Comparative analysis of bacterial content, levels of lipopolysaccharides and lipoteichoic acid in symptomatic and asymptomatic endodontic infections at different stages of endodontic treatment

Endodontic therapy seeks to prevent and when necessary to eliminate endodontic infections. The persistence of microorganisms to the mechanical chemical preparation or the intracanal medication is considered the main cause of the failures of the endodontic treatment. In these cases Enterococcus faecalis is the most commonly found microorganism. Photodynamic therapy (PDT) emerges as a promising adjuvant therapy to conventional endodontic treatment to eliminate these persistent microorganisms. This therapy consists in the association of a specific light source with a photosensitizer in the presence of oxygen, producing cytotoxic species to the microorganisms. Thus, the present article aims at a review of the literature on the use of photodynamic therapy in endodontics, a test that is shown to assist in the preparation of the root canal.

Background. The main etiology of endodontic treatment failure is caused by bacteries that stay in the root canal. E.faecalis is a bactery that is found as an etiology of endodontic treatment failure. Cell wall of this bacteria is containing Lipoteichoic acid (LTA). LTA can penetrate into the periradicular tissue, act as endotoxin in host and cause periradicular inflammation then lead to bone destruction. Bone destruction occurs due to the inflammation process that is mediated by immune system. The important cell in the process of bone destruction is osteoclast. Bone destruction is marked by the form of osteoclast that is called osteoclastogenesis. NFATc-1 and osteocalcin play important things in osteoclastogenesis. Purpose. The aim of this study is to know about the expression of NFATc-1 and osteocalcin during the periapical bone destruction due to induction of E.faecalis. Method. This study used laboratory experimental with the post test only control group design. A total of 54 male rats were randomly divided into 2 main groups, which each main group had 3 subgroups. Group A (control) : every tooth was induced only by sterile BHIb. Group A had 3 subgroups (A Control day 3, 10, and 21), group B : every tooth was induced by 10 μl BHI-b E.faecalis ATCC212(106 CFU), it was contained 3 sub groups (B day 3,10, and 21). The animals were sacrificed based on their days scheduled group and prepared for histological examination of periapical bone, then we did the immunohistochemistry followed by calculation on the light microscope. Result. The analysis revealed that the expression of NFATc-1 and osteoclast increased significantly in group B when E.faecalis was induced. Conclusion. From this study we know that the expression of NFATc-1 and osteocalcin are increasing during the periapical bone destruction that induced by E.faecalis.

The importance of endodontics is presented within our own concept of Dentistry Sustainable Development (DSD) consisting of three inseparable elements; i.e., Advanced Interventionist Dentistry 4.0 (AID 4.0), Global Dental Prevention (GDP), and the Dentistry Safety System (DSS) as a polemic, with the hypothesis of the need to abandon interventionist dentistry in favour of the domination of dental prevention. In view of the numerous systemic complications of caries that affect 3−5 billion people globally, endodontic treatment effectively counteracts them. Regardless of this, the prevention of oral diseases should be developed very widely, and in many countries dental care should reach the poorest sections of society. The materials and methods of clinical management in endodontic procedures are characterized. The progress in the field of filling materials and techniques for the development and obturation of root canals is presented. The endodontics market is forecast to reach USD 2.1 billion in 2026, with a CAGR of 4.1%. The most widely used and recognized material for filling root canals is gutta-percha, recognized as the “gold standard”. An alternative is a synthetic thermoplastic filler material based on polyester materials, known mainly under the trade name Resilon. There are still sceptical opinions about the need to replace gutta-percha with this synthetic material, and many dentists still believe that this material cannot compete with gutta-percha. The results of studies carried out so far do not allow for the formulation of a substantively and ethically unambiguous view that gutta-percha should be replaced with another material. There is still insufficient clinical evidence to formulate firm opinions in this regard. In essence, materials and technologies used in endodontics do not differ from other groups of materials, which justifies using material engineering methodology for their research. Therefore, a detailed methodological approach is presented to objectify the assessment of endodontic treatment. Theoretical analysis was carried out using the methods of procedural benchmarking and comparative analysis with the use of contextual matrices to virtually optimize the selection of materials, techniques for the development and obturation of root canals, and methods for assessing the effectiveness of filling, which methods are usually used, e.g., in management science, and especially in foresight research as part of knowledge management. The results of these analyses are presented in the form of appropriate context matrices. The full usefulness of the research on the effectiveness and tightness of root canal filling using scanning electron microscopy is indicated. The analysis results are a practical application of the so-called “digital twins” approach concerning the virtual comparative analysis of biomaterials used in endodontic treatment.

Objective. To describe a pattern of antibiotic prescription during root canal treatment procedures based on the reports of Lithuanian general dental practitioners. Materials and methods. Questionnaires concerning endodontic treatment were sent to all 2850 Lithuanian dental practitioners registered on the database of the Lithuanian Dental Chamber. Only the questions containing general information and individual decisions about antibiotic prescription for endodontic treatment were selected for the analysis. Results. The response rate was 53.8%. Of the 1532 responses, 1431 questionnaires received from licensed general dental practitioners were analyzed. More than 60% of the respondents reported prescribing antibiotics in cases of symptomatic apical periodontitis. Majority of the respondents (83.9%) reported symptomatic apical periodontitis with periostitis being a clear indication for the prescription of antibiotics. Nearly 2% of the respondents reported prescribing antibiotics in cases of symptomatic pulpitis. With an increasing duration of professional activity, an increase in the proportion of the respondents reporting prescription of antibiotics was observed. Amoxicillin was the most preferable antibiotic during endodontic treatment, followed by amoxicillin with clavulanic acid. An increase in reporting penicillin prescription and decline in prescribing amoxicillin and amoxicillin with clavulanic acid with regard to the increasing age of respondents was observed. Conclusions. Lithuanian dentists, particularly those with longer duration of professional activity, tended to exceed general recommendations for the antibiotics prescription in cases of endodontic infections. However, a trend toward prescription of broader spectrum antibiotics by younger dentists, compared with those with longer professional activity, was observed.

Introduction: Photodynamic therapy (PDT) has emerged as a powerful tool for conventional endodontic treatment, capable of eradicating microorganisms present in endodontic infections. Despite this, the use of PDT in the surgical endodontic treatment is not well-known. Case Report: This paper describes the case of a patient with asymptomatic apical periodontitis in a mandibular incisor submitted to surgical endodontic treatment associated with PDT. After conventional procedures, photosensitizer methylene blue (0.01%) was applied to the retrograde cavity for 3 minutes, followed by irradiation with diode laser (100 mW and 660 nm) for 3 minutes. Six months after the procedure, the patient was asymptomatic, and the radiographic examination showed healthy periradicular tissues. Conclusion: The association of PDT with surgical endodontic treatment was effective, suggesting that this therapy may provide additional benefits to patients when compared to the conventional surgical technique.
