Abstract Background and Study Aims The traditional percutaneous placement of dorsal root ganglion (DRG) electrodes may not be eligible for every patient. In this tertiary spine surgery and interventional pain therapy center, alternative neurostimulation implantation techniques were developed and applied where standard percutaneous approaches failed or were contraindicated. Case presentation Three alternative implantation techniques can be used: (1) open surgical placement of DRG leads, (2) two-lead insertion via a lateral to medial transforaminal approach (level L3), and (3) percutaneous approach with two leads close to the spinal nerves L4 (peripheral nerve stimulation). Results The placement of the leads occurred without complications and resulted in similar expected outcomes as with the common percutaneous technique with long-term stable pain suppression at 7 months and 1 year. Conclusions In patients in whom the DRG cannot be approached by the standard percutaneous approach, at least three alternatives may be used in experienced hands resulting in stable pain suppression of similar magnitude.

Background: The mortality risk increases greatly in patients with cancer if they are infected with severe acute respiratory syndrome coronavirus 2. The new American Society of Clinical Oncology (ASCO) and the European Society of Medical Oncology (ESMO) guidelines for the COVID-19 pandemic suggested modifications to the standards of care to reduce harm from treatment. However, it is unclear whether these changes suit the wishes of patients. Methods: We conducted a survey of patients with gastrointestinal cancer who were undergoing active chemotherapy in our ambulatory therapy center between August 17 and September 11 2020. The survey comprised 18 questions on five topics: patient characteristics, lifestyle changes, disturbance in their psychological health, thoughts on the cancer treatment, and infection control in the hospital. Results: Among the 63 patients who received the questionnaire, 61 participated in the study. The COVID-19 pandemic has led to changes in their lifestyles and substantially impacted their psychological wellbeing. The incidence of anxiety and insomnia has considerably increased during the pandemic. However, female patients and patients aged 70 years or older reported no notable differences. There was no significant difference in the responses to the questions regarding thoughts on the cancer treatment. Conclusion: Our study revealed that the COVID-19 pandemic has substantially impacted patients’ lifestyles and psychological wellbeing. However, most patients preferred to continue their usual treatment without any change to their treatment plan. It is important to involve the patient in the decision-making process when formulating treatment goals.

Abstract The Medipix3, a hybrid pixel detector with a silicon sensor, has been evaluated as a beam instrumentation device with proton and carbon ion measurements in the non-clinical research room (IR1) of MedAustron Ion Therapy Center. Protons energies are varied from 62.4 to 800 MeV with 104 to 108 protons per second impinging on the detector surface. For carbon ions, energies are varied from 120 to 400 MeV/amu with 107 to 108 carbon ions per second. Measurements include simultaneous high resolution, beam profile and beam intensity with various beam parameters at up to 1000 FPS (frames per second), count rate linearity and an assessment of radiation damage after the measurement day using an x-ray tube to provide a homogeneous radiation measurement. The count rate linearity is found to be linear within the uncertainties (dominated by accelerator related sources due to special setup) for the measurements without degraders. Various frequency components are identified within the beam intensity over time firstly including 49.98 Hz with standard deviation, σ = 0.29, secondly 30.55 Hz σ = 0.55 and thirdly 252.51 Hz σ = 0.83. A direct correlation between the number of zero counting and noisy pixels is observed in the measurements with the highest flux. No conclusive evidence of long term radiation damage was found as a result of these measurements over one day.

Nowadays, the huge amount of patient’s data significantly increases with respect to the time in repositories and data mining is increasingly used as an emerging research area in medical fields for extracting useful and previously unknown insights/patterns from the repository data. These unknown patterns/hidden insights can help in discovering new knowledge hidden in these data repositories. From the observation, different ARV regimens were ordered for different patients. However, combination of these drugs causes different side effects on the patients. It has been observed that there was a lack of predictive studies and designed models available in hospitals specifically ART Centers that accurately determine or classify the patient’s ARV regimen to TDF + 3TC + EFV, TDF + 3TC + NVP, AZT + 3TC + ATV/R, AZT + 3TC + LPV/R, TDF + 3TC + LVP/R, TDF + 3TC + ATV/R, 8888, and ABC + 3TC + LPV/R. In order to solve these kinds of problems, we built an accurate classifier system or model using parameters like Patient Age, Patient Encounter Day, Patient Encounter Month, Patient Encounter Year, Patient Weight, Patient CD4 Count Adult, Patient TB Screen, Patient Following WHO Stage, Patient CD4 Percent Child, Patient Regimen Specify, Patient Regimen, and so on. The general objective of this research was predictive modeling for the patient’s ARV regimen class through data mining techniques so as to improve them. The study used the CRIPS-DM methodology to find and interpret patterns in repositories. A decision tree (J48 and Random Forest) algorithm was used for classification. Using all tested classifiers, the investigation of the study shows that the total accuracy was more than 60%. On the other hand, among different classifications, class H (ABC + 3TC + LPV/R) has shown the worst prediction. But it was revealed that the J48 classifier relatively produces higher classification accuracy for the D (AZT-3TC-NVP) regimen. Here, classification depended on the selected parameters, which revealed that prediction accuracy value differed among all classifiers and the selected attributes. Finally, the study concluded that data mining can be used as a significant technique to discover patient regimen based on salient affecting factors with 96.1% precision achieved. Ensemble learning resolves the categorizing models of greater anticipating performance with different learning algorithms. This model aligned with sentimental investigation to magnify the appearances of the dataset either from the social media or from primary data collection. The empirical investigation with different parameters shows the detailed improvement of their learning methods.

Background:Adeno-associated virus (AAV)-mediated gene therapies are rapidly advancing to the clinic, and AAV engineering has resulted in vectors with increased ability to deliver therapeutic genes. Although the choice of vector is critical, quantitative comparison of AAVs, especially in large animals, remains challenging. Methods:Here, we developed an efficient single-cell AAV engineering pipeline (scAAVengr) to simultaneously quantify and rank efficiency of competing AAV vectors across all cell types in the same animal. Results:To demonstrate proof-of-concept for the scAAVengr workflow, we quantified - with cell-type resolution - the abilities of naturally occurring and newly engineered AAVs to mediate gene expression in primate retina following intravitreal injection. A top performing variant identified using this pipeline, K912, was used to deliver SaCas9 and edit the rhodopsin gene in macaque retina, resulting in editing efficiency similar to infection rates detected by the scAAVengr workflow. scAAVengr was then used to identify top-performing AAV variants in mouse brain, heart and liver following systemic injection. Conclusions:These results validate scAAVengr as a powerful method for development of AAV vectors. In this retrospective descriptive study we focus on cases of three patients who underwent phage therapy procedures at Eliava Phage Therapy Center (EPTC) in Tbilisi, Georgia. Patients with chronic infectious diseases related to Pseudomonas aeruginosa (two patients, lower respiratory tract infection (LRTI)) and Klebsiella pneumoniae (one patient, urinary tract infection (UTI)) are among those very few EPTC patients whose pathogens persisted through phage therapy. By looking at bacterial strains and personalized phages used against them we tried to point towards possible adaptation strategies that are employed by these pathogens. Genome restriction-based Pulsed Field Gel Electrophoresis (PFGE) profiling of strains isolated before and after phage therapy hints towards two strategies of adaptation. In one patient case (Pseudomonas aeruginosa related lung infection) bacterial strains before and after phage therapy were indistinguishable according to their PFGE profiles, but differed in their phage susceptibility properties. On the other hand, in two other patient cases (Pseudomonas aeruginosa related LRTI and Klebsiella pneumoniae related UTI) bacterial adaptation strategy seemed to have resulted in diversification of infecting strains of the same species. With this work we want to attract more attention to phage resistance in general as well as to its role in phage therapy.
