Background. Modern digital technologies use can solve many problems of Russian healthcare system. The digital medicine development should be accompanied by the informatization of all participants of the process of providing medical services. Analysis and assessment of the changes taking place in society and in the health care system itself in digitalization terms are necessary for making effective management decisions aimed at improving the population health. Purpose. The purpose of this study is to analyze the digital medicine state and prospects for the development. Materials and methods. The analysis of foreign and domestic literature on digital medicine was used to identify current trends in this area and the variety of technologies used. The data analysis from public opinion polls gave an idea of the main problems of Russian healthcare. The study of Internet resources, like information and discussion platforms for medical professionals, as well as survey data, made it possible to identify the main aspects of the informatization process in the healthcare system. Results and discussion. There were highlighted: the main advantages of information technologies use in the healthcare sector, the priority areas for the digital medicine short term development, the level of understanding of telemedicine among healthcare workers, the main areas of discussion regarding digital medical technologies, as well as important problems, risks and trends in this area. Conclusion. The authors identified eight promising areas for the digital medicine short term development. The study of Internet resources has shown that medical social networks are an important channel for healthcare informatization, including professional adaptation, especially of young specialists, to work with new technologies. The Internet also allows physicians to expand their professional capabilities with the help of software products for laboratory research, diagnosis and treatment. All participants of the medical and related industries are actively entering the digitalization process, striving to create a unified system for electronic data exchange. The main problem that slows down the development of digital medicine in Russia and which needs to be addressed at the state level is ensuring data security. A key measure to solve this problem is information about digital health opportunities through social media. At the content creation level there must be built trust to this channel.

Introduction. The development of effective digital medicine tools is an intensive and complex process that requires the interdisciplinary efforts of a wide range of experts, from scientists and engineers to ethics experts and lawyers. Digital medicine products have great potential for improving medical measurement, diagnosis and treatment. One of the main challenges for the healthcare sector is to address the issue of fast, convenient and secure exchange of information about patients’ health. Service-oriented architectures of such products may accomplish many of the challenges facing healthcare systems. The purpose of the paper is to develop an information and software module ExchangeDMD to ensure the accumulation, storage and exchange of diagnostic medical data in accordance with modern medical information standards to maintain the interoperability function as one of the leading principles of digital medicine. Results. A special adaptive architecture of digital medicine infrastructure has been developed, which enables an integrated solution of data exchange between participants of providing medical services, which is carried out with the help of web services. The specifics of different types of medical information are analyzed and taken into account in accordance with the access regime for its processing. The module structure has been developed and implemented in software, which has three main levels: central virtual storage (virtual data center to implement certain functions), remote administration segment (technical support and administration network) and user segment (mobile devices and user-patient applications). Conclusions. The ExchangeDMD information and software module is designed to ensure the accumulation of patient data, integration between the various units within the system, as well as to ensure the management of this data by health care personnel. The ExchangeDMD module is built using the international standard HL7 CDA, which enables formalizing electronic medical records using RIM (information model links) to attract the necessary directories and classifiers when creating medical records and documents.

The development of the health sector around the world is linked to digital technologies, because there is a need to optimize the processes of medical care for the population. Every year there is an increase in this market by a quarter. The use of digital technologies helps to improve health care. The management and organization system implements unified effective systems using the technological capabilities of digitalization in the health care sector. The relevance of the study is related to the need to change the outdated health management system with access to a modern level of technical and administrative support for medical services.The actual tool of digital medicine is cooperative forms of network interaction. We are exploring the further development of digital medicine in the short term, what opportunities can be presented and what results can be obtained by residents. Network communications play an important role in uniting professional medical communities. More than 65% of people between the ages of 21 and 35 have become participants in network technologies, and their number is constantly increasing. Administrative and management staffs are most active in discussions (43%) on the Internet and all respondents have an idea about telemedicine. Patients have the opportunity to get advice by contacting doctors online. The next stage in the development of information technologies is associated with increasing the speed and volume of transmitted data, which will help to predict critical conditions that threaten the patient’s health as quickly as possible. Now there is a discussion about the possibility of not just consulting, but also making diagnoses when patients contact the doctor, discussing in which cases a personal meeting with the doctor is necessary and under what conditions it is enough to provide maximum information about the state of health for diagnosis. Introduction. To ensure the effective delivery of health services, it is important to provide accessible mechanisms for interaction between different levels of health care and patients. mHealth, eHealth and other technological developments, such as telemedicine, constitute a new digital health paradigm. The purpose of the paper is to develop the formalized components of the digital medicine integrating environment to ensure effective interaction between patients, family physicians and health care workers at various levels. Results. The general structure of information flows of the integrative environment of digital medicine is described and the formalized representation of health care business processes are developed.An analysis of the characteristics of the main actors actively involved in medical care was provided, and the access level to medical information of each participant, which is an obligatory condition, in particular in the case of mobile medicine, was determined. To ensure the storage of this information, an infologicalmodel of the database was developed, indicating the internal links between the blocks of the database. Model elements are defined for each block: classes, attributes and operations (using UML). Conclusions. Formalized representation of eHealth business processes using mHealth reflects the interaction of participants in these processes (patient and different groups of health professionals) and information flows that arise in this interaction. To ensure reliable mechanisms for the digital medical data exchange, access levels were prescribed for each participant included in the eHealth system.

This study aimed to analyze the possibilities that modern digital technologies offer to the fields of health, psychotherapy, psychology and self-help. A review of various studies in the field of digital medicine with statistical data was conducted. The conclusions that researchers from different countries came to were similar: digital health applications are a practical solution that can be used to improve mental and psychological health. Moreover, digital psychotherapy can serve as a preventive tool to avert the development of mental disorders, increase the ability to deal with stress and mental problems, grow self-confidence, and more. The results of a study that was carried out using a Russian multimedia tool Master Kit are discussed. After using the program for three months, its users advanced in their ability to create an image of the desired outcome (a goal) and develop a plan to reach it. There is a noticeable difference in how goal-oriented the users became and their ability to independently self-train. Experimental data suggest that the tool can be effectively used to form and transform personal beliefs through a self-training format. More than half of the subjects talked about their satisfaction with the method, and more than one-quarter of them noted that their expectations from working with the program were fully satisfied.

The rapid development of new technologies has created interesting and unexpected possibilities in e-health, and digital platforms have become widespread, connecting users, experts, and practitioners of the health world. This triggered our investigation into the relationship between the engagement platforms used by 293 doctors with various specializations, their satisfaction, and the dimensions of social sustainability in the healthcare sector. The research focused on professional interaction and its sphere of action in engagement platforms, defined as virtual contact points for exchanging information, thus increasing the co-creation of value between physicians and patients. In order to verify our hypothesis, a health digital platform called paginemediche.it was used, and the two dimensions of engagement and sustainability were considered, examining their causal relationship and evaluating their effects on physician loyalty in terms of the re-use of the digital platform by doctors. Our results, using a multiple linear regression analysis, showed that the social sustainability of the digital health platform was directly influenced by online engagement, generating a positive effect on physician loyalty. In particular, the human dimension of social sustainability proved to be decisive for the re-use of the platform.

Background. The implementation of medical digital technologies can provide better accessibility and flexibility of healthcare for the public. It encompasses the availability of open information on the health, treatment, complications, and recent progress on biomedical research. At present, even in low-income countries, diagnostic and medical services are becoming more accessible and available. However, many issues related to digital health technologies remain unmet, including the reliability, safety, testing, and ethical aspects. Purpose. The aim of the review is to discuss and analyze the recent progress on the application of big data, artificial intelligence, telemedicine, block-chain platforms, smart devices in healthcare, and medical education. Basic Design. The publication search was carried out using Google Scholar, PubMed, Web of Sciences, Medline, Wiley Online Library, and CrossRef databases. The review highlights the applications of artificial intelligence, “big data,” telemedicine and block-chain technologies, and smart devices (internet of things) for solving the real problems in healthcare and medical education. Major Findings. We identified 252 papers related to the digital health area. However, the number of papers discussed in the review was limited to 152 due to the exclusion criteria. The literature search demonstrated that digital health technologies became highly sought due to recent pandemics, including COVID-19. The disastrous dissemination of COVID-19 through all continents triggered the need for fast and effective solutions to localize, manage, and treat the viral infection. In this regard, the use of telemedicine and other e-health technologies might help to lessen the pressure on healthcare systems. Summary. Digital platforms can help optimize diagnosis, consulting, and treatment of patients. However, due to the lack of official regulations and recommendations, the stakeholders, including private and governmental organizations, are facing the problem with adequate validation and approbation of novel digital health technologies. In this regard, proper scientific research is required before a digital product is deployed for the healthcare sector.

The purpose of the study is to assess the perception of various aspects of digital medicine by the youth segment of consumers, the degree of readiness to consume its services and the level of involvement in this process. The last few years have become a turning point for medicine, if we talk about the number of new directions, emerging techniques and the introduction of digital technologies. Digital medicine uses information and communication technologies to solve health problems of different groups of patients. Its capabilities help doctors and patients to analyze the development of the disease, calculate health risks. The segment of medical gadgets of digital medicine offers consumers a variety of products. Artificial intelligence and wearable devices are becoming common means of organizing the treatment process and monitoring the human condition. Young people, as a rule, more actively perceive any novelties and include еthem in their daily life. Is it really so? If we consider Russian young consumers, since foreign experience indicates their rather high interest.Materials and methods of research. The presented study of the perception of the youth audience and readiness to use digital medicine is based on the methodology, proposed by the venture Fund Rock Health, dedicated to digital health. This Fund conducts research, aimed at improving the quality, safety and accessibility of modern medicine. The study is conducted by online survey of consumers who have access to the Internet at home, at work or through cell phones. The questionnaire was adapted for the Russian target audience. The results of the research of the consulting company Accenture, which regularly studies the trends in the consumption of medical technologies with the use of artificial intelligence, robotics and gadgets for self-diagnosis, were used as a factual basis. Data of research of the analytical company CB Insights, forming annual forecasts about the changes in healthcare, and also materials of joint research of the company Econsultancy and the developer of IT technologies for medicine Adobe are considered. The article presents some data of the report of the American company Change Healthcare, which has a strategic partnership with Google Cloud.Results of the research on the one hand, show the high involvement of young people in the field of digital technologies. The survey includes 380 respondents, 89% are smartphone users and 83% regularly visit social networks. However, they are quite wary of digital medicine, requiring the control of personal data. They trust their doctor more than mobile apps and Internet resources; they are reluctant to buy wearables, little understanding of what is telemedicine. They are limited to searching for medical information on the Internet only about services, doctors, medicine and treatment technologies to find out general information.Conclusion. The content analysis shows that digital medicine is becoming a popular trend in the development of modern healthcare. New technologies are being introduced, the key areas of development of which are focused on the Internet of things for medicine (IoT), artificial intelligence, mobile apps, genome editing, telemedicine, blockchain projects and cloud computing. The Russian market is slowly developing. Young consumers are not yet active in the consumption of digital health services and products. The key reasons for this are low awareness of the benefits and concerns about the security of the personal data.

Digitalization of healthcare delivery is rapidly fostering development of precision medicine. Multiple digital technologies, known as telehealth or eHealth tools, are guiding individualized diagnosis and treatment for patients, and can contribute significantly to the objectives of precision medicine. From a basis of “one-size-fits-all” healthcare, precision medicine provides a paradigm shift to deliver a more nuanced and personalized approach. Genomic medicine utilizing new technologies can provide precision analysis of causative mutations, with personalized understanding of mechanisms and effective therapy. Education is fundamental to the telehealth process, with artificial intelligence (AI) enhancing learning for healthcare professionals and empowering patients to contribute to their care. The Gulf Cooperation Council (GCC) region is rapidly implementing telehealth strategies at all levels and a workshop was convened to discuss aspirations of precision medicine in the context of pediatric endocrinology, including diabetes and growth disorders, with this paper based on those discussions. GCC regional investment in AI, bioinformatics and genomic medicine, is rapidly providing healthcare benefits. However, embracing precision medicine is presenting some major new design, installation and skills challenges. Genomic medicine is enabling precision and personalization of diagnosis and therapy of endocrine conditions. Digital education and communication tools in the field of endocrinology include chatbots, interactive robots and augmented reality. Obesity and diabetes are a major challenge in the GCC region and eHealth tools are increasingly being used for management of care. With regard to growth failure, digital technologies for growth hormone (GH) administration are being shown to enhance adherence and response outcomes. While technical innovations become more affordable with increasing adoption, we should be aware of sustainability, design and implementation costs, training of HCPs and prediction of overall healthcare benefits, which are essential for precision medicine to develop and for its objectives to be achieved.
