Myocardial interstitial fibrosis is an important contributor to the development of heart failure. Type 3 p90 ribosomal S6 kinase (RSK3) was recently shown to be required for concentric myocyte hypertrophy under in vivo pathological conditions. However, the role of RSK family members in myocardial fibrosis remains uninvestigated. Transgenic expression of α-tropomyosin containing a Glu180Gly mutation (TM180) in mice of a mixed C57BL/6:FVB/N background induces a cardiomyopathy characterized by a small left ventricle, interstitial fibrosis, and diminished systolic and diastolic function. Using this mouse model, we now show that RSK3 is required for the induction of interstitial fibrosis in vivo. TM180 transgenic mice were crossed to RSK3 constitutive knockout ( RSK3−/−) mice. Although RSK3 knockout did not affect myocyte growth, the decreased cardiac function and mild pulmonary edema associated with the TM180 transgene were attenuated by RSK3 knockout. The improved cardiac function was consistent with reduced interstitial fibrosis in the TM180; RSK3−/− mice as shown by histology and gene expression analysis, including the decreased expression of collagens. The specific inhibition of RSK3 should be considered as a potential novel therapeutic strategy for improving cardiac function and the prevention of sudden cardiac death in diseases in which interstitial fibrosis contributes to the development of heart failure.

Background: Radiography is widely used for diagnosis of congestive heart failure as it enables non-invasive assessment of cardiac size, shape and pulmonary vasculature. So, the present study was conducted to record the radiographic changes in dogs with congestive heart failure.Methods: Fifty-one dogs with cardiac insufficiency brought to Teaching Veterinary Hospital of GADVASU, showing one of the clinical signs of chronic cough, dyspnea, exercise intolerance, abdominal distension, syncope and cyanosis were selected and were subjected to Lateral and Ventro-dorsal chest radiography. Result: Dilated cardiomyopathy was present in 24 dogs. Radiographically, pulmonary edema, cardiomegaly, vessel congestion were more common in dilated cardiomyopathy (DCM). Valvular diseases were present in 16 dogs and radiographically left atrial (LA) dilatation was present in dogs with valvular diseases. Pericardial effusions were present in 11 dogs showing enlarged globoid heart radiographically.

Sudden cardiac death is one of the leading causes of death in the western industrial nations. Most people are affected by coronary heart disease (coronary heart disease, CHD) or heart muscle (cardiomyopathy). These can lead to life-threatening cardiac arrhythmias. If the heartbeat is too slow due to impulse or conduction disturbances, cardiac pacemakers will be implanted. High-frequency and life-threatening arrhythmias of the ventricles (ventricular tachycardia, flutter or fibrillation) cannot be treated with a pacemaker. In such cases, an implantable cardioverter-defibrillator (ICD) is used, which additionally also provides all functions of a pacemaker. The implantation of a defibrillator is appropriate if a high risk of malignant arrhythmias has been established (primary prevention). If these life-threatening cardiac arrhythmias have occurred before and are not caused by a treatable (reversible) cause, ICD implantation will be used for secondary prevention. The device can stop these life-threatening cardiac arrhythmias by delivering a shock or rapid impulse delivery (antitachycardic pacing) to prevent sudden cardiac death. Another area of application for ICD therapy is advanced heart failure (heart failure), in which both main chambers and / or different wall sections of the left ventricle no longer work synchronously. This form of cardiac insufficiency can be treated by electrical stimulation (cardiac resynchronization therapy, CRT). Since the affected patients are also at increased risk for sudden cardiac death, combination devices are usually implanted, which combine heart failure treatment by resynchronization therapy and the prevention of sudden cardiac death by life-threatening arrhythmia of the heart chambers (CRT-D device). An ICD is implanted subcutaneously or under the pectoral muscle in the area of the left collarbone. Like pacemaker implantation, ICD implantation is a routine, low-complication procedure today.

Long term of heart failure symptoms will affect the patient functional status and ability to do self-care appropriately. Decreasing functional status and self-care ability will certainly affect the quality of life of patients. This study aimed to determine the relationship of self-care with the quality of life of heart failure patients that undergoing treatment at Heart Polyclinic of Mangusada Hospital. The research design was descriptive correlational using cross sectional approach. The study was conducted in January-February 2019. Total respondent participated were 61 respondents and were taken using a purposive sampling technique. Self care of heart index (SCHFI) questionnaire and the Minnesota Living With Heart Failure Questionnaire (MLHFQ) were used. The results showed that the average of self-care score was 45.25 and the average score of the quality of life was 60.67. Analysis using the Spearman Rank test obtained p-value = 0,000 and r = 0.506. It can be concluded that self care had a significant correlation on quality of life patients with heart failure in Mangusada Hospital. We recommended that nursing service providers play an active role in providing education and motivation to heart failure patients to improve self-care abilities.
