BACKGROUND: Although the instrument-assisted manual (IM) technique has been widely utilised to improve soft tissue and joint mobility, its therapeutic benefits and underlying neuromechanical mechanisms remain unknown compared to those of conventional static stretching (SS) and hold-relax (HR) manual techniques. OBJECTIVE: This study aimed to compare the effects of the SS, HR, and IM techniques on muscle activity, kinematics, and strength during deep squatting in limited ankle dorsiflexion (DF) syndrome. METHODS: This was a cross-sectional randomised controlled study including 39 adults divided into three groups: SS group: 13, HR group: 13, IM group: 13. Outcome measures were the tibialis anterior (TA): gastrocnemius (GCM) balance ratio and ankle, knee, hip and thoracolumbar junction angles. TA muscle strength was analysed to evaluate the limited ankle DF. Analysis of variance was performed, with P< 0.05. RESULTS: The TA:GCM balance ratio, ranges of motion of ankle DF and knee flexion, and TA muscle strength in the IM group improved significantly compared to that in either the SS group or HR group (P< 0.05). CONCLUSIONS: Our novel results demonstrated that IM was most effective in normalising TA:GCM balance, ankle DF range of motion, and TA muscle strength during deep squatting in adults with limited ankle DF.

Introduction: Regional differences in activation rates may contribute to the electrical substrates that maintain atrial fibrillation (AF), and estimating them non-invasively may help guide ablation or select anti-arrhythmic medications. We tested whether non-invasive assessment of regional AF rate accurately represents intracardiac recordings.Methods: In 47 patients with AF (27 persistent, age 63 ± 13 years) we performed 57-lead non-invasive Electrocardiographic Imaging (ECGI) in AF, simultaneously with 64-pole intracardiac signals of both atria. ECGI was reconstructed by Tikhonov regularization. We constructed personalized 3D AF rate distribution maps by Dominant Frequency (DF) analysis from intracardiac and non-invasive recordings.Results: Raw intracardiac and non-invasive DF differed substantially, by 0.54 Hz [0.13 – 1.37] across bi-atrial regions (R2 = 0.11). Filtering by high spectral organization reduced this difference to 0.10 Hz (cycle length difference of 1 – 11 ms) [0.03 – 0.42] for patient-level comparisons (R2 = 0.62), and 0.19 Hz [0.03 – 0.59] and 0.20 Hz [0.04 – 0.61] for median and highest DF, respectively. Non-invasive and highest DF predicted acute ablation success (p = 0.04).Conclusion: Non-invasive estimation of atrial activation rates is feasible and, when filtered by high spectral organization, provide a moderate estimate of intracardiac recording rates in AF. Non-invasive technology could be an effective tool to identify patients who may respond to AF ablation for personalized therapy.

Background:RANK-ligand is essential for osteoclast development, activation, and survival and it is a key mediator of increased osteoclast activity in rheumatoid arthritis (RA). Denosumab is a monoclonal antibody that binds RANK-ligand.Objectives:The aim of this study was to evaluate the effects of denosumab on bone mineral density (BMD) and to define a contribution of factors: anamnesis, clinical/laboratory markers, glucocorticoids (GC) intake, etc. on the response to therapy with denosumab in women with RA and osteoporosis (OP).Methods:66 postmenopausal women (mean age 59,6±7,4) with RA (mean duration 17,7±10,4 years) and OP received s/c denosumab 60 mg every 6 months pro 12 months. RF-positive were 72%, ACCP – 74% of patients. 34 (49%) patients continued GC. At baseline and after 12 months it was carried out the dual energy x-ray absorptiometry at 3 sites: lumbar spine (L1-L4), hip neck (HN) and distal forearm (DF) and x-ray of hands and feet (Sharp/van der Heijde (SVH) score). The Statistica 6.0 was used.Results:After therapy it was noted the increase (р < 0,05) of BMD in L1-L4 and HN, a tendency to increase (р =0,0529) in DF. Mean BMD (L1-L4) before\after the treatment was 0,821 ± 0,104 g/cm2vs 0,864 ± 0,110 g/cm2, at HN was 0,625 ± 0,089 g/сm2vs 0,639 ± 0,088 g/сm2, at DF was 0,498 ± 0,090 g/сm2vs 0,503 ± 0,089 g/сm2. The mean change of BMD (%) after 12 months at L1-L4 was +4,6%, at HN +2,8%, at DF +0,7%. Positive response (increase or stabilization of BMD) was noted in 89% patients at L1-L4, 67% - at HN and 60% - at DF. Analysis of influence of factors on the response to therapy showed that positive response on therapy in NH and DF was associated with RF-positivity. The distinct contribution to the negative response in L1-L4 and HN was associated with GC intake (previous intake more than 3 months in the anamnesis) and purpose of the GC after menopause onset. Fractal dimension (Df) has been identified as indirect measure in quantifying the complexity of retinal vessel network which is useful for early detection of vascular changes. Reliability studies of Df measurement on retinal vasculature, has been conducted on retinal images processed by using semi-automated software which only permits image with 45ᵒ field of view (FOV). Smartphone-assisted fundus camera retinal image has a maximum 30ᵒ FOV which warrant manual processing in measuring the Df. Retinal blood vessels need to be manually segmented to produce binary images for retinal vasculatures Df measurement. Therefore, this study was conducted to determine the intragrader and intergrader reliability of manual segmentation of the retinal vasculature Df measurement from retinal images taken using a smartphone-assisted fundus camera Forty-five retinal images were captured using the Portable Eye Examination Kit Retina (Peek Retina™, Peek Vision Ltd, UK). Suitable image for Df analysis were selected based on gradable retinal image criteria which included; i) good image focus, ii) centered position of optic nerve head (ONH) and iii) significant blood vessel visibility. The images were cropped 0.5 disc diameters away from disc margin and resized to 500x500 pixels using GNU Image Manipulation Program Version 2.8.18 (GIMP, The GIMP Team, United States). Retinal vessels were manually traced by using layering capabilities for blood vessel segmentation. Df values of segmented blood vessels were measured by using Image J (National Institutes of Health, USA) and its plugin software, FracLac Version 2.5. Intragrader and intergrader reliability was determined by comparing the Df values between; two readings measured one week apart by a grader and readings from two different graders, respectively, using intraclass correlation coefficient (ICC) and Bland-Altman graphical plots. Intragrader agreement for retinal Df showed good reliability with ICC of 0.899 (95% CI: 0.814–0.945). Bland Altman analysis indicated good agreement between Df values at different grading time (mean difference 0.0050; 95% CI:-0.0001–0.0101). Intergrader reliability for retinal Df was high with ICC of 0.814 (95% CI: 0.459–0.919). Bland Altman plot revealed good intergrader agreement for retinal Df between two graders with a bias value of 0.0158 (95% CI: 0.0092–0.0223). In conclusion, manual segmentation of retinal image captured by smartphone-assisted fundus camera has good reliability (0.75 < ICC < 0.9) for Df analysis to study the morphology of retinal vasculatures.


Portable Eye Examination Kit retina (Peek Retina™, Peek Vision Ltd, UK) and 3D Printed Ophthalmoscope (3DPO) were identified to have acceptable image for retinal evaluation, however the retinal images quality in term of blood vessels visibility between both devices was uncertain. This study was conducted to compare the quality of image based on blood vessels visibility between Peek Retina and 3DPO for fractal dimension (Df) analysis. In this study, a total of 40 retinal images (nPEEK=20, n3DPO=20) of 20 participants were captured on random eyes. The best retinal images with good focus and significant retinal blood vessels visibility of Peek Retina and 3DPO were selected for image quality analysis. The retinal images were cropped approximately following the size of the cornea and resized to 900 by 900 pixels of resolution using GNU Image Manipulation Program Version 2.8.18 (GIMP). The images were randomly sorted as Retinal Image Quality Assessment (RIQA) generated by Google form. Likert scale was implemented to assess the preferences scale of retinal image quality in determining the visibility of retinal vasculature to be traced with four choices of response options (1; very difficult, 2; difficult, 3; easy and 4; very easy). Prior to the retinal image assessment, ten optometrists were asked to experience retinal blood vessels tracing and consequently evaluate the 40 images by choosing the scale options (1 to 4) based on visibility retinal blood vessels. Mann-Whitney test indicated that the blood vessel tracing was easier for Peek Retina (median = 3) than for 3DPO (median = 2), p < 0.0001. Retinal image captured by Peek Retina was rated as very easy (43.5%) for blood vessels tracing as compared to the image from 3DPO (17.0%)Error! Reference source not found.. Only 1.5% of the image captured by PEEK was considered as a very difficult for blood vessel tracing. Difficult and easy preference scales of blood vessel tracing for PEEK were 16.5% and 38.5% respectively. 34% of 3DPO retinal image was graded as difficult for blood vessel tracing followed by 28.5% (easy), 20.5% (very difficult) and 17.0% (very easy). The results indicate that a retinal image photographed by Peek Retina was more preferable in tracing retinal vascular network for Df analysis as compared to 3DPO.

Stock identification of fish plays a significant role in management of fishery resources. It is considered as the precursor to species conservation and imperative for declining fish populations. The stock structure of Clupisoma garua (Hamilton, 1822) populations of Ganga riverine system was studied using truss network analysis. A total of 306 fish specimens obtained from river Ganga in Allahabad, Farakka and Triveni were used to construct the truss network. The truss variables were subjected to principal component analysis (PCA) in which the first component explained 46.71% and the second 14.66% representing body depth and caudal regions of fish, respectively. Bivariate plots of PCA and discriminant function (DF) analysis revealed that it is a single continuous stock in the study area of the river Ganga and the misclassification of DF indicated only a marginal morphological difference. The results indicated the need for a uniform management strategy appropriate for population of C. garua in the river Ganga.

The Arabian red shrimp (Aristeus alcocki Ramadan, 1938) is a deep-sea penaeoid shrimp that forms a major commercial fishery in the Indian coast. However, the spawning population of this species along the Indian coast is poorly known. To study this, stock structure of A. alcocki using truss morphometry was employed. A total of 1842 matured specimens were collected from five geographical locations (Tuticorin (SET), Chennai (SEC), Nagapattianam (SEN), Sakthikulangara (SWS), and Kalamuku (SWK)) along the Indian coast. Thirty-nine truss distances were extracted from each specimen and analyzed by multivariate methods (i.e., principal component analysis (PCA), discriminant functions (DF), and hierarchical cluster analysis). The results of the PCA indicated that the first two components cumulatively explained >70% (female: 72.1%; male: 71.5%) of the total morphometric variation. Stepwise DF analysis indicated that abdominal variables significantly discriminated the populations at different locations. The results clustered the five samples into a minimum of two groups: samples from SWK clustered in group I, whereas rest of the samples clustered in group II. Morphometric variation between the groups was significant for each sex. Significant differences between the groups may be attributed to geographical and environmental conditions, suggesting separate management strategies for resource sustainability.

Background: Previous studies have suggested a relationship between neuroanatomical and neurofunctional hippocampal alterations and episodic memory impairments in multiple sclerosis (MS) patients. Objective: We examined hippocampus volume and functional connectivity (FC) changes in MS patients with different episodic memory capabilities. Methods: Hippocampal subfield volume and FC changes were compared in two subgroups of MS patients with and without episodic memory impairment (multiple sclerosis impaired (MSi) and multiple sclerosis preserved (MSp), respectively) and healthy controls (HC). A discriminant function (DF) analysis was used to identify which of these neuroanatomical and neurofunctional parameters were the most relevant components of the mnemonic profiles of HC, MSp, and MSi. Results: MSi showed reduced volume in several hippocampal subfields compared to MSp and HC. Ordinal gradation (MSi > MSp > HC) was also observed for FC between the posterior hippocampus and several cortical areas. DF-based analyses revealed that reduced right fimbria volume and enhanced FC at the right posterior hippocampus were the main neural signatures of the episodic memory impairments observed in the MSi group. Conclusion: Before any sign of episodic memory alterations (MSp), FC increased on several pathways that connect the hippocampus with cortical areas. These changes further increased when the several hippocampal volumes reduced and memory deficits appeared (MSi).
