In Chapter 8 we turn to Co-futuring Kinship—the ways in which past and present practices inform how the future of the kinship for care at a distance. This is particularly important for “super-aging” contexts like Japan in which one in three is of 80 years old. Chapter 8 sets the picture for discussion around digital health in which mobile media is fully imbricated in. Discussions around a “silver bullet” in the form of a mobile app still dominate despite the fact that there is much work into the need for social, rather than technological, solutions.

AbstractMore and more people and organisations who are addressing complex sustainability challenges are turning to systems change practices. They are looking to get to grips with complexity and to better understand how to use their resources, position and influence to address the challenges. These people are working across civil society, philanthropy, business, international development, government and beyond. Many hope that adopting this emerging practice will give them the answers to the long held questions of – How do I know where to intervene? How do I know that what I am doing is the ‘right’ thing? Am I using my resources for their greatest effect? Once we have set ambitious goals around issues like inequality and climate change, how do I know I am creating impact?. In 1999 Donella Meadows wrote a paper entitled Leverage points: places to intervene in a system to help translate the work of systems dynamics into understanding where a small amount of energy might have a greater effect. Ever since, practitioners have been chasing these elusive leverage points trying to understand how this might be made useful and practical. There is, however, no silver bullet to changing a system. At Forum for the Future and through the School of System Change, we work on a number of different projects such as the Protein Challenge and Boundless Roots Community as well as collaborate on, coach and co-inquire with others such as the Marine CoLAB, Oneless, Lankelly Chase Foundation. In this paper we seek to build on systems change ideas and theories, using Forum for the Future experience of working with these ideas in practice, and offer actionable knowledge (Coghlan 2007) to other change makers who are grappling with these questions. This paper provides four qualities that help us understand the dynamics of a changing system, and how potential in these dynamics might be identified and be translated into strategy and interventions. I explore and illustrate these through cases and examples and raise the question about how change makers might value what we measure when understanding impact in the context of a changing system.

AbstractDigital Health-Enabled Community-Centered Care (D-CCC) represents a pioneering vision for the future of community-centered care. Utilizing an artificial intelligence-enabled closed-loop digital health platform designed for, and with, community health workers, D-CCC enables timely and individualized delivery of interventions by community health workers to the communities they serve. D-CCC has the potential to transform the current landscape of manual, episodic and restricted community health worker-delivered care and services into an expanded, digitally interconnected and collaborative community-centered health and social care ecosystem which centers around a digitally empowered community health workforce of the future.

This paper presents a case study on security and privacy implications on the design of a mobile application in digital health, the DeStress Assistant (DeSA) app, which utilizes sensing technologies and capabilities of the Internet of Things (IoT). An analysis of the applicable legislative framework is provided and selected challenges encountered during the app design are discussed, which are related with the practical implications of provisions of the international and national legislation for software applications in general as well as medical devices and handling of sensitive data in particular. We provide insights into design choices, including different possible scenarios for classification of a mobile app as a medical device and the pertaining legal risks the app developer is faced with as a consequence of possible legal obligations, and different possibilities of specifying the intended use. Also, we propose two designs of a mechanism that enables secure sharing of the patient’s health-related observations from the DeSA app with a medical professional within a treatment context. The first mechanism provides secure submission of health-related observations into a hospital information system, whereas the second mechanism enables secure short-term sharing of observations without storage.

“Reuse [software] engineering is a process where a technology asset is designed and developed following architectural principles, and with the intent of being reused in the future” (Bean, 1999). “If programming has a Holy Grail, widespread code reuse is it with a silver bullet. While IT has made and continues to make laudable progress in our reuse, we never seem to make great strides in this area” (Grinzo, 1998). “The quest for that Holy Grail has taken many developers over many years down unproductive paths” (Bowen, 1997). This article is an overview of software reuse methods, particularly object oriented, that have been found effective in business systems over the years.

The Internet and digital media are increasingly seen as having enormous potential for solving problems facing healthcare systems. This chapter traces emerging “digital health” uses and applications, focusing on the political economy of data. For many people, the ability to access their own data through social media and connect with people with similar conditions holds enormous potential to empower them and improve healthcare decisions. For researchers, digital health tools present new forms of always-on data that may lead to major discoveries. Technology and telecommunications companies hope their customers? data can answer key health questions or encourage healthier behavior. At the same time, Gina Neff argues that digital health raises policy and social equity concerns regarding sensitive personal data, and runs a risk of being seen as a sort of silver bullet instead of mere technological solutionism.

Abstract The panellist Anant Jani will discuss about: What is the value of digital health? There is much hope and hype surrounding the potentially transformative effect that digital health tools can have in health and care systems but it is very difficult to ascertain the true value that digital health tools currently deliver or could deliver in the future. Compounding this uncertainty is the diverse, large and ever-changing digital health landscape - there are currently over 300,000 health and care apps on the market place in comparison to the less than 2000 drugs health and care systems normally have to deal with. In this session, we highlight how the quadruple value framework, recently endorsed by the EU Commission, can be used to help rationalize the digital health ecosystem by promoting the interventions that have the greatest potential to promote primary, secondary or tertiary prevention while optimising resource utilisation.

ObjectivesWe aimed to explore the opinion leader empowered patients’ relationship with their medical professionals, their experiences and beliefs about technologies, and how they see the future. We also attempted to determine whether technologies, the access to it or patient empowerment are the main driving forces behind these changes.DesignA qualitative interview study analysed with interpretative phenomenological analysis.SettingAll interviews were conducted and recorded individually with the same trained interviewer via a Skype call.ParticipantsThe study is based on qualitative, semistructured interviews with 11 opinion leader empowered patients from six countries including UK, USA, Australia, Sweden, South Africa and Ireland.ResultsWe identified four superordinate themes emerging from e-patients’ experiences: (1) impact of technology, (2) the meaning of empowerment, (3) the changing physician–patient relationship and (4) expectations for the future. The relationship e-patients have with their physicians is based on efficient communication, proactivity, the desire for asking questions and the use of technologies. The interviews have shown that the rapid development of technology has fundamentally changed the lives of these e-patients, and technology eventually is transforming the physician–patient relationship into a partnership. Regarding the future of the physician–patient partnership, e-patients emphasised that change will rather be cultural than technological.ConclusionsThe interviews have shown that cooperation between technology and healthcare is not enough on its own: the most decisive factor is the return of the human touch and reciprocal communication. All of these suggest that technology is an important ally in the ‘renaissance of medicine’ that starts to treat patients as it should have always had.
