Phobic Dispareunia: a Case Report

A case of phobic dyspareunia is described. We are talking about a 22-year-old girl whose genital pains first arose 2 years ago during intercourse with a man whose penis length was 25 cm. She had only 5 intercourses with him, and then because of these pains interrupted the relationship with him. Now she is dating a 22-year-old guy who lives in another city, and they come to visit each other. Genital pains and burning in the vagina occur in her not only during intercourses with him and after their end, but even when they are in different cities. In the latter case, they arise when she is planning a trip to another city, involving sexual contacts with her boyfriend (wakes up at night and experiences genital pain and burning sensation “to tears”). Because of this, she cannot sleep. When such a trip is not supposed, then she does not have pain and burning sensation in the genitals. It so happens that the patient comes to her boyfriend, but they do not perform vaginal intercourse, and everything is limited to petting and oral sex. This is preceded by her request to him, and he is sympathetic to her problem. But pains in the genitals occur even with cunnilingus, if not only the clitoris but also the vagina is involved (“bakes, pricks, gives birth to hedgehogs”). She reports that it so happens that pains arise when she comes to her boyfriend, but the matter has not yet come to sexual contact. Before intercourses, she notes the presence of a fear of pain associated with their implementation. This fear is accompanied by tension in the muscles of the body and difficulty in breathing. Currently, sexual desire is sharply weakened, which can be explained by the presence of dyspareunia. During the examination by the gynecologist, no profile pathology was revealed in the patient. Appropriate cognitive influences and 10 sessions of hypnosuggestive programming were carried out. As a result of the therapy, she stopped avoiding sexual intercourses, began to live a regular sex life, pain during and after intercourses practically ceased to arise, libido was increased.

Circumferential dissection of deep fascia as ancillary technique in circumcision: is it possible to correct phimosis increasing penis size?

Background Phimosis is the inability to retract the preputium downward over the glans penis. Despite the various techniques of preputial plasty described in literature, the most performed surgical treatment is still the conventional circumcision. Methods In this paper we retrospectively reviewed data of a homogeneous population of 36 consecutive adult patients who underwent phimosis correction by circumcsion with dissection of the Deep Fascia. Patients were followed up by one independent plastic surgeon that measured penis length and circumference in nonerected state preoperatively and at 6 month time postoperatively. Results The Wilcoxon Signed Rank Test showed a significant (p < 0.0001) difference between the two groups both in terms of length and circumference. Conclusions In conclusion, the ancillary technique we described leads to an increase of penis size, is safe and easy to perform and does not increase significantly operative time nor complication rate to the conventional procedure.

Is the penile golden ratio an indicator of normal sperm count and normal hormonal status? A prospective-observational study

Background and Objective: The constant phi number (1.618…) is accepted as the indicator of esthetic beauty and harmony in numerous fields of natural sciences and is also known to be effective in various biological systems in the human body such as skeletal system and facial anatomy, respiratory system, and cardiovascular system. In this study, we aimed to compare the anthropometric proportions obtained from penile measurements and the phi-value and to investigate whether this number could be an indicator of fertility. Material and Methods: This study included 200 infertile male patients. Penile ratio (PR) was calculated by dividing the corpus penis length by the glans penis length. Patients were divided into two equal groups: Group-I included patients with a PR that was closest to the constant phi-value (n = 100) and Group-II included patients with a PR that was the most distant to the phi-value (n = 100). Spermiogram findings, and serum hormone levels were compared between the two groups. Results: Mean age was 27.37 ± 3.45 years, median penile length was 9.1 (7.1-10.2) cm, and median PR was 2.06 (1.65-2.64). In group I and II, an abnormal spermiogram was detected in 41% and 48% (P = 0.319) and hormonal dysfunction was present in 28% and 26% of the patients (P = 0.750), respectively. Overall sperm counts and sperm concentrations were significantly higher in group I compared to group II. Conclusion: Although PR may not be an indicator of normal spermiogram or hormone status, patients with a closer PR to the phi value are likely to have more favorable spermiogram values.

Peningkatan Pengetahuan Tentang Puberitas Pada Siswa Kelas 4, 5, 6 SD Negeri Sukawening Kecamatan Ciwidey Kabupaten Bandung

Abstrak: Setiap anak pasti mengalami pertumbuhan dan perkembangan secara alami, baik secara fisik, mental dan kematangan organ reproduksi mulai berfungsi dan karakteristik seks sekunder mulai muncul pada remaja tersebut. Usia pubertas pada anak remaja sekitar usia 10 tahun sampai 20 tahun. Pada anak perempuan masa ini ditandai dengan menstruasi (menarche), pertumbuhan payudara, tumbuhnya rambut di daerah kemaluan, sedangkan pada anak laki-laki pada masa pubertas ditandai dengan perubahan suara yang disertai dengan tonjolan kerongkongan (Adam’s apple), perubahan panjang penis, dan tumbuhnya rambut kemaluan. Pada awal memasuki masa pubertas, seorang remaja biasanya membutuhkan banyak informasi mengenai perkembangan, pertumbuhan dan perubahan yang dialaminya sehingga anak mencari informasi dengan bertanya kepada orang tua, teman, atau orang-orang yang berada di sekitar lingkunganya, tidak semua orang sekitar lingkungan remaja bisa membantu permasalahan yang dihadapi remaja sehingga mereka mencari dengan cara sendiri, misalnya dengan bertanya pada orang dewasa lainnya, dari majalah, atau bahkan dari internet. Tujuan kegiatan pengabdian masyarakat yang di lakukan oleh Sekolah Tinggi Ilmu Immanuel bekerjasama dengan SD Sukawening adalah untuk meningkatkan pengetahuan anak remaja tentang pubertas, dan mempersiapkan lebih dini datangnya masa pubertas sehingga diharapkan anak lebih siap menghadapi perubahan secara fisik dan psikologis yang akan terjadi nanti sehingga dampak negatif pubertas tidak terjadi pada anak remaja. Metode kegiatan pelaksanaan tersebut meliputi memberikan pendidikan kesehatan tentang pubertas pada anak remaja kelas 4.5.6 dan pemutaran video. Hasil kegiatan pengabdian masyarakat antara lain, pengetahuan anak meningkat tentang pubertas, anak sudah memahami perubahan yang akan terjadi pada masa pubertas baik secara fisiologis maupun secara psikologis, anak mengetahui bagaimana cara menghadapi masa pubertas dan kemana mencari informasi yang benar tentang pubertas jika mengalami masalah atau ada hal yang ingin di tanyakan tentang perubahan yang dialaminya. Peserta yang mengikuti kegiatan sebanyak 60 orang. Abstract: Adolescence experiences growth and development naturally, physically, mentally and the reproductive organs begin to function and secondary sex characteristics appeared. The age of puberty in adolescents is around the age of 10 to 20 years. The female period is being marked by menstruation (menarche), breast and pubic hair growth, while in male, puberty is characterized by voice changes accompanied by Adam's apple protrusion, changes in penis length, and pubic hair. At the beginning of puberty, adolescence usually needs a lot of information about the development, growth, and changes, thus they seek information by asking parents, friends, or people around. Sometimes, people around them will not be able to help; consequently, they search by themselves, such as asking other adults, read magazines, or even from the internet. The purpose of community service activities carried out by Immanuel School of Health Science in collaboration with SD Sukawening were to increase adolescent knowledge about puberty, and prepare for the early arrival of puberty; thus, they are expected to be better prepared facing physical and psychological changes; so that the negative impact puberty does not occur in them. The method of implementation involved providing health education about puberty and video screening to school-aged students in grades 4, 5, 6. The results of community service activities to adolescents such as increasing knowledge about puberty; understanding changes physiologically and psychologically; how to deal with puberty, and where to find correct information if they experience problems or to fulfill curiosity regarding puberty problems. There are sixty respondents participated during the program.

Morphometrics of the indian false vampire bat (Megaderma lyra) from district Jhelum, Pakistan

During the present study thirteen Megaderma lyra bats were observed roosting in dark, domed shaped room of Rohtas Fort, district Jhelum. Out of these, six specimens were captured from the roosting site, using hand net. All captured specimens were male. These bats were identified through their unique facial features, an erect and elongated nose-leaf, large oval ears that joined above the forehead and no tail. Mean head and body length of captured specimens was 80 mm, forearm length was 67 mm while average lengths of 3rd, 4th and 5th metacarpals were 51.73 mm, 55.17 mm and 60.42 mm, respectively. Mean skull length was 29.84 mm, breadth of braincase was 12.77 mm. Average Penis length of two specimens was 6.6 mm and total bacular length was 3.08 mm respectively. This is the first record of Megaderma lyra from district Jhelum.

Crustacean Reproductive Records

Crustacean reproductive traits are highly diverse, and this chapter illustrates some of the most extreme cases, placing them in the context of the more typical crustaceans. It highlights, for example, the male and female records of size and age, the “hottest” and “coolest” reproducers, the longest penises, the largest sperm and eggs, the smallest and largest brood sizes, the longest mate guarding, the most massive sexually selected weapons, the flashiest courtship, the most fathers per brood, the longest incubation of broods, the smallest and largest larvae, the longest larval duration, the longest dormancy of eggs, and the oldest fossil evidence of penis, sperm, brood care, and larvae. Using these illustrious case studies, this chapter briefly examines the adaptive advantages of these extremes and discusses why few species have evolved unusual reproductive traits. Crustaceans indeed appear to hold animal records with respect to relative penis length, aflagellate sperm length, dormant egg viability and fossil ages of penis, giant sperm, and brood care. These captivating examples may be of applied importance in terms of restoring human-altered ecosystems (resurrection ecology using egg banks) and in management strategies of important fisheries.
