Enterprise Risk Management (ERM) is an effective technique in managing risk within an organization strategically and holistically. Risk culture relates to the general awareness, attitudes and behaviours towards risk management in an organisation. This paper presents a conceptual model that shows the relationship between risk culture and ERM implementation. The dependent variable is ERM implementation, which is measured by the four processes namely risk identification and risk assessment; risk treatment; monitor and consult; communicate and consult. The independent variables under risk culture are risk policy and risk appetite; key risk indicators; accountability; incentives; risk language and internal relationships. This study aims to empirically test the relationship between risk culture and ERM implementation among Malaysian construction public listed companies. Risk culture is expected to have direct effects and significantly influence ERM. This study contributes to enhance the body of knowledge in ERM especially in understanding significant of risk culture that influence its’ implementation from Malaysian perspective.

Historically, a substantial proportion of women have been unduly detained in conditions of high security in England due to the lack of suitable alternative facilities. There have been no studies that have comprehensively examined the individual needs of these women in relation to their placement needs. A cross sectional survey was undertaken of the individual and placement needs of all females detained in the three high security psychiatric hospitals (HSPHs) in England on 18th October 1999. Whilst there are clearly differences in accommodation needs among the women, those requiring lower secure services are significantly less likely to have needs in relation to violence (OR 0.29, 95% CI 0.14-0.62) and drugs (OR 0.29, 95% CI 0.12-0.67) when length of stay is taken into account. They also have lower security, treatment and dependency needs than women still requiring HSPH treatment. A significant reorganisation of services is required. There is a need to gain further understanding of the relationships between traumatic histories, clinical presentation and antisocial behaviour and how these impact on assessed risk, treatment and care needs and outcome. The special needs of these women should be considered a priority when developing treatment and care packages.

Background Oral anticoagulation (OAC) can reduce stroke risk at the cost of increased bleeding risk in those with atrial fibrillation (AF). Observational data have shown that higher-risk patients with AF most likely to benefit from OAC are less likely to receive OAC at hospital discharge. Methods We used data from ORBIT-AF Registry, a cohort of 9,589 AF patients enrolled among 173 participating outpatient practices. OAC was defined as warfarin or dabigatran use at study enrollment. Stroke and bleeding risk were calculated using the CHADS2 and ATRIA scores, respectively. Results The study population had a mean age of 73.5 years; 57.8% were men. Overall, 76.4% of patients received OAC. Use of OAC rose with increasing CHADS2 stroke risk, from 67% for CHADS2 <1 to 80% for CHADS2 ≥2 (p<0.0001). OAC use fell slightly with increasing ATRIA bleeding risk, from 77% for ATRIA score ≤3 to 74% with ≥5 (p=0.002 for trend). Among patients with low bleeding risk, rates of OAC increased commensurate with stroke risk (p<0.0001 for interaction; see figure). Higher bleeding risk tended to decrease rates of OAC among patients with a CHADS2 score ≥2 (p=0.13 for interaction). Conclusions In community-based outpatients with AF, use of OAC rose with increasing thromboembolic risk and declined with higher bleeding risk. These findings suggest that the risk-treatment paradox may be less that previously reported. Provision of OAC in community practice appears to appropriately consider patients' stroke and bleeding risks. Further research is required to understand how quality improvement initiatives can further improve stroke prevention.

Lipid and lipoprotein disorders are well known risk factors for atherosclerosis and its complications. The level of atherogenic LDL-cholesterol (LDL-C) is directly related to an increased risk of occurrence and progression of ischemic heart disease. Epidemiological and clinical studies have shown that the use of statin therapy to decrease LDL-C can significantly reduce the incidence of mortality, major coronary events and the need for revascularization procedures in the different groups of patients. The findings of a large meta-analysis conducted by the Cholesterol Treatment Trialists? (CTT) collaborators showed that every 1.0 mmol/l reduction of atherogenic LDL-C is associated with a 22% reduction in cardiovascular diseases mortality and morbidity. However, despite the impressive results of the benefits of statin therapy, the EUROASPIRE study showed that about 50% of patients with ischemic heart disease did not achieve target LDL-C levels. According to the new ESC/EAS Guidelines for the Management of Dyslipidaemias in patients with a very high cardiovascular risk, treatment goal should be to decrease LDL-C below 1.8 mmol/l or ?50% of initial values. In the majority of patients that can be achieved by statin therapy. For this reason an adequate choice of statins is of crucial importance, whereby the needed reduction in atherogenic LDL-C, after the identification of its target level based on the assessment of total cardiovascular risk, can be achieved.

Patent foramen ovale is strongly associated with cryptogenic stroke. Variousclinical trials has shown the association between cryptogenic stroke andincidence of undelrying patent foramen ovale, these trials also shown thedecrease of cryptogenic stroke incidence with the treatment of patentforamen ovale Lesion. In the absence of absolute contraindications, patientswith patent foramen ovale are advised to undergo closure. Preproceduralexaminations such as trans esophageal echocardiography and pretreatmentwith anticoagulants are required to prevent peri and postprocedural adverseevents. Currently, patent foramen ovale Closure can be done through apercutaneous access with minimal risk. Treatment of patent foramen ovalecan help decrease future incidences of strokes

Background/ Aim. Transition from standard to highly conformal radiation therapy techniques, requires implementation of complex advanced dosimetry. The aim of the work was comparison of dosimetric parameters of 3DCRT and VMAT plan, as well as complications after treatment in relation to dosimetric parameters at gynecological cancer patients. Methods. Forty-nine gynecological cancer patients were included in the study. All patients were planned for 3D CRT, but due to unacceptable doses to organs at risk, treatment plans for IMRT or VMAT were generated for 21 patients. The patients were prescribed 50.4 Gy/28 fractions (4) and 45 Gy/25 fractions (45 patients). The coverage of PTV and doses to organs at risk were recorded. PTV margins were evaluated for both techniques according to the Van Herk formula. Results. ICRU 83 criteria were fulfilled in all 3DCRT /VMAT/IMRT plans providing optimal coverage of PTV. Doses to OARS: in average, the V45Gy in small bowel in IMRT/VMAT plans was four times smaller than the same of 3DCRT plans. The V45Gy of small bowels was in average 49.4cm3 in IMRT/VMAT plans, while in 3DCRT plans it was 211.6 cm3. In case of femoral head, significant reduction in V30Gy (10.8 % vs. 33.1%) and mean dose in case of IMRT/VMAT plans was recorded (30.4 Gy in 3DCRT vs 23.6 Gy). Rectum was planned with significantly lower dose in terms of V30Gy (79.5% vs 95.2%) in IMRT/VMAT plans. Bladder was better spared in VMAT plans in terms of V40Gy (51% vs. 91%), but maximum dose was higher in VMAT plans than in 3DCRT (50.1 Gy to 48.1 Gy in average). For all OARs there is statistically significant difference registered at p>0.05. Toxicities recorded in VMAT and 3DCRT patients include mainly radiation induced cystitis and enteritis. Patients treated with 3DCRT generally have longer recovery time. Homogeneity index was 0.11 for VMAT plans and 0.09 for 3DCRT plans. Conclusions. Analysis of dosimetric parameters revealed significant differences in normal tissue doses for same 3DCRT and VMAT patient, which confirmed necessity for implementation of advanced techniques for as many patients as possible.
