Abstract Background This post hoc analysis assessed clinical and functional responses to tofacitinib monotherapy, tofacitinib + methotrexate (MTX), and adalimumab + MTX, in patients with rheumatoid arthritis enrolled in the ORAL Strategy study, including evaluation of patient-level data using cumulative probability plots. Methods In the 12-month, phase IIIb/IV ORAL Strategy study, patients with rheumatoid arthritis and an inadequate response to MTX were randomized to receive tofacitinib 5 mg twice daily (BID), tofacitinib 5 mg BID + MTX, or adalimumab 40 mg every other week + MTX. In this post hoc analysis, cumulative probability plots were generated for mean percent change from baseline (%∆) in the Clinical Disease Activity Index (CDAI; clinical response) and mean change from baseline (∆) in the Health Assessment Questionnaire-Disability Index (HAQ-DI; functional response) at month 12. Median C-reactive protein (CRP) levels by time period were summarized by CDAI remission (≤ 2.8) status at months 6 and 12. Results Data for 1146 patients were analyzed. At month 12, cumulative probability plots for %∆CDAI and ∆HAQ-DI were similar across treatments in patients with greater response. At lower levels of response, patients receiving tofacitinib monotherapy did not respond as well as those receiving combination therapies. With tofacitinib + MTX, numerically higher baseline CRP levels and numerically larger post-baseline CRP reductions were seen in patients achieving CDAI remission at months 6 and 12 vs those who did not. Conclusions These results suggest that patients with a greater response did well, irrespective of which therapy they received. Patients with lesser response had better outcomes with combination therapies vs tofacitinib monotherapy, suggesting they benefitted from MTX. High pre-treatment CRP levels may be associated with better response to tofacitinib + MTX. Trial registration ClinicalTrials.gov, NCT02187055. Registered on 08 July 2014.

State defense threats that arise as a result of the development of the global, regional and national strategic environment also include social conflicts that are still widely found in various parts of Indonesia. The preparation of a national defense strategy from an early age is very much needed to face the worst possibility and the government must hold a total war. The involvement of all national resources is needed including human resources, social conflict is a serious problem in the development of the defense force, so a solution must be found. This paper uses a phenomenological qualitative descriptive method and the theory of defense policy implementation by Lukman Yudho Prakoso to analyze the current problems. The research results show that there is still a need to carry out good handling in handling social conflicts that are integrated, can communicate interactively between entities, and implement transparent, controlled and accountable activities. So that the conclusion can be formulated that it is necessary to instill the value of State defense in handling social conflicts, so that it is hoped that awareness will arise in the community for responsibility in defending the State. Another conclusion is that a strategy is needed to build the best network and the involvement of an entity that functions as a Driver Force for handling social conflicts in supporting military campaign strategies.

Background/Objective:  Distracted driving due to cellphone use can lead to serious and fatal consequences. In 2017, an estimated 14% of all distraction-affected fatal crashes in the U.S. were related to the driver’s cellphone use. Many states have passed distracted driving laws to help reduce the number of unfortunate outcomes related to a driver’s cellphone use. It is important to evaluate whether these laws are truly effective at building safer roads. The main objective of this proposed study is to execute a systematic review of literature published in the last 20 years in order to further the understanding of how distracted driving laws affect drivers’ cellphone usage and its associated outcomes.  Proposed Methods:   A systematic review will be conducted using the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) statement established by Moher et al. The various steps that will be taken to properly select and analyze articles include eligibility criteria, information sources and search strategy, study selection, evaluation of selected articles, data collection, and summary measures.   Expected Results:   After implementing the various steps from the PRISMA statement, certain articles will be selected to fulfill the research objective. It is expected that most of the publications selected for this systematic review will be in support of distracted driving laws. It is also expected that these laws are in fact effective at reducing drivers’ cellphone use and various negative outcomes, such as crashes.   Conclusions/Potential Impact:   A systematic review will be completed utilizing the proposed protocol written in the summer of 2020. If needed, additional details and changes will be applied to the protocol before the study officially begins. This systematic review may help us gain an understanding of the effects of distracted driving laws, which may impact the approach to creating future policies and interventions intended to promote safer driving roads.

Problem and goal. Since we are going through a key moment in terms of technological advances and their effect on the labour field, the investigation of the on-the-job training processes gains a predominant role. Moreover, the need to research an approach that promotes a balance between the four spheres of development of learning - economic, sustainable, community and personal - is emerging. This study was conceived as a result of this urgency, aiming to respond to the following problem: to what extent does a training model, based on the VPL (Validation of Prior Learning) approach and supported by principles of the educational sciences, can contribute to individual empowerment and, consequently, to an improvement in the organizational quality of Portuguese SMEs (Small and Medium-sized Enterprises)? Methodology. The division of the research into two parts - Study 1 and Study 2 - allowed the collection and analysis of structural data (Study 1) that served as a basis for the definition of a dynamic strategy (Study 2). Although undertaken at different times, they have thus proved to be complementary. In fact, the design of the training model was based, firstly, on the results obtained through exploratory interviews with key informants and, secondly, on the data obtained throughout the training process that involved ninety-two employees from ten Portuguese SMEs from diverse categories and sectors. Results. The training model, which emerged from the interaction between the theoretical reflection and the practical context, comprises five components: distance learning, recognition of skills, language skills, intercultural skills and an individual action plan. Conclusion. This study, being an important contribution to the topic of on-the-job training in Portuguese SMEs, at the same time opens the way for the continuation of the research.
