Various causes can be responsible for tooth loss. In general, caries, periodontal disease, facial trauma, pathology of the jaws, among other causes, could lead to the loss of a tooth or a group of teeth. As a consequence, the stimuli that participate in bone maintenance are compromised and bone reduction occurs gradually, making it difficult to use conventional prostheses. Fortunately, technological advances applied to dental implantology have allowed us to perform full-arch prosthetic treatments, managing to rehabilitate the form, function, esthetics and lost self-esteem in patients with severe atrophy of the jaws. The objective of this chapter is to describe the key and current aspects in full-arch rehabilitation with dental implants.

1. Background: Loss of permanent teeth is very common, affected chewing function, speech and aesthetics; restoration of missing teeth with dental implant has several advantages, but we need thoroughly study the clinical and X ray features at the position at missing teeth, then to select the type of implant and make the best plan for the dental implant patients. 2. Materials and method: Cross-section descriptive study. From January 2009 to November 2010, study with 56 patients with 102 implants of MIS and Megagen systems at the Faculty of Odonto-Stomatology, Hue College of Medicine and Pharmacy and Vietnam-Cuba Hospital in Ha Noi. 3. Results: Distributed equally in male and female, common ages 40 – 59 (55,4%), the majority of missing teeth occurs in the lower jaw (63,8%) and especially, the teeth 36 and 46 (25,4%). The majority of missing teeth due to dental caries, dental pulp and apical diseases (64,7%) of the molar teeth (51,9%); the most position of missing tooth have enough bone for dental implants (87,3%), time of tooth loss and bone status in the position of tooth loss are related to each other (p < 0,01). Diameter and length of implant usually used 4.0 – 6.0 mm (63,7%) and 8.5 – 13.0 mm (83,3%). 4. Conclusion: Clinical and X ray features of edentulous patients has an important role in determining the type of implants and treatment planning of dental implants. Periodontal disease is the most common chronic inflammatory disease seen in humans. It is a major public health concern, and in its severe form affects approximately 10.8% or 743 million people aged 15−99 worldwide. Trends such as the rise of smoking in developing countries, the obesity and diabetes epidemic, coupled with an ageing population with greater tooth retention, are all likely to increase the burden of periodontitis still further in the UK and worldwide. Consequences of periodontitis include hypermobility of teeth, tooth migration, drifting and eventual tooth loss. Tooth loss can directly affect the quality of life of a person in terms of reduced functional capacity, self-esteem and social relationships. CPD/Clinical Relevance: This article reports the prevalence of periodontal disease in the UK and worldwide, along with the consequences of periodontitis. The importance of timely diagnosis to avoid litigation is discussed, as is the importance of effective management of periodontitis in order to improve patients' oral health-related quality of life.

<p class="abstract"><span lang="EN-US">Severe periodontal disease often leads to tooth loss, necessitating prosthetic rehabilitation to restore function and aesthetics. The concept of perio-prosthetic treatment using extensive bridges of cross-arch design was introduced approximately 30 years ago. Long term follow-up studies have shown that teeth with reduced periodontal support can be used as abutments for extensive fixed prostheses, provided periodontal disease had been treated successfully and an effective recall program had been instituted to prevent periodontal disease recurrence. Implants along with cross arch bridges in controlled periodontitis subject have never been used together for complete full mouth rehabilitation.</span></p>

ABSTRACT Aim and background In clinical practice, we come across patients with traumatically amputated or congenitally missing partial or complete fingers that can be restored using microsurgical replantation or transplantation procedures. However, in some cases this might not be possible due to systemic or local factors and the lost or missing part has to be replaced prosthetically to offer psychological and functional wellbeing. These prostheses can be constructed with various materials like acrylics or silicone retained with the help of auxiliary aids. However, these prostheses cause some hindrance in performing functions like writing, typing, etc. The aim of the present trial was to ameliorate the existing design of implant supported finger prosthesis. Technique Distal phalange of middle finger replaced with implant supported silicone finger prosthesis is modified by utilizing a metal framework to support silicone material to improve rigidity while working. Conclusion and clinical significance We could achieve a good function, esthetics and tactile sensibility with this modified design. Whenever, feasible this design can improve the performance and patients feel a deep sense of satisfaction and improved self-esteem with this modified prosthesis. Resume. According to epidemiological studies, there is still an increased prevalence of periodontal disease among the population of different regions of the world, with increasing intensity and aggressiveness. Long-term chronic generalized lesions of recurrent and progressive inflammatory-dystrophic process lead to the destruction of alveolar processes, pathological mobility of teeth, tooth loss and dentofacial dysfunctions. One of the most characteristic symptoms of generalized periodontitis is the presence of traumatic occlusion. Depending on the degree of pathological changes in periodontal tissues, the size and topography of dentition defects, the replacement of dentition pathologies should be planned by splint-type orthopedic structures and computerized analysis of occlusal relationships with T-Scan III system. The aim of the study is to improve the effectiveness of orthopedic treatment of periodontal diseases complicated by defects of the dentition with the control of occlusal relationships with a computerized T-Scan III system. Materials and methods. We have examined 87 persons aged between 30 and 69 years with generalized periodontitis of I – III severity, complicated by partial loss of teeth of different sizes and topography, and a control group of the same age in 10 patients with intact dentition and physiological forms of occlusion without clinically diagnosed periodontal diseases. Computerized analysis of occlusion indices was performed by the T-Scan III system. We have investigated the stereotype of functional loads according to the following quantitative and qualitative indicators: the index of asymmetry of force between the parts (%); time of occurrence of the maximum amount of dental contacts (sec); time of disclusion (sec); the presence of premature occlusal contacts on the natural teeth and orthopedic constructions, and the presence of changes in the direction of the trajectory of the total vector of occlusal load.  Orthopedic rehabilitation of patients in the case of periodontal diseases of the I-II degree of severity and the presence of small defects of the dentition, we conducted by splint-type orthopedic structures, dental implants, and artificial crowns of our own design with discrete occlusal loading (Licence for Effective Model No 143278, Ukraine, MPK А61С 13/007, u 2019 11487: Artificial Crown with Support on a Dental Implant). Patients with periodontal diseases of the II – III severity, complicated by defects of dentitions with different size and topography, the dental prosthetics has been performed by traditional removable and combined orthopedic splint-type constructions, and removable splint-type dentures of their own design: "Removable Prosthesis of Splint Type" (Licence for Effective Model No 130980 Ukraine, IPC (2006.01) А61С 13/007, u 2018 03039, 10.01.2019).  Computerized analysis of occlusion indicators for the patients of I and II clinical groups and the III control group has been performed before and after orthopedic treatment by T-Scan III system. Results of the research. The results of investigation for functional occlusion have demonstrated that the index of relative strength asymmetry between the sides of the dentition in patients with generalized periodontitis complicated by dentition defects increased significantly and led to nonphysiological redistribution of loads compared with indices obtained in patients from control group due to the lack of correspondence for occlusal contacts. These indices are equal to 16.7 ± 3.8% in patients of I group and 18.9 ± 4.8% in group II compared to the similar control indicators – 4.7 ± 1.01%, respectively (p≤0.01). It should be noted that these indicators have differed from the control group indicators according to the severity of the pathological process. Rational prosthetics with removable and non-removable orthopedic structures, including on the base of dental implants, helps to optimize occlusal relations. Conclusions. Occlusographic studies performed in patients with generalized periodontitis having different severity and complicated by partial tooth loss have showed increased index of relative strength asymmetry between the sides of chewing, the presence of premature contacts and incorrect location of the trajectory of the total occlusal load vector. The dynamics of pathological changes in occlusion indices corresponds to the degree of generalized periodontitis, which indicates the necessity of opportune replacement of dentition defects, taking into account the occlusal relations. Rational prosthetics helps to optimize occlusal relations.

The role of occlusion in the progression of periodontal disease remains a controversial subject. Occlusal force, which is a mechanical stress applied to tissues, has always been considered to not initiate, nor accelerate, periodontal attachment loss resultant from inflammatory periodontal disease. This chapter outlines this controversy in great detail, from the perspective that the absence of a validated occlusal force and timing measuring device that can quantify the occlusion, has contributed to the confusion and questions that exist in the scientific community about the relationship between both periodontal disease and peri-implantitis, and the occlusion. The development of a new occlusal measurement technology that records and analyzes precise and reproducible relative occlusal contact force levels in real-time, independent of a clinician's subjectivity, is helping to change the scientific opinion regarding occlusion's role in periodontal and peri-implant supporting tissue loss. The T-Scan 10 system is particularly adapted for treating patients who demonstrate tissue loss combined with occlusal issues. Indeed, after having controlled the major etiologic and risk factors of periodontal disease and peri-implantitis, adjusting the occlusion after active tissue and implant therapy favors healing. The outcome of periodontal treatment aimed at compromised teeth and dental implants, combined with occlusal force excess control from computer-guided targeted occlusal adjustments, is highly predictable, and is characterized by less inflammation, a decrease of probing depths, and the stabilization of bone levels around teeth and dental implants.

Development of the self is a vital aspect during the period of adolescence. Interaction with peers contributes to the development of various aspects of self. Due to the technological advances in today's times, adolescents interact with their peers through social media sites and portals. It is essential to study this development in light of the increasing use of social media by adolescence. Thus, the study aimed at developing an item pool to tap the construct of social media influencing self-esteem of adolescents following the procedure of tool construction. Participants included adolescents ranging between 16 to 18 years of age, who have at least one social media account for personal use. There were 110 participants for the first phase and 397 participants for the second phase of the study. The scale has eight items with the overall reliability of .7. It indicates a fitting measure of self-esteem influenced by social media, with looking-glass self theory, according to which individuals develop their self, based on their perceptions of others responses to their behaviour.
