Improving maternal and child survival it’s an important integral part of health care. A large number of deaths in Africa emanate from preventable diseases and largely in the first month of life. Majority of deaths were from preventable causes such as pneumonia, diarrhea and malaria and all accounting to 14.9%, 9.2% and 7.3% respectively. In an endeavour to the prevent and promote healthcare system, the government of Botswana came up with a special programme geared toward protecting the locals against common diseases. The government of Botswana, through the Ministry of Health (MoH), introduced the Accelerated Child Survival and Development (ACSD) strategic plan intervention with a specific focus on reducing the ‘under five mortality rate’ (U5MR). One of the high-impact interventions for reducing the U5MR is the Integrated Management of Childhood Illness (IMCI) strategy. The strategic goal of IMCI is to reduce death, illness, and disability and to promote improved growth and development among children under 5 years of age. Child welfare clinic and Nutrition has been discovered as one of the strategy to promote child’s growth and development. This strategy can be effective if it is implemented with IMCI and Immunisation.

AbstractObjectiveTo investigate the nutritional impact of a community-based programme that focused on social cohesion and action.DesignThe change in nutritional status of children aged 12–60 months was examined over a period of 3 years in Makueni District in Eastern Province of Kenya in six communities in which an intervention programme of Participatory Learning and Action was introduced and in ten communities in which only basic preparations were made but no intervention was started.SettingThe intervention was part of the Government of Kenya Community Based Nutrition Programme and was supported by the Government of Denmark.SubjectsChildren aged 12–60 months.ResultsAmong communities without intervention there were similar levels of underweight (mean Z-score: −1·63 v. −1·50 (NS); % with Z-score < –2: 36·6 % v. 34·5 % (NS)) and stunting (mean Z-score: −2·0 v. −1·99 (NS); % with Z-score < –2: 44·3 % and 47·4 % (NS)) at baseline and after 3 years. By contrast, among communities who had received interventions, there were significant improvements after 3 years in the levels of underweight (mean Z-score: −1·66 v. −1·37 (P < 0·02); % with Z-score < –2: 42·9 % v. 31·4 % (P < 0·035)) and stunting (mean Z-score: −2·05 v. −1·59 (P < 0·05); % with Z-score < –2: 52·7 % v. 39·7 % (P < 0·02)).ConclusionsThe results indicate considerable potential for using Participatory Learning and Action as a community-based approach to effectively address child undernutrition. It is suggested that these interventions are developed, implemented and evaluated more widely as a mean of tackling childhood undernutrition and improving child survival and development.

RESUMOObjetivo: identificar na literatura nacional dificuldades enfrentadas pelo enfermeiro na prática do manual Atenção Integrada às Doenças Prevalentes na Infância. Método: revisão integrativa, com dados coletados nas bases de dados Lilacs, Medline e Biblioteca Virtual SciELO. 13 artigos atenderam aos critérios de inclusão e os Descritores para nortear a pesquisa foram: Estratégia Saúde da Família; Enfermagem; Saúde da Criança; Crescimento e Desenvolvimento; Mortalidade Infantil; Indicadores de Mortalidade. Resultados: a pesquisa constatou dificuldades encontradas pelos enfermeiros mediante sua prática na Estratégia Saúde da Família na aplicação à Atenção Integrada às Doenças Prevalentes na Infância, seja no aspecto de estrutura da rede de atenção primária, seja por parte da falta de conhecimento no desenvolvimento do protocolo. Conclusão: foi de relevância enfocar a prática do enfermeiro ao manual da Atenção Integrada às Doenças Prevalentes na Infância para favorecer cuidados e desenvolvimento à saúde da criança, indicando que a aplicação desta prática ainda não é exercida por todos os profissionais, tendo em vista fatores que envolvem desde barreiras no setor de trabalho até falta de capacitação profissional. The development of posyandu is the right strategy to foster child survival and development. Growth and development of toddlers who are not monitored properly and experience interference will not be repaired in the next period. In fact, only the growth of infants is monitored and evaluated continuously, but the development of children under five has not been well monitored and evaluated. The priority of the guidance program that is carried out, namely on the aspect of service quality, this service activity aims: 1) increase the knowledge of cadres about posyandu, growth and development of toddlers, as well as early detection of toddlers 'growth and development, 2) increase the cadres' ability to detect early growth and development toddler The activity was carried out with a participatory approach method, namely active involvement of the trained partners in each stage and development activities, which had been carried out through counseling, training, discussion, brain storming and mentoring. The results of service activities are increasing cadre knowledge about posyandu, growth and development of toddlers, and early detection of growth and development of toddlers; increased ability of cadres in early detection of toddler growth; the ability of cadres to provide counseling is related to nutritional status problems found in toddlers.

Background Acute diarrheal disease causes over 5 milion deathsworldwide in children under 5 years old, mostly because of failureto detect and treat dehydration properly and immediately.Objective To determine the validity of clinical diagnosis of dehy-dration according to Integrated Management of Childhood Illness(IMCI) as a diagnostic test for dehydration in children under fiveyears old.Methods A prospective observational study was done for clinicaldiagnosis in dehydration. Children aged 2 months-5 years old withdiarrhea and or vomiting who visited the primary health centers inYogyakarta were enrolled. Clinical diagnosis was made by atrained IMCI nurses. Dehydration was defined as the presence ofdecreasing consciousness, very sunken eyes, slow reaction whenoffered a drink, and decreased skin elasticity. Ninety-five per-cents confidence intervals (CI) were calculated, and the signifi-cance was assessed by X 2 .Results There were 148 children aged 2 months to 5 years oldenrolled this study. Clinical dehydration used according to IMCIproduced sensitivity of 91% (95% CI 83;98), specificity of 82%(95% CI 75;89), positive predictive value (PPV) of 70% (95% CI58;77), negative predictive value (NPV) of 94% (95% CI 90;98),positive likehood ratio of 5.17 (95% CI 3.37;7.94), and negativelikehood ratio of 0.11 (95% CI 0.04;0.27).Conclusion Clinical diagnosis for dehydration according to IMCIis sufficiently accurate as a diagnostic test for dehydration in chil-dren under 5 years old

The Quebec Water Policy was launched in November 2002 in support of reform of the water governance. One of the government commitments is to gradually implement watershed-based management for 33 major watercourses located primarily in the St. Lawrence plain. At the local and regional levels, watershed organizations are responsible for implementing integrated management, from a sustainable-development perspective, by preparing a master plan for water (MPW), which will include watercourses, lakes, wetlands and aquifers. These watershed organizations rely on public consultation, as well as local and regional expertise, on the responsibilities for water of the municipalities and regional county municipalities of the territory, as well as those of the ministries and other government agencies. They are also required to observe national priorities regarding protection, restoration, and development of water resources and to comply with relevant guidelines, directives, standards, regulations, and legislation. The role of watershed organizations is to act as planning and consultation tables. Government representatives are present, on the initial process, as the facilitator and for scientific and technical support. They do not have, at this moment, any voting or decisional rights. After two years, integrated water management mobilized water stakeholders on watersheds and they are on their way to initiating their first MPW.
