untuk mempertahankan gigi kaninus memulai pembedahan. Dengan berbagai pertimbangan, penderita pada akhirnya menjalani operasi pengambilan gigi kaninus dan gigi supernumerary di Bagian Bedah Mulut RSGM Prof. Soedomo. Langkah-langkah diagnosis, operasi dan berbagai kemungkinan komplikasi juga turut disertakan di dalam pembahasan. Kesimpulan: Pengambilan gigi kaninus dan gigi supernumerary yang terpendam merupakan pilihan perawatan jika tidak memungkinkan untuk dilakukan exposure pada impaksi gigi kaninus pada maksila. Background: Impacted canines is the second most impacted tooth after third molar impaction, approximately 12%-15% of the population present with impacted canines. A supernumerary tooth is one that is additional to the normal series and can be found in almost any region of the dental arch. The incidence of an impacted canines as a sequent with a supernumerary tooth is very rare. Purpose: The aim of this case report is to add more information about a minor surgery due to canine and supernumerary tooth impaction for orthodontic treatment. Case and Management: We reported a case of a woman, 38 years old who have noticed a malocclution through the forwardness movement of her anterior teeth, due to the present of impacted canine and supernumerary. The patient consulted from orthodontic department to oral and maxillofacial department for further assessment, treatment and also the probability for surgical exposure of impacted canine. We have decided to do odontectomy for the impacted canine and supernumerary tooth as well at Oral and Maxillofacial department, Prof. Soedomo Hospital. The diagnosis process, exposure of impacted canine considerations are also discussed. Conclusion: the odontectomy for impacted canine and supernumerary teeth had performed as last options if there is impossible to do an exposure of an impacted canine.

Background: The permanent maxillary canine is the most impacted tooth after third molars. There are many possible surgical techniques to approach impacted canines. The literature reports that high-intensity laser therapies (HILTs) can be used for opercolectomy of an impacted tooth. The aim of this study is to propose a new orthodontic-surgical approach using a laser for the disinclusion of palatally impacted canines. Methods: Nine patients presented maxillary primary canine persistence in the dental arch. Orthopanoramic X-ray and cone beam computed tomography (CBCT) showed the impaction of permanent maxillary canines. Surgical exposure was performed using a diode laser (Raffaello, DMT, Lissone, Italy, 980 nm + 645 nm). No orthodontic devices were applied for impacted tooth traction. Canine movement was monitored at 1, 8, 16 weeks post-surgery with photos and a CS3500 intraoral scanner (Carestream Dental, Atlanta USA) to evaluate their autonomous eruption. Results: No complications were observed. In all the cases, complete disimpaction of the treated canines was obtained in only four months. Conclusions: Impacted canine exposure with a diode laser has many advantages if compared with traditional surgery. The pre-orthodontic exposure and autonomous eruption of impacted canines provided simplified and predictable outcomes. The absence of traction and the reduced time for orthodontic treatment increased compliance during the orthodontic alignment.

Impaksi palatal kaninus sering mengakibatkan keluhan secara estetis. Faktor genetik merupakan faktor yang dominan serta beberapa faktor yang lain yaitu diskrepansi lengkung gigi, ukuran gigi, retensi gigi desidui, kerusakan dini, pencabutan dini , posisi yang abnormal benih gigi, agenese incisivus lateral dan kista. Tujuan laporan kasus ini adalah untuk memberikan informasi tentang perawatan kasus impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg. Pasien perempuan umur 15 tahun, datang ke klinik ortodonsia RSGM Prof. Soedomo FKG UGM. Pemeriksaan subyektif, pasien terganggu dengan keadaan gigi depan yang maju dan bercelah. Pemeriksaan obyektif menunjukkan adanya rudimenter, agenese gigi incisivus lateral kanan dan kiri rahang atas, impaksi palatal kaninus kiri atas. Transposisi kaninus kanan atas ke ruang incisivus lateral kanan atas, pergeseran midline. Maloklusi angle kelas I dengan relasi skeletal kelas I dengan bimaksiler retrusif disertai protrusif incisivus maksila dan retrusif incisivus mandibula. Perawatan diawali dengan pencabutan gigi rudimenter. Tahap I menggunakan multiloop archwire untuk leveling dan unraveling gigi anterior, koreksi pergeseran midline. Tahap berikutnya adalah pemasangan button pada kaninus impaksi untuk mengaitkan kawat ligatur pada archwire yang berfungsi untuk menarik kaninus impaksi palatal pada lengkung gigi. Perawatan ortodontik pada kasus dengan impaksi palatal kaninus rahang atas pada maloklusi kelas I skeletal menggunakan alat cekat teknik Begg dapat dilakukan dengan hasil perawatan yang baik. ABSTRACT: Repositioning of Palatally Impacted Canine in Orthodontic Treatment Using Begg Fixed Appliance. Palatally impacted canine often leads to esthetic complaints. Genetic factor is dominant followed by such other factors as dental arch discrepancy, tooth size, retention of deciduous teeth, early decay, premature extraction, abnormal position of tooth germ, lateral incisor agenesis, and cysts. This article provides information about the treatment of palatally impacted maxillary canine case in a skeletal class I malocclusion using Begg fixed appliance technique. A 15-year-old female patient came to the orthodontia clinic of RSGM Prof. Soedomo FKG UGM. The subjective examination found that the patient was disturbed by her protrusive, gapped front teeth. Then, the objective examination indicated the presence of rudimentary, lateral incisor agenesis of right and left upper jaw, and upper left palatally impacted canine. In addition, there was a transposition of upper right canine to lateral incisor area as well as a midline shift. Angle class I malocclusion with class I skeletal relationship and bimaxillary retrusion along with maxillary incisor protrusion and mandibular incisor retrusion also occurred. The treatment began with rudimentary tooth extractions. The first stage used a multiloop archwire for leveling and unraveling of anterior teeth as well as correction of midline shift. The button attached to the impacted canine could tie the ligature wire to the archwire that served to attract the palatally impacted canine in the dental arch. The orthodontic treatment in cases of palatally impacted maxillary canine with skeletal class I malocclusion using Begg fixed appliance technique can be applied with a good treatment result.

Orthodontic movement of a maxillary canine into the dental arch may be a therapeutic challenge, and it is necessary to analyse a case in detail and to evaluate therapeutic methods, including a difficult decision of tooth extraction in all cases.<b> Aim</b>. The paper aims to discuss orthodontic indications for maxillary canine extraction based on available literature. <b>Material and methods</b>. The PubMed database and the following key words were used for studies: canine impaction, maxillary impacted canine and canine extraction. Literature was supplemented with publications from reviewed journals not included in the PubMed database. 50 papers were selected from the literature list. <b>Results, review and discussion</b>. A decision to extract a maxillary canine should be made based on a detailed clinical examination and analysis of orthodontic records, including photographs, diagnostic models and radiological scans. CBCT imaging test should be performed as standard before treatment in case of impaired tooth eruption. Factors that should be especially considered include: tooth anatomy, periodontal status, position in the bone, occlusion, condition of the adjacent teeth and any pathological processes in the tooth vicinity. Before making a therapeutic decision a patient should be informed about available therapeutic options according to current medical knowledge, as well as about advantages and disadvantages of each of them. <b>Summary</b>. Factors favouring extraction of impacted canines or canines with impaired eruption include: unfavourable position (a horizontal position of a tooth in the bone, position in the zone IV and V according to Kurol, large distance from the edge of the alveolar process), anatomical abnormality (root angulation, external or internal resorption), root ankylosis, patient’s age above 30 years and good occlusion without a canine in the dental arch with the need to extract a premolar for its movement into the dental arch. <b>(Baszak J, Sobieszczańska A, Dunin-Wilczyńska I. Background & Objective: Impacted teeth are one of the most common reasons for visit- ing dentists and maxillofacial surgeons. During clinical and radiographic examinations and evalu- ations, the dentist sometimes realizes that the patient’s problem is due to the impaction of one or more teeth. Therefore, it is necessary for every dentist to make the necessary and correct decision for the treatment of such teeth. One of the best ways to diagnose impacted teeth is first of all the absence of that tooth in the desired location and clinical evaluation of the patient and secondly to check the radiographic view of the desired area. Impacted teeth can be the source of many prob- lems for various reasons, so in most cases, their extraction is recommended. Knowing how to place the impacted tooth and determining its type and other characteristics of the impaction can help the dentist in choosing the appropriate treatment-surgical method and also prevent complications during surgery. Proper and timely diagnosis of impaction as well as determining its position (buc- cal-palatal-intermediate) in the jawbone can reduce damage to adjacent structures and also affect the treatment plan. Because accurate detection of impacted tooth positions is possible with radiog- raphy, the most accurate radiograph to examine the impacted tooth is cbct images. Therefore, due to possible occlusion injuries and the effect of occlusion pattern on the treatment of these teeth, it is important to study the types of impaction patterns. Therefore, this study was presented with the aim of investigating the impaction pattern of maxillary canine teeth in Ardabil with cbct archive. Materials and Methods: In this descriptive cross-sectional study, CBCT images of 239 patients who met the inclusion criteria were evaluated. Due to the lack of statistics, due to the lack of statistics of maxillary impacted canine teeth in Ardabil, the counting method was used for sampling and stereotypes were identified as maxillary impacted canine teeth as the sample size. The obtained data were analyzed in SPSS24 software. Results: Among the impacted canine teeth studied, 63.2% had palatal, 17.5% buccal and 19.3% had intermediate pattern. Also, 11.2% had damage to adjacent structures and 88.8% had no damage to adjacent structures. Among the types of damage to adjacent structures, 46.9% caused root re- sorption of the first premolars, 34.5% caused root canal resorption, and 18.6% had other damage to adjacent structures. Of these, 50.4% had root curvature and 49.6% had no root curvature, of which 12.1% had severe curvature, 38.7% had mild curvature and 49.2% had severe curvature. Also, the average angle of the incised canine to the lateral incisor was 41.7%. 9% of impacted canine teeth had a root resorption of lateral incisors and 9% did not have a root resorption of lateral incisors. Conclusion: Dentists should treat cases such as maxillary latent canine such as: Examine the occlusion pattern, angle to adjacent teeth, damage to adjacent structures, curvature of the incised canine root, and root resorption of adjacent teeth.

Tooth impaction is the retardation in the eruption pattern of a tooth. One of the most commonly impacted teeth is the maxillary canine. However, impaction of mandibular canines is not as common as maxillary canines. Treatment of such impacted teeth usually involves surgical exposure, followed by bonding of an orthodontic attachment to facilitate extrusive movement of the impacted tooth. However, some side-effects on other teeth can be expected which includes the intrusion and tipping of adjacent teeth. In order to prevent side-effects on the adjacent teeth, we present a modified uprighting spring used to extrude an impacted canine.

In orthodontics, the impaction of maxillary canine is a common problem. Treatment for this clinical hitch usually requires surgical exposure of the impacted tooth, followed by orthodontic traction to align the tooth in the arch. To expose labially impacted canine, several methods are used in literature. In this article, we throw light on an innovative technique showing the surgical procedure for uncovering impacted canine by using a punch biopsy instrument.

Objectives: Maxillary impacted canines have an important place in orthodontics for the reason of being a common anomaly and its treatment is time consuming and difficult. The aim of this study was to investigate the mandibular indexes and cephalometric parameters in adolescents with impacted maxillary canines. Materials and Methods: The radiographs of 4026 patients aged 12–19 years old were scanned and a total of 155 patients were included in the study. Subjects were divided into three groups according to the permanent maxillary canines: Bilateral impacted canine (BIC) (52 subjects; mean age 15.02 ± 1.74 years), unilaterally impacted canine (UIC) (51 subjects; mean age 14.89 ± 1.39 years), and the control (52 subjects; mean age 14.84 ± 1.65 years) groups. Skeletal, soft tissue and dental measurements were performed on cephalometric radiographs. Mandibular index analyzes including the panoramic mandibular index (PMI), mental index (MI), antegonial index (AI), and gonial index (GI) measurements and also angular measurements of impacted maxillary canines were performed on panoramic radiographs. Data were analyzed statistically and P < 0.05 was considered significant. Results: Maxillary skeletal and dental measurements showed retrusive positioned maxillary dental arch in the sagittal direction in the impacted canine groups (P < 0.05). PMI and MI values were greater in the BIC and UIC groups than in control group (P < 0.05). There was no statistically significant difference in AI value among groups (P < 0.05). GI value of BIC group was found statistically lower than other groups (P < 0.05). Conclusion: Individuals with impacted maxillary canines have more retrusive maxillary dental arch and soft-tissue components. Furthermore, mandibular cortex was thicker in subjects with impacted maxillary canine than individuals with not and clinicians should keep in mind this when examine the panoramic radiographs in terms of early diagnosis of impacted maxillary canine teeth.

Complete tooth eruption, especially canine, will affect facial esthetic and the harmony. Surgical procedure will helpto expose an impacted tooth determining orthodontically into dental arch. Clinical and radiographic examinationshould be taken for locating the position of the tooth. The flap was designed differently between labially or palatallyimpacted canine. A 16 years old female came to clinic with Class III Angle dental and skeletal malocclusion with animpacted of upper left canine. She was treated with fixed orthodontic appliance using edgewise technique andsurgical exposure using closed-eruption technique. The proper diagnosis and management between oral andmaxillofacial surgery, and orthodontics will guarantee good result.
