We prospectively studied SARS-CoV-2 transmission at schools in an era of Variants of Concern (VoCs), offering all close contacts serial viral asymptomatic testing up to 14 days. Of 229 school close contacts, 3 tested positive (1.3%), of which 2 were detected through asymptomatic testing. Most secondary transmission (90%) occurred in households. Routine asymptomatic testing of close contacts should be examined in the context of local testing rates, preventive measures, programmatic costs, and health impacts of asymptomatic transmission.

In October 2016, Public Health England was initially notified of four cases of cryptosporidiosis among users of two swimming pools. We investigated to identify further cases, the outbreak source, and ensure the implementation of appropriate control measures. Probable primary cases had diarrhoea and reported swimming in the pools 1–12 days prior to illness; confirmed cases were verified by the reference laboratory. Secondary cases had contact with primary cases 1–12 days prior to illness. We identified twenty-two cases: eleven were primary (eight confirmed) and eleven were secondary (five confirmed). Four cases were infected with C. parvum (different gp60 subtypes); all were primary and swam at two pools. Seven primary and secondary cases were infected with C. hominis gp60 subtype IdA16, and all were associated one pool. Failings in pool water treatment and management were identified that likely contributed to the load on the filters and their efficiency. Our investigation identified a complex outbreak, with secondary transmission, involving exposures to two swimming pools. C. hominis IdA16 is rare; it has been isolated from only three previous UK cases. We hypothesize that C. hominis cases arose from a common exposure, and the C. parvum cases were likely sporadic. This investigation highlights the value of integrating epidemiology and microbiology to investigate clusters of Cryptosporidium cases, defining the extent of the outbreak and the likely transmission pathways.

Abstract Background Understanding household transmission dynamics of infectious diseases can help develop mitigation strategies. Traditional methods of population-level disease surveillance do not capture household transmission. Data collected from smartphone-connected thermometers that can differentiate among individuals in a household can be used to study these characteristics. Using this technology, we estimated the household secondary attack rate (SAR) of febrile illness, assessed its correlation with CDC-reported influenza-like illness (ILI) and COVID-19 case incidence, and identified risk factors for secondary transmission. Methods We conducted a retrospective cohort study among 596,096 febrile illness index cases recorded from August 1, 2016 to January 20, 2021 in households with two or more individuals in all 50 states. Fevers were measured using the Kinsa Smart Thermometer and mobile device app. Secondary cases were defined as household members who recorded a fever 1–10 days after an index case. We calculated SAR prior to and during the COVID-19 pandemic within the study period, and assessed correlation to ILI and COVID-19 case incidence using Spearman’s rank correlation coefficient. Bivariate and multivariable mixed logistic regression models were used to identify risk factors for secondary transmission. Results SAR in the pre-COVID-19 period was 5.9% (95% CI: 5.8%–6.0%) during flu season (November to April), and 3.7% (95% CI: 3.6%–3.7%) in flu off-season, and weekly SAR was significantly correlated with ILI reported from CDC (ρ=0.84, p< 0.001). Secondary transmission was 40% more likely to occur in households where the index case’s initial temperature was ≥ 39.1°C. During the COVID-19 period, SAR was 3.3% (95% CI: 3.3%–3.4%), and daily SAR was significantly correlated with national daily COVID-19 incidence rates (ρ=0.86, p< 0.001). Households in census tracts with >50% essential workforce were 50% more likely to experience secondary transmission. Conclusion Household SAR was highly correlated with ILI and COVID-19 cases. Capturing household transmission of febrile illness through routine public health surveillance may identify risk factors for infectious disease transmission, allowing for more targeted interventions. Background: Dialyzed patients are vulnerable to coronavirus infection disease 2019 (COVID-19). The incidence and outcome of COVID-19 in hemodialysis (HD) patients in Taiwan remain unclear. A series of preventive measures were executed to combat COVID-19 transmission among HD patients. Methods: We carried out a series of forward-looking and practical preventive strategies of COVID-19 control in our HD center. Incidences of COVID-19 of our HD unit were compared with those of national and local estimates from a community outbreak from 15 May to 30 June 2021. Prognostic factors associated with mortality were analyzed. Results: The national incidence of COVID-19 was 0.062%; being highest in Taipei City (0.173%), followed by New Taipei City (0.161%) and Keelung (0.083%). The overall incidence in Keelung HD patients was 0.666%. One patient of our HD center contracted COVID-19 from the household; however, we have contained secondary transmission in our HD center by implementing strict preventive measures. The mortality rate of HD patients in Keelung was 66.6%. The median Ct value of HD patients was 17.53 (11.75–27.90) upon diagnosis. The deceased patients had a higher cardiac/thoracic ratio than alive (0.61 vs. 0.55, p = 0.036). Conclusions: Taking aggressive and proactive infection preventive measures impedes the secondary transmission of COVID-19 in HD facilities. COVID-19-associated mortality was high in HD patients, being the high cardiac-thoracic ratio, an important prognostic factor for clinical outcome of infected HD patients.

Background: We aimed to quantify the risk of transmission of SARS-CoV-2 in the school setting by type of school, characteristics of the index case and calendar period in the Reggio Emilia province (RE), Italy, from school reopening in September 2020 until the beginning of April 2021. The secondary aim was to estimate the promptness of contact tracing. Methods: A population-based analysis of surveillance data of all COVID-19 cases occurring in RE, Italy, from September 1, 2020, to April 4th, 2021, for which a school contact and/or exposure was suspected. Indicator of the delay in contact tracing was computed as the time elapsed since positivity of the index case and the date on which the swab for classmates was scheduled (or most were scheduled). Results: Overall, 30,426 and 13,571 contacts among classmates and teachers/staff, respectively, were identified and received recommendation for testing; 43,414 (98.7%) performed the test. Secondary transmission occurred in about 40% of the investigated classes, and the overall secondary case attack rate was 4%, slightly higher when the index case was a teacher, but with almost no differences by type of school and stable during the study period. Promptness of contact tracing increased during the study period, reducing the time from index case identification and testing of contacts from 7 to 3 days, as well the ability to identify possible source of infection in the index case (from 42% in September/October, to 22% in November, to 50% in December-April). Conclusions: Despite the spread of the Alpha variant during the study period in RE, the secondary case attack rate remained stable from school reopening in September 2020 until the beginning of April 2021.

Background: Roles of children and adolescents in spreading coronavirus disease 2019 (COVID-19) in the community is not fully understood.Methods: We analyzed the data of 7,758 children and adolescents with COVID-19 and characteristics of secondary transmission generated by these cases using case information published by local governments. Ratio of pediatric and adolescent cases generating secondary transmission was calculated for various social settings.Results: The incidence of COVID-19 was 24.8 cases per 105 population aged between 0 and 9 years, and 59.2 among those aged between 10 and 19 years, which was lower than that among individuals of all age groups (79.6 per 105 population) between January 15 and October 31, 2020. The proportion of cases generating secondary cases was 8.3% among infants and young children in nursery schools and kindergartens, 16% among children and adolescents attending primary schools, 34% among those attending junior high schools, 43% among those attending high schools, 31% among those attending professional training colleges, and 24% in those attending universities. Households were the most common setting for secondary transmission.Conclusion: The risk of generating secondary cases might be limited among pediatric and adolescent cases with COVID-19, especially in settings outside households. Effectiveness of traditional mitigation measures (e.g., school closures) to suppress COVID-19 transmissions should be carefully evaluated.

Abstract Cases of SARS-CoV-2 acquisition after vaccination with BNT162b2 have been described but the risk of secondary transmission from fully-vaccinated individuals remains ill-defined. Herein we report a confirmed transmission of SARS-CoV-2 alpha variant (B.1.1.7) from a symptomatic immunocompetent woman 4 weeks after her second dose of BNT162b2, despite anti-spike seroconversion.
