Abstract Background: Large inequalities in child health remain in Nepal, with caste, ethnicity and sex being major determinants of deprivation and negative outcomes. The purpose of this study was to explore whether key demographics on under 5s were associated with health seeking behaviours, utilisation of health care, and treatment received.Methods: Data came from Integrated Management of Neonatal & Childhood Illness (IMNCI) records of 23 health centres across five districts. After digitising the paper records, district, ethnicity, sex, age and temperature of the child were used to predict the number of days taken to seek medical care for Acute Respiratory Infection (ARI), diarrhoea and fever. In addition to this, correct diagnosis and subsequent treatment of pneumonia was assessed against IMNCI guidelines, again using the demographic factors of interest to predict these outcomes.Results: From 116 register books spanning 23 health centres, 33,860 child patient records were considered for analysis. The median age of attendance was 16 months (Inter-Quartile Range= 9, 30), while there were more male children that attended (55.8% vs. 44.2% for females). There were statistically significant differences for the time taken to attend a health centre between different districts for ARI, diarrhoea and fever, with children in the remote Humla and Mugu districts taking significantly longer to present at a health facility after the onset of symptoms (all p<0.012). Children from underprivileged ethnic groups, Madhesi and Dalit, were less likely to be given a correct diagnosis of pneumonia (p=0.001), while males were more likely to receive a correct diagnosis than females (73% vs. 67%, p=0.001). This sex difference remained in the adjusted regression models for diagnosis of pneumonia (p=0.011) but not for treatment of pneumonia (p=0.202).Conclusions: Significant demographic differences were found based on ethnicity, sex, and district when examining health seeking behaviours for ARI, diarrhoea, and fever. Significant associations were seen for these same factors when exploring accuracy of diagnoses of pneumonia, but not for treatment. This study has emphasised the importance of a digitalised healthcare system, where inequalities can be identified without the reliance on anecdotal evidence.

AbstractMutations in ANO3 have recently been identified as an autosomal dominant cause of dystonia (dystonia-24). Since then, the phenotypic spectrum has also been extended in children. Here, we reported a case of a 10-year-old Turkish girl child patient with a novel variant (NM_001313726: c.221dupA, p.Tyr74*), who exhibited tremor with mild dystonia. This report expands the phenotype caused by ANO3 variants and reveals an essential clinical aspect for patients and medical staff.

PurposeThis study suggest the development of a wearable orthotic device pattern that can reduce pain and deformation, and help in the normal development of children with cerebral palsy. Such a pattern enables daily wear before hip subluxation occurs, to prevent hip dislocation and subluxation.Design/methodology/approachThis study set the design line by carrying out cell work on the actual model, then proceeded with the first pattern design. The final version of the second orthotic device was designed by conducting discussions with experts and the patient's guardian, with the device fitted to the child patient. The evaluation of the second orthotic device used the virtual model to check the pressure area and level through virtual fitting. An evaluation was then conducted with the device fitted to the child patient, to verify the functionality and suitability of the final pattern.FindingsFollowing the initial fitting evaluation, the second pattern was presented after modifying and supplementing issues such as movement suitability with posture change, position change of the great trochanter when wearing a diaper, pressure control of the X-shaped band on the genital area and thigh abduction. The master pattern of the final version of the second orthotic device was proposed after confirming that the femoral head of the hip joint was stably fixed, and the compression was applied through a verification based on the virtual fitting using the virtual model, and with the device fitted to the child patient.Originality/valueWith this study, it is expected that the process and design plan for the development of wearable orthotic device patterns for the persons with disabilities impaired mobility can be used as a basic resource to create devices that merge the clothing and medical fields.

AbstractA 3.5-year-old male child patient with mild developmental delay presented with history of acute onset fever, encephalopathy, and dyskinesia. The patient was investigated for common etiologies and was managed supportively. His neuroimaging was suggestive of vanishing white matter (VWM) disease which was confirmed by clinical exome sequencing. The child had an eventful hospital stay followed by near-total recovery after 4 weeks. The case attempts to sensitize readers about the current perspectives pertaining to VWM disease.

AbstractMultisystem inflammatory syndrome in children (MIS-C) is a severe disease that can lead to death. There is no definitive treatment for MIS-C yet. It has been reported that intravenous immunoglobulin, intravenous methylprednisolone, fluid supplements, antibiotics, inotropics, extracorporeal membrane oxygenation (ECMO), plasmapheresis, biological therapy, and anticoagulation therapy can be used for treatment. In this article, we presented an 8-year-old girl child patient who survived due to timely administered ECMO and combined therapies including plasmapheresis without any sequela despite her life-threatening condition due to MIS-C.

Abstract In a 6-year-old child patient, transthoracic echocardiography revealed a large saccular structure (with anechogenic content) in the left atrium, near the fossa ovalis, and diagnosis of the left atrial septal pouch was made. The left atrial septal pouch is a kangaroo pouch-like structure on the left side of the interatrial septum, opened into the left atrial cavity without a connection between the left and right atria. It occurs when the foramen ovale is absent but the septum primum and septum secundum are only partially fused. The left atrial septal pouch is believed to be present in 47% of population. In many cases, the pocket on the atrial septum is small and it could not be detected by transthoracic echocardiography. Our description is uncommon because we diagnosed a very large septal pouch. Based on our knowledge, this is the youngest reported case of the left atrial septal pouch and the longest follow-up described in this type of congenital heart malformation.

Nowadays, the huge amount of patient’s data significantly increases with respect to the time in repositories and data mining is increasingly used as an emerging research area in medical fields for extracting useful and previously unknown insights/patterns from the repository data. These unknown patterns/hidden insights can help in discovering new knowledge hidden in these data repositories. From the observation, different ARV regimens were ordered for different patients. However, combination of these drugs causes different side effects on the patients. It has been observed that there was a lack of predictive studies and designed models available in hospitals specifically ART Centers that accurately determine or classify the patient’s ARV regimen to TDF + 3TC + EFV, TDF + 3TC + NVP, AZT + 3TC + ATV/R, AZT + 3TC + LPV/R, TDF + 3TC + LVP/R, TDF + 3TC + ATV/R, 8888, and ABC + 3TC + LPV/R. In order to solve these kinds of problems, we built an accurate classifier system or model using parameters like Patient Age, Patient Encounter Day, Patient Encounter Month, Patient Encounter Year, Patient Weight, Patient CD4 Count Adult, Patient TB Screen, Patient Following WHO Stage, Patient CD4 Percent Child, Patient Regimen Specify, Patient Regimen, and so on. The general objective of this research was predictive modeling for the patient’s ARV regimen class through data mining techniques so as to improve them. The study used the CRIPS-DM methodology to find and interpret patterns in repositories. A decision tree (J48 and Random Forest) algorithm was used for classification. Using all tested classifiers, the investigation of the study shows that the total accuracy was more than 60%. On the other hand, among different classifications, class H (ABC + 3TC + LPV/R) has shown the worst prediction. But it was revealed that the J48 classifier relatively produces higher classification accuracy for the D (AZT-3TC-NVP) regimen. Here, classification depended on the selected parameters, which revealed that prediction accuracy value differed among all classifiers and the selected attributes. Finally, the study concluded that data mining can be used as a significant technique to discover patient regimen based on salient affecting factors with 96.1% precision achieved. Ensemble learning resolves the categorizing models of greater anticipating performance with different learning algorithms. This model aligned with sentimental investigation to magnify the appearances of the dataset either from the social media or from primary data collection. The empirical investigation with different parameters shows the detailed improvement of their learning methods.

Objectives: This study aimed to evaluate perceptions of undergraduate dental students and graduate dentists towards behaviouralguidance techniques in paediatric dentistry. Materials & Methods: 225 subjects with previous experience in treating paediatricpatient were identified and invited to fill up a Likert-style questionnaire with 15 questions in total. The data was evaluated for descriptive analysis and statistical analysis using Pearson’s chi-square test and Fisher’s exact test(significance level, p<0.05). Results: 158 respondents were included in this study. 47.4% think that communication is the most effective non-pharmacological method of behavioural guidance, 62.6% and 45.6% think that tell-show-do is the most practical and most successful behaviour shaping techniques, respectively. 43.7% responded neutral to the use HOME or physical restraints in controlling child patient, 81.0% almost never used HOME method. 87.3% had explained procedures of dental treatment to child patientsduring treatment, 89.2% had used euphemism on the child. 40.5% of child patientssometimes struggled when receiving their treatment. 33.5% required some effort to get their child patient treated. 79.1% had never usedprotective stabilization method. 75.9% think that their working environment is favourable and conducive for child patients. 39.2% think that desensitization is the most effective method to reduce dental fear and anxiety in child patient. 59.5% preferpresence of parents during treatment. 56.35% of frightened cry was most commonly seen clinically. 73.4% would not consider further studies in paediatric dentistry. Conclusion: Generally, the respondents demonstrated satisfactory knowledge and acceptance of various behavioural guidance techniques in paediatric dentistry, but with some deficiencies.
