Objectives: A retrospective study was carried out to analyse the two wheeler accidents that presented over a one year period to a single institution and to study the pattern of limb injuries according to zones assigned to the limb. The mode of injury in the two wheeler accidents and the differences in limb injuries between the driver and the pillion riding passenger were also studied.Methods: All the road traffic accidents that could be attributed to two wheelers were obtained with the help of trauma records and the nominal registers and ward records. Limb injuries were then assigned to various anatomical zones that were created for the purpose of the study. The upper limb and lower limb injuries were separately analysed for differences between the driver and the pillion rider.Results: There were 242 two wheeler accidents seen between 1st jan 2008 to 31st dec 2008, of these 110 had limb injuries. 96 cases were eventually included and of these 67 were drivers. The most common mode of injury was a skid and fall of the vehicle ( 42%). Pillion riders were injured more often in collisions between two and four wheelers. The commonest injury in drivers was in wrist and hand regions of the upper limb and the tibia and ankle regions of the lower limb. In the pillion rider, lower limb injuries were commonest in the tibia and ankle whereas in the upper limb shoulder and wrist injuries predominated.Conclusion: Two wheeler accidents cause significant morbidity in both the driver and pillion rider, the tibia and ankle are commonly injured in both the populations whereas minor differences in upper limb injury are seen between the two groups.Keywords: Road traffic accidents; epidemiology; pattern of injuries.DOI: 10.3126/ajms.v1i1.2794Asian Journal of Medical Sciences Vol.1(1) 2010 p.14-15

<p class="abstract"><strong>Background:</strong> Injuries of lower limb due to road traffic accidents are challenging to reconstruct due to paucity of tissues and damage to vasculature. Recurrent trophic ulcers are also difficult to reconstruct due to stigmata of previous surgeries. When local and regional flaps are not available, free flaps become the best option. The success of free flap depends on the presence of healthy recipient vessels and microsurgical expertise. In cases where the free flaps have failed or when there is no available soft tissue for local flaps, cross leg flaps are a method of resurfacing and salvaging the limb.</p><p class="abstract"><strong>Methods:</strong> This Study was done from January 2013 to January 2016 in Rajiv Gandhi Government General Hospital, Chennai. 20 patients were included in the study and the flaps used were conventional cross leg and cross leg reverse superficial sural artery flap.<strong></strong></p><p class="abstract"><strong>Results:</strong> All flaps survived. There was one case of wound dehiscence managed by re-insetting, two cases with marginal necrosis which were managed conservatively. One case of partial loss which was managed by STSG and one case of sinus formation due to tuberculosis managed with ATT. Remaining flaps had no complications.</p><p class="BodyA"><strong>Conclusions:</strong> Cross leg flaps are still relevant in the present era of microsurgery. They can be the primary choice of reconstruction or as a workhorse when free flaps have failed or other loco regional flaps are already exhausted. Immobilisation and two stage reconstruction are few disadvantages which when weighed against limb salvage are acceptable. </p>

Background: In Bangladesh fatality rate due to road traffic accidents is rising sharply day by day. At least 2297 people were killed and 5480 were injured in road traffic accidents within 1st six months of 2017.Whereas in the previous year at 2016 at least 1941 people were killed and 4794 were injured within the 1st six months. No survey has been reported in Bangladesh yet correlating ADHD as a reason of impulsive driving which ends up in a road crash.

COVID 19 is undeniably one of the deadliest diseases that humanity has ever seen. It continues to affect the lives and livelihood of people appallingly across the world. Maximum discussions focus towards the apprehension of catching the infection, dwelling in homes, overpopulated nursing homes and shut down of all kinds. But, here let’s discuss the positive side of COVID 19 pandemic.As COVID 19 has spread its influence all over the world, affected countries have either announced lockdown or have implemented severe restrictions in their respective countries. Because of this, everyone dwells in their homes. Thus, exercising social distancing and functioning from home. All of the above is directed at restricting the transmission of coronavirus and expectantly ostracising the fatality from COVID 19. These transformations have also brought about some unanticipated emanations; some good things have come out of the pandemic as well. Positive effects of COVID 19 are seen on reduced road traffic, and road traffic accidents lowered levels of air pollution which has to lead to lowered heart attack rates and rejuvenating environment. Crime rates have fallen, and expenses are reduced in most places. Community action, communication amongst families, behaviour, sanitation, hygiene, online and distance education has positively impacted by COVID 19 pandemic. COVID 19 despite a bane for humans, can be thought of a boon for living beings. The habitats and elements have been purified with the stringent use of petrochemical products. To breathe fresh air and to consume purified water is a boon by itself. Now, it is time for humans to lead a caring life to every bounty bestowed on them by Nature. This thoughtful and considerate life will give hope for a healthy, stress-free life.
