PT Gasses Logistic is one of the companies that operates in the downstream oil and gas industry that focuses on providing logistic services and other support. From oil, gasses, to lubricant, together with their partnership, PT Gasses Logistic are responsible of transporting those goods to their consumers across Indonesia. Also, PT Gasses Logistic is the subsidiary/child company from one of the biggest state-owned enterprises in Indonesia. The core business of PT Gasses Logistic is transportation, that is why every type of problem in transportation needed to be solved quickly. Through this research, it is identified 4 problems by using Pareto Chart. The problems are work & traffic accident, losses while loading & unloading operation, risk in damaging MESRAN MIN 40, and risk in damaging RORED EPA MIN 90. Those problems will be evaluated with risk management framework and analyse further using Six Sigma DMAIC method. Risk management framework will be used to identify the risk rating for each risk, which will be used when planning the mitigation strategies. Six Sigma DMAIC method will identified what is the problem, how it the performance regarding the process related to the problem, what is the root causes, how to improve it, and how to control it. After using the risk management framework and Six Sigma DMAIC method, it is found that the root causes of the problems, which is identified by using fishbone diagram tools, is People and Machine. The author then provided the risk mitigation strategies solution of improvement, the implementation plan, and how to control it. With this research, the author hopes that it will help reducing/mitigating any losses received by PT Gasses Logistic incurred by those problems.

Abstract Phosphating is most common method for surface treatment and finishing of ferrous and nonferrous metal. It has excellent corrosion resistance, wear resistance, adhesion and lubricating properties besides its economic values and speed of application. Steel is one of the cheapest materials used in various industries and it requires corrosion resistance therapy. This study aims to provide possible solution to the rusty issue on carbon steel product by improving coating stability using Defining–Measuring–Analysing–Improving–Controlling (DMAIC) method. The result proposed that zinc phosphate coating material has to replace current iron phosphate coating due to the former is more corrosion time resistant than that of the latter.

In line with the increasingly fierce industry competition, all companies strive to make continuous improvements to increase added value and reduce waste which will impact the company's ability to maintain its existence in the future. One of the problems found in the tire manufacturing industry is the quality problem of the occurrence of defects in the mixing process which is dominated by the viscosity out standard on the compound steel breaker. In this study, analysis and improvement of the defect problem were carried out using Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA), and Multiple Linear Regression (MLR) to test the correlation between the root causes found to the main problem. Based on the results of the analysis found thirteen root causes where the factor of variation in material viscosity and the suitability of determining the design process has the largest Risk Priority Number (RPN) value and has a strong correlation to defects that occur based on hypothesis testing. Furthermore, improvements are made using the DMAIC method on all factors that affect the occurrence of defects. As the result, the improvement can be effective in reducing the defect to 34.5% and achieve the expected target.

PurposeThe purpose of this case study is to develop a lean six sigma (LSS) define–measure–analyze–improve–control (DMAIC) procedure to optimize the willingness to respond (WTR) of Louisiana-based law enforcement officials (LEO) to disasters.Design/methodology/approachVarious LSS tools were used to implement DMAIC to clearly define the problem of WTR, measure the self-reported WTR through a survey, perform statistical analysis on the measured data to identify significant variables to WTR, brainstorm issues and improvements with stakeholders, develop mitigation strategies, implement a pilot solution survey and develop control measures.FindingsLouisiana LEO showed an average of 73.9% of WTR to all disasters. Seven significant variables influenced WTR, which are prior experience, transportation, duty to organization, risk to self, conflicting roles, training and incentive pay. The results from pilot solutions showed that utilizing incentive pay, adequate training and personal protective equipment (PPE) increased WTR from 0.5% up to 16%.Originality/valueThis study developed and validated a unique procedure to improve LEO WTR to disasters, providing a set of DMAIC tools and concepts that can be used by other emergency response agencies. This research was performed during the COVID-19 pandemic and after Hurricane Laura impacted Louisiana.

PurposeOnline spiral pipe manufacturing is one of the most common processes for producing water and gas transmission pipes. Weld quality and pipe circumstances are the most important qualitative characteristics of these pipes determining the overall cost of production. The purpose of this paper is describing how to implement Six Sigma on the production of spiral welded pipes with a real case study.Design/methodology/approachFirst, the capability of the pipe production process has been performed by using the defects per million opportunities to allow for comparison and analysis of the project effectiveness. Then, the variation pattern of this index, after an improvement project, has been investigated. Finally, Taguchi's loss functions are used to evaluate the effectiveness of these projects.FindingsThe results represent a significant improvement in the production process and a reduction in production costs regarding both weld quality and pipe circumstances. Due to the successful implementation of Six Sigma project in welding and pipe quality improvement, the Define, Measure, Analyze, Improve and Control (DMAIC) method can be used effectively in defining weld and pipe quality.Originality/valueIn this proposed work, for the first time, research on the implementation of Six Sigma in the pipeline and welding industries on two online spiral production machines has been investigated. The DMAIC method has been used for the first time to improve the dimensional quality and weld quality of spiral pipes.

Service companies, especially in the logistics sector during the pandemic, are currently very much needed by many people. The increasing demand for package and mail delivery services is increasing day by day, must be balanced with service and timely delivery to the recipient. PT. Pos Indonesia (Persero) Kantor Pos Pemeriksaan (KPRK) Mojokerto 61300 is a company engaged in courier and logistics services which has a target to be able to provide better services for customers. However, the Mojokerto Inspection Post Office (KPRK) still has several problems, including the problem of delivery failure parcels and letters to recipients or what is commonly called the failure of the Standard Delivery Time. The purpose of this research is to minimize the occurrence of delivery failures parcels and letters and to increase Standard Delivery Time. Data obtained from observations, interviews and direct documentation in the field. The method used in this problem is to use the DMAIC method (Define, Measure, Analyze, Improve, Control). The results of the analysis and discussion through several calculations on the check sheet and Pareto diagram, it is known that delivery failed packages and letters for three months amounted to 3,406 items with the highest problem factors being incomplete addresses and moving recipients. To minimize these problems, human resources must be improved and improved.Perusahaan jasa khususnya bidang logistik pada masa pandemi saat ini sangat dibutuhkan oleh banyak masyarakat. Mewabahnya permintaan jasa pengiriman paket dan surat semakin hari semakin meningkat harus diimbangi dengan pelayanan dan pengiriman tepat waktu kepada penerima. PT. Pos Indonesia (Persero) Kantor Pos Pemeriksaan (KPRK) Mojokerto 61300 merupakan perusahaan bergerak dibidang jasa kurir dan logistik yang memiliki target untuk dapat memberikan pelayanan jasa yang lebih baik bagi pelanggan. Namun Kantor Pos Pemeriksaan (KPRK) Mojokerto masih memiliki beberapa permasalahan diantaranya permasalahan gagal antar kiriman paket dan surat kepada penerima atau biasa disebut ketidakberhasilan Standar Waktu Penyerahan (SWP). Tujuan penelitian ini adalah untuk meminimalisir terjadinya gagal antar kiriman paket dan surat serta meningkatkan Standar Waktu Penyerahan. Data diperoleh dari hasil observasi, wawancara dan dokumentasi langsung di lapangan. Metode yang digunakan dalam permasalahan ini adalah dengan menggunakan metode DMAIC (Define, Measure, Analyze, Improve, Control). Hasil dari analisis dan pembahasan melalui beberapa perhitungan pada check sheet dan diagram pareto maka diketahui gagal antar kiriman paket dan surat selama tiga bulan berjumlah 3.406 item dengan faktor permasalahan tertinggi alamat yang tidak lengkap dan penerima pindah. Untuk meminimalisirkan permasalhan tersebut maka SDM harus dilatih dan ditingkatkan.

Abstrak. Dunia industri yang semakin berkembang akan mengakibatkan banyaknya persaingan. Perus-ahaan yang dapat bersaing adalah perusahaan yang dapat menjaga  kualitas produknya dengan baik sehingga dapat memenuhi kepuasan konsumen. Maka dari itu diperlukan pengendalian kualitas. Penelitian ini difokuskan pada penurunan cacat pada produk batu bata merah di CV. Ghatan Fatahillah dengan metode Six Sigma-DMAIC (Define, Measure, Analyze, Improve, dan Control). Dari hasil penelitian didapatkan DPMO sebesar 36.212 dengan nilai sigma 3,29. Jenis cacat yang paling sering terjadi pada batu bata merah yaitu pecah/patah yaitu sebesar 4.327 atau 59,81% dari total keseluruhan produk yang cacat.  Hasil dari tahap analyze dengan fishbone diagram, ditetapkan penyebab dari pecah/cacat, yaitu: pekerja kurang paham standar kualitas, tidak adanya pemeriksaan produk, pekerja kurang paham prosedur kerja, kinerja mesin tidak stabil, pekerja kurang teliti,  jumlah penggilingan tidak menentu, dan komposisi batu bata tidak menentu. Maka perlu dilakukan usulan perbaikan agar dapat mengurangi jumlah produk yang cacat pada batu bata merah. Kata kunci: DMAIC, DPMO, Pengendalian Kualitas, Six Sigma Abstract. The growing industrial world will result in a lot of competition. Companies that can compete are companies that can maintain the quality of their products well so that they can meet customer satisfaction. Therefore, quality control is needed. This research is focused on reducing defects in red brick products at CV. Ghatan Fatahillah with the Six Sigma-DMAIC method (Define, Measure, Analyze, Improve, and Control). From the research results obtained DPMO of 36,212 with a sigma value of 3.29. The type of defect that most often occurs in red bricks is broken / broken, which is 4,327 or 59.81% of the total defective products. The results of the Analyze stage with the fishbone diagram showed that the causes of breakage / defects were determined, namely: workers do not understand quality standards, lack of product inspection, workers do not understand work procedures, unstable machine performance, workers are not careful, the number of mills is erratic, and composition erratic bricks. So it is necessary to make improvement proposals in order to reduce the number of defective products in red bricks. Keywords: DMAIC, DPMO, Quality Control, Six Sigma
