In recent years there has been growing evidence to suggest a major role of basophils alongside eosinophils and mast cells in allergic inflammation. The aim of this study was to analyze the dynamics of the basophil phenotypes after the use of magneto-la-ser therapy in children with atopic bronchial asthma. Materials and methods. A total of 66 children with mild persistent atopic bronchial asthma (aged 6 to 18 years old) were examined. Group A included 34 children who received magneto-laser therapy together with basic asthma treatment (low dose of inhaled corticosteroids). Group B included 32 children who received only basic asthma therapy. The level of CD203с + , CD203с + CD63 + , CD203с + IgE + basophils was determined in peripheral blood in the beginning of the study, after 2 weeks and after 3 months. Results. A statistically significant decrease in the absolute levels of CD203с+CD63+ and CD203с + IgE + basophils and in the relative level of CD203с + IgE + among all CD203с + basophils was determined in group A after magneto-laser therapy. The comparison of group A and group B indices revealed a significant difference between the relative level of CD203с + IgE + basophils after 12-15 days from the beginning of the study. This indicator was significantly lower in group A than in group B (p<0.05). The absolute level of CD203с + IgE + basophils was significantly lower in group A in comparison with group B after 82-90 days (p<0.05). Conclusions. Magneto-laser therapy can change the phenotype of basophils in children with atopic bronchial asthma, causing suppression of proallergic pa-rameters. Considering these results there is reason to believe, that it is possible to use this method as an additional immunocorrective treatment in patients with basophilic phenotype of atopic asthma.

BACKGROUND: Cytokines including Interleukin-5 play a key role in orchestrating the chronic inflammation of asthma. We aimed to determine the level of serum IL-5 in partly controlled atopic asthma in children and to assess the effect of different therapies on their levels.METHODS: The study included 40 children aged 6-12 years with partly controlled asthma. Cases were randomly divided into two groups; group ‘A’ receiving Leukotriene modifiers and group ‘B’ receiving inhaled corticosteroids; each for two months. They were compared to 20 healthy non-asthmatic, matched controls. Serum IL-5 was measured for cases on the first visit and two months after therapy. Absolute eosinophilic count and serum Ig-E were determined. Pulmonary function testing was performed using spirometer at the beginning and two months after regular therapy.RESULTS: Serum Interleukin-5 was significantly increased in asthmatic children during exacerbation and was significantly decreased after treatment. ROC curve analysis showed significant difference of IgE and PEFR after treatment with leukotriene modifier only.CONCLUSION: Serum IL-5 seems to have a role in asthma pathogenesis. Efficiency of the two therapies (ICs & LTA) was similar in this group of patients. Both treatments led to significant decline in serum IL-5, IgE levels and eosinophilic count.

Aim: To find the effectiveness of laser therapy and ultrasound therapy along with muscle energy technique in treatment of Trapezitis. Materials and Methods: Non-equivalent quasi experimental study design was used in this study. Total of 30 subjects with trapezitis were selected using non probability convenient sampling technique.30 Subjects was divided into two groups by lot system. Group A received laser therapy and Group B received ultrasound therapy and for both the group muscle energy technique was given. The outcome measures are Neck Disability Index (NDI) for measuring Functional Disability. Data collected and tabulated was statistically analyzed. Result: Statistical analysis of post-test, Neck Disability Index (NDI) revealed that there is statistically significant difference seen between Group A and Group B. Conclusion: From the result, it has been concluded that Laser therapy with muscle energy technique (Group A) was more effective than Ultrasound therapy with muscle energy technique (Group B) on improving functional ability in subjects with Trapezitis.

The aim of the research was to study the phenotype of eosinophils in children with atopic bronchial asthma and their role as diagnostic criteria of the disease. Materials and methods. We have examined 130 children aged from 6 to 18 years old with atopic bronchial asthma. The control group consisted of 40 healthy children from 6 to 18 years old, who didn’t have allergic diseases. During the research the relative and absolute levels of eosinophils, the level of eosinophils carrying FcεRI and FcεRII (CD23) – receptors were established. Results. The relative level of eosinophils in asthmatic children was 6.00% [3.00; 8.00], the absolute level – 417.00 cells/μL [232.00; 636,00], which is significantly higher compared to control group, where the relative level was 2.50% [1.00; 3.00%] and the absolute level was 166.50 cells/μl [86.00; 213.00] (p<0.001). The level of eosinophils carrying the FcεRI receptor was 420,00 cells/μl [250,00; 660,00], which is significantly higher than in the control group – 200,00 cells/μl [140,00; 240.00] (p<0.001). The relative level of eosinophils carrying CD23 receptor on their surface was 62.20% [35.40; 76.60] and the absolute level –223.37 cells/μl [105.30; 375.24]. The results are significantly higher than those of the control group: 25.45% [14.30; 30,60] (p<0,001), and 30,88 cells/μl [25,63; 42.84] (p <0.0001) respectively. It was found in the ROC-analysis that there is high evidence of presence of atopic bronchial asthma if absolute levels of eosinophils carrying CD23 receptor was 73.008 cells/μl or more or the relative index – 35.10% and more. Conclusions. The relative and absolute levels of eosinophils, as well as eosinophils carrying FcεRI and CD23 receptors are statistically significantly higher in atopic bronchial asthma than in control group patients. Increased number or relative index (73.008 cells/μl or ≥ 35.10% respectively) of eosinophils carrying CD 23 receptors can be considered as a diagnostic criterion of atopic bronchial asthma.

Background: Fixed-dose combinations of Inhaled corticosteroids (ICS) and Long acting beta agonist (LABA) are established and widely used treatment for bronchial asthma when ICSs as monotherapy are ineffective. This study attempted to compare the efficacy of salmeterol and fluticasone with formoterol (newer LABA) and fluticasone in patients of bronchial asthma.Methods: An open label, randomized, prospective, parallel and comparative study of eight-week duration was conducted on 80 patients of bronchial asthma, with the collaboration of Department of pharmacology and Department of Tuberculosis and Chest Diseases Hospital, Government medical college, Amritsar. Patients in Group A were treated with 2 actuations of Formoterol and Fluticasone (6/125µg) twice daily and group B patients were treated with 2 actuations of Salmeterol and Fluticasone (50/125µg) twice daily for 8 weeks with metered dose inhaler (MDI). Patients in group A and B were assessed on day zero, 4 weeks and 8 weeks for clinical assessment and computerized spirometry for FVC, FEV1, FEV1/FVC and PEFR.Results: In group A mean±SD of FEV1 statistically significantly increased (<0.001) after eight week of therapy (1.50±0.12) from its baseline values (1.34±0.11). Similarly, in group B mean ± SD of FEV1 statistically significantly increased (<0.001) after eight weeks (1.48±0.13) from its baseline values (1.36±0.12). There was statistically significant (<0.001) improvement in other parameters of spirometry in patients of both the groups.Conclusions: It was observed that both the combination of Fluticasone + Formoterol and Fluticasone + Salmeterol are effective in the treatment of bronchial asthma.

The aim of our study was to analyze the effect of recreational swimming on lung function and bronchial hyperresponsiveness (BHR) in patients with mild persistent asthma. This study included 65 patients with mild persistent asthma, who were divided into two groups: experimental group A (n = 45) and control group B (n = 20). Patients from both groups were treated with low doses of inhaled corticosteroids (ICS) and short-acting β2 agonists salbutamol as needed. Our program for patients in group A was combined asthma education with swimming (twice a week on a 1-h basis for the following 6 months). At the end of the study, in Group A, we found a statistically significant increase of lung function parameters FEV1 (forced expiratory volume in 1 sec) (3.55 vs. 3.65) (p < 0.01), FVC (forced vital capacity) (4.27 vs. 4.37) (p < 0.05), PEF (peak expiratory flow) (7.08 vs. 7.46) (p < 0.01), and statistically significant decrease of BHR (PD200.58 vs. 2.01) (p < 0.001). In Group B, there was a statistically significant improvement of FEV1 3.29 vs. 3.33 (p < 0.05) and although FVC, FEV1/FVC, and PEF were improved, it was not significant. When Groups A and B were compared at the end of the study, there was a statistically significant difference of FVC (4.01 vs. 4.37), FEV1 (3.33 vs. 3.55), PEF (6.79 vs.7.46), and variability (p <0.001), and statistically significantly decreased BHR in Group A (2.01 vs. 1.75) (p < 0.001). Engagement of patients with mild persistent asthma in recreational swimming in nonchlorinated pools, combined with regular medical treatment and education, leads to better improvement of their parameters of lung function and also to more significant decrease of their airway hyperresponsiveness compared to patients treated with traditional medicine

Abstract Background Retinopathy is a common and specific microvascular complication of diabetes. Clinical and experimental findings suggested that choroidal vasculopathy in diabetes may play a role in the pathogenesis of diabetic retinopathy. Spectral-domain optical coherence tomography (SD-OCT) can analyze choroidal structure and histopathological changes in diabetes. Purpose To compare choroidal thickness (CT) before and after panretinal photocoagulation (PRP), modified grid laser (MGP) and combined (panretinal photocoagulation & modified gird laser) using SD-OCT. Design Comparative prospective non-randomized case study, included 30 eyes of 30 patients from both sexes aging from 15 to 60 years. All patients were from Ain-Shams University Hospitals outpatient clinics. Patients were divided into 3 equal groups: Group A Patients with PDR underwent PRP, Group B: Patients with macular edema (ME) underwent MGP, Group C: Patients with PDR&ME underwent combined (PRP & MGP). All cases underwent SD-OCT with CT measurement before and at 1 & 3 Mns after laser therapy with assessment of best corrected visual acuity (BCVA). Results Mean age of participant was 50.83 ± 5.34 (SD). The mean CT in group A at baseline was 329.00 ± 36.08 μm (SD). This was increased to 336.40 ± 35.64 μm at 1 month then was decreased to 309.10 ± 38.87 μm at 3 Mns after laser therapy. This was showed highly statistically significant difference (P-value = 0.000). In group B the CT at baseline was 328.90 ± 19.69 μm. This was decreased to 313.90 ± 17.83 μm & 288.90 ± 23.10 μm after 1 & 3 months respectively with highly statistically significant difference (P-value = 0.000). While in group C the mean CT at baseline 329.80 ± 21.06 μm. This was decreased to 318.10 ± 19.25 μm, 300.40 ± 17.98 μm at 1 & 3 months after combined laser respectively. This was showed highly statistically significant difference (P-value = 0.000). There was no statistically significant difference between the studied groups regarding preoperative and postoperative (after 1&3 Mns) CT. There was no statistically significant difference observed between the studied groups regarding preoperative BCVA (P-value = 0.437). While P-value showing highly statistically significant difference between the studied groups regarding postoperative BCVA (P-value = 0.000). Conclusion The Choroidal thickness decreased in all 3 studied groups; suggesting that all laser techniques may reduce choroidal vascular permeability or cause atrophy of choroidal vessels over a 12-week period.

Background: This study was undertaken because of paucity of literature regarding outcomes of inhaled formeterol/fluticasone versus formoterol/mometasone in asthmatic patients.Methods: Fifty newly (male/female) recruited cases of bronchial asthma were diagnosed on the basis of spirometry. The patients were allocated to two groups viz group A and B. Group-A received mometasone furoate/formoterol (200/10 μg OD) and Group-B received fluticasone/formoterol (200/10 μg OD) respectively. The drugs were administered through metered-dose inhaler (MDI).Results: The mean FEV1/FVC ratio recorded (64.40±9.01) before starting the treatment has significantly changed to (68.92±8.58) after starting the treatment. Mean forced expiratory volume (47.56±14.73%) noted before the use of bronchodilator also changed to mean FEV1 63.98±15.17. Mean forced expiratory volume recorded before treatment (55.02±5.01) in a group who were treated with formoterol/mometasone combination changed to (72.06±5.86) after treatment. However, the mean forced expiratory volume recorded before treatment 54.92±4.47 in a group who were treated with formoterol/fluticasone combination changed to 75.48±5.03 after the treatment. While comparing the two treatment regimens, it is evident from the results that there is no significant difference in FEV1 between the groups. However, the post bronchodilator FEV1 was significantly (p<0.001) higher among the patient group which were treated with fluticasone/formoterol combination than the group who were treated with mometasone/formoterol combination. No significant adverse effect of either of two regimens was observed thus showing that both the combinations are comparatively safe for use.Conclusions: This study reveals that both the treatment regimens showed a significant improvement in lung functions without any significant adverse event.

Background: Functional tricuspid valve regurgitation secondary to left-sided valve disease is common. DeVega repair is simple, but residual regurgitation with subsequent impairment of the right ventricular function is a concern. This study aims to compare tricuspid valve repair using DeVega vs. ring annuloplasty and their impact on the right ventricle in the early postoperative period and after six months. Methods: This is a prospective cohort study of 51 patients with rheumatic heart disease who underwent tricuspid valve repair for secondary severe tricuspid regurgitation. Patients were divided into two groups: group A; DeVega repair (n=34) and group B; ring annuloplasty repair (n=17). Patients were assessed clinically and by echocardiography before discharge and after six months for the degree of tricuspid regurgitation, right ventricular diameter and tricuspid annular plane systolic excursion (TAPSE). Results: Preoperative echocardiographic assessment showed no difference in left ventricular end-systolic diameter, end-diastolic diameter, ejection fraction and right ventricular diameter, however; group A had significantly better preoperative right ventricular function measured by TAPSE (1.96 ± 0.27 vs1.75 ± 0.31 cm; p=0.02). Group B had significantly longer cardiopulmonary bypass time (127.65 ± 13.56 vs. 111.74 ± 18.74 minutes; p= 0.003) and ischemic time (99.06 ± 11.80 vs. 87.15 ± 16.01 minutes; p= 0.009). Pre-discharge, there was no statistically significant difference in the degree of tricuspid regurgitation, but the right ventricular diameter was significantly lower in group B (2.66 ± 0.41 and 2.40 ± 0.48 cm; p=0.049). After six months of follow up, the degree of tricuspid regurgitation (p= 0.029) and the right ventricular diameter were significantly lower in the ring annuloplasty group (2.56 ± 0.39 and 2.29 ± 0.44 cm; p=0.029). Although there was a statistically significant difference in preoperative TAPSE, this difference disappeared after six months. Conclusion: Both DeVega and ring annuloplasty techniques were effective in the early postoperative period, ring annuloplasty was associated with lesser residual regurgitation and better right ventricular remodeling in severe functional tricuspid regurgitation than DeVega procedure after 6-months of follow up.

Objective: To study the effect of short-term use of oral contraceptive pills on intra-ocular pressures of women of childbearing age.   Methods: It was a comparative observational study, conducted at Arif memorial teaching hospital and Allied hospital Faisalabad for a period of six months. Hundred female subjects were divided into two groups of 50 each. Group A, included females, who had been taking oral contraceptive pills (OCP) for more than 6 months and less than 36 months. Group B, included 50 age-matched controls, who had never used OCP. Ophthalmic and systemic history was taken. Careful Slit lamp examination was performed and intraocular pressures (IOP) were measured using Goldman Applanation tonometer. Fundus examination was done to rule out any posterior segment disease. After collection of data, we analyzed and compared the intra ocular pressures between the two groups by using ANOVA in SPSS version 21.   Results: Average duration of using OCP was 14.9 months. There was no significant difference of Cup to Disc ratios between the two groups (p= 0.109). However, significant difference was noted between the IOP of OCP group and controls. (p=0.000). Conclusion: OCP significantly increase IOP even when used for short time period.
