Pathogenetic features of periodontitis as complications of orthodontic treatment

The article presents the results of clinical and microbiological studies and immunological features of the state of periodontal tissues in patients with moderate periodontitis, which developed as a complication of orthodontic treatment. It was found that at the later stages of orthodontic treatment with the development of periodontitis, a complex of persistent changes in the periodontal tissues at different levels of pathogen-esis was revealed in patients. In this connection, in order to plan the complex treatment of patients with periodontitis, which has developed as a complication of orthodontic treatment, and to predict the course of the disease, it is necessary to influence the established and mutually supportive links and indicators of pathogenesis.

The aim of the study is to improve the results of complex treatment of patients with gingival recession on the basis of validation of new, highly effective methods using autologous mesenchymal stem cells. A complex treatment was conducted in 93 patients with an anatomical recession of the gum in combination with dentoalveolar anomalies at the age of 20 to 29 years. Patients were divided into 3 groups depending on the treatment used. All patients underwent periodontological treatment. In the 2nd group, patients underwent periodontological treatment including orthodontic equipment, in the 3rd group, they underwent orthodontic treatment, and a cell transplant based on a mixture of autologous mesenchymal stem cells and sterile bioplastic collagen material “Collost” was used. A comprehensive assessment of the periodontal tissue of the three groups showed that among the patients who received the standard treatment complex, a good condition of the periodontal tissues in 1 to 2 years after treatment was noted only in 16.7 % of the patients of the 1st group. Inclusion of orthodontic treatment made it possible to obtain good results in 25 % of patients in the 2nd group, which is 8 % more than in the 1st group. At the same time, good treatment results were revealed in 94 % of the 3rd group patients, who underwent periodontological and orthodontic treatment using autologous mesenchymal stem cells, which confirms the need for measures aimed to normalize the position of teeth, dentition and bite with the use of small orthodontic long-acting forces, to improve the microcirculation of periodontal tissues, and to restore the contour of marginal dreams. Inclusion of autologous mesenchymal stem cells in the complex treatment of patients with gingival recession is accompanied by a 1.6-fold decrease in the prevalence and gum recession (1.15 mm), and a 2.6-fold increase in the tissue volume, by the stimulation of periodontal microcirculation processes (the capillary periodontal pressure is increased by a factor of 1.1 time and the intensity of periodontic microcirculation is increased by a factor of 3.1 times), as well as by the normalization of the position of teeth, dentition and occlusion, which allowed one to obtain good long-term results in 94 % of patients.

To date, various methods of qualitative and quantitative assessment of dental-facial anomalies have been described in the medical literature. They describe the causes of dental anomalies, determine the localization of disorders, consider the functional state of the dental system, as well as quantify the degree of violations of facial aesthetics, occlusion, morphology and function in various dental-facial anomalies. To assess the effectiveness of orthodontic treatment, it is necessary to conduct a quality assessment in patients with dentofacial anomalies who have completed orthodontic treatment. The essence of the proposed method is that on the basis of tables developed and structured according to several blocks, an assessment of the quality of orthodontic treatment is proposed, including an assessment of the aesthetics of the face and smile, articulation and occlusal ratios, malocclusion anomalies in the sagittal, vertical and transversal directions, the state of periodontal tissues and hard tissues of the teeth, as well as the state of the temporomandibular joint in patients who have completed orthodontic treatment. The assessment of the quality of orthodontic treatment consists of three blocks: assessment of the quality of the results of orthodontic treatment, assessment of the process of orthodontic treatment, assessment of the maintenance of medical records. According to the results of filling in which, a conclusion is made about the quality of the orthodontic treatment. The overall assessment of the quality of orthodontic treatment is obtained by summing up the results of the three assessments received, and the assessment of the quality of clinical work is crucial. The method developed by us allows us to make an expert assessment of the process of orthodontic treatment, the quality of its results and the maintenance of medical records in patients who have completed orthodontic treatment. The article presents the results of the assessment of the quality of orthodontic treatment in the clinics of St. Petersburg.

Inclusion of autologous mesenchymal stem cells in the complex treatment of patients with gingival recession is accompanied by a 1.6-fold decrease in the prevalence and gum recession (1.15 mm), and tissue volume increased 2.6-fold, stimulation of periodontal microcirculation processes (in terms of capillary the periodontal pressure was 1.1 times and the intensity of periodontal microcirculation was 3.1 times), as well as the normalization of the position of teeth, dentition and occlusion, which allowed to obtain good long-term results in 94% of patients.

Relevance. Currently, researchers pay close attention to the study of chronic diseases that have a direct or indirect impact on the state of the patient’s dental status. It is known that chronic inflammatory diseases of the oral organs are, often simultaneously, permanent foci of infection that affect the state of other systems and organs. The general list of such diseases includes: chronic gingivitis, chronic generalized periodontitis, glossitis, gingivostomatitis.Aim. The aim of the study was to study the effectiveness of complex conservative therapy, including natural therapeutic factors of Crimea, in patients with chronic inflammatory diseases of the oral organs, occurring against the background of somatic pathology.Materials and methods. The objects of the study were periodontal tissues in 70 people with chronic generalized periodontitis (CGP) and gingivostomatitis, occurring against the background of gastrointestinal (GI) diseases.Results. The results obtained indicate that complex treatment with local application of hydrotherapy named “PhytoBiol” and thermal Saka mineral water (in a dilution of 1:4), combined with periodontal dressings according to the recipe proposed by us, cause persistent remission of these diseases, contribute to the improvement of periodontal tissues and oral mucosa. Thus, complex treatment of inflammatory diseases of periodontal tissues in patients in the main group (n = 35) led to long-term remission in 82.33% of cases, while in the control group the result was 69.5% (at p < 0.05).Conclusions. The positive results, which were obtained, give grounds to recommend the proposed complex of treatment and prevention of exacerbations of this chronic disease for use in practical health care.

The results of a study of the effectiveness of laser photodynamic therapy in complex treatment of periodontal diseases have been presented. Dental indices OHI-S, SBI, PMA, PI and the polarography method have been used as clinical criteria for assessing the state of periodontal tissues.

Ukraine is a unitary state, yet historically various regions, oblasts, districts, and local areas have different levels of economic development. To secure sustainable economic and social development and provide social services guaranteed by the state for each citizen according to the Constitution, the mechanism of redistribution between revenues and expenditures of oblasts, regions, and territories through the budgets of a higher level is used. The paper aims to research the peculiarities of improving interbudgetary relations in conditions of authorities’ decentralization. The paper defines the nature of interbudgetary relations. The basic and reverse subsidies to Ukraine and Lvivska oblast are analyzed. The advantages and disadvantages the communities face at changing approaches to balancing local budgets are determined. Regulative documents that cover the interbudgetary relations in Ukraine are analyzed. Special attention is paid to the problem of local finances reforming, including the development of interbudgetary relations. The scheme of the economic interbudgetary relations system in Ukraine is developed. The ways to improve the system of interbudgetary relations in Ukraine are suggested. The negative and positive aspects, advantages, and disadvantages of the system of interbudgetary relations in Ukraine require the following improvements. 1. It is necessary to avoid the complete budget alignment in the process of budgets balancing by interbudgetary transfers as the major objective. 2. The interbudgetary transfers should be distributed based on a formal approach. 3. The changes have to be introduced to the calculation of medical and educational subsidies in terms of financial standard of budget provision to avoid the money deficit for coverage of necessary expenditures. 4. There is a need to improve interbudgetary relations at the levels of districts, villages, towns, and cities of district subordination. 5. Improvement of the mechanism of targeted benefits provision, their real evaluation, and control for the use of funds.

Background: The crevicular fluid analysis represents a useful diagnosis tool, with the help of which noninvasive cellular metabolic activity can be analyzed. The aim of the study is to investigate comparatively IL1β and IL6 in the gingival crevicular fluid of clinically healthy adolescents and young adults during the acute phase of orthodontic treatment. Methods: Gingival crevicular fluid was collected from 20 patients (aged between 11 and 28) undergoing orthodontic treatment. Measurements were taken before (T0) and after 24 h after distalization forces were activated (T1). IL1β and IL 6 were analyzed using Elisa tests. The statistical tests used were two-sided t tests. Results: Between the two time periods there was a significant raise both in the crevicular fluid rate (0.57 µL at T0 vs. 0.95 µL at T1, p = 0.001) and in IL1β levels (15.67 pg/µL at T0 vs. 27.94 pg/µL at T1, p = 0.009). We were able to identify IL6 only in a third of the sites. There is a significantly increased level of ILβ at T1 in adolescents, more than in young adults (42.96 pg/µL vs. 17.93 pg/µL, p = 0.006). Conclusions: In the early stage of orthodontic treatment, the periodontal tissues of adolescents are more responsive to orthodontic forces than those of young adults.

Aim: To evaluate the periodontal status before and after orthodontic treatment and to analyze the confounding factors associated with it. Materials and Methods: The present retrospective study analyzed pre- and posttreatment records of a total of 60 patients. Intraoral digital photographs before and after the orthodontic treatment were used to measure the following three periodontal variables: (a) width of the keratinized gingiva, (b) gingival recession, and (c) the status of interdental papilla. The ImageJ 46 software imaging program was used to perform the required measurements. The clinical data were then analyzed in an association with the participants’ demographic data, the type of orthodontic tooth movement, and other confounding factors. The data obtained were manually entered into the statistical package and analyzed using a significance level set at P < .05. Descriptive statistics, paired t-tests, and Pearson’s correlation analysis were used to identify differences among the studied variables. Results: Among the 60 patients, the majority (66.7%) were females. Most cases of the treated malocclusion were Class II, and most patients underwent intrusion and retrusion movements. The width of keratinized gingiva increased for most sites of upper teeth. In contrast, the lower teeth showed a mix of improvement and compromised width of keratinized gingiva. As a measure of gingival recession, the difference of clinical crown height revealed the tendency toward a reduced clinical crown height for most sites. Upper left lateral incisors and canines showed significant values with regard to keratinized gingival width measurements and tooth movements such as extrusion and intrusion movements. Likewise, with regard to the clinical crown height, upper right central incisors showed significant differences when correlated with the interincisal angle, lower incisors to the mandibular plane, and upper incisors to the NA line. The upper lateral incisors also showed significant correlations to certain cephalometric measurements. Conclusions: The orthodontic treatment was found to exert a significantly positive impact on the surrounding periodontium, particularly in the upper canine areas. Likewise, various types of tooth movement were found to positively affect the periodontium.
