Significance of Abuse Chemical Contamination of Returnable Dairy Containers: Hazard Assessment

Certain pesticide residues reabsorbed into milk (or water) contents when stored in LEXAN® resin and polyethylene plastic returnable milk containers that had been exposed to diluted pesticide products, washed, and subsequently filled. The significance of these laboratory test results is comprehensively discussed by attempting to characterize the nature of exposure to pesticide residues from a washed returnable dairy container, as opposed to exposure to milk containing pesticide residues of non-container origin. Laboratory findings are compared to actual public use experience with returnable plastic dairy containers. A hazard assessment is presented, which includes a comparison of quantities of pesticide residues found extracting with existing Federal milk tolerance standards, food tolerance standards, unavoidable contaminant food additive regulations, World Health Organization and Safe Drinking Water Committee acceptable daily intake values. The probability of purchase of milk that is contaminated with pesticide residues, where the washed returnable dairy container is the source, is presented.

Abstract Approximately 200 citrus samples from markets of the Valencian Community (Spain) were analyzed to establish their residue levels in 12 organophosphorus pesticide residues during the 1994-1995 campaign. The organophosphorus pesticides carbophenothion, chlorpyriphos, chlorfenvinphos, diazinon, ethion, fenitrothion, malathion, methidation, methylparathion, phosmet, quinalphos, and tetradifon were simultaneously extracted by matrix solidphase dispersion and determined by gas chromatography- mass spectrometry using selected ion monitoring mode. A total of 32.25% contained pesticide residues and 6.9% exceeded the European Union Maximum Residue Levels (MRLs). The pesticides found in the samples with residues above MRLs were carbophenothion, ethion, methidathion, and methyl parathion. Lower level residues of these and the other pesticides studied (except diazinon) were frequently found. The estimated daily intake of the 12 organophosphorus pesticide residues during the studied period was 4.87 x 10−4 mg/kg body weight/day. This value is lower than the provisional tolerances dairy intakes proposed by the Food and Agriculture Organization and the World Health Organization.

Background: Environmental contamination with elevated levels of copper (Cu), cobalt (Co), iron (Fe), zinc (Zn), lead (Pb), chromium (Cr6+), cadmium (Cd), and nickel (Ni)—all states of which are found in Uganda—raises health risk to the public. Pb, Cr6+, Cd, and Ni for instance are generally considered nonessential to cellular functions, notwithstanding the importance of the oxidative state of the metals in bioavailability. As such, we aimed in this study (i) to evaluate heavy metal concentrations in four vegetables from a typical open-air market in Uganda, (ii) to assess the safety of consuming these vegetables against the World Health Organization (WHO) recommended limits of heavy metals consumption, and (iii) to formulate a model of estimated daily intake (EDI) among consumers in the country. Methods: This was a cross-sectional study conducted in five georeferenced markets of Bushenyi district in January 2020. Amaranthus, cabbages, scarlet eggplants, and tomatoes were collected from open markets, processed, and analyzed by atomic absorption spectrometry. Modeled EDI, principal component (PCA) and cluster analysis (CA) were conducted to identify relationships in the samples. Results: The levels of essential elements in the four vegetables were found to fall from Co > Cu > Fe > Zn. Those of non-essential metals were significantly higher and followed the pattern Cd > Cr > Pb > Ni. The highest EDI values were those of Cu in scarlet eggplants, Zn in amaranthus, Fe in amaranthus, Co in amaranthus, Pb in cabbages, total Cr in scarlet eggplant, Cd in cabbages and tomatoes, and Ni in cabbages. In comparison to international limits, EDIs for Zn, Cu, Co and Fe were low while Ni in cabbages were high. PCA showed high variations in scarlet eggplant and amaranthus. The study vegetables were found to be related with each other, not according to the location of the markets from where they were obtained, but according to their species by CA. Conclusion: The presence of non-essential elements above WHO limits raises policy challenges for the consumption and marketing of vegetables in the study area. Furthermore, low EDIs of essential elements in the vegetables create demand for nutritious foods to promote healthy communities.

Patients with mesangial proliferative lupus glomerulonephritis (World Health Organization class II) are generally believed to have only mild to moderate proteinuria and normal renal function. However, there have been several reports of patients with mesangial lupus with nephrotic-range proteinuria. In this report, we present two additional cases and review the literature. Of seven reported cases, persistent nephrotic syndrome was observed in four, morphologic transformation occurred in three, and all but one presented with varying degrees of azotemia. These cases reinforce the concept that in systemic lupus erythematosus, laboratory findings may not correlate well with the underlying glomerular lesion, and therefore, the renal biopsy is an essential clinical tool in the approach to lupus nephritis.

Objective : Coronavirus disease 2019 (COVID-19) has been declared as an international public health emergency by the World Health Organization (WHO), with outbreaks in over 200 countries and causing over 390,000 deaths globally. ACE-2 receptors are highly expressed in the upper and lower gastrointestinal system, providing a prerequisite for SARS-CoV-2 infection in the gastrointestinal tract. In addition, over half of the COVID-19 patients have viral nucleic acid detected in their feces and almost one-quarter of the cases, the stool samples test positive even when respiratory samples are negative. The aim of this systematic review is to summarize literature and to evaluate the clinical characteristics of patients with positive viral RNA stool test for COVID-19 and if there is a possibility of fecal-oral transmission of SARS-CoV-2 virus.Method : This systematic review has been registered in PROSPERO (CRD42020183049). A systematic search of the literature for observational study and randomized control trial was conducted in PubMed central and Google Scholar through May 5th, 2020. Three reviewers independently searched and selected. The risk of bias was evaluated using Newcastle-Ottawa Quality assessment tool.Results : 340 articles were screened, then from which eight articles were selected. Of eight articles that were included in this study, we sought for three main categories of the clinical manifestation; gastrointestinal, respiratory, and others. Each study was reviewed systematically to gain demographic data and evidence regarding the possibility of fecal oral transmission in SARS-CoV-2. Two studies reported prolongation of positive stool test results after the respiratory specimen conversion to negative which support the theory of fecal oral transmission.Conclusion : In conclusion, diarrhea, cough, and fever are the most common clinical manifestations in COVID-19 patients with positive RNA stool test results. Fecal oral transmission may be possible due to the ACE-2 receptors in the lining of the gastrointestinal tract. RNA stool test should be used as addition in discharging COVID-19 patients.

Over the last years, the detection of pesticide residues in the official food surveillance programs of Chile has been increased, mainly in fresh vegetables such as tomatoes and lettuces. The Metropolitana Region of Chile presents the highest detections in the country. The lack of evaluations of toxicological risks in human health have increased uncertainty of the potential effects of pesticides exposures in the Chilean population. This research aims to determinate health risks assessment of pesticide residues associated to tomatoes and lettuces produced in Metropolitana Region. The findings of this study reveal that tomatoes and lettuces cultivated in the MR show more than 50% of samples with one or multiple pesticides residues. From the total samples, 16% were over the Chilean Maximum Residue Levels (MRLs). The main pesticides detected in tomatoes and lettuces were methamidophos, methomyl, difenoconazole, cyprodinil and boscalid. The results obtained using the official data of the Ministry of Health of Chile (MINSAL) compared to the World Health Organization (WHO), describe relevant risks through the Estimated Daily Intakes (EDI), Hazard Quotients (HQ) and Hazard Index (HI) for the Chilean population due to high concentrations of methamidophos, methomyl and cyprodinil. More restrictions for the use of methamidophos, methomyl, difenoconazole, cyprodinil and boscalid and effective control programs should be implemented in order to mitigate the impacts on the Chilean population.

Polychlorinated dibenzo-<em>p</em>-dioxins (PCDDs) and polychlorinated dibenzo-<em>p</em>-furans (PCDFs) are unintentionally formed during inefficient combustions and as a by-product. Due to their resistance to degradation and their toxic effect on health, PCDD/Fs are listed by the Stockholm Convention as persistent organic pollutants (POPs). Once released into the atmosphere, most of them are adsorbed to air particles and transported away from sources in atmosphere. India signed the Stockholm Convention India agreeing thereby to reduce and eliminate the use of POPs. The German agency for Technical Cooperation helped develop facilities for monitoring POPs at a national level in Delhi. This paper presents the data generated during a training assignment for Central Pollution Control Board officials at the German laboratory. Air borne particulate matter (&lt;PM<sub>10</sub>) was collected from 6 different locations in Delhi, India and analyzed in a German laboratory for 17 congeners of PCDD/Fs. The concentrations of &Sigma;PCDD/Fs ranged between 1720-9010 fg m<sup>-3</sup> (mean 5559 fg m<sup>-3</sup>) and their toxic equivalency values ranged from 67 to 460 fg I-toxic equivalent quantities (TEQ) m<sup>-3</sup>, with an average of 239 fg I-TEQ m<sup>-3</sup> which was lower than the ambient air standards. The dominant congeners were octachlorinated dibenzo-p-dioxin (OCDD), octachlorinated dibenzo-p-furans (OCDF), 1,2,3,4,6,7,8-heptachlorinated dibenzo- p-furans, and 1,2,3,4,6,7,8-heptachlorinated dibenzo-p-dioxin. The contributions of individual homologs for &Sigma;PCDDs/Fs I-TEQ was in the order of OCDD (31%)&gt;HCDF (21%)&gt;hexachlorodibenzofurans (13%)=OCDF (13%)&gt; HCDF (12%) and other individual congeners contribute less than 5%. High chlorinated congeners contributed with more than 80% for &Sigma;PCDD/Fs I-TEQ. Rough estimates of tolerable daily intake (TDI) shows low health risk of exposure to &Sigma;PCDD/Fs with inhalation of 0.098 pg I-TEQ kg<sup>1</sup>day<sup>1</sup> for adult and 0.152 pg TEQ kg<sup>-1</sup>day<sup>-1</sup> for children, which is much lower than World Health Organization recommended TDI for dioxins.

HCH isomers, endosulfan, malathion, chlorpyrifos, and methyl-parathion were monitored in human milk samples from Bhopal, Madhya Pradesh. The endosulfan concentrations were highest and exceeded the S-HCH, chlorpyrifos, and malathion concentrations by 3.5-, 1.5-, and 8.4-fold, respectively. Through breast milk, infants consumed 8.6 times more endosulfan and 4.1 times more malathion than the average daily intake levels recommended by the World Health Organization. A correlation analysis (r values) between mothers' age and the content of the chemicals accumulated in breast milk indicated a substantial degree of correlation for malathion (r–0.5). The other chemicals showed low to negligible correlation with donor age.

AbstractPreeclampsia is the 2A retrospective chart review of women who presented for evaluation of hypertension in pregnancy during 2010. Demographic information, medical history, symptoms, vital signs, and laboratory results were collected. Bivariate analysis was used to investigate associations between predictors and the outcome.Of the 481 women in the sample, 22 were identified as having abnormal laboratory test results (4.6%). Women who reported right upper quadrant pain or tenderness had significantly increased likelihood of having laboratory abnormalities compared to those without the complaint.Only a small percentage of women evaluated were determined to have abnormal laboratory findings, predominantly among women with severe preeclampsia. Right upper quadrant pain or tenderness was positively correlated with laboratory abnormalities. The restriction of laboratory analysis in women with clinical evidence of severe disease may be warranted – a broader study should, however, first be used to confirm our findings.
