Zusammenfassung Ziel Überprüfung von Effekten einer oralen Ergänzung von Mangan (Mn) auf die Mn-Konzentration im Blut laktierender Warmblutstuten und die Untersuchung der Beziehung zwischen der Mn-Versorgung der Mutterstuten und ihrer Saugfohlen. Material und Methoden Für die 90-tägige Fütterungssphase wurden laktierende Stuten in 3 Gruppen eingeteilt und erhielten täglich ein Placebo (n = 11) oder eine Mn-Zulage von 560 mg Mn als Mn-Sulfat (n = 11) oder Mn-Chelat (n = 11) zusätzlich zur Mn-Aufnahme aus der Grundration (Heu ad libitum, Totale Mischration: Mn-Aufnahme ~100 mg/kg Trockenmasse). In 14-tägigen Abständen wurden Blutproben von den Stuten und Fohlen entnommen. Die Mn-Bestimmung im Serum und Vollblut sowie in der Stutenmilch erfolgte mittels Massenspektrometrie mit induktiv gekoppeltem Plasma, die Mn-Analyse in repräsentativen Futterproben mittels Atomabsorptionsspektrometrie. Zur Datenauswertung diente die Software IBM SPSS Statistics 27 (IBM Deutschland GmbH, Ehningen). Ergebnisse Während der Supplementierung zeigten die Stuten im Vollblut (Median: 15,6 µg/l; 25.–75. Perzentil: 12,8–18,5 µg/l) 10-fach höhere Mn-Konzentrationen als im Serum (Median: 1,54 µg/l; 25.–75. Perzentil: 1,20–1,90 µg/l). Die Fohlen wiesen 16,4-fach höhere Mn-Konzentrationen im Vollblut (Median: 21,3 µg/l; 25.–75. Perzentil: 16,7–28,1 µg/l) im Vergleich zum Serum (Median: 1,50 µg/l; 25.–75. Perzentil: 1,30–1,70 µg/l) auf. Die Mn-Vollblutspiegel der Fohlen entsprachen der 1,6-fachen Mn-Konzentration ihrer Mutterstuten. Die Milch enthielt eine mediane Mn-Konzentration von 0,012 mg/kg Frischmasse. Die Mn-Supplementierung hatte keinen Effekt auf die Mn-Spiegel im Blut von Stuten und ihren Fohlen. Schlussfolgerung und klinische Relevanz Die Mn-Supplementierung beeinflusste die Mn-Konzentrationen im Blut nicht, es unterschieden sich aber die Mn-Gehalte zwischen Serum und Vollblut. Ferner lagen die Mn-Spiegel im Vollblut der Fohlen höher als die der Stuten, obwohl die Mn-Konzentrationen – unabhängig von der Supplementierung – in der Stutenmilch niedrig waren. Aufgrund des geringen Mn-Gehalts der Milch ist eine Mn-Zufuhr, vorzugsweise über das Raufutter, notwendig, um die Mn-Versorgung der Fohlen sicherzustellen.

This study aimed to assess the cytotoxicity of commercially available adhesive strategies—etch-and-rinse (Adper™ Scotchbond™ 1 XT, 3M ESPE, St. Paul, MN, USA, SB1), self-etch (Clearfil™ SE Bond 2, Kuraray Noritake Dental Inc., Tokyo, Japan, CSE), and universal (Scotchbond™ Universal, 3M Deutschland GmbH, Neuss, Germany, SBU). MDPC-23 cells were exposed to adhesives extracts in different concentrations and exposure times. To access cell metabolic activity, viability, types of cell death, and cell cycle, the MTT assay, SRB assay, double labeling with annexin V and propidium iodide, and labeling with propidium iodide/RNAse were performed, respectively. Cultures were stained with May-Grünwald Giemsa for qualitative cytotoxicity assessment. The SB1, CSE, and SBU extracts determined a significant reduction in cell metabolism and viability. This reduction was higher for prolonged exposures, even for less concentrated extracts. CSE extracts significantly reduced the cell’s metabolic activity at higher concentrations (50% and 100%) from 2 h of exposure. After 24 and 96 h, a metabolic activity reduction was verified for all adhesives, even at lower concentrations. These changes were dependent on the adhesive, its concentration, and the incubation time. Regarding cell viability, SBU extracts were the least cytotoxic, and CSE was significantly more cytotoxic than SB1 and SBU. The adhesives determined a reduction in viable cells and an increase in apoptotic, late apoptosis/necrosis, and necrotic cells. Moreover, on cultures exposed to SB1 and CSE extracts, a decrease in the cells in S and G2/M phases and an increase in the cells in G0/G1 phase was observed. Exposure to SBU led to an increase of cells in the S phase. In general, all adhesives determined cytotoxicity. CSE extracts were the most cytotoxic and were classified as having a higher degree of reactivity, leading to more significant inhibition of cell growth and destruction of the cell’s layers.

BACKGROUND In-patient management like ordering medical tests is time-consuming and burdens physicians on the ward with non-clinical tasks. We developed and implemented a smartphone-based mobile application (MA) that uses speech recognition for the ordering of radiological examinations. While we could show that the MA supported process was faster, its acceptance and usability remaines unknown. OBJECTIVE We examined the user experience (1), user acceptance (2) and the determinants of user acceptance (3) of a new mobile, speech recognition guided in-patient management MA. METHODS A comprehensive questionnaire based on the short version of the User Experience Questionnaire (UEQ-S) and the Unified Theory of Acceptance and use of Technology (UTAUT) was given to all physicians at the Department of Trauma and Plastic Surgery at the University Hospital of Wuerzburg , Germany. The user experience with the new MA was compared to the usual desktop application (DA) workflow embedded in the clinical information system (i.s.h.med, Cerner Health Services Deutschland GmbH, Berlin, Germany). The domains of user experience consisted of overall attractiveness, pragmatic quality, and hedonic quality. For the determinants of the acceptance model, we employed hierarchical regression analysis. RESULTS Twenty-one out of 30 physicians (mean age 34±8 years, 62% male) completed the questionnaire (response rate 70%). Compared to the conventional DA workflow, the new MA showed superior overall attractiveness (mean difference +2.15±1.33), pragmatic quality (mean difference +1.90±1.16), hedonic quality (mean difference 2.41±1.62; all p<.001). The user acceptance measured by the UTAUT (mean 4.49, SD 0.41; min. 1, max. 5) was also high. Performance expectancy (beta=0.57, p=.02) and effort expectancy (beta=0.36, p=.04) were identified as predictors of acceptance explaining 65.4% of its variance. CONCLUSIONS There is huge potential in reducing the physicians’ burden of administrative tasks by adopting innovative DHI like our speech recognition guided MA in daily ward routine. With this study, we could illustrate that physicians are more than willing to implement such innovative mobile health solutions.

Abstract Background: Widely available rapid testing is pivotal to the fight against COVID-19. Real-time reverse transcription-polymerase chain reaction (rRT-PCR) remains the gold standard. We compared two frequently used commercial rapid diagnostic tests (RDTs) for SARS-CoV-2-antigens, the SD Biosensor SARS-CoV-2 Rapid Antigen Test (Roche Diagnostics) and the Panbio COVID-19 Ag Rapid Test (Abbott Diagnostics), against rRT-PCR for SARS-CoV-2 detection. Methods: We compared the tests in 2215 all-comers at a diagnostic centre between February 1 and March 31, 2021. rRT-PCR-positive samples were examined for SARS-CoV-2 variants. Findings: 338 participants (15%) were rRT-PCR-positive for SARS-CoV-2. The sensitivities of Roche-RDT and Abbott-RDT were 60.4% and 56.8% (P<0.0001) and specificities 99.7% and 99.8% (P=0.076), respectively. Sensitivity inversely correlated with rRT-PCR-derived Ct values. Unadjusted, the RDTs had higher sensitivities in individuals referred by treating physicians and health departments than those tested for other reasons, in persons without comorbidities compared to those with comorbidities, in individuals with symptoms suggesting COVID-19, and in the absence of SARS-CoV-2 variants compared to Alpha variant carriers. The associations of sensitivity with clinical symptoms and the SARS-CoV-2 genotype were robust against adjustment for Ct values. Assuming that 10 000 symptomatic individuals are tested, 500 of which are truly positive, the RDTs would generate 38 false-positive and 124 false-negative results. Assuming that 10 000 asymptomatic individuals are tested, including 50 true positives, 18 false-positives and 34 false- negatives would be generated. Interpretation: The sensitivities of the two RDTs are unsatisfactory. This calls into question whether their widespread use is effective in the ongoing SARS-CoV-2 pandemic. Die Idee für einen digitalen Adventskalender kam auf, um die Mitarbeiter in den Pflegeeinrichtungen gerade in der Vorweihnachtszeit zu unterstützen. Durch die steigenden Zahlen, die Mehrbelastung und die sozialen Einschränkungen wurde in diesen schweren Zeiten psychisch wie physisch mehr abverlangt als zuvor. Die Übermittlung der digitalen Inhalte sollte die Interessenten auf einfachem Weg erreichen und Informationen und Tipps für einen gesünderen Arbeitsalltag vermitteln. Die technische Vorbereitung und die Details der Übermittlung werden im Folgenden beschrieben. Mit Kreation und Produktion der einzelnen Adventskalender-Türchen haben wir unsere Kurzvideoagentur, die Snack-Content Company (SCC) Deutschland GmbH, beauftragt. Von ihr wurde folgendes Konzept erarbeitet und umgesetzt.

The review of the applications for grants of the National Research Foundation of Ukraine revealed evidence that many applicants-clinicians are not aware of the principles of Good Clinical Practice (GCP) that are implemented in different forms in the vast majority of research involving human subjects across the globe. In the essay, based on the experience of researching at Danylo Halytsky Lviv National Medical University (Ukraine), Loma Linda University (USA), Friedrich Schiller University Jena (Germany), as well as industry (Sanofi-Aventis-Deutschland GmbH., Frankfurt am Main, Germany), the author shares his thoughts about the gaps in education, training, regulations and administrative flaws that prevent more rapid progress in clinical research in Ukraine. Despite the complexity of the problem, implementation of relatively simple steps at institutional and/or national levels, as suggested, can boost the efficiency of research, increase the quality of applications and move clinical research in Ukraine to a qualitatively different level
