BackgroundRecent studies suggest that purging disorder (PD) may be a common eating disorder that is associated with clinically significant levels of distress and high levels of psychiatric co-morbidity. However, no study has established evidence of disorder-related impairment or whether distress is specifically related to PD rather than to co-morbid disorders.MethodThree groups of normal-weight women [non-eating disorder controls (n=38), with PD (n=24), and with bulimia nervosa (BN)-purging subtype (n=57)] completed structured clinical interviews and self-report assessments.ResultsBoth PD and BN were associated with significant co-morbidity and elevations on indicators of distress and impairment compared to controls. Compared to BN, PD was associated with lower rates of current and lifetime mood disorders but higher rates of current anxiety disorders. Elevated distress and impairment were maintained in PD and BN after controlling for Axis I and Axis II disorders.ConclusionsPD is associated with elevated distress and impairment and should be considered for inclusion as a provisional disorder in nosological schemes such as the Diagnostic and Statistical Manual to facilitate much-needed research on this clinically significant syndrome.

This study explored the associations between individuals presenting different problematic eating behaviors (Objective/Subjective binge-eating—OBE/SBE—and Compulsive/Non-compulsive grazing—C_Grazing/NC_Grazing) and eating disorder related symptoms. About 163 pre- and 131 post-bariatric patients were assessed. Assessment included: Face-to-face clinical interview to assess binge-eating and grazing episodes, and self-report measures to assess eating disorder symptomatology, psychological distress, and negative urgency. OBE and NC_Grazing were the problematic eating behaviors most and least associated with psychopathology, respectively. OBE and C_Grazing uniquely accounted for the significant variance in the most disordered eating variables. Our findings emphasize the need for the conceptualization of grazing behavior in the spectrum of disordered eating.

This study extended research on the specificity of the effects of the carbon dioxide (CO₂) challenge by examining panic reactivity in participants with bulimia nervosa (BN) (n=15) compared to those without bulimia nervosa (n=31). All participants completed self-report measures assessing state and trait anxiety, depression, anxiety sensitivity (AS), distress tolerance (DT), discomfort intolerance (DI), and eating disorder features. They subsequently breathed two vital capacity inhalations; room air and 35% CO₂-enriched air. Reactivity to room air was not different between groups. However, participants with BN displayed greater reactivity to CO₂ compared to the participants with BN. AS, DI, and DT could not be tested as potential mediators in the association between diagnostic group and reactivity because these constructs were not associated with reactivity. Eating disorder features and frequency of binges and purges were also not associated with reactivity. Detailed implications and suggestions for further research are discussed.

OBJECTIVEThere is increasing evidence of the impact of ultra-processed foods on multiple metabolic and neurobiological pathways, including those involved in eating behaviours, both in animals and in humans. In this pilot study, we aimed to explore ultra-processed foods and their link with disordered eating in a clinical sample. METHODSThis was a single site, retrospective observational study in a specialist eating disorder service using self report on the electronic health records. Patients with a DSM-5 diagnosis of anorexia nervosa (AN), bulimia nervosa (BN) or binge eating disorder (BED) were randomly selected from the service database in Oxford from 2017 to 2019. The recently introduced NOVA classification was used to determine the degree of industrial food processing in each patient’s diet. Frequencies of ultra-processed foods were analysed for each diagnosis, at each mealtime and during episodes of bingeing.RESULTS71 female and 3 male patients were included in the study. 22 had AN, 25 BN and 26 had BED. Patients with AN reported consuming 55% NOVA-4 foods, as opposed to approximately 70% in BN and BED. Binge foods were 100% ultra-processed.DISCUSSIONFurther research into the metabolic and neurobiological effects of reducing ultra-processed food intake on bingeing behaviour is needed.

Clinicians and researchers have several approaches with which to assess eating disorder and related symptomatology, including interviews, self-report instruments, and behavioral measures. The purpose of this chapter is to describe a process, based on a functional approach, that will help assessors to develop assessments and choose instruments for eating disorders and eating-related problems. This approach takes into account both theoretical and practical concerns and allows assessors to individualize their assessments depending on their particular needs. This process starts with broad considerations about the context in which the assessment is to be given and ends with the choice of specific instruments to be used.

Abstract Objectives Both the relative reinforcing value (RRV) of food, or the motivation to obtain food, and eating disorder (ED) pathology have been shown to independently predict weight gain. However, less is known about how the interaction between the RRV of food and ED pathology predicts weight gain over time. Therefore, the purpose of this study was to examine the combined effects of the RRV of food and ED pathology on weight change over 6 months in a sample of adolescents. Methods Participants included 77 12–14 year old adolescents participating in a longitudinal study examining factors that predict changes in weight status. Data presented are from baseline and 6 months. The RRV of food was assessed using a computer task. Participants earned points for energy dense food by pressing a mouse button on a computer across escalating schedules of reinforcement. Participants were classified as “low” or “high” in RRV based on a median split of their total responses. Global ED pathology was assessed at baseline using adolescent self-report on the Eating Disorder Examination Questionnaire. Participants were classified as “low” or “high” in ED pathology based on a median split of their global ED pathology. Four groups were created: low ED pathology/low RRV (n = 20), high ED pathology/low RRV (n = 23), low ED pathology/high RRV (n = 15), and high ED pathology/high RRV (n = 19). Height and weight were measured at both baseline and 6 months and used to calculate zBMI. ANOVA was used to examine differences in zBMI change over 6 months by RRV/ED pathology group. Results zBMI change from baseline to 6 months differed by RRV/ED pathology group (P < .05). Changes in zBMI over 6 months were as follows: - 0.025 ± 0.298 for low ED pathology/low RRV; 0.010 ± 0.322 for high ED pathology/low RRV; - 0.095 ± 0.181 for low ED pathology/high RRV; and 0.186 ± 0.268 for high ED pathology/high RRV. Follow-up contrasts revealed that the high ED pathology/high RRV group experienced greater zBMI changes than the other groups (ps < 0.05). Conclusions While the RRV of food and ED pathology are both independently associated with weight gain, the current study indicates that there may be something unique about the combination of high RRV of food and high ED pathology related to risk of weight gain. Future work is needed to identify strategies to limit weight gain in this vulnerable population. Funding Sources National Institutes of Health.

ObjectivesThis study examined formal and informal healthcare use (HCU) in community women with disordered eating, and associations of HCU with mental health-related quality of life (MHRQoL), psychological distress, mental health literacy (MHL) and eating disorder (ED) symptoms over time.HypothesisWe hypothesised that HCU would lead to improvement in ED symptom severity, MHRQoL, MHL and psychological distress.Design, setting, participantsData were from years 2, 4 and 9 of a longitudinal cohort of 443 community women (mean age 30.6, SE 0.4 years) with a range of ED symptoms, randomly recruited from the Australian Capital Territory electoral role or via convenience sampling from tertiary education centres. Data were collected using posted/emailed self-report questionnaires; inclusion criteria were completion of the HCU questionnaire at time point of 2 years (baseline for this study). HCU was measured using a multiple-choice question on help seeking for an eating problem. To test the effect of HCU over time on MHRQoL (Short Form-12 score), psychological distress (Kessler Psychological Distress Scale score), ED symptom severity (Eating Disorder Examination Questionnaire score) and ED–MHL, linear or logistic mixed-effects regression analyses were used.Results20% of participants sought ED-specific help at baseline; more than half of participants sought help that was not evidence based. HCU at baseline was significantly associated with improved MHRQoL and ED symptom severity and decreased psychological distress over time (Cohen’s d all >0.3, ie, small). HCU was not significantly associated with MHL over time. The predictive ability of the fitted models ranged from 32.18% to 42.42% for psychological distress and MHL treatment, respectively.ConclusionsFormal and informal HCU were associated with small improvements in ED symptoms, MHRQoL and psychological distress but not with improved MHL. Informal services in ED management should be investigated further along with efforts to improve ED–MHL.

Objective: To identify the diagnostic subtypes of eating disorders (EDs), the psychiatric comorbid diagnoses, and associated specific and nonspecific psychopathology in a series of 120 adolescents undergoing standardized assessment for an ED. Method: Consecutive patients referred to our large pediatric hospital for ED assessment completed a semistructured diagnostic interview for children and adolescents. The following self-report scales were administered to assess specific and nonspecific psychopathology: the Children's Depression Inventory (CDI), the Brief Symptom Inventory (BSI), the Eating Disorder Inventory 2 (EDI-2), and the Family Assessment Measure (FAM-III) of family functioning. Results: Female subjects with a mean age of 14.5 years and a mean body mass index (BMI) of 18.1 comprised 93% of the sample. The restrictive subtypes of anorexia nervosa (AN) (43%) and eating disorder not otherwise specified (EDNOS) (16%) were the most common diagnoses. Patients with restricting symptoms (R) could be grouped together because they were more similar to each other with respect to self-report symptoms of psychopathology than they were to patients with binge/purge (B/P) symptoms and vice versa. Patients with R endorsed significantly fewer subjective symptoms, both ED-specific and nonspecific, and rated their families' functioning better than did B/P patients. Comorbid, current major depressive disorders and dysthymic disorders occurred in 66% of subjects, but depressive, dysthymic, and oppositional disorders occurred in 96% of those with B/P symptoms. Severity of the CDI was the best single discriminator between R and B/P subjects. Conclusions: Adolescents with EDs in the early stage of their illness are similar to adults with EDs in the following ways: they meet the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria for subtypes of EDs (excluding amenorrhea) and commonly have comorbid psychiatric disorders, especially depressive disorders. Patients with B/P symptoms can be distinguished from restricting subjects because they endorse significantly more ED-specific and nonspecific psychopathology and have a higher frequency of comorbid Axis I diagnoses (especially depressive disorders) than restricting patients. Oppositional defiant disorder (ODD) occurs more commonly in adolescents with EDs associated with B/P symptoms.
