AbstractInhibition of the eukaryotic initiation factor 5A activation by the spermidine analogue GC7 has been shown to protect proximal cells and whole kidneys against an acute episode of ischaemia. The highlighted mechanism involves a metabolic switch from oxidative phosphorylation toward glycolysis allowing cells to be transiently independent of oxygen supply. Here we show that GC7 decreases protein expression of the renal GLUT1 glucose transporter leading to a decrease in transcellular glucose flux. At the same time, GC7 modifies the native energy source of the proximal cells from glutamine toward glucose use. Thus, GC7 acutely and reversibly reprogrammes function and metabolism of kidney cells to make glucose its single substrate, and thus allowing cells to be oxygen independent through anaerobic glycolysis. The physiological consequences are an increase in the renal excretion of glucose and lactate reflecting a decrease in glucose reabsorption and an increased glycolysis. Such a reversible reprogramming of glucose handling and oxygen dependence of kidney cells by GC7 represents a pharmacological opportunity in ischaemic as well as hyperglycaemia-associated pathologies from renal origin.

In this work, the cytotoxic behavior of six ruthenium(II) complexes of stoichiometry [(η6-p-cymene)RuCl2L] (I-VI), L = 4-cyanopyridine (I), 2-aminophenol (II), 4-aminophenol (III), pyridazine (IV), and [(η6-p-cymene)RuClL2]PF6; L = cyanopyridine (V), L = 2-aminophenol(VI) towards three cell lines was studied. Two of them, HeLa and MCF-7, are human carcinogenic cells from cervical carcinoma and human breast cancer, respectively. A comparison with healthy cells was carried out with BGM cells which are monkey epithelial cells of renal origin. The behavior of complex II exhibits selectivity towards healthy cells, which is a promising feature for use in cancer treatment since it might reduce the side effects of most current therapies.

The term Munchausen Syndrome (MS) was first described in 1951 by Asher to characterize individuals who intentionally produce signs and symptoms of a disease. People with this syndrome pretend to be sick and tend to look for treatment, without secondary gain, in different health services. A report of an 18-year-old woman in renal lithiasis research is reported and, at the end of the investigation, it was concluded that the "stones" were not of renal origin. The MS is rare and promotes massive health care spending, with unnecessary and costly examinations, exposing patients to iatrogenics. An underdiagnosed disease with little knowledge of health professionals. Key-words: Munchausen Syndrome; Urinary calculosis; Iatrogeny.


The case of a 70-year-old man, with no previous pathological history, who consulted for post-traumatic injury to the scalp; it was managed as a benign lesion and it was taken to surgical resection with a histopathology and immunohistochemistry study compatible with clear cell neoplasia of metastatic renal origin. Extension studies were performed with findings of pulmonary nodules that suggested metastatic disease and with a finding of right renal mass. Radical nephrectomy is performed confirming the diagnosis of renal cell carcinoma subtype clear cells. A review of the literature was carried out, which shows that a skin lesion may be the first symptom of an internal neoplasm, with breast cancer being the most frequent one reported in the literature in cutaneous metastases. The finding of a posttraumatic injury to the scalp as the first sign of a clear cell carcinoma of renal origin is very rare. The diagnosis of skin metastases requires a histopathology and immunohistochemistry study, which must be timely because it may be the first link in the research chain to detect an occult neoplasm. In conclusion, the surgeon who faces a soft tissue tumor on the scalp must consider the characteristics of the lesion, the patient’s risk factors in an articulated way that allows him to suspect malignancy in a skin lesion and that allows him diagnose a previously unknown and asymptomatic neoplasm.

We present a case of an 85-year-old woman diagnosed with uncomplicated pyelonephritis, who was treated with intravenous ceftriaxone. Her chronic medications were phenprocoumon, diltiazem and bisoprolol. During the infectious phase, the patient presented tachycardia – despite high-dose beta-blocker treatment – and developed left acute heart failure, with acute renal failure (pre-renal origin). After introduction of furosemide diuretic therapy, clinical conditions improved and better control of the volemic status and heart rate was achieved. Several days after ceftriaxone and digoxin therapy initiation, worsening multiple non-blanching palpable purpuric lesions with bullae and papules, limited to the lower extremities, were noted. Skin biopsy was performed and a diagnosis of leucocytoclastic vasculitis, with associated panniculitis, was made. Ceftriaxone was discontinued and systemic corticosteroids were introduced, with a clear improvement in the cutaneous condition.
