<p>Actual, continuously available information on the accuracy of forecasts can support both weather services and users of forecasts in quality assurance during operations and identify systematic weaknesses. Comparing the forecast success of different forecasting methods allows decision makers in the weather service and on the user side to evaluate the cost-benefit ratio of available forecasting approaches, be it different models, DMO and post-processing, or different providers. Finally, in addition to on-off experiments for version comparison, the success of developments to the forecast system can be seen in the comparison of time series of verification results against those of other forecasts. </p><p>From the development of the forecasting process to daily operations to the use of forecasts in subsequent industry applications, stakeholders have very different questions about the quality of weather forecasts. From the weather room, there is a particular need for up-to-date information on the previous day's forecast success and rapid access to case verification analyses following unusual events. Especially in B2B, case-specific comparison with the success of other forecasts is also in demand. For management, on the other hand, longer-term trends in forecast quality are the focus of interest. Finally, users often base their choice of a forecasting provider not only on procurement costs and convenience of access, but also take into account the current forecast accuracy of their relevant parameters, in their region, in the forecast horizon relevant to them. Especially weather-sensitive industries such as road weather services, energy production and transmission, but also media often agree with forecast suppliers on continuous monitoring of forecast quality. </p><p>We present different perspectives and questions and show possible answers as use cases in a verification portal.</p>

AbstractThe authors review the main concepts regarding the importance of cleaning/disinfection of ultrasonography probes, aiming a better comprehension by practitioners and thus enabling strategies to establish a safe practice without compromising the quality of the examination and the operator productivity. In the context of biosafety, it is imperative to assume that contact with blood or body fluids represents a potential source of infection. Thus, in order to implement cleaning/disinfection practice, it is necessary to understand the principles of infection control, to consider the cost/benefit ratio of the measures to be implemented, and most importantly, to comprehend that such measures will not only benefit the health professional and the patient, but the society as a whole.

Obesity is a serious health problem that has increased in recent decades. It is a chronic disease responsible for serious physical, psychological and social problems, reaching to alter the quality of life of people who suffer from it. Bariatric surgery is the best treatment for obesity even in its most severe levels, since in addition to reducing excess weight, it achieves a high rate of improvement and remission of metabolic comorbidities, improving the quality and expectation of life of the operated patients. Within bariatric procedures, laparoscopic gastric bypass and laparoscopic sleeve gastrectomy are the most performed worldwide. Quality of life is defined as, the perception that the obese person has of their physical, psychological, and social limitations and the reduction of opportunities. The success of the surgery consists of changing the quality of life of the patient. The results should not be evaluated only according to the initial loss or late weight gain, complications or sequelae of one or another technique or the subsequent need for cosmetic surgery, but a series of factors derived from the patients themselves should be taken into account, which together define the quality of life and even the cost / benefit ratio.

Obesity represents one of the most challenging public health problems of our century. It accounts for approximately 5% of deaths worldwide, mostly owing to cardiovascular disease and its associated complications. Cardiovascular noninvasive imaging may provide early accurate information about hypertrophy and ischemia/fibrosis in obese subjects. Echocardiography and nuclear cardiology have serious limitations in obese subjects owing to poor acoustic window and attenuation artifacts, respectively. Coronary computed tomography angiography can provide information about obstructive coronary disease; however, the use of radiation is a serious disadvantage. Finally, cardiac magnetic resonance (CMR) holds the promise of an “all in one” examination by combining evaluation of function, wall motion/thickness, stress rest/perfusion, replacement and diffuse fibrosis without radiation. Future studies are required to document the cost/benefit ratio of the CMR in the evaluation of cardiovascular risk in overweight/obese children and adolescents.

To the Editor.— Two recent reviews in Pediatrics1,2 provide much interesting information on the effect of home visits on the health of women and children. However, I was disappointed not to find in either article more than token reference to the cost of the programs reviewed. In this day of increasingly scarce health care resources, we no longer have the luxury of evaluating programs such as these without detailed consideration of their cost-benefit ratio.

В настоящее время информация о состоянии атмосферы широко используется как при составлении прогнозов погоды, так и при решении многих экономических задач. Существенная особенность метеорологических явлений - их пространственно-временная изменчивость. Это вызывает необходимость такой организации наблюдений, которая позволит своевременно отметить возникновение явления и проследить за ходом его развития. Статистика показывает, что в период с 1997 по 2000 годы произошло порядка 193 авиационных инцидентoв, которые обусловлены недостатками метеорологического обеспечения полетов. Поэтому задача повышения качества метеорологического обеспечения является актуальной. На основе модельных и практических экспериментов показано, что беспроводной канал связи LoRaWaN способен объединить весь поток данных, поступающих с удаленных метеорологических датчиков, которые используют разные протоколы для взаимодействия друг с другом. В ходе выполнения эксперимента получены положительные результаты, доказывающие возможность организации таких каналов связи для задач метеообеспечения аэродромов. Также приводится структурная схема для организации канала связи. При этом не требуется прокладка кабельных коммуникаций связи, когда стоимость такого вида работ, включая согласование, зачастую значительно превышает стоимость самого метеорологического оборудования At present, information on the state of the atmosphere is widely used both in the preparation of weather forecasts and in solving many economic problems. An essential feature of meteorological phenomena is their spatial and temporal variability. This makes it necessary to organize observations in such a way that will allow timely noting the occurrence of the phenomenon and monitoring the progress of its development. Statistics show that in the period from 1997 to 2000 there were about 193 aviation incidents, which were caused by shortcomings in the meteorological support of flights. Therefore, the task of improving the quality of meteorological support is urgent. This article shows, based on model and practical experiments, that the LaRaWAN wireless communication channel is able to combine the entire data stream coming from remote meteorological sensors using various communication protocols among themselves. During the experiment, I obtained positive results, proving the possibility of organizing such communication channels for the tasks of meteorological support of airfields. A block diagram for the organization of a communication channel is also provided. At the same time, the laying of cable communications is not required, when the cost of this type of work, including coordination, often significantly exceeds the cost of the meteorological equipment itself

The paper has studied the economic viability of improved technology (Introduced under NAIP component-3) for extraction of cashew kernel from cashew nut in Bastar region of Chhattisgarh, India. Cost concept has been used to calculate economics of cashew kernel. The technology (Boiling, steaming, cutting, drying, and peeling) has been found viable over conventional practices (Traditional manual separation by stone or hammer) on account of higher recovery of 40 percent and cost reduction by 29.71 percent. Overall net profit per unit (One unit includes one boiler, one steamer, two cutter, one dryer, six peelers and cost of land, depreciation and interest on working capital) in the case of improved technology has been estimated to be Rs 7.32 lakh. Cost of production in machine extraction practices was 202.80 Rupees per kilogram of cashew in spite of traditionally practiced 288.56 Rupees per kilogram. The cost benefit ratio was found higher in machine extraction (1.57) as compare to traditionally practiced (0.169). The mechanical decortications and separation could not only save time and money, also reduced women drudgery (due to manual breaking by stone or hammer to separate kernel). The technology has been found suitable for promotion of entrepreneurship on the processing of cashew kernel from cashew nut in the production catchments which otherwise is not properly utilized. The objective is to reinforce the importance of blood reinfusion as a cheap, safe and simple method, which can be used in small hospitals, especially those in which there is no blood bank. Moreover, even with the use of devices that perform the collection and filtration of blood, more recent studies show that the cost-benefit ratio is much better when autologous transfusion is compared with blood transfusions, even when there is injury to hollow viscera and blood contamination. It is known that the allogeneic blood transfusion carries a number of risks to patients, among them are the coagulation disorders mediated by excess enzymes in the conserved blood, and deficiency in clotting factors, mainly the Factor V, the proacelerin. Another factor would be the risk of contamination with still unknown pathogens or that are not investigated during screening for selection of donors, such as the West Nile Fever and Creutzfeldt-Jacob, better known as "Mad Cow" disease. Comparing both methods, we conclude that blood autotransfusion has numerous advantages over heterologous transfusion, even in large hospitals. We are not against blood transfusions, just do not agree that the patient's own blood is discarded without making sure there will be enough blood in stock to get him out of the hemorrhagic shock.

Farmers are faced with challenges of producing enough food and the use of traditional methods seems not to keep pace with the ever-growing demand of the populace thus creating increased concern in food scarcity. Although it has been identified that smart tools will enhance the production pace needed in the Agricultural sector, unfortunately, most of these tools are designed for farmers without their inputs, thus creating tools that are not meeting demands. This study focused on a farmer-centered design, development, and deployment approach to improving farm productivity. The design thinking approach was used to identify the specific need of the farmers in selected areas, ideas were created using brainstorming sessions involving experts in the field, and prototypes were developed and deployed to evaluate the impact performance. The result shows that the proposed system improved the cost-benefit ratio of crop farming from 2.14 to 2.26. This is a 12% productivity increase.

ABSTRACT:The triptans represent a major advance in migraine therapy but their cost per dose greatly exceeds that of many older treatments. There is evidence that for a significant proportion of migraine patients these new drugs can show a positive cost benefit and also improve quality of life. Cost benefit would be expected to be greatest in patients with more severe migraine attacks.

Background: Family meetings facilitate the exploration of issues and goals of care however, there has been minimal research to determine the benefits and cost implications. Aims: To determine: (1) if family caregivers of hospitalised patients referred to palliative care who receive a structured family meeting report lower psychological distress (primary outcome), fewer unmet needs, improved quality of life; feel more prepared for the caregiving role; and receive better quality of end-of-life care; (2) if outcomes vary dependant upon site of care and; (3) the cost-benefit of implementing meetings into routine practice. Design: Pragmatic cluster randomised trial involving palliative care patients and their primary family caregivers at three Australian hospitals. Participants completed measures upon admission (Time 1); 10 days later (Time 2) and two months after the patient died (Time 3). Regression analyses, health utilisation and process evaluation were conducted. Results: 297 dyads recruited; control ( n = 153) and intervention ( n = 144). The intervention group demonstrated significantly lower psychological distress (Diff: –1.68, p < 0.01) and higher preparedness (Diff: 3.48, p = 0.001) at Time 2. No differences were identified based on quality of end of life care or health utilisation measures. Conclusions: Family meetings may be helpful in reducing family caregiver distress and enhancing their preparedness for the caregiving role and it appears they may be conducted without increased hospital health utilisation impacts; although opportunity costs need to be considered in order to routinely offer these as a standardised intervention. Additional health economic examination is also advocated to comprehensively understand the cost-benefit implications.
