Background: Brucellosis, a major health problem in developing countries, is a multisystem infection with a broad spectrum of clinical manifestations. Hematological complications, ranging from an intravascular coagulopathy to mild homeostasis disorders (such as gammopathy), have been reported in brucella infection. These signs and symptoms may lead to misdiagnosis of brucellosis with other hematological diseases. Case: A 65-year-old male whose occupation was shepherding was referred to our hospital as a known case of multiple myeloma with continuous fever, muscle weakness, and night sweating after taking 2 courses of chemotherapy. The laboratory diagnosis of multiple myeloma had been based on the observation of a high percent of plasma cells in the bone marrow aspiration. At follow- up, the result of patient's fever workup, with 2 sets of blood cultures, was positive for Brucella melitensis. Isolated brucella was confirmed as B. melitensis by 16S rRNA sequencing. Brucellosis serologic test was performed by agglutination test and positive results were obtained. The patient was discharged with the cessation of fever and general improvement after the end of the parental treatment phase of brucella bacteremia. Conclusions: Brucella infection may cause a severe disease, mimicking a primary hematological disease, which could complicate the correct diagnosis. In brucellosis cases, due to the wide range of symptoms, in addition to cultivation and serological methods, molecular methods should also be used to prevent inappropriate diagnosis and additional costs.

Objective: Brucellosis- one of the major zoonotic diseases, still remains an uncontrolled problem, in regions of high endemicity. Ophthalmic brucellosis is not studied and overlooked in most developing countries.  Considering the severe outcome of undiagnosed ophthalmic brucellosis, in this paper we made attempts to find out whether this disease still remains a health problem in a South East Asian developing country, where study of this disease is largely neglected.  Methods This study was carried out over a one year period from January 2015 to December 2015. Blood samples were collected from clinically confirmed cases of uveitis and they were subjected to five serological and one genus specific molecular investigations, for the detection of Brucella infection.Results Out of 20 uveitis cases, 4 (20%) cases confirmed as brucellar uveitis, by serological tests followed by PCR confirmation. After treatment of brucellosis all the four patients were recovered uneventfully.Conclusion: Brucella infection involving the eye is still a significant problem in South East Asian countries; hence in all uveitis cases in this reason brucellosis should be excluded by available laboratory tests.Government of India (Dept. of Biotechnology the major zoonotic diseases, still remains an uncontrolled problem, in regions of high endemicity. Objective: Brucellosis is a zoonotic disease seen widely around the world. Although many aspects and treatment of this disease is well known, peritoneal involvement and ascites is not well established so far. Material and Methods: This study retrospectively enrolled 346 adult patients (aged >17 years) with acute Brucellosis attending Hepatology Clinic, Van Yuzuncu Yil University, between April 2013 and May 2016. Characteristics of those with and without ascites were analyzed using Pearson correlation coefficients and Chi-Square test in SPSS software system. Results: Of the 346 cases, 20 (5, 7%) had ascites. Those with ascites had significantly higher transaminase, cholestatic enzyme and amylase levels compared to those without ascites. Conclusions: We conclude that acute Brucella infection can lead to a unique low gradient ascites probably resulting from pancreatic leakage followed by peritoneal accumulation of serum proteins.
