Some bacteria are capable of forming highly resistant spores when environmental conditions are not adequate for growth. Depending on the genus and species of the bacterium, these endospores are resistant in varying degrees to heat, cold, pressure, enzymatic degradation, ionizing radiation, chemical sterilants, physical trauma and organic solvents. The genus Clostridium, responsible for botulism poisoning, tetanus, gas gangrene and diarrhea in man, produces endospores which are highly resistant. Although some sporocides can kill Clostridial spores, the spores require extended contact with a sporocidal agent to achieve spore death. In most clinical situations, this extended period of treatment is not possible nor practical. This investigation examines Clostridium sporogenes endospores by light, transmission and scanning electron microscopy under various dormant and growth conditions, cataloging each stage in the germination and outgrowth process, and analyzing the role played by the exosporial membrane in the attachment and germination of the spore.

Granuloma annulare (GA) is a benign inflammatory skin condition of unknown etiology. Various clinical forms of GA including localized, generalized, subcutaneous, patch and perforating types, have been described. Although the etiology and pathogenesis of granuloma annulare are obscure, there is much evidence for an immunological mechanism. Precipitating factors are insect bites, sunburn, photochemotherapy, drugs, physical trauma, acute phlebitis and sepsis after surgery.[1] Some authors have proposed that it might be associated with a variety of underlying conditions such as thyroid disorders, diabetes mellitus and positive tuberculin skin test.[2] For definitive diagnosis, a biopsy should be performed. We report a case of generalized GA with serpiginous borders on the dorsal aspects of the hands and arms along with an association with diabetes.

Monica Ali was born in 1967 in Dhaka, Bangladesh, but grew up in England. Her English mother met her Bangladeshi father at a dance in northern England in the 1960s. Despite both of their families` protests, they later married and lived together with their two young children in Dhaka. This was then the provincial capital of East Pakistan which after a nine-month war of independence became the capital of the People`s Republic of Bangladesh. On 25 March 1971 during this civil war, Monica Ali`s father sent his family to safety in England. The war caused East Pakistan to secede from the union with West Pakistan, and was now named Bangladesh.

This chapter analyses the Imagines of Philostratos, which are underpinned by a desire to teach readers how to view art and how to speak about viewing images. An important aspect of this is teaching what those who feel (among other things) pleasure and pain might look like. This interest in pleasure and pain is reflected in a number of important tableaux in which Philostratos presents images of physical trauma and violation. In these instances, Philostratos explores the relationship between pain and trauma, showing how it is influenced by, and helps to mould, the individual’s emotional context, the physical beauty of the protagonists, and the reader’s pleasure at the image.

Abstract Background The course and corresponding characteristics of quality of life (QOL) domains in trauma population are unclear. Our aim was to identify longitudinal QOL trajectories and determine and predict the sociodemographic, clinical, and psychological characteristics of trajectory membership in physical trauma patients using a biopsychosocial approach. Methods Patients completed a questionnaire set after inclusion, and at 3, 6, 9, and 12 months follow-up. Trajectories were identified using repeated-measures latent class analysis. The trajectory characteristics were ranked using Cohen’s d effect size or phi coefficient. Results Altogether, 267 patients were included. The mean age was 54.1 (SD = 16.1), 62% were male, and the median injury severity score was 5.0 [2.0—9.0]. Four latent trajectories were found for psychological health and environment, five for physical health and social relationships, and seven trajectories were found for overall QOL and general health. The trajectories seemed to remain stable over time. For each QOL domain, the identified trajectories differed significantly in terms of anxiety, depressive symptoms, acute stress disorder, post-traumatic stress disorder, Neuroticism, trait anxiety, Extraversion, and Conscientiousness. Discussion Psychological factors characterized the trajectories during 12 months after trauma. Health care providers can use these findings to identify patients at risk for impaired QOL and offer patient-centered care to improve QOL.

BackgroundTransgender, nonbinary and gender-expansive (TGE) people face barriers to abortion care and may consider abortion without clinical supervision.MethodsIn 2019, we recruited participants for an online survey about sexual and reproductive health. Eligible participants were TGE people assigned female or intersex at birth, 18 years and older, from across the United States, and recruited through The PRIDE Study or via online and in-person postings.ResultsOf 1694 TGE participants, 76 people (36% of those ever pregnant) reported considering trying to end a pregnancy on their own without clinical supervision, and a subset of these (n=40; 19% of those ever pregnant) reported attempting to do so. Methods fell into four broad categories: herbs (n=15, 38%), physical trauma (n=10, 25%), vitamin C (n=8, 20%) and substance use (n=7, 18%). Reasons given for abortion without clinical supervision ranged from perceived efficiency and desire for privacy, to structural issues including a lack of health insurance coverage, legal restrictions, denials of or mistreatment within clinical care, and cost.ConclusionsThese data highlight a high proportion of sampled TGE people who have attempted abortion without clinical supervision. This could reflect formidable barriers to facility-based abortion care as well as a strong desire for privacy and autonomy in the abortion process. Efforts are needed to connect TGE people with information on safe and effective methods of self-managed abortion and to dismantle barriers to clinical abortion care so that TGE people may freely choose a safe, effective abortion in either setting.

The rights of a child stem from the dignity and uniqueness of a child as an individual and are entitled to every young human being. For the first time children's rights were included in the Geneva Declaration in 1924, and their full collection was enshrined in the Convention on the Rights of the Child prepared by the United Nations in 1989. A child has the right to live without violence. In addition to physical trauma, the effects of sexual abuse of a child disrupt normal psychosexual development. Bullying of children is also punished. The law allows gentle discipline only by parents. Mental bullying, so-called systematic harassment of the child, humiliation, ridicule or insult. Sexual violence is a particular issue here. Sexual violence is defined by WHO as an abuse of children for sexual pleasure by adults or elder peers. The article discusses some issues of medical opinion in the case of sexual violence against a child. This is especially difficult for a disabled child. The knowledge of an experienced clinician, yet without the knowledge about rules of jurisprudence and with shortcomings of basic legal knowledge, is not always enough for competent opinion-making.
