Topic relevance. The dental surgeon often is left alone with the dilemma: to save or remove the third lower molars. The justification of indications for tooth extraction must have objective criteria. It should be noted that a patient with this problem does not always have the opportunity to receive comprehensive advice from an orthodontist. In view of this, the solution to this problem requires the search for radiological data, which are the most available today and would help to justify treatment tactics for preservation or removal of the tooth. The conditions of tooth preservation in the dentition are most favorable with a slight medio-distal inclination and a vertical position of the third lower molars. However, this does not exclude the possibility of tooth impaction, which is often accompanied by periodic inflammatory phenomena in the tissues adjacent to the tooth and motivates both the patient and the dental surgeon to solve this problem. The aim of our study was to research of radiographic indicators of orthopantomography of the mandible to predict the vertical retention of the third lower molars. Material and methods of research. The platforms of Google Scholar, Research Gate and PubMed are used for research and analysis of the scientific literature on the application of objective criteria of mandibular orthopantomography data in the prediction of vertical impaction of the third lower molars. Impaction of mandibular molars is the result of a number of factors, including heredity, racial characteristics, muscle function, the nature of the food consumed, insufficient growth of the mandible, the size of the retromolar space. The accurately determination the positions of the molars and the size of the retromolar space of the mandible by available methods of X-ray diagnosis (the most common of which is the method of orthopantomography of the jaws) are necessary to prediction of tooth retention. The accuracy of imaging and measurements on orthopantomograms of the jaws was studied by both domestic and foreign authors. The angle of inclination of lower wisdom tooth, the size of the retromolar space of the mandible and the mesiodistal size of the teeth are determined on OPTG of jaws in the vast majority of publications. The age aspect of patients is also important for prediction of tooth impaction. In particular, most researchers state the fact that up to 20 years of life there is a significant probability of changing the position of the teeth during their eruption and therefore it is quite appropriate to wait. To predict the nature of the impaction of the third lower molars, the authors provide indicators of the angle of the tooth, the structure of their roots, the degree of impact and age of the patient. The publication of Maria Mersedes Gallas-Torreira and co-authors, which compared two methods of linear measurements on OPTG – Olive-Basford’s and Olmos ’methods have greatest interest. Both methods can be used to predict of impaction of lower wisdom tooth. However, if the prognostic estimate is based on orthopantomogram data rather than cone-beam computed tomography, the Olmos` method demonstrates a lower error rate and a more plausible prediction. To substantiate the algorithm for predicting the vertical retention of third lower molar in order to improve treatment tactics for the preservation or removal of the tooth which based on the obtained search data and the results of our own clinical observations and it is planned in the future.

Abstract Background Virtual surgery planning (VSP) is believed to reduce inaccuracies in maxillary positioning compared to conventional surgery planning (CSP) due to the elimination of face-bow transfer and laboratory steps. However, there is still a lack of comparative studies for the accuracy of splint-based maxillary positioning in CSP versus VSP. Therefore, the objective of this retrospective, observational study was to compare if splints produced by VSP and CSP reach postoperative outcomes within clinically acceptable limits. Methods The planned and actual postoperative results of 52 patients (VSP: n = 26; CSP: n = 26) with a mean age of 24.4 ± 6.2 years were investigated by three-dimensional (3D) alignment with planning software. The conventional treatment plan was digitized, so that the evaluation of both methods was performed in the same manner using the same coordinate system. Inaccuracies were measured by sagittal, vertical and transversal deviations of the upper central incisors and the inclination of the maxillary occlusal plane between the planned and achieved maxillary positions. Results Both methods demonstrated significant differences between the planned and actual outcome. The highest inaccuracies were observed in vertical impaction and midline correction. No significant differences between CSP and VSP were observed in any dimension. Errors in vertical and sagittal dimension intensified each other. Conclusions In conclusion, splint-based surgeries reached similar results regardless of the applied planning method and splint production.

Introduction: Maxillary canines are usually impacted teeth the second after third molar. Frequency of impacted maxillary canines is about 1 to 3% of population (in 85% of the cases palataly, and in 15% of the cases bucaly). The aim of this research was to find the frequency of bucal and palatal impacted maxillary canines in relation to sex and position of impacted tooth. Matherial and method: We analyzed three-dimensional computed tomography images (CBCT) of 64 examinees (aged 12 to 33 years) with 80 unerupted maxillary canines to plan the orthodontic therapy. Results: Among 64 examines with impactions, 80 impacted maxillary canines are diagnosed. The average age of all respondents in the survey is 16,3±4,3. Of 64 examinees in research 41(64.1%) were female which is significantly more important than the representation of male examinees whose number was 23(35.9%). Frequency of unilateral impactions of maxillary canines is three times higher 48 (75%) in comparison to bilateral impactions 16 (25%). Of total 80 impacted maxillary canines 19 (23.75%) were bucal impacted, 3 (2.75%) in the middle of alveolus and 58 (72.5%) palatally. With the bucal and palatal impaction usually was represented partially vertical impaction (70%); completely horizontal impaction (10%) was represented only in the palatal impaction. Conclusion: The analysis of CBCT images of maxilla shows a statistically significant frequency of partially vertical, unilateral and paltally localized impacted maxillary canines.

Objective: This study aimed to identify radiographic parameters on routine orthopantomograms to aid in early diagnosis of palatally impacted canines. Design: Split-mouth case-control study. Setting: Single-centre university dental hospital. Methods: Thirty-three individuals with palatally impacted canines and non-impacted contra-lateral teeth were recruited. Radiographic variables angle A, angle B, angle C, distance D, sector class and depth of vertical impaction were measured for both groups. Mann–Whitney U test and Spearman’s correlation coefficient were used to analyse significant differences and association between variables with sector class and depth of vertical impaction, respectively. Logistic regression analysis determined radiographic parameters as predictors of palatal impaction. Results: Significant differences were seen between sector class, depth of vertical impaction and the radiographic parameters between cases and controls ( P ⩽ 0.001). For cases, strong correlation was found for angle C between the impacted canine and occlusal plane (r = −0.28, P = 0.022) and perpendicular distance D of impacted canine to the occlusal plane (r = 0.81, P ⩽ 0.001) with the depth of vertical impaction. Logistic regression analysis showed the odds of canine impaction increase with an increase in angle B and angle C. Conclusions: Our study showed that greater depth of vertical impaction, sector class, increased perpendicular distance and reduced angulation from occlusal plane resulted in palatal impaction of permanent maxillary canines. The odds of palatal canine impaction increase nearly twofold with every 10 increase in angle B and angle C.

Objectives: To evaluate the epidemiological patterns of third molar impaction in a cohort of patients living in the north of Saudi Arabia. Materials and Methods: A retrospective cohort study comprised of analysing 2550 Orthopantomograms (OPGs) belonging to patients who attended Aljouf University College of Dentistry between September 2013 and December 2015. OPGs were examined to determine the frequency of third molar impaction, their levels of eruption and angulations. Mixed effects logistic regression analysis was performed to calculate adjusted odds ratios. Data were weighted by age and sex based on population regional estimates. Results: 1551 patients (60.8%) with a mean age of 33.5 years-old (95%CI: 32.9 to 34) demonstrated 2650 impacted third molars. Third molars were more likely present in patients aged from 20 to 39 years-old (p<0.001); and in mandible more than maxilla (p<0.001). It showed highest vertical impaction and higher impaction rate in mandible than maxilla. Level A impaction was the most common among other levels by 1365 (53.5%). Vertical impaction was the most common pattern (1354 patients; 53.1%). Mesioangular impaction ranked second in mandible, while distoangular impaction ranked second in maxilla. There was no statistically significant difference between males and females concerning impaction frequency, depth levels and angulations. Conclusion: Impacted third molars is still a public health concern among youth and young adults. Vertically impacted mandibular third molars with their occlusal plane at the same level as the occlusal plane of adjacent tooth is the most prevalent pattern of third molar impaction in the northern region of Saudi Arabia.

Category: Ankle Arthritis, Trauma Introduction/Purpose: The supination-adduction ankle fractures often showed vertical impaction of the tibial medial plafond. The malunion of these fractures cause the varus ankle deformity and secondary ankle arthritis. This retrospective control study looked at the use of supramalleolar osteotomy combined with intra-articular osteotomy in patients with malunited supination- adduction ankle fractures. Methods: Twelve patients were treated with malunited supination-adduction ankle fractures between January 2013 and December 2014. All of these patients had the varus ankle deformity and secondary ankle arthritis. Supramalleolar osteotomy combined with intra-articular osteotomy were underwent for the reconstruction surgery. The visual analog scale (VAS) score for pain during daily activities, Olerud and Molander Scale scores, subjective satisfaction survey rating and the modified Takakura classification stage were obtained. Ten patients were available for follow-up at a mean of 35.4 months (range, 28 to 40 months). Results: Average postoperative Olerud and Molander Scale score 24 months after surgery was 83±10 compared with 60±14 preoperatively. The mean VAS score decreased from 7±2 preoperatively to 2±2 at the latest follow-up. Six patients rated their result as excellent, 3 as good and 1 as fair. No significant difference in the modified Takakura classification stage was observed between the preoperative and the last follow-up. Conclusion: The use of supramalleolar osteotomy combined with intra-articular osteotomy was an effective option for the treatment of malunited supination-adduction ankle fractures.

OBJECTIVESThe aim of this study was to evaluate the relation of distal caries of second molar with patterns of impacted third molar.METHODOLOGYA retrospective study was carried out from January 2017 to June 2017.A total of 160 patients and 276 impacted teeth were selected with an age > 18 years. A single researcher assigned the diagnosis of caries and patterns of impacted teeth through clinical notes and orthopantomogram(OPG)which was reviewed by Head of Department. Winter’s and Pell and Gregory classification was used for impaction classification. The data was analyzed using SPSS 22.0 version. P ≤ 0.05 was considered as statistically significant.RESULTSFemale (56.3%) were predominant than male (43.8%) with mean age presentation was 24.2 ± 4.7 years.271 mandibular second molars were present. Vertical (64.5%) impaction was found to be the most frequent followed by mesioangular (19.6%). Pearson correlation showed that vertical impaction was significantly related to distal caries of second molar(Right side, r=.262 p=.002 and Left side, r=.240 p=.006).CONCLUSIONVertical impaction was positively related with distal caries therefore prophylactic removal of lower impacted teeth is recommended.

Background: The mandibular third molar is the most common tooth to become impacted than other teeth, often causing more complications in the process of growing, influence to the lives of patients. Removal of wisdom teeth is one of the most common dental surgical procedures, is an invasive intervention caused significant injury to the bone and soft tissue, so hard to avoid possible complications after surgery. Understand the relationship between degree of impaction and healing after mandibular wisdom tooth surgery will bring many benefits to surgeons and patients. The purpose of this study is to evaluate the degree of impaction of mandibular third molar and explore the relationship between healing after mandibular wisdom tooth surgery with the degree of impaction. Materials and method: The study consisted of 40 patients, average age 29.38 ± 9.8 a impacted mandibular third molar and cause complications, tooth extraction is indicated. All are to be attended to, determine the degree of impaction, wisdom tooth surgery. Evaluate the healing through pain, swelling, limited mouth open after 2 days, 1 week and 2 weeks. Result: Correlation of wisdom teeth to ramus and adjacent teeth mainly type I (45%) and type II (35%), type III accounted for the lowest rates (20%). Relative depth of wisdom teeth in the bone mainly position A (45%), B position also accounts for a significant proportion (35%), positions C (20%). Correlation of wisdom teeth axis to adjacent teeth: mesioangular impactions (50%) and horizontal (30%), distoangular impaction (2.5% and 5%), vertical impaction (12, 5%). Pain, swelling, limited mouth open only happen the first two days in groups I and II, group III of symptoms until a week after surgery. Conclusion: There is correlation between the degree of impaction and healing after mandibular wisdom tooth surgery. Degree of as much impaction, the level of pain, swelling, limited mouth open higher. Key words: mandibular third molar; impaction

✓ The authors describe a lumbar spine fracture that is characterized on anteroposterior x-ray views by separation of the pedicular shadows. It is almost invariably associated with posterior interlaminar herniation of the cauda equina through a dorsal dural split, and anterolateral entrapment or amputation of the nerve root. The fracture is unstable and requires internal fixation and fusion at the time of neurolysis. Fractures meeting these criteria should be explored as soon as the patient's condition permits. Myelography is usually unnecessary and may be contraindicated in some cases. The postulated mechanism of injury is hyperextension with vertical impaction and rupture of the ring made up of the lamina, pedicle, and vertebral body. The ring is fractured in several places in a manner similar to that seen in “Jefferson fracture” of C-1. The special anatomical relationships of the thoracolumbar junction and the plane of the lumbar facets are also discussed.
